탄광부 진폐증자의 흉부 방사선 사진상 소음영의 밀도에 따른 혈청 단백 농도 및 전기영동 분획상 (Serum Protein Concentrations and Electrophoretic Patterns by Categories of Small Opacity Profusions in Coal Workers' Pneumoconioses)

  • The concentrations of serum proteins fractions and their electrophoretical patterns were investigated in 135 patients with coal workers' pneumoconiosis who participated in confirmative examination for pneumoconiosis in December 1989. Their radiographical profusions were classified as 1/0 or more. Agarose film and phosphoric acid-sodium hydroxide buffer(pH 9.6) were used for electrophoresis. Concentration of each protein fractions and electrophoretical patterns seemd to be equivalent to reference values. Serum ${\alpha}_1$- and $\beta$-globulin concentrations, however, were significantly different (p<0.50) among categories of small opacity profusions and showed the lowest level in the group of category 1. Albumin concentratins decreased and ${\alpha}_2$- globulin concentrations increased significantly (p<0.05) in the group of complicated with pulmonary tuberculosis. $\gamma$-globulin concentrations were not varied by category of profusions nor by pulmonary tuberculosis complication.

  • Carbon monoxide diffusing capacity(DLco) was evaluated before and after nifedipine administration in coal workers' pneumoconiosis by the size of radioopacity. Nifedipine was administered to 18 men and 17 men of small round opacity group and large opacity group respectively. Placebo was administered to 19 men and 15 men of small and large opacity group respectively. In large opacity group DLco was increased after nifedipine administration.   • The purpose of this study is to analyze occupational history of coal miners with pneumoconiosis. In this study, occupational characteristics of 49 coal miners with pneumoconiosis were compared with those of 45 coal miners without pneumoconiosis but in similar age category($43{\sim}52$ years of age) based on interview survey. Various indices on occupational characteristics were developed for the following areas: duration of employment, perception of working condition, working density, dust concentration, temperature, humidity, and experience of respirator wearings. Perception of working condition were measured in 5 points scales but experiences of respirator wearing was measured in 3 points scale. Each index was multiplied by duration of employment. From the analysis, only the experience of respirator wearing showed statistically significant difference between the two groups. Therefore, respirator wearing seemed to be effective in reducing occurrence of pneumoconiosis.

탄광부 진폐증자의 혈액 중 납, 철 및 아연 농도 (Concentrations of lead, iron, and zinc in blood of coal wokers' pneumoconiosis patients)

  • Lead, iron, and zinc concentrations in whole blood were determined by atomic absorption spectrophotometry, using a simple one-step dilution procedure, which were measured in 3 groups, 98 officers unexposed to dust or to metal, 58 coal miners without pneumoconiosis, and 113 coal workers' pneumoconiosis (CWP) patients. The results were as follows : 1. The precisions (C. V.%) of lead, iron, and zinc in blood were $12.65{\pm}6.95%,\;1.47{\pm}1.25%\;and\;6.35{\pm}3.34%$, respectively. 2. Lead and zinc in blood showed the log-normal distribution unlike iron in blood which showed normal distribution. 3. Lead, iron, and zinc concentrations in blood of 3 groups were follows : There was significant difference of concentration for zinc in blood by groups statistically. 4. The difference of lead, iron, and zinc concentrations in blood was not significant (p>0.05) by profusion on chest radiographs.

  • The medical record of ECC and pulmonary function test of 297 cases who were dead at hospital or admitted as coal workers' pneumoconiosis were used for the study of the relationship between forced expiratory volume in one second($FEV_{1.0}$) and ECG sign of pulmonale. The incidence of T wave inversion in $V_1$ lead was significantly increased as $FEV_{1.0}$ decrease. The incidence of T wave inversion in $V_1$ lead was over the half in the group of $FEV_{1.0}$ less than 0.7l.

Levels of Exhaled Breath Condensate pH and Fractional Exhaled Nitric Oxide in Retired Coal Miners

  • Inhaled inorganic dusts, such as coal, can cause inflammation and fibrosis in the lungs, known as pneumoconiosis. Diagnosis of pneumoconiosis depends on morphological changes by radiological findings and functional change by pulmonary function test (PFT). Unfortunately, current diagnostic findings are limited only to lung fibrosis, which is usually irreversibly progressive. Therefore, it is important that research on potential and prospective biomarkers for pneumoconiosis should be conducted prior to initiation of irreversible radiological or functional changes in the lungs. Analytical techniques using exhaled breath condensate (EBC) or exhaled gas are non-invasive methods for detection of various respiratory diseases. The objective of this study is to investigate the relationship between inflammatory biomarkers, such as EBC pH or fractional exhaled nitric oxide ($FE_{NO}$), and pneumoconiosis among 120 retired coal miners (41 controls and 79 pneumoconiosis patients). Levels of EBC pH and FENO did not show a statistically significant difference between the pneumoconiosis patient group and pneumoconiosis patients with small opacity classified by International Labor Organization (ILO) classification. The mean concentration of $FE_{NO}$ in the low percentage $FEV_1$ (< 80%) was lower than that in the high percentage (80% $\leq$) (p = 0.023). The mean concentration of $FE_{NO}$ in current smokers was lower than that in non smokers (never or past smokers) (p = 0.027). Although there was no statistical significance, the levels of $FE_{NO}$ in smokers tended to decrease, compared with non smokers, regardless of pneumoconiosis. In conclusion, there was no significant relationship between the level of EBC pH or $FE_{NO}$ and radiological findings or PFT. The effects between exhaled biomarkers and pneumoconiosis progression, such as decreasing PFT and exacerbation of radiological findings, should be monitored.

