잔존 총배설강 기형 수술 후 발생한 요도-질 누공에 대한 전방 시상 경직장적 접근 술식 (Anterior Sagittal Transrectal Approach, ASTRA) 1 례보고 (Anterior Sagittal Transrectal Approach (ASTRA) for Urethrovaginal Fistula after Total Repair of Persistent Cloaca - 1 Case Report -)

  • The authors applied anterior sagittal transrectal apporach (ASTRA) for the repair of urethrovaginal fistula which developed after total repair of persistent cloaca. The patient had been diagnosed to have persistent cloaca, double uterus and double vagina, and received PSARP, excision of right-side uterus and vagina, and left vaginal switch operation at 22 months old. After operation, the patient admitted several times due to frequent urinary tract infection and ectopic stone formation in bladder and neovagina. Urethro-neovaginal fistula was confirmed by cystoscopy and corrected with ASTRA. Postoperative voiding cystourethrogram showed no fistula tract. ASTRA showed improved surgical field, minimized ureterocystic damage, and preserved perirectal nerve due to limited incision of rectum.

Aerobic bacteria from oral cavities and cloaca of snakes in a petting zoo

  • It is important to identify the bacteria in snakes because they can cause disease; importantly, bacteria such as Stenotrophomonas maltophilia, Escherichia coli, Proteus vulgaris etc. could be pathogens especially in hospitalized, debilitated hosts, and immunocompromised patients. To analyze the distribution of snakes' bacteria in petting zoo, samples from 20 snakes were collected from 2002 to 2008. Nine bacteria species were isolated from both oral and cloaca while four and six species were identified only from oral and cloaca, respectively. Except for Actinobacter sp., all of the identified strains are opportunistic pathogens, and most of them can cause nosocomial infections in humans. Present results indicate that prevalence of various zoonotic bacterial strains in snakes could be involved in potential transfer of these bacteria into caretakers and other animals. Therefore, it needs to examine the antibiotic resistance of these pathogens to prevent outbreaks.

  • Persistent cloaca in a female newborn is one of the most complex and challenging anorectal malformation. The incidence is about 10% of all anorectal anomalies. Treatment of cloacal malformations has evolved during the past 40 years. However, it still remains one of the most difficult operations. In 1997, Pena reported a new procedure called "Total urogenital mobilization". We have repaired 3 cases of cloacal anomaly using "Total urogenital mobilization". The operations were performed between age 15 and 28 months. The length of the common channels was 2.5 - 3.0 cm. Two cases had double vaginas and one of them also had double uterus. The operation time was 4-5 hours and no major complications occurred. Functional and cosmetic results were satisfactory.

Impact of quorum quenching bacteria on biofouling retardation in submerged membrane bioreactor (SMBR)

  • Membrane biofouling is a critical operational problem that hinders the rapid commercialization of MBRs. Quorum quenching (QQ) has been investigated widely to control membrane biofouling and is accepted as a promising anti-fouling strategy. Various QQ strategies based on bacterial and enzymatic agents have been identified and applied successfully. Whereas, this study aimed to compare indigenously isolated QQ strain i.e., Enterobacter cloaca with well reported Rhodococcus sp. BH4. Both bacterial species were immobilized in polymeric beads and introduced to two different MBRs keeping the overall beads to volume ratio as 1%. Efficiencies of these strains were monitored in terms of prolonging the membrane filtration cycle of MBR, release of extra-cellular polymeric substances, membrane resistivity measurements and mineralization of signal molecules and permeate quality. Indigenous strain (Enterobacter cloaca) was added to $QQ-MBR_E$ while Rhodococcus sp. BH4 was introduced to $QQ-MBR_R$. QQ bacterial embedded beads showed enhanced filtration cycles up to 1.4 and 2.3 times for $QQ-MBR_E$ and $QQ-MBR_R$ respectively as compared to control MBR (C-MBR). Soluble EPS concentration of 52 mg/L was observed in C-MBR while significantly lower EPS concentration of 20 and 10 mg/L was witnessed in $QQ-MBR_E$ and $QQ-MBR_R$, respectively. Therefore, substantial reduction in biofouling showed the effectiveness of indigenous strain.

후장중복증을 동반한 변형 총배설강외반증 (A Cloacal Exstrophy Variant Associated with Hindgut Duplication)

  • Diphallus is a rare congenital anomaly and is frequently associated with duplication of the urinary tract and rectosigmoid, and commonly associated with vertebral anomalies. Remzi reported less than 100 cases of duplication of all or a portion of the penis, but about 10 cases of complete diphallus with exstrophy of cloaca was reported, and a case of complete diphallus associated with hindgut duplication was reported, and complete diphallus with displacement of bladder associated hindgut duplication and imperforate anus was not reported in Korea. We experienced a case of the complete diphallus associated with displacement of bladder, hindgut duplication, and imperforate anus as a variant of cloacal exstrophy. A review of published cases suggests that this may be the first example of a complete diphallus with displacement of bladder coexisting with the hindgut duplication and imperforate anus.

