여름철 동남권 전통시장에서 판매되는 즉석섭취 튀김어묵의 미생물학적 오염도 조사 (Investigation of Microbial Contamination in Fried Fish Paste, a Ready-To-Eat Food Distributed from Traditional Markets in Korea's Southeast Region During Summer)

  • This study assessed microbial contamination in seven fried fish pastes sold in Southeast Asian traditional markets during summer. It measured viable cell count, coliforms, Escherichia coli, fungi, and Staphylococcus spp. It also qualitatively analyzed Vibrio parahaemolyticus, Salmonella spp., Bacillus cereus, Listeria monocytogenes, and Clostridium perfringens. The average viable cell count, coliforms and fungi were detected as 6.34 (3.84-8.13), 2.16 (1.00-3.55), and 3.92 (1.85-7.74) log10 CFU/g, respectively. Staphylococcus spp. was detected at 4.59 (2.10-7.63) log10 CFU/g. Some samples had high contamination levels: viable cell count (8.13 log10 CFU/g), fungi (7.74 log10 CFU/g) and S. aureus (7.63 log10 CFU/g). However, E. coli was not detected in any samples (ND, <1 log10 CFU/g). V. parahaemolyticus, Salmonella spp., B. cereus, L. monocytogenes, and Cl. perfringens were also not detected in the samples. The microbial contamination data provide insight into managing microbial contamination and ensuring the safety of fried fish pastes in traditional summer markets.

Detection of Enteropathogens in Human Immunodeficiency Virus and Non-Human Immunodeficiency Virus-Infected Children with Acute Diarrhea in an Indonesian Tertiary Hospital Using Multiplex Real-Time Polymerase Chain Reaction

  • Purpose: Diarrhea is one of the leading causes of mortality in children living in developing countries. The etiology of acute diarrhea in each healthcare center varies depending on place, time, and population. This study aimed to identify pathogen patterns in human immunodeficiency virus (HIV)-infected and non-HIV children suffering from acute diarrhea, using multiplex real time reverse transcriptase polymerase chain reaction (RT-PCR), in an Indonesian tertiary hospital. Methods: This cross-sectional study was conducted at Dr. Cipto Mangunkusumo National Hospital from March 2019 to April 2020. Results: The study showed that multiplex RT-PCR results were positive in 58.9% of the specimens, with more positive results in HIV-infected children than in non-HIV-infected children (70% vs. 54.7%). Altogether 72 enteropathogens were detected from all specimens. Enteropathogens in non-HIV children with acute diarrhea consisted of bacteria (70.6%) and viruses (29.4%) with a predominance of enteroaggregative Escherichia coli (25.4%), followed by Campylobacter spp. (11.8%), enteropathogenic E. coli (9.8%), Norovirus GII (7.8%), and Clostridium difficile (7.8%). Enteropathogens in HIV-infected children consisted of viruses (57.1%), bacteria (28.6%), and parasites (14.3%) comprising Norovirus GII (24%), Cryptosporidium spp. (14.3%), Campylobacter spp. (14.3%), Norovirus GI (14.3%), and Astrovirus (14.3%). Cryptosporidium spp. was the only parasite found in this study and was found only in HIV-infected children. In non-HIV children with acute diarrhea, most pathogens were invasive bacteria, while in HIV-infected children, more viral and parasite infections occurred, primarily caused by opportunistic pathogens. Conclusion: The pattern of enteropathogens can help clinicians determine further examinations and appropriate empirical antimicrobial therapy for the patient.

광주지역 한우 분변 내 설사병 병원체 조사 (Prevalence of enteropathogens in the feces from diarrheic Korean native cattle in Gwangju area, Korea)

