Influence on Amputee Gait by the Ankle Joint Alignment (발목관절 조절각도가 절단환자의 보행에 미치는 영향)

  • Socket pressure distributions with gait analysis of a trnsfemoral and a transtibial prostheses were measured in order to assess an optimal socket fitting and function. Ankle joint was aligned by the neutral and the dorsi/plantar flexed positions. Compared to dorsi and plantar flexed positions of ankle joint, cadence and walking speed increased with the neutral ankle joint alignment. Other gait parameters were close to the normative data with the neutral ankle joint alignment. For the transfemoral amputee, dorsiflexed alignment of the ankle joint created high pressure on the lateral aspect of the socket, on the other hand, plantarflexed alignment resulted in increased pressure on the medial aspect of the socket. For the transtibial amputee, dorsiflexed alignment of the ankle resulted in high pressure on the antero-lateral aspect of the socket during mid-stance, but apltarflexion of the ankle joint showed slight increases in pressure at the same location in the socket. The present study clearly demonstarted that malalignment of a prosthesis results in localized increasesing pressure within the socket. Proper slignment of the prosthesis is required in order to acquire an appropriate socket-limb interface as well as the proper gait.

Analysis of trunk angle and muscle activation during chest compression in 119 EMTs (가슴압박시 구급대원의 체간 각도와 근활성도 분석)

  • Purpose: We aimed to investigate trunk angle and muscle activation of the extremity and back to evaluate the effect of chest compression on work-related musculoskeletal disorders in 119 emergency medical technicians (EMTs). Methods: Eighteen 119 EMTs performed 2-minute chest compression without interruption on a cardiopulmonary resuscitation manikin, during which we measured changes in the trunk and shoulder joint angles, muscle activation (triceps brachii, biceps brachii, erector spinae, gluteus maximus, pectoralis major, rectus abdominis, and rectus femoris) and chest compression accuracy. Results: The decrease in trunk angle by trunk muscle activation was the highest in event 2, the major direction of chest compression. Both shoulder joint angles had no significant difference. Muscle activation of the triceps brachii (p < .01), biceps brachii (p < .05), rectus abdominis (p < .05) and rectus femoris (p < .01) significantly increased during the compression phase compared with the decompression phase, with the rectus femoris showing an increase of 19%. Muscle activation of the erector spinae significantly increased in the decompression phase compared with the compression phase (p < .01). Conclusion: 119 EMTs mainly use the triceps brachii, biceps brachii and pectoralis major muscles during chest compression.

An Analysis on the Prehospital Care of 119 Emergency Medical Service squads in Gyeonggi Area and Improvement Methods - Focusing on Prehospital Care - (경기도지역 119안전센터의 구급활동 현황과 개선방안 - 현장 응급처치 내역을 중심으로 -)

  • Purpose : The purpose of this study was to analyze the prehospital care report prepared by EMS squads of 119 center in Gyeonggi area and to identify the status of prehospital care activities, problems and improvement possibilities of the emergency care. Method : Five hundred copies of prehospital care report prepared by EMS squads in thirty-eight 119 centers under 18 fire stations in Gyeonggi area from March to April in 2008 were randomly chosen for the analysis. Data abstracted according to the purpose of the study were input and the analysis of prehospital care activities were performed using SPSS-WIN(ver. 16) statistics package. Results : 1. Total 500 cases of prehospital emergency care activities were reviewed. By sex, females were 219 (43.8%) and males were 281 (56.2%). The places of reporting were home (57.8%). According to the type of emergency, 281 cases (56.2%) were caused by disease and 291 (43.8%) were from other causes such as traffic accidents or incidental injuries. 2 The average time needed for the 119 EMS squad to arrive at the scene after being reported was 7.29 minutes. The time used at the scene for the emergency care was 7.3 minutes in average and the time to arrive at the hospital was 25.4 minutes in average. 3. In patient evaluation, in 68% of the cases more than two vital signs were measured at one time and emergency patients were 31%, and non-emergency patients were 69%. 4. In one EMS activity, average 2.15 cases of emergency care were provided to a patient. The cases where two kinds of emergency care were given were 14.4%, which is the most frequent cases. When reviewing the details of the emergency cares given to the patient including multiple cares, trivial cards (taking it easy and giving comfort) were the most frequent one as 40.6% and the medical direction of the doctor was given in only one case out of 500 cases. 5. In patient evaluation and emergency care, vital signs were provided to emergency patients at significantly higher rate comparing to the non-emergency patients. The number of emergency care performance was significantly higher in emergency cases. In emergency dispatch, the cases that EMT (1st class) was on board was 86.2%. When comparing the cases when the 1st class EMT was on board and otherwise, the cases with the presence of 1st class EMT showed more vital signs were detected but there was no significant difference in the number of emergency cares provided. Conclusion : It seemed that the on-scene emergency care did not satisfy the expectations. So it is necessary to enforce the cooperation between the elements, the qualifications of the 119 EMS squads and to improve the prehospital working environment in order to provide the better medical service at any time.

