인지적 간호중재 (Cognitive Nursing Intervention)

  • Nursing as it is practiced and taught, historically has been viewed as natural science. There are new movements to create a paradigm of nursing in the human sciences. A natural science methodologies elicit quantitative data from observable phenomena and reveal causal relationships. Human science methodology is the study of Unitary Man's participative experience in a situation, the simultaneity paradigm. In a theory of nursing rooted in human science assumptions about man and health are synthesized, and the practice of nursing is continuously expanded through research. To find independent nursing interventions especially cognitive nursing intervention models it has been necessary to consult a multitude of journals and text sources for references, and no one nursing textbook can be used in help patients achieve nursing goals. The goals of nursing in the simultaneity paradigm focus on the quality of life from the person's perspective. Cognitive interventions based on the person's perspective were selected from those that colleagues deemed appropriate to nursing and those that were identified in the nursing literature. They were supportive nursing care, reminiscence, meditation, relaxation and imagery. Nurses have been reluctant to make diagnoses, implement actions, and assume responsibility for this actions and this is of concern but can be understood because nurses have had little exposure to action and lack an intervention armamentarium from with which to choose actions to achieve nursing goals. Efforts in nursing education and nursing service are required to remedy this problem. Nurses must be convinced of the challenge and excitement associated with autonomous functioning. It is a characteristic of the true professional. Traditionally, nursing has prescribed one method for handling a situation. Fundamental nursing texts usually only present one way to handle a situation, because alternative interventions to achieve a client goal may be available. Considerably more research is necessary before these can be prescribed. However, unless a first step is taken, progress will not be made. The quality of health care or nursing care is enhanced when nurses transform dilemas into commited action. This is apparent from widespread experiences of nurses.

초발 정신증 환자에서 Mismatch Negativity를 이용한 1년 간의 예후 예측 연구 (Predicting Prognosis in Patients with First Episode Psychosis Using Mismatch Negativity : A 1 Year Follow-up Study)

  • Objectives : It has been shown that early intervention is crucial for favorable outcome in patients with schizophrenia. However, development of biomarkers for predicting prognosis of psychotic disorder still requires more research. In this study, we aimed to investigate whether baseline mismatch negativity (MMN) predict prognosis in patients with first episode psychosis (FEP). Methods : Twenty-four patients with FEP and matched healthy controls (HCs) were examined with MMN at baseline, and their clinical status were re-assessed after 1 year. Repeated-measures analysis of variance was performed to compare baseline MMN between the two groups. Multiple regression analysis was used to identify factors predicting prognosis in FEP patients during the follow-up period. Results : MMN amplitudes at baseline were significantly reduced in patients with FEP compared to healthy controls. In the multiple regression analysis, baseline MMN amplitude significantly predicted later improvement of performances on digit span and delayed recall of California Verbal Learning Test. However, baseline MMN did not predicted improvement of clinical symptoms. Conclusion : These results indicate that MMN may be a possible predictor of improvement in cognitive functioning in patients with FEP. Future study with larger sample and longer follow-up period would be needed to confirm the findings of the current study.

초등학생의 리듬 재산출 능력과 인지기능 수준 간 상관관계 (Correlation Between Rhythm Reproduction Task Performance and Cognitive Function in School-Aged Children)