  • Calf diarrhea is a common disease in young claves and is still a major cause of productivity and economic loss in livestock farms. Fecal samples from Korean native cattle (n=100) with diarrhea from 64 farms in Gwangju area, Korea from september 2017 to December 2018 were examined for shedding of important protozoan parasitic, viral and bacterial pathogens using culture, rapid test kit and PCR methods. Of 57 (89.1%) of the 64 Korean native cattle farms examined had samples infected with at least one of the investigated pathogens. Among 100 fecal samples, 88 samples were positive for at least one the twelve pathogens and 51 samples were simultaneously positive for two or more pathogens by culture and PCR assay. Bovine group A rotavirus (BRV) was the most common pathogen, found in 43/100 (43.0%) samples on 32/64 (50.0%) farms. Subsequently, kobuvirus (30.0%), pathogenic E. coli (29.0%), bovine parvovirus (17.0%), Giardia spp. (13.0%), Eimeria spp. (10.0%), Clostridium perfringens type A (8.0%), bovine torovirus (8.0%), bovine viral diarrhea virus (6.0%), bovine coronavirus (5.0%), bovine norovirus (2.0%) and Cryptosporidium spp. (2.0%) were detected. Nebovirus, kırklareli virus, bovine adenovirus, Salmonella spp. and intestinal parasites were not detected. Of the 72 calves sampled in this age group, 64 (88.9%) samples were positive for at least one enteropathogen. BRV was identified in 34/72 (47.2%) samples from 27/48 (56.3%) farms. Subsequently, pathogenic E. coli (30.6%), kobuvirus (29.2%), BPaV (22.2%), Giardia spp. (15.3%), Eimeria spp. (9.7%), BVDV (6.9%), Cl. perfringens type A (6.9%), BCoV (4.6%) and Cryptosporidium spp. (2.8%) were detected in fecal samples. A total of ninety-six strains of E. coli were isolated from one hundred fecal samples collected from Korean native cattle with diarrhea. The presence of stx1, stx2, eaeA, LT, STa, STb, ehxA, saa, F4, F5(K99), F6, F17, F18 and F41 genes in the isolates was investigated by PCR. Out of ninety-six E. coli isolates screened for specific genes, 30 strains E. coli were identified to harbor shiga toxin-producing E. coli (STEC) 7 (7.3%), enterohemorrhagic E. coli (EHEC) 8 (8.3%), enteropathogenic E. coli (EPEC) 6 (6.3%), enterotoxigenic E. coli (ETEC) 2 (2.1%) and STEC/ETEC hybrid 7 (7.3%). This study provides epidemiological estimates of the prevalence of Korean native cattle's enteropathogens in Gwangju area, Korea, which would be used for cattle farmers and veterinarians to select appropriate therapeutic method.

2015년부터 2018년까지 일개 이차병원에서 동정된 소아 급성 위장염 원인 병원체의 분자진단과 역학의 임상적 연구 (Molecular Detection and Epidemiology of Etiologic Agents among Children with Acute Gastroenteritis at a Secondary Hospital from 2015 to 2018)

  • 목적: 급성 위장염의 대부분의 경우 원인 병원체가 확인되지 않는다. 최근 들어 발달한 multiplex reverse transcriptase-polymerase chain reaction (RT-PCR) 검사는 장염 병원체 검출에 도움을 줄 수 있다. 이 연구는 multiplex RT-PCR을 이용해, 소아 장염환자에서 병원체의 역학을 조사하고자 하였다. 방법: 2015년 5월부터 2018년 6월까지 대한민국 서울의 2차병원에서 급성 위장염으로 진단받은 소아 환자의 대변에서 병원체를 확인하기 위해 multiplex RT-PCR 검사를 시행하였다. 결과: 바이러스 병원체에 대한 1,366개의 대변 검체 중, 483개(35.3%)에서 1개 이상의 병원체가 분리되었다. A군 로타바이러스는 106건(7.8%)에서 확인되었으며, 양성률은 3.0% (8/263)에서 16.7% (48/288)까지 매년 증가했다(P<0.001). 노로바이러스 GII는 가장 흔한 바이러스성 병원체였고(263/1366, 19.3%), 3년간 양성률은 증가하지 않았다. 세균성 병원체에 대한 304개의 대변 검체 중 캄필로박터(32/304, 10.5%)는 가장 흔한 세균성 병원체였으며, 그 다음으로 Clostridium difficile (toxin B) (22/304, 7.2%), 살모넬라균(17/304, 5.6%)이었다. 이 균들의 양성률은 연구기간 동안 증가하지 않았다. 결론: 로타바이러스 백신 도입 이후 노로바이러스 GII가 소아 장염에서 주요한 병원체였지만, 연구기간 동안 로타바이러스 감염 환자가 증가했고, 특히 2018년에는 급증했다. 따라서 새로운 로타바이러스 균주의 등장 가능성을 포함한 추가 연구가 필요하다. 캄필로박터는 소아 세균성 장염의 주요 원인이며, 적절한 치료를 위해 이 균의 임상적 특성을 고려하고 지속적 감시가 필요하겠다.