진폐증에서 비전형적 형태의 진행성 종괴성 섬유증 1예 (An Unusual Form of Progressive Massive Fibrosis In Pneumoconiosis)

  • Pneumoconiosis, very popular to coal miners who are exposed to coal dusts dominantly, was introduced in the 19th century to describe lung diseases consequent to the inhalation of mineral dusts. Coal workers' pneumoconiosis(CWP) colloquially called "black lung" in the United States, is a distinct pathologic entity resulting from the tissue reaction to deposits of dust include the coal macule, which associated with focal emphysema constitutes the characteristic lesion of simple CWP and complicated CWP or progressive massive fibrosis(PMF). Coal mining are also associated with chronic bronchitis, chronic airflow limitation, and/or generalized emphysema. Progressive massive fibrosis lesions may imprint on and obliterate airways and vessels, and cavitation is not uncommon, being the consequence of ischemic necrosis or mycobacterial infections. We report a case which is unusual form of progressive massive fibrosis to be differentiated from lung carcinoma. It is a rapid growing PMF with ischemic necrosis. By the studies which are about risk of having progressive massive fibroois, it is predicted to be 1.4%. And the other study shows that simple pneumoconiosis clearly predisposed to PMF, with five year attack rates of 13.9%, 12.5%, 4.4% and 0.2% among men with categories 3, 2, 1, and 0 respectively at the start of the risk periods.

만성폐쇄성폐질환을 동반한 탄광부진폐증자의 혈청 중 LD 및 CRP 농도 (The Serum Levels of LD and CRP in Patients of Coal Workers' Pneumoconiosis with Chronic Obstructive Pulmonary Disease)

  • 탄광부진폐증(CWP)과 만성폐쇄성폐질환(COPD)은 석탄분진에 노출되는 탄광부의 폐에서 나타나는 만성적 폐 염증의 특성을 가진다. 이번 연구의 목적은 대조군 27명, CWP 40명, COPD를 동반한 CWP 30명 등 총97명을 대상으로 염증지표로서 혈청 중의 LD와 CRP 수준을 비교하고자 하였다. 혈청 중의 LD 평균 농도(165.7 vs 184.6 U/L, p=0.016)와 CRP 평균 농도(0.08 vs 0.15 mg/dL, p=0.002)는 CWP군에서 유의하게 높았다. 혈청 중의 CRP 평균 농도(0.10 vs 0.19 mg/dL, p=0.008)는 COPD군에서 높았다. 연령을 통제한 공분산분석에서, 혈청 중 CRP 평균 농도는 대조군, CWP군 및 CWP+COPD군 간에 통계적으로 유의한 차이가 있었으며(0.07 vs 0.13 vs 0.19 mg/dL, p=0.005), CWP+COPD 군의 혈청 CRP 평균 농도가 대조군보다 유의하게 높았다(p=0.001). 이러한 결과는 높은 수준의 혈청 중 CRP는 탄광부 이직근로자에서의 CWP 및 COPD와 관련이 있는 것으로 보인다.

요양병원 진폐 환자의 심폐증상, 수면의 질 및 우울에 관한 연구 (Cardiopulmonary Symptoms, Quality of Sleep, and Depression in the Hospitalized Patient with Pneumoconiosis)

  • Purpose: This study was to examine the cardiopulmonary symptoms, quality of sleep, and depression, and to identify the influencing factors in the hospitalized coal worker's pneumoconiosis patient. Methods: 137 hospitalized patients who received pneumoconiosis treatment were conveniently selected. A descriptive correlational study was conducted. Data were collected using structured questionnaires and were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients and multiple regression analysis. Results: The mean score of cardiopulmonary symptoms was relatively high ($3.97{\pm}0.56$, out of 5). The 5 and over score of quality of sleep, sleep disturbance was 94.2%, and the reported depression was 95.6%. The levels of depression depended on the oxygen therapy and family/acquaintance visiting. The depression was significantly correlated with the cardiopulmonary symptoms and quality of sleep. The influential factors affecting depression were cardiopulmonary symptoms and quality of sleep, which explained about 34.1% of the variance. Conclusion: The results of this study indicate that nursing interventions are needed to reduce depression, and to improve cardiopulmonary symptoms and quality of sleep in the hospitalized pneumoconiosis patient. These results can provide for nursing intervention to facilitate reduction of depression.