The Effects of Empowerment of the paramedics on Critical Thinking (1급 응급구조사의 임파워먼트가 비판적 사고에 미치는 영향)

  • Purpose : This study aimed to examine the effects of empowerment of the paramedics on critical thinking and provide the basic materials for the qualitative improvement of emergency medical service. Methods : This study collected data with the paramedics working at general hospital and hospital emergency room in G metropolitan city, B metropolitan city, U metropolitan city, D metropolitan city and J province from July 1 to 31, 2008. Total 180 questionnaires were collected and 152 were used for final analysis, and the following results were obtained through statistical analysis using SPSS 12.0 program. Results : 1) Average score of empowerment was 4.44 out of 6 and according to average score by areas, significance was 4.94, capacity 4.92, self-determination 4.28, and effective-ness 3.62. 2) Empowerment by general characteristics was statistically significant in age (F = 3.313, p < 0.05), the final scholastic attainments(F = 2.436, p < 0.05), and salary(F = 1.695, p < 0.01). 3) Average score of critical thinking was 3.12 out of 7 and according to scores by areas, maturity was highest as 3.71, followed by no prejudice as 3.70, systemicity as 3.14, pursuit of truth as 3.05, much curiosity as 2.93, critical thinking and self-confidence as 2.92, and analysis as 2.91. 4) Critical thinking by general characteristics was statistically significant in marital status (F = 15.695, p < 0.01) and the final scholastic attainments (F = 2.606, p < 0.05). 5) Correlations between empowerment and critical thinking showed positive correlations as Pearson's correlation coefficient r = 0.400 and positive correlation in all areas including empowerment and critical thinking were found (r = 0.116-0.710). 6) The effect of empowerment on critical thinking was statistically significant in p < 0.01 and was explained as $R^2=0.155$. In the effects of empowerment on critical thinking, significance, capacity and self-determination were statistically significant in p < 0.01, effectiveness in p < 0.05 and were explained as R2 = 0.244. Conclusion : From the above results, it was found that empowerment had the influence on critical thinking, so it was considered that hospital managers and emergency room chiefs must develop and operate education and training program based on the concept of empowerment, maintenance and management strategies.

Construct Validation of the Short Sensory Profile-2 (SSP-2) for Children With Autism Spectrum Disorder (자폐스펙트럼 장애 아동에 대한 단축형 감각 프로파일-2(Short Sensory Profile-2)의 구성타당도 연구)

  • Objective : The purpose of this study was to verify the construct validity of Short Sensory Profile-2 (SSP-2) for children with Autism Spectrum Disorder (ASD). Methods : Data were collected from SSP-2 for 120 parents of ASD children. Raw data were analyzed by applying the Rasch analysis to the goodness fit of person and item, item difficulty, rating scale, and separation reliability of SSP-2. Results : 7 persons in sensory processing area and 8 persons in behavioral response area were inappropriate criteria and excluded from the analysis. Item goodness-of-fit analysis determined that the If the Mnsq value is between 0.6 and 1.4 and the Z value is outside the ±2 range for nonconformity. this study All items in the instrument were found to have appropriate criteria. Item difficulty analysis in sensory processing area was high in item 13 (.48 logit) and low in item 10 (-.54 logit). In the behavioral response area, item 25 (1.58 logit) was high and item 30 (-.68 logit) was low. In the rating scale analysis, it was found that the 3-point scale is more appropriate than the 5-point scale. The separation reliability of sensory processing area was .90 and the behavioral response area was .95. Conclusion : This study verified the construct validity of SSP-2 and expected to be applied as a useful evaluation tool for children with ASD.

The Effects of Levelers on Electroplating of Thin Copper Foil for FCCL (전기도금법을 이용한 FCCL용 구리박막 제조시 레벨러의 영향 연구)

  • In recent days, the wire width of IC is narrowed and the degree of integration of IC is increased to obtain the higher capacity of the devices in electronic industry. And then the surface quality of FCCL(Flexible Copper Clad Laminate) became increasingly important. Surface defects on FCCL are bump, scratch, dent and so on. In particular, bumps cause low reliability of the products. Even though there are bumps on the surface, if leveling characteristic of plating solution is good, it does not develop significant bump. In this study, the leveling characteristics of additives are investigated. The objective of study is to improve the leveling characteristic and reduce the surface step through additives and plating conditions. The additives in the electrodeposition bath are critical to obtain flat surface and free of defects. In order to form flat copper surface, accelerator, suppressor and leveler are added to the stock solution. The reason for the addition of leveler is planarization surface and inhibition of the formation of micro-bump. Levelers (SO(Safranin O), MV(Methylene Violet), AB(Alcian Blue), JGB(Janus Green B), DB(Diazine Black) and PVP(Polyvinyl Pyrrolidone) are used in copper plating solution to enhance the morphology of electroplated copper. In this study, the nucleation and growth behavior of copper with variation of additives are studied. The leveling characteristics are analyzed on artificially fabricated Ni bumps.

