  • The JAXA's TechCLEAN project(2003-present) is summarized, with the interim technical achievements. TechCLEAN is collateral program with the NEDO engine project to accelerate R&D work of small passenger aircraft engine as well as to develop innovative environment technologies applicable to the future improvements. In the project NOx reduction, CO2 reduction and noise reduction are targeted. Component level researches and system demonstrator validation are planned with test facility renovation and demonstration base engines.

  • Ultrasonography (US) of the elbow is an increasingly utilized modality for a variety of diagnoses. In this brief review, US findings for the pathologic conditions of forearm and elbow are described. The most common pathologies discussed here include distal biceps tendon and triceps tendon lesions, medial and lateral epicondylopathies, ulnar collateral ligament tears, ulnar nerve subluxation, joint effusions, and intra-articular bodies.

A Comparative Study on Responses to Korean Version Questionaires on Respiratory Symptoms (호흡기계(系) 질환의 역학적(疫學的) 조사방법 개발에 관한 연구(I) -번역설문서 응답양상(應答樣相)에 대한 비교평가-)

  • Korean versions of British Medical Research Council (MRC), Cornell Medical Index(CMI), and American Thoracic Society 78 (ATS-DLD-78) respiratory questionaires were compared with each other, and were tested the stability, in terms of test-retest reliability, of each questions by self-administration of those to 156 medical students. The results obtained and conclusions drawn are as follow: 1. The degree of agreements between responses to the comparable questions of CMI vs MRC, and of CMI vs ATS-DLD-78 were not satisfactory. There were, however, $71{\sim}100$ per cent of agreement between responses to the questions on Cough, Wheezing, Phlegm, Breathlessness, and Chest illness of ATS-DLD-78 vs MRC questionaire. And the ATS-DLD-78 tended to yield greater number of positive responses than MRC (See Table 4). 2. All of the coefficient of stability of each questions in 3 questionaires were statistically significant, ranged $77{\sim}100$ per cent, except that of the question on episode of cough and phlegm in ATS-DLD-78 questionaire (See Table 5-1). The question is composed of two collateral conditions, 'lasting for 3 weeks or more' and 'each year'. 3. It can be insisted that the Section-B questions of CMI is not proper for use in epidemiologic survey on respiratory illness. And rather than MRC, the ATS-DLD-78 questionaire deserves to prefer to be used in epidemiologic studies on respiratory illness. 4. In question-wording, especially, of inquiring past experience, it is possible to lessen the reliability of the question that including collateral conditions such as 'the duration lasted of symptoms', and moreover, of which words are not common usage. For example, for Korean '10days' or 'half a month' is more familiar time unit rather than 'week'.

Determinants of Capital Structure of Korea Listed Firms (우리나라 상장기업(上場企業)의 자본구조(資本構造) 결정요인(決定要因)에 관한 연구(硏究))

  • The Purpose of this study is to test empirically the determinants of capital structure of the Korea Listed Firms. In order to accomplish the purpose of this study, both literature survey and empirical test have been made. For the empirical test, agency and asymmetric information factors as well as traditional ones have been throughly reviewed. Traditional factors tested in this study include firm-size, collateral value of the assets, business risk, tax, non-debt tax shields, and industry effects. Agency and asymmetric information factors include growth opportunities, the percentage of outstanding equity held by inside stockholders, and the number of inside stockholders. From the results of the cross-sectional regression analysis, the adjusted R-square is 1931%, and the overall F-value indicates significance. For the analysis period, the signs of the variables except business risk are as predicted. Firm-size, collateral value of the assets, and business risk significant at the.01-.05 level. In order to determine the influence of industry factors on the financial leverage, a total of 8 dummy variables are added to the regression model. The adjusted R-square inclosed by 4.2% for the first analysis period(1983-1985) and 6% for the second analysis period(1986-1987). This suggests that industry factors are significant in explaining the variations in financial leverage across firms. In order to pursue the influence of agency and asymmetric information factors on the financial leverage, again the cross-sectional regression analysis is done for the middle size firms gruop(n=40). The adjusted R-square increased by 9.8% for the first analysis period(1983-1985) and 6.1% for the total analysis period(1983-1987), and all the signs was as predicted. But both the variables except the number of inside stockholders was not significant.

Angiographic and Clinical Factors Related with Good Functional Outcome after Mechanical Thrombectomy in Acute Cerebral Artery Occlusion

  • Objective : The aim of this study is to investigate good prognostic factors for an acute occlusion of a major cerebral artery using mechanical thrombectomy. Methods : Between January 2013 to December 2014, 37 consecutive patients with acute occlusion of a major cerebral artery treated by mechanical thrombectomy with stent retrievers were conducted. We analyzed clinical and angiographic factors retrospectively. The collateral flow and the result of recanalization were sorted by grading systems. Outcome was assessed by National Institutes of Health Stroke Scale (NIHSS) and modified Rankin Scale (mRS) at 90 days. We compared the various parameters between good and poor angiographic and clinical results. Results : Twenty seven patients demonstrated good recanalization [Thrombolysis in Cerebral Infarction (TICI) 2b or 3] after thrombectomy. At the 90-day follow up, 19 patients had good (mRS, 0-2), 14 had moderate (3-4) and four had poor outcomes (5-6). The mRS of older patients (${\geq}75years$) were poor than younger patients. Early recanalization, high Thrombolysis in Myocardial Infarction risk score, and low baseline NIHSS were closely related to 90-day mRS, whereas high TICI was related to both mRS and the decrease in the NIHSS. Conclusion : NIHSS decreased markedly only when recanalization was successful. A good mRS was related to low initial NIHSS, good collateral, and early successful recanalization.