  • 본 연구는 아동의 리듬 수행 능력과 인지 능력 간 상관관계를 알아보고자 하였다. 이를 위해 평균 연령 9세의 초등학생 59명을 대상으로 iPad 기반 리듬 수행 검사, 숫자 외우기 검사(DST), 아동 색선로 검사(CCTT), 키보드를 활용한 자가탭핑 검사를 진행하였다. 검사 결과, 리듬 재산출의 정확도(accuracy)를 측정하는 리듬 수행 검사 1단계 10개 문항은 자가탭핑 평균속도(s)와 유의한 상관관계가 나타났으며, 리듬 수행 검사 2단계에서 측정하는 그룹핑(grouping) 능력은 CCTT 검사 결과와 유의한 상관관계가 있었다. 특히 문항 특성에 따른 결과를 분석한 결과 일정한 리듬이 두 번 이상 연속으로 반복되어 제공되는 리듬꼴의 정확한 수행은 자가탭핑 속도와 부적 상관관계가 있었으며, 분할박이 많은 리듬꼴의 리듬 수행력은 CCTT, 즉 집행기능과 유의한 상관관계가 있는 것으로 나타났다. 본 연구 결과 리듬 재산출 과제가 초등학교 학령기 아동의 인지 기능 검사 결과와 유의한 상관관계에 있다는 것을 확인하였다. 또한 아동이 산출하는 자가탭핑 속도, 즉 내적 리듬 특성이 인지 기능과 상관관계에 있음을 알 수 있었다. 본 연구는 학령기 아동의 리듬 수행력과 관련된 기초 자료를 제시하고, 이러한 리듬수행력이 인지 기능의 측정 지표가 될 수 있는 가능성이 있음을 시사한다. 본 연구에서 도출된 결과를 통해 추후 리듬꼴의 특징 및 리듬 과제의 난이도에 따른 리듬 수행력이 학령기 아동의 인지적, 정서행동적, 음악적 특성을 다각적으로 평가할 수 있는 가능성 또한 제언되었다.

고령자를 위한 실감객체기반 인지재활 시스템의 개발과 사용성 연구 (Development and Usability of a Cognitive Rehabilitation System Based on a Tangible Object for the Elderly)

  • 목적: 본 연구의 목적은 다양한 인지기능 수준을 가진 고령자를 위한 실감객체 기반 인지재활시스템을 개발하고, 사용성을 검증하는 것이다. 방법: 사용성 연구는 시스템의 강점과 약점을 확인하기 위해 15명의 환자 집단과 4명의 작업치료사 집단에 설문조사를 실시하였으며 응답에 근거한 수정을 통해 시스템을 보완 및 개선하였다. 응답자들은 세 종류의 훈련-실행기능 훈련, 기억력 훈련, 집중력 훈련을 20분~30분간 경험해본 후, 구조화된 설문에 응답하였으며 설문의 영역은 각 훈련 영역별 콘텐츠, 중앙처리장치이자 화면 제시 매체인 태블릿 PC 및 실감객체를 포함하는 하드웨어, 시스템 이용에 따른 만족도로 구성되었다. 결과: 설문 응답 결과 환자 집단과 작업치료사 집단 모두에게 가장 흥미로웠던 영역은 실행기능으로 나타났고 가장 흥미롭지 않은 영역은 집중력으로 나타났다. 하드웨어 사용성 설문에서 태블릿 PC의 화면의 크기가 부적절하다는 응답이 6건, 실감객체의 크기가 부적절하다는 응답이 5건으로 나타났다. 시스템 이용에 대한 만족도 설문에서 전체 응답자의 40%가 시스템에 만족하는 것으로 응답하였다. 결론: 본 시스템은 훈련과제를 수행하기 위해 필요한 실감객체 도구의 조작이 용이하다는 점과 사용자의 요구에 따라 인지훈련의 영역을 쉽게 선택하여 훈련할 수 있다는 점, 사용자의 수행도에 의해 난이도가 자동으로 조정된다는 점을 특징으로 갖는다. 본 시스템에서 입력장치로 사용되는 실감객체도구는 양손에 쥐고 사용함으로써 훈련환경을 자연스러운 일상에서의 도구 사용과 유사하도록 고안하였다. 본 시스템은 지역사회 인지 재활전문가 부족의 대안으로 사용가능 할 것으로 기대된다.