Intelligent Diagnosis Assistant System of Capsule Endoscopy Video Through Analysis of Video Frames (영상 프레임 분석을 통한 대용량 캡슐내시경 영상의 지능형 판독보조 시스템)

  • Capsule endoscopy is one of the most remarkable inventions in last ten years. Causing less pain for patients, diagnosis for entire digestive system has been considered as a most convenience method over a normal endoscope. However, it is known that the diagnosis process typically requires very long inspection time for clinical experts because of considerably many duplicate images of same areas in human digestive system due to uncontrollable movement of a capsule endoscope. In this paper, we propose a method for clinical diagnosticians to get highly valuable information from capsule-endoscopy video. Our software system consists of three global maps, such as movement map, characteristic map, and brightness map, in temporal domain for entire sequence of the input video. The movement map can be used for effectively removing duplicated adjacent images. The characteristic and brightness maps provide frame content analyses that can be quickly used for segmenting regions or locating some features(such as blood) in the stream. Our experiments show the results of four patients having different health conditions. The result maps clearly capture the movements and characteristics from the image frames. Our method may help the diagnosticians quickly search the locations of lesion, bleeding, or some other interesting areas.

Development and Evaluation of Separation Anxiety Scale for Mothers Using Child Care Facilities (보육시설 이용 어머니의 격리불안 측정 도구 개발 및 평가)

  • The aim of this study was to develop and validate an instrument to measure the separation anxiety of mothers who use child care facilities. The scale was developed as follows: initial items generated, validity and reliability tests performed, and final items extracted. The initial items were developed through a literature review and interviews. The content validity was verified by five experts. Using a final questionnaire, the data was collected from 204 mothers who used child care facilities with a convenient sampling method. The construct validity was evaluated by item analysis, exploratory factor analysis and confirmatory factor analysis. The results of confirmatory and exploratory factor analyses showed that the separation anxiety consisted of 6 factors: the mother's guilty feelings (5-items), safety of child care facilities (5-items), trust of teachers (5-items), child's anxiety (4-items), interaction anxiety with the child (4-items), and management of child care facilities (3-items). The separation anxiety scale for mothers using child care facilities consisted of 26 items. The reliability of the developed scale was examined. The Cronbach's ${\alpha}$ coefficient was 0.93. These results show that this scale is a valid and reliable instrument for measuring the separation anxiety level of mothers who use child care facilities.

Nurses' Perception of Flexible Working and Its Operational Guideline (간호사의 시간선택제 근무에 대한 인식과 근무방안)

  • The purpose of the study was to explore the nurses' perception of flexible working and present an operational guideline for implementation. The data were collected from a survey of 532 nurses conducted from 10 February to 24 February, 2015, using simple random sampling. SPSS 21.0 was used to analyze the data. This study found that the demanding for shorter hour shifts was highest ($M=3.89{\pm}0.87$) and 242 participants (46.2%) intended to apply the shift, followed by a fixed night shift, fixed weekend shift, and 12 hour shift. The common reasons identified were the poor environment of shift work and the challenges of child care. The increased time for personal development, relaxation activity to improve the quality of life and child care was highlighted and work-life balance was noted as a positive impact of flexible working. The negative impacts of flexible working were being likely to become temporary workers. These results can help establish uniform working conditions of nurses in the future by providing a standardized operational guideline as a precondition for reasonable compensation and working environment according to a flexible working pattern.

Formation of Zn(II) Complexes of Physiological Buffer Amino alcohols in Aqueous Solution (수용액 중에서 생리학적 완충제 아미노 알코올과 Zn(II) 이온과의 착물 형성)

  • The complex formation from Zn(II) ion and 2-(2-hydroxyethylamino)-2-(hydroxymethyl)-1,3-propanediol (Monotris), Bis(2-hydroxyethyl)imino-tris(hydroxymethyl)methane(Bistris) in aqueous solution at $25^{\circ}C$ and at an ionic strength of 0.10 M have been studied potentiometrically. For the Zn(II)-Monotris system, in the Monotris (L) complex $ZnL^{2+}$, one of the hydroxyl oxygen atoms as well as the amine nitrogen of the ligand are coordinated. In basic media, the coordinated hydroxyl group is deprotonated. For the Zn(II)-Bistris system, in the Bistris(L) complex $ZnL^{2+}$, two of the hydroxyl oxygen atoms as well as the amine nitrogen of the ligand are coordinated. In basic media, one of the coordinated hydroxyl groups is deprotonated. In very high basic media, an additional hydroxyl group undergoes deprotonation. The equilibrium constants for the formation of $ZnL^{2+}$, $ZnLH_{-1}{^+}$, $ZnLH_{-2}$, $Zn_2L_2H_{-2}{^{2+}}$ and $Zn_2L_2H_{-3}{^+}$ have been determined.
