  • Single pass radionuclide cardiac angiography was performed in 67-year old woman due to dyspnea and chest discomfort. The study showed markedly dilated ascending aorta and collateral ciculations from left sudclavian vein, and confirmed aortic aneurysm with partial obstruction of superior vena cava.

  • For development of low fuel consumption vehicle, design of engine can he changed and it brings collateral effect. Abnormal vibration is one of bad effect of high efficiency engine and it can be got over by active engine mount system. For development of active engine mount system, an adequate experimental equipment is required which contain characteristic of real engine movement. To make adequate experimental equipment, regulated force vibrating system including characteristics of vehicle chassis is required.

  • Anomalous origin of the coronary artery from the pulmonary artery is a rare congenital coronary artery disease and the origin of the left coronary artery from the pulmonary artery represents the commonest form of these unusual lesions. Because of differences in symptomatology, clinical course and prognosis, this malformation has been divided into infant type[Bl-and-White-Garland syndrome] and adult type on the basis of the absence or presence of collateral circulation between the right and left coronary artery. The latter type has been reported relatively few cases. A 21-year-old male was admitted to the Yeungnam University Hospital, due to study of incidentally noticed heart murmur. At that time he was asymptomatic and past medical history was noncontributory. Chest roentgenogram was within normal limit and electrocardiogram was consistent with hypertrophy of left ventricle. Echocardiogram and aortogram demonstrated markedly dilated and tortuous right coronary artery and anomalous origin of the left coronary artery from the pulmonary artery. To prevent arteriosclerosis, progressive myocardial infarction, infection and aneurysmal rupture, Takeuchi operation which establish a two coronary system by transpulmonary arterial reconnection of the anomalous left coronary artery was done. Postoperative course was uneventful.

  • Pulmonary atresia with ventricular septal defect and major aortopulmonary collaterals is a rare and complex lesion in which great morphologic variability exists regarding the sources of pulmonary blood flow. We report a case of this disease with no true central pulmonary arteries in a 9-month-old-boy successfully treated by one-stage complete unifocalization and repair from a midline stemotomy approach.

  • Although attention to terrorism has increased sharply in recent years within many countries, it is by no means a new phenomenon. Majority of these countries have limited regulations or guidelines about terrorism. LOP (Level Of Protection) can be consider as a first step. This paper seeks to present a process to determine LOP and allowable damage. LOP is determined by asset value reason why it should be based on cost. The asset value is defined as "cost induced when asset is damaged". For example, the collateral damage outside the facility should be taken in consideration in the asset value. Allowable structural damage is assigned depending on LOP.

  • The shape of combustion nozzles varies from large diameter to small diameter ones. In the case of small nozzle, nozzle exit can be easily winkled or damaged in machining process. Femtosecond laser is a micro machining technology that is able to drill a small nozzle without damaging the nozzle exit. In this experiment, a small nozzle of combustion was fabricated by using a femtosecond laser. The fabricated nozzle of combustion provided a very small nozzle diameter with clean nozzle exit without wrinkling or collateral damage.

  • The authors have performed operations on vascular system of 108 cases from 1972 through 1979, and analyzed the diseases, surgical procedures and results. They were 1. Arterial system; 45 cases P.D.A. : 20 Occlusive diseases : 13 Coarctation of aorta : 4 Aneurysm : 4 A-V fistula : 2 Trauma : 2 2. Venous system; 6 cases Esophageal varix : 4 S.V.C. syndrome : 1 Varicose vein : 1 3. Arteriovenous shunt for hemodialysis; 57 cases Of the arterial diseases, the worst results came from Burger`s disease. For the bleeding esophageal varices, we have performed ligations of varices or collateral circulations rather than emergency splenorenal shunt with good results. External A-V shunt for hemodialysis had much more complications than the A-V fistula.

  • Anomalous origin of right coronary artery from pulmonary artery is a rare congenital cardiac anomaly. Anomalous right coronary artery often be a incidental finding without serious cardiovascular sequale. The only characteristic physical finding is a continuous murmur with diastolic accentuation. There are no diagnostic EKG or chest X-ray changes. Diagnosis is made best by selective left coronary arteriography showing retrograde filling of right coronary artery from collateral vessels. Here, we present a case of twenty-four months old aged girl with anomalous origin of right coronary artery combined with pulmonary stenosis. This is the first pediatric patient with anomalous right coronary artery and the first patient to have surgical correction for this malformation.

  • Pulmonary atresia with VSD is uncommon congenital anomaly with high mortality in neonatal period. Recently we experienced surgical correction of 2 cases of pulmonary atresia with VSD. The first case was 7-year old female patient and diagnosed as pulmonary atresia with VSD combined PDA. So, total correction was undertaken which consisted of PDA ligation, patch repair of VSD, transannular enlargement of RVOT with woven Dacron vascular graft, and closure of PFO. Postoperative systemic Rt. ventricular and radial artery pressure ratio was 0.44 and her postoperative course was uneventful. The second case was 6-year old male patient diagnosed as pulmonary atresia with VSD and large systemic-pulmonary collateral arteries. There were two large systemic-pulmonary collaterals, one was simply controlled by ligation, but the other was considered to supply Rt. upper lung. So end to side anastomosis was performed to the RVOT patch. Postoperative systolic Rt. ventricular and radial artery pressure ratio was 0.54. During the follow up period he showed clinical picture of Rt. heart failure, which is relatively well controlled with anticongestive therapy.

  • The transcranial Doppler(TCD) is a technique for measuring blood flow velocity of intracranial and extarcranial arteries. This examination based on Doppler effect which was first formulated in 1842 by the Austrian physicist Christian Doppler. In 1982, Rune Aaslid first maked 2MHz pulsed probe and recording intracranial vessels with transcranially. There are six criteria utilized in gaining positive identification of the intracranial vessels. The six criteria are as follows l)acoustical windows 2)depth of sample volume 3)direction of flow 4)spatial relationship of ACA and MCA bifurcation 5)mean velocity and 6)response common carotid artery compression and/or oscillation test. The affected factors for TCD examination are angle of insonation, posture of subject, age, gender, hematocrit, metabolic factors, and cardiac output. Clinical application of TCD are detection of stenosis, occlusion, emboli, thrombsis in intracranial and extracranial arteries and evaluation of cerebral arterovenous malformation, collateral capacity in the circle of Willis, ischemia cerebrovascular disease, stroke patient and vertebrobasilar system.