Comparison of Airborne Bacterial Communities from a Hog Farm and Spray Field

  • Airborne bacteria from hog farms may have detrimental impacts on human health, particularly in terms of antibiotic resistance and pathogen zoonosis. Despite human health risks, very little is known about the composition and diversity of airborne bacteria from hog farms and hog-related spray fields. We used pyrosequencing analysis of 16S rRNA genes to compare airborne bacterial communities in a North Carolina hog farm and lagoon spray field. In addition, we isolated and identified antibiotic-resistant bacteria from both air samples. Based on 16S rRNA gene pyrosequence analysis, Actinobacteria, Bacteroidetes, Firmicutes, and Proteobacteria were the dominant phyla in airborne bacterial communities from both hog farm and spray field sites. Within the Firmicutes genera, Clostridium spp. were more abundant in the hog farm, whereas Staphylococcus spp. were higher in the spray field. The presence of opportunitic pathogens, including several Staphylococcus species and Propionibacterium acnes, was detected in both bioaerosol communities based on phylogenetic analysis. The isolation and identification of antibiotic-resistant bacteria from air samples also showed similar results with dominance of Actinobacteria and Proteobacteria in both hog farm and spray field air. Thus, the existence of opportunistic pathogens and antibiotic resistant bacteria in airborne communities evidences potential health risks to farmers and other residents from swine bioaerosol exposure.

A Review of Interactions between Dietary Fiber and the Gastrointestinal Microbiota and Their Consequences on Intestinal Phosphorus Metabolism in Growing Pigs

  • Dietary fiber is an inevitable component in pig diets. In non-ruminants, it may influence many physiological processes in the gastrointestinal tract (GIT) such as transit time as well as nutrient digestion and absorption. Moreover, dietary fiber is also the main substrate of intestinal bacteria. The bacterial community structure is largely susceptible to changes in the fiber content of a pig's diet. Indeed, bacterial composition in the lower GIT will adapt to the supply of high levels of dietary fiber by increased growth of bacteria with cellulolytic, pectinolytic and hemicellulolytic activities such as Ruminococcus spp., Bacteroides spp. and Clostridium spp. Furthermore, there is growing evidence for growth promotion of beneficial bacteria, such as lactobacilli and bifidobacteria, by certain types of dietary fiber in the small intestine of pigs. Studies in rats have shown that both phosphorus (P) and calcium (Ca) play an important role in the fermentative activity and growth of the intestinal microbiota. This can be attributed to the significance of P for the bacterial cell metabolism and to the buffering functions of Ca-phosphate in intestinal digesta. Moreover, under P deficient conditions, ruminal NDF degradation as well as VFA and bacterial ATP production are reduced. Similar studies in pigs are scarce but there is some evidence that dietary fiber may influence the ileal and fecal P digestibility as well as P disappearance in the large intestine, probably due to microbial P requirement for fermentation. On the other hand, fermentation of dietary fiber may improve the availability of minerals such as P and Ca which can be subsequently absorbed and/or utilized by the microbiota of the pig's large intestine.

Clinical Characteristics and Ultrasonographic Findings of Acute Bacterial Enterocolitis in Children