우울 발생요인에 관한 이론적 고찰 (Theoretical Models of Causative Factors in Depression : A Review of the Literature for Nursing)

  • This literature review was undertaken to explore theoretical models of depression for their potential usefulness in nursing research and practice. Depression has bean accounted for by numerous theories or models of causation ; 11 theories selected from psychology, medicine and psychoanalysis and supported by empirical or experimental research were reviewed. These theories identify a variety of precipitating and predisposing factors that may affect the individual's depression. Aggression - turned - inward theory, object loss theory, ego functioning theory, personality organization theory, behavioral theory, learned helplessness theory, cognitive theory, genetic factors, and biological theories conceptualize predisposing factors. Only life stressors theory identifies precipitating facotrs. Each of these theories contributes to an understanding of depression, but many of them use overlapping and interrelated factors. It is also evident from recent. research that there are multiple causes for depression involving an interactive effect among predisposing and precipitating factors that are both biological and psychological in origin. That is, a single theory is not useful, but perhaps a unified theory could be developed that would be helpful to nursing. This review points to the need for continuing development and testing of theories that would integrate the multiple conceptualizations of depression.

  • Visual perception is a complex process engaging many different aspects of brain functioning. Like other cognitive functions, the extensive cortical distribution and complexity of visual perceptional activites make them hihgly vulnerable to brain injury. Dectection and characterization of perceptual disorders require a careful clinical assessment as well as the application of selected neuropsychological tests. In this article we reviewed neuropsychological assessment of visual perception and constructional abilities. And the principal visuospatial disorders are discussed, the associated neuropsychiatric disorders are presented.

  • Mental retardation(MR) is one of the most common developmental disabilities, which is characterized by deficits in intellectual and adaptive functions. Most children with MR have cognitive limitation in the mild range. With respect to the etiology, it is believed that genetic and environmental factors are interrelated and show variable pictures. Most children with MR present with speech and language delay during the early years. The diagnosis can be made by clinical features and neuropsychological tests of intelligence and adaptive functioning. The treatment is limited, but many associated problems are amenable to multidisciplinary interventions. The article will review the recent advances in the management of MR and other neurodevelopmental disorders in children.

  • Attention deficit hyperactivity disorder (ADHD) is a neurobehavioral developmental disorder characterized by a persistent pattern of inattention and/or hyperactivity, as well as forgetfulness, poor impulse control or impulsivity, and distractibility. The recommended evaluation includes documenting the type and severity of ADHD symptoms, verifying the presence of normal vision and hearing, screening for comorbid psychological conditions, reviewing the child's developmental history and school performance, and applying objective measures of cognitive function. Prevailing opinion characterizes ADHD as a disorder of executive function attributable to abnormal dopamine transmission in the frontal lobes and frontostriatal circuitry. A clearly defined etiology remains unknown, but studies suggest a strong genetic link. The aim of treatment is to decrease symptoms, enhance functionality, and improve well-being for the child and his or her close contacts. Stimulants remain the pharmacological agents of first choice for the management of ADHD, and psychosocial, behavioral and educational strategies that enhance specific behaviors may improve educational and social functioning in children with ADHD.

서비스 로봇을 위한 리액티브 감정 생성 모델 (Design of Reactive Emotion Process for the Service Robot)

  • Emotion interaction between human and robot is an important element for natural interaction especially for service robot. We propose a hybrid emotion generation architecture and detailed design of reactive process in the architecture based on insight about human emotion system. Reactive emotion generation is to increase task performance and believability of the service robot. Experiment result shows that it seems possible for the reactive process to function for those purposes, and reciprocal interaction between different layers is important for proper functioning of robot's emotion generation system.

도시가계의 사교육비 지출규모의 결정요인 -일반계 고등학생의 과외사교육비를 중심으로- (Factors Which Determine the Amount of Private Educational Expenditures of urban Households in Korea: With Special Reference to Extra-class Private Educational Expenditures of General High School Students)

  • Relationships between socio-demographic variables and parents' education invest-ment motivation variables and extra-class private education expenditures were examined using a sample of 602 households in Inchon Korea. Tobit analysis was used. Singal-cognitive motivation parents' duty motivation household income housing asset father's occupation class and number of children had significant effects on extra-class private education expenditures on general high school students. To alleviate overspending in extra-class private education diploma pursuing society has to be changed and formal school education need to be properly functioning. Finally parents seek new child education paradigm for the creative labor demand in the next millenium.
