Cases Report on Conservative Treatment for the Chronic Closed Lock of Temporomandibular Joint

  • Some treatment methods have been proposed for patients with chronic closed lock of temporomandibular joint. We report a conservative treatment for patients who had chronic closed lock of temporomandibular joint and who did not want surgical treatment. Two patients who had been treated in the Template clinic, Soonchunhyang University Bucheon Hospital, are the subjects of this report. The subjects had chronic closed lock symptom for over 3 months after an onset of locking; conventional therapies had no effect. The subjects were treated by making them wear a Template appliance while sleeping and exercise for 10 hours a day. After periodic follow-up, significant improvement was observed for Template treatment in terms of the maximal mouth opening range. When conventional therapy is expected to be ineffective, The Template appliance can be used as conservative treatment for temporomandibular disorders patients with chronic closed lock of temporomandibular joint.

Treatment Outcome of Patients with Closed Lock by Conservative Therapy (보존적 처치에 따른 폐구성 과두걸림환자의 예후)

  • In order to evaluate the outcome of conservative treatment for acute or chronic closed lock, 127 patients were subjected at the Dept. of Oral Medicine, PNUH, from 1991. All the symptoms were analyzed and the results according to treatment modalities were compared before and after treatment. 1. Closed lock patients were more often seen in women of twenty and thirty. 2. The most common reasons for treatment were functional pain, limitation of mouth opening, restricted laterotrusion to non-effected side, deflection on opening but, on the contrary, noise was the least. 3. Most of closed lock patients were curedin six months. 4. Conservative treatment including stabilization splint produced better results. 5. Functional pain and limitation of mouth opening were relieved and maximumm comfortable opening was expanded, but joint noise decreased in the chronic and increased in the acute.

A Study on the Mandibular Movements in the Patients with TMJ Lock Closed (악관절 폐구성 과두걸림 환자의 하악운동에 관한 연구)

  • The author examined the patterns and various ranges of mandibular movements in TMJ lock closed patients in the frontal, sagittal and horizontal plane and obtained the following results. 1. In the frontal trajectory, the mean amount of maximum mouth opening was 24.4mm and the opening paths were deviated to the affected side in 87.1% of the patients. The mean amount of maximum laterotrusion to the affected side was 10.4mm and that of non-affected side was 7.5mm. There was a significant difference between them(p<0.001). 2. In the sagittal trajectory, the mean amount of the maximum protrusion was 7.0mm, the mean amount of the maximal retrusion was 1.0mm 3. In the horizontal trajectory, the pattern of laterotrusion showed asymmetry: the mean length of non-affected side was smaller than that of the affected side. Protrusive path were deviated to the affected side in 64.5% of the patients, the mean degree of deviation was 16.4$^{\circ}$. The mandibular movements of TMJ lock-closed patients can be characterized by decreased range of mouth opening, protrusive movement, and laterotrusive movement to the non-affected side and also characterized by deviated opening and protrusive path to the affected side.

  • Arthrocentesis is a simple, less invasive, inexpensive, and highly efficient procedure for closed lock of the temporomandibular joint with regard to the lack of recurrence of symptoms during extended periods of follow-up and significiant improvement in pain and jaw function. It can be performed under local anesthesia. Arthrocentesis closed lock provide sustained normal joint function and marked pain relief. This study is the clinical outcome of arthrocentesis for closed lock of the temporomandibular joint. 160 patients(169 joints) who had experienced sudden-onset, persistent limited mouth opening were the subjects of this study. Arthrocentesis of the upper compartment of the affected TMJ was performed using normal saline. As results, at 3-60 months postarthrocentesis maximum mouth opening(MMO) had increased from mean of 27.7mm to 43.5mm, contralateral movements(CLM) from mean 5.74mm to 9.55mm, midline deviation during mouth opening from mean 3.04mm to 0.69mm. In 130 cases there was a history of joint noises, in 52 cases all noise had ceased after procedures.

미세직경 악관절경을 이용한 악관절 closed lock 환자의 치료

  • Visually guided irrigation and lysis(VGIL) using temporomandibular joint(TMJ) arthroscope is useful for decreasing pain and increasing the functional mobility of TMJ. Also it demonstrated similar effectiveness comparing with arthrocentesis. Arthroscopy permits intracapsular inspection that is imperative not only for identification of morphological characteristics of joint space, but also for adequate irrigation and lysis of specific joint space. Conventional 2.3mm diameter arthroscope with 2.7mm catheter was too wide. So it was traumatic and uncomfortable to manipulate on temporomandibular joint space, especially on TMJ internal derangement patient with reduced space. We report our clinical experience on 6 TMJ closed lock patients who were treated with new 1.2mm fiberscope at Chung Hoon Dental Clinic between March 2003 and August 2003. Also we present clinical advantage & disadvantage of new system with literature review.

  • The temporomandibular joint arthrocentesis is indicated the 'closed lock' due to anterior disc displacement without reduction. It can be easily carried out under local anesthesia with little complications. We performed arthrocentesis to the 24 patients, 9 patients who suffered from acute or chronic closed lock with anterior disc displacement, 2 patients from temporomandibular joint dysfunction related to systemic disease and 13 patients from joint dysfunction with pain. We present the common clinical aspect of arthrocentesis that operator can be easily faced with and possible modifications of this method.

  • In general, treatment of the patients with closed lock divides into a conservative and a surgical treatment. Surgical treatment has been often applied in case that occlusal splint therapy was not effective on the patient with closed lock. In recent, some clinicians reported good results with mandibular manipulation. Three patients complained limitation of month opening-(mean mouth opening was 22.3mm) and TMJ pain. Articular discs were displaced anteriorly on MRI. Two patients didn't improve the symptoms with long term occlusal splint therapy. We applied mandibular manipulation after injection with 2% lidocaine into the upper joint space of the affected TMJ and directly inserted occlusal splint to all patients. At the follow-up check, mean mouth opening was 41.7mm. TMJ pain decreased, condyle and disc relationship was improved functionally on MRI.

Initial Frequency Preset Technique for Fast Locking Fractional-N PLL Synthesizers

  • This paper presents a fast locking technique for a fractional-N PLL frequency synthesizer. The technique directly measures $K_{VCO}$ on a chip, computes the VCO's target tuning voltage for a given target frequency, and directly sets the loop filter voltage to the target voltage before the PLL begins the normal closed-loop locking process. The closed-loop lock time is significantly minimized because the initial frequency of the VCO are put very close to the desired final target value. The proposed technique is realized and designed for a 4.3-5.3 GHz fractional-N synthesizer in 65 nm CMOS and successfully verified through extensive simulations. The lock time is less than $12.8{\mu}s$ over the entire tuning range. Simulation verifications demonstrate that the proposed method is very effective in reducing the synthesizer lock time.

  • Purpose : To evaluate the efficacy of arthrocentesis for treatment of closed lock. Material : 42 patients were diagnosed to closed lock by physical, radiographic examriation and undergone arthrocentesis. All patients have a pain and mouth opening limitation on affected site. Method : Arthrocentesis was done under conscious sedation and local infiltration anesthesia, normal saline and some steroid was injected on upper compartment of tempormandibular joint. After pumping and lavage, manual reduction procedure of anterially displaced disc was done. All the patients wear an anterior repositioning splint just after arthrocentesis. The result of arthrocentesis was assessed by pain and difference of mouth opening. Result : Difference of mouth opening after arthrocentesis was improved to 18.85mm and pain was gradually decreased. All patients were worn stabilization type splint after mean 1.84 months. 6 patients had relapse of mouth opening limitation so done arthrocentesis again and delivered good results.