  • Purpose: This study clarified the bacterial pathogens currently causing acute infectious enterocolitis (AIE) in children and evaluated the clinical characteristics and ultrasonographic findings according to the different pathogens. Methods: Medical records regarding age, sex, clinical symptoms, laboratory data, identified enteropathogens, ultrasonographic findings, treatment, and outcome of 34 patients who were diagnosed with AIE via stool examination using multiplex polymerase chain reaction (PCR) or culture, were retrospectively reviewed. Results: Twenty-four patients (70.6%) were male. The mean age of the patients was $8.5{\pm}6.2$ (range, 1.1-17.1) years. Six bacterial pathogens were isolated: Salmonella species (spp.) (32.4%), Campylobacter spp. (20.6%), verotoxin-producing Escherichia coli (14.7%), Staphylococcus aureus (11.8%), Clostridium difficile (8.8%), and Shigella spp. (2.9%). Abdominal pain occurred in all patients regardless of pathogen. The patients infected with Salmonella were older than those infected with verotoxin-producing E. coli (p<0.05). C-reactive protein levels were higher in patients with Salmonella and Campylobacter infections than in those with verotoxin-producing E. coli infection (p< 0.05), the other clinical and laboratory data were indistinguishable between pathogens. Ultrasonography demonstrated diverse involvement of bowel segments according to pathogen. Wall thickening of both the ileum and the entire colon was the most common lesion site regardless of pathogen. Conclusion: Various bacterial agents cause AIE and the symptoms are diverse symptoms, however, all most children recovered spontaneously. Use of multiplex PCR on stool samples warrants improvement of its sensitivity for diagnosis of enteropathogenic bacteria. Ultrasonographic examination is useful for diagnosis of AIE; it can also detect the disease extent and severity.

High Frequency of Enteric Protozoan, Viral, and Bacterial Potential Pathogens in Community-Acquired Acute Diarrheal Episodes: Evidence Based on Results of Luminex Gastrointestinal Pathogen Panel Assay

  • Infectious diarrhea is endemic in most developing countries. We aimed to investigate the protozoan, viral, and bacterial causes of acute diarrhea in Taif, Saudi Arabia. A cross-sectional prospective 1-year study was conducted on 163 diarrheal patients of various ages. Stool samples were collected, 1 per patient, and tested for 3 protozoa, 3 viruses, and 9 bacteria with the Luminex Gastrointestinal Pathogen Panel. Overall, 53.4% (87/163) of samples were positives (20.8% protozoa, 19.6% viruses, 2.8% bacteria, and 9.8% mixed). Rotavirus (19.6%), Giardia duodenalis (16.5%), and Cryptosporidium spp. (8.5%) were the mostly detected pathogens. Adenovirus 40/41 (4.2%), Salmonella (3%), Shiga toxin-producing Escherichia coli (3%), and Entamoeba histolytica (2.4%) were also detected. Norovirus GI/II, Vibrio cholerae, Yersinia enterocolitica, and Clostridium difficile toxin A/B were not detected in any patients. All pathogens were involved in coinfections except E. histolytica. Giardia (5.5%) and rotavirus (3%) were the most commonly detected in co-infections. Enterotoxigenic E. coli (2.4%), Campylobacter spp. (2.4%), E. coli 0157 (1.8%), and Shigella spp. (1.2%) were detected in patients only as co-infections. Infections were more in children 0-4 years, less in adults <40 years, and least >40 years, with statistically significant differences in risk across age groups observed with rotavirus (P<0.001), Giardia (P=0.006), and Cryptosporidium (P=0.036) infections. Lastly, infections were not significantly more in the spring. This report demonstrates the high burden of various enteropathogens in the setting. Further studies are needed to define the impact of these findings on the clinical course of the disease.

2004년 충남지역 의료기관의 설사환자 가검물에서 분리된 병원성미생물 감염실태에 관한 조사연구 (Investigation for the Infectious Diarrhea by Pathogenic Microorganism from Hospitals in ChungNam Province in 2004)