  • Subject : The physiological characteristic of Eight Extra Meridians. Objective : This study research physiological characteristic of Eight Extra Meridians that differentiate from the physiological characteristic of Twelve Regular Meridians. Method : First, we researched the meaning of "There are Eight Extra Meridians and they don't be arrested by Twelve Regular Meridians" in Nanjing 27Nan compared with the contents of regular Meridians circulation in Huangdineijing. Second, we studied on the origin of Eight Extra Meridians and researched their route. Third, from these researches we drew some physiological characteristics of Eight Extra Meridians. Conclusion : In Huangdineijing, the regular Meridians circulation include the route of Governor Meridian, Conception Meridian, and Heel Meridian. So the sentence in Nanjing 27Nan is contradictory to the contents of Huangdineijing. The origin of Eight Extra Meridians could be found in Huangdineijing. The collateral Meridians of the Uterus and Epiglottis Meridian are specifically formulated to supplying for the Uterus or Epiglottis. Eight Extra Meridians have third qualities of Meridian, collateral Meridian, and the solid viscera keep the Essence Gi, so named 'Extra'. And they have an intimate association with Extraordinary Organs. They place at the middle axis of human body, thus do higher physiological function that control and regulate the function of Twelve Regular Meridians and Five Viscera & six Bowels for adaptation to the environment.

  • Cerebral dural sinus thrombosis(CDST) has been described as a rare disease with a variety of patho-etiological factors. The diagnosis of CDST is difficult due to various symptoms and signs, none of which is specific to CDST. But timely diagosis is critical for effective management. The introduction and widespread use of computed tomography(CT), magnetic resonance imaging(MRI) and cerebral angiography made early diagnosis of CDST possible. In particular, MR venography is the most useful tool for establishing a correct diagnosis quickly. In early literature, mortality ranked between 30% and 50% but in more recent series it is between 5.5% and 30%. With the advent of diagnostic and therapeutic tools, early diagnosis and proper management has made the prognosis better. The appropriate therapy for CDST, however, has been the subject of much cortroversy. Individual variations of the venous system and collateral vessels are key factors to decide the proper treatment. In this report, we present two cases with symptomatic CDST treated without open surgical or direct endovascular interventions with good outcome.

Spinal Dural Arteriovenous Fistulas : Clinical Experience with Endovascular Treatment as a Primary Therapeutic Modality

  • Objective : The aim of this study was to evaluate the efficacy of endovascular therapy as a primary treatment for spinal dural arteriovenous fistula (DAVF). Methods : The authors reviewed 18 patients with spinal DAVFs for whom endovascular therapy was considered as an initial treatment at a single institute between 1993 and 2006. NBCA embolization was considered the primary treatment of choice, with surgery reserved for patients in whom endovascular treatment failed. Results : Surgery was performed as the primary treatment in one patient because the anterior spinal artery originated from the same arterial pedicle as the artery feeding the fistula. Embolization was used as the primary treatment modality in 17 patients. with an initial success rate of 82.4%. Two patients with incomplete embolization had to undergo surgery. One patient underwent multiple embolizations, which failed to completely occlude the fistula but relieved the patient's symptoms. Spinal DAVF recurred in two patients (one collateral development and one recanalization) during the follow-up period. The collateral development was obliterated by repeated embolization, but the patient with recanalization refused further treatment. The overall clinical status improved in 15 patients (83.3%) during the follow-up period. Conclusion : Endovascular therapy can be successfully used as a primary treatment for the majority of patients with spinal DAVFs. Although it is difficult to perform in some patients, endovascular embolization should be the primary treatment of choice for spinal DAVF.

Eleven Cases on Peripheral Arterial Injuries (말초동맥손상 11례 보고)

  • Eleven cases of peripheral arterial injuries treated at Korea University Woo Sok Hospital during these 3 years and 3 months from Feb. 1971Eleven cases of peripheral arterial injuries treated at Korea University Woo Sok Hospital during these 3 years and 3 months from Feb. 1971 to May 1974 were reviewed. Common causes of injuries were stab wound, automobile accidents and iatrogenic injuries during operation. Of the 11 arterial injuries 3 were femoral artery which` was the commonest in frequency and the next was 2 cases of brachial artery. The most frequent type of injury was transection. Laceration, contusion and spasm was also recognized. The need to operate immediately following an injury was emphasized. Operative procedures were end to end anastomosis and saphenous vein graft in 5 and 2 cases, respectively. Other cases were undergone multiple suture ligature due to staphyllococcal infection, insertion of polyethylene catheter, and lateral suture after thromboendarterectomy. Fractures and extensive soft tissue damage associated with arterial injuries with widespread destruction of the collateral circulation aggravated the situation and complicated the amputation of lower extremity in 4 cases. The factors influencing the amputation were time lag, presence of associated injuries and complications.[KTCS 1974;1:109-116] Common causes of injuries were stab wound, automobile accidents and iatrogenic injuries during operation.