  • The abnormal male Newhampshire of five months age with three hindlimbs was examined macroscopically and radiographically The external feature included two normal wings, two normal hindlimbs and an underdeveloped extra hindlimb, which was attached to the pelvic region. The extra hindlimb consisted of an underdeveloped femur like bone, tibiotarsus likebone, tarsometatarsus like bone and three rows of digits with claw. The femur like bone was fused with pelvic girdle like bone. The bones of extra hindlimb were fused into one except one row of digit, which was attached to the tibiotarsus like bone. Three ceca and one cloaca with two vents were also observed.

  • Herein, we report an exceptionally rare case of a 25-year-old woman with cloacal exstrophy/omphalocele-exstrophy-imperforate anus-spinal defects (OEIS) syndrome achieving a viable pregnancy despite many gastrointestinal and genitourinary malformations and multiple respective corrective operations. The patient was born with two vaginas, two uteruses, four ovaries, an imperforate anus, a large omphalocele including bowel and bladder exstrophy, and diaphysis of the pubic rami. This patient is the only documented OEIS patient not to have tethered spinal cord as an anomaly, perhaps contributing to her successful pregnancy. After experiencing preeclampsia with severe features at 35 weeks, the baby was born via cesarean section.

  • 한국의 한 환자의 비뇨기계로부터 배출된 철선충이 신종으로 확인됨에 따라 진주철선충 (신칭) (Parachordodes megareolatus, n. sp.)으로 기재하였다. 이 신종의 형태적 특성은 표피에 작은 혹 (areole)이 돋아있고 이들이 큰구멍혹 (megareole)들과 연계되어 있는 것이다. 표피에 분포된 큰구멍혹이 이 신종이 갖고 있는 특성중의 하나이다. 일면. 본 충의 속 (genus)이 갖고 있는 특성중의 하나는 수컷 복면의 총배설강(cloaca)이 개구되는 전면에 강모 (bristle)가 나 있고 그 배열이 모두 융합되어 있다는 것이다.

닭에 있어서의 Caecostomy (맹장으로 튜브를 삽입하는) 기술 (A Technique for Caecostomy in the Chicken)

  • 닭 맹장으로의 뇨의 역류량을 측정하기 위해서 Caecostomy (맹장으로 튜브를 삽입하는 수술)기술이 개발되었다. 시술 이틀 뒤부터 맹장에 튜브가 장착된 닭은 20m1의 따뜻한 생리적 식염수를 이용하여 격일간격으로 10일 동안 맹장에 장착된 튜브를 환류시켰다. 시술후 닭은 일시적으로 식욕과 활기가 감퇴되었지만, 몇일내에 정상으로 회복되었다. 수술로 인해 생긴 상처는 시술 10일내에 아물었다. 맹장에 장착된 튜브와 총배설강에 외과적으로 시술된 프라스틱 채집병을 이용하여서 배설물이 매일 모아졌다. 사후검사에서 맹장에 삽입되었던 고무튜브주위를 둘러싸고 있는 완벽한 맹장이 확인되었다.

닭의 괴사성 장염에 대한 병리학적 연구 (Pathological changes on naturally occuring necrotic enteritis in chicken)

  • From January of 1991 to December of 1992, 42 chickens collected from 21 poultry farms and also diagnosed as necrotic enteritis(NE) was examined clinical signs, gross and histopathological findings. Main clinical signs were characterized by decreased appetite, mild to severe depression, reductance to move, ruffled feathers, greenish to yellow-browinish diarrhea sometimes including blood. As progressed, diseased chickens showed feces mixed with necrotic debris which detached from the intestinal mucosa and mostly resulted in the death. In chronic cases, there were dirty feathers around cloaca due to diarrhea and notably retarded growth. Principle gross lesions were usually confined to the jejunum and ileum, especially toward the lower part of Meckel's diverticulum. The part of small intestine was frequently distended with gas, and also showed mucosal congestion and hemorrhages with varying degrees. Sometimes, the intestinal mucosa was thickened, and also covered with fibronecrotic psuedomembrane. In addition, there were focal necrosis and severely multifocal ulcreation in the mucosa of small intestine. Major histopathological findings included villous necrosis and erosion of the small intestine covering with lots of bacterial colonies, inflammatory cell infiltration in the lamina propria, and dilatation and hyperplasia of crypts. Luminal exudate contained bacterial colonies, fibrin, erythrocytes, and desquamated epithelium. Thirteen(61.9%) out of 21 NE-occurring farms were complicated with intestinal coccidiosis.