  • 2004년 1월부터 12월까지 충남지역 5개 지정 협력병원에서 수거한 설사환자의 대변가검물 787건에 대한 병원성세균, 설사바이러스 및 원충에 대한 검사를 실시하여 다음과 같은 결론을 얻었다. 병원성 세균 10종에 대한 검사결과 Salmonella spp. 27건, 병원성대장균 20건(EHEC 3건, EPEC 9건, ETEC 8건), Clostridium perfringens 18건, Staphy-lococcus aureus 6건, Vibrio parahaem-olyticus 4건과 Shigella spp. 4건 등 79건이 분리되었다. 병원성 바이러스 4종에 대한 검사결과 rotavirus 115건, norovirus 55건, astro-virus 5건, rotavirus 4건, norovirus 4건의 혼합감염, adenovirus 3건, rotavi-rus 2건과 astrovirus 2건의 혼합감염으로 총 190건이 검출되었다. 원충 3종에 대한 검사결과 Entam-oeba histolytica 5건, Giardia lamblia 1건으로 6건이 검출되었다. 병원성세균은 10세 이하에서 26.8%, 61세 이상에서 45.6%, 바이러스는 10세 이하에서 65.8%, 61세 이상에서 17.4% 그리고 원충은 10세 이하에서 83.3%, 61세 이상에서 16.7%로 모두 10세 이하와 61세 이상에서 높은 검출을 보였다. 특히, 5세 이하의 어린이 경우 세균은 20.3%, 바이러스는 63.7% 그리고 원충은 83.3%로 높은 검출을 보였다. 병원성세균은 월별, 계절별로 분리율 차이가 미미하나 1월과 3월에 각각 2.5%로 가장 적게 분리되었고 8월에 16.7%로 가장 많이 분리되었다. 병원성 바이러스는 9월에 1.6%로 가장 적게 4월에 14.7%로 가장 많이 검출 되었다. Salmonella spp.는 ampicillin, chloramphenicol, tetracycline, nalidixic acid, ticarcillin에 비교적 높은 내성을 나타냈고, Shigella spp.는 ampicillin, trimethoprim/sulfamethoxazole, chloram-phenicol, tetracycline, ampicillin/sulbactam, ticarcillin에 비교적 높은 내성을 나타냈으며, 병원성대장균은 ampicillin, ceph-alothin, gentamicin, tetracycline, nalidi-xic acid, ampicllin/sulbactam, ticarcillin에 비교적 높은 내성을 나타내었다.

혈액배양 양성검체에서 패혈증 원인균 신속동정을 위한 Vitek MS 시스템의 유용성 평가 (An Evaluation of Vitek MS System for Rapid Identification of Bacterial Species in Positive Blood Culture)

  • 본 연구에서는 계대배양과 세균 동정 시험에 소요되는 시간을 단축하고, 혈류감염의 새로운 검사 방법을 모색하여 간단하고 신속 정확한 동정 결과를 도출할 목적으로 질량분석기를 이용하여 다음과 같은 결과를 얻었다. 혈액배양에서 한 가지 세균만 배양된 검체는 총 254개였으며, Vitek 2에서 208주(81.8%)의 세균을 동정되었으며, 45주는 동정되지 않았다. 동정된 세균 중 그람양성 세균이 146개(57.5%), 그람음성 세균이 108개(42.5%)이었다. 전체적으로 233개는 종(species) 수준까지 동정 되었으며, 21개는 속(genus)수준까지 동정 되었다. 동정 오류는 Propionibacterium acnes를 Clostridium bifermentans로 동정 되었다. 균종별로는 enterobacteriaceae, glucose non-fermentative bacilli (GNFB), staphylococci의 정확도는 각각 81/83 (97.6%), 12/15 (80.0%), 72/85 (84.7%)로 나타났다. Vitek 2에 의한 표준법과 Vitek MS에 의한 직접법에 의한 동정의 일치율은 81.8%이었으며, 45주는 동정되지 않았다. 동정되지 않은 세균들의 대부분은 그람양성 세균(n=37)이었고, 그람양성 세균은 streptococci (14), coagulase-negative staphylococci (CNS) (11), enterococci (3), Staphylococcus aureus (2), Micrococcus spp. (2), Bacillus spp. (2) 그리고 Actinomyces odontolyticus, Finegoldia mag na, Peptostreptococcus spp.가 각각 1건씩 이었다. 결과 보고 시간은 기존의 검사 방법보다 24~72시간까지 단축되었다. 산소성 배양과 무산소성 배양 사이의 동정률의 차이는 없었으나 무산소성 배양을 사용하면 용해 과정이 필요 없어 검체 준비 시간을 단축할 수 있었다. 이상의 연구 결과로 혈액배양병에서 직접 동정하는 방법은 정확하고 결과 보고 시간이 신속하여 환자의 치료에 매우 유용할 것이라 생각된다. 향후 추가적인 연구에서는 본 연구에서 정확성이 부족했던 사슬알균(streptococci)과 혈장응고효소 음성 포도알균(CNS)을 동정하기 위한 방법을 더욱 개선할 필요가 있을 것으로 사료된다.
