The reliability of Anycheck device related to healing abutment diameter

  • PURPOSE. The purpose of this in vitro study was to examine the reliability of the Anycheck device and the effect of the healing abutment diameter on the Anycheck values (implant stability test, IST). MATERIALS AND METHODS. Thirty implants were placed into three artificial bone blocks with 10 Ncm, 15 Ncm, and 35 Ncm insertion torque value (ITV), respectively (n = 10). (1) The implant stability was measured with three different kinds of devices (Periotest M, Osstell ISQ Mentor, and Anycheck). (2) Five different diameters (4.0, 4.5, 4.8, 5.5, and 6.0 mm) of healing abutments of the same height were connected to the implants and the implant stability was measured four times in different directions with Anycheck. The measured mean values were statistically analyzed. RESULTS. The correlation coefficient between the mean implant stability quotient (ISQ) and IST value was 0.981 (P<.01) and the correlation coefficient between the meant periotest value (PTV) and IST value was -0.931 (P<.01). There were no statistically significant differences among the IST values with different healing abutment diameters. CONCLUSION. There was a strong correlation between the Periotest M and Anycheck values and between the ISQ and IST. The diameter of the healing abutment had no effect on the Anycheck values.

A study on perceived value of work among the clinical dental hygienists and its determinants (임상치과위생사들의 일가치감 및 그 결정요인에 관한 연구)

  • This study, to investigate the state of PVW among the clinical dental hygienists and its determinants, has polled 322 clinical dental hygienists working in Daegu, Kyung buk province, and Ulsan. Data were collected by self-administrated questionnaire. A SPSS 12.0 for Windows was used for statistical analysis. The obtained results were as follows: 1. The average of PVW among the respondents was 3.43. 2. The area average of PVW determinants was as follows; job significance, 3.31, process efficiency, 3.42, product usefulness, 3.32, vision realizability, 3.53, social recognition, 3.23 and the total average, 3.36. 3. The married have higher PVW than the unmarried in all areas(p=.012, p=.008, p=.036, p=.007). 4. As for working experience, in the case of perceived self value, those having 1~2 years experience have the highest PVW(p=.000), apart from perceived self value, those having over 6 years experience have the highest PVW in all areas(p=.015, p=.001, p=.002). Relating to the task, those working in the patient consulting and managing part have the highest PVW, relating to the annual salary, those having over 25 million won have the highest PVW(p=.000, p=.000, p=.001, p=.000). 5. The relationship between PVW and PVW determinants, in all respects, is meaningful, so it was found that the higher determinants are, the higher PVW is. 6. The whole explanation variate of PVW determinants was meaningful in PVW, perceived self value, value fulfillment emotion, respectively representing 38.8%, 55.6%, 61.5%(pE.001). PVW of clinical dental hygienists should be considered as a significant criterion of indivisual and organizational effectiveness. To improve effectually PVW of the clinical dental hygienists, there is need to establish culture in a system of hospital that redound to their social recognition.

Utility Analysis for Pegfilgrastim in DLBCL Patients on R-CHOP Regimen (항암치료를 받는 미만성거대비세포 림프종 환자에서 페그필그라스팀에 대한 효용성 평가)

  • Objective: This study was designed to compare pegfilgrastim and filgrastim in diffuse large B-cell lymphoma (DLBCL) patients treated with a rituximab with cyclophosphamide, hydroxydaunorubicin, oncovin, and prednisone (R-CHOP) regimen in terms of clinical efficacy and cost-effectiveness. Method: Clinical efficacy was measured by trough level of absolute neutrophil count (ANC), days of ANC under 50% of baseline value, days of ANC under 90% of baseline value, duration of ANC recovery to baseline value, days of ANC less than $0.5{\times}10^9cells/L$, and difference of peak and trough level of ANC during 1 cycle of R-CHOP regimen. To evaluate cost-effectiveness, total prices of used filgrastim and pegfilgrastim within 1 cycle of R-CHOP were analyzed. Results: In terms of clinical efficacy, trough level of ANC and days to ANC recovery showed statistical significance. The median trough levels of ANC with administration of filgrastim and pegfilgrastim were 0.18 and 1.94 (p = 0.021), respectively, and the median durations of ANC recovery to baseline value were 5.5 days and 2 days (p = 0.023), respectively. For the median days of ANC under 50% of baseline value, days of ANC under 90% of baseline value, days of ANC less than $0.5{\times}10^9cells/L$, and difference of peak and trough level of ANC during 1 cycle of R-CHOP, the pegfilgrastim group performed better than the filgrastim group. However the difference was not statistically significant. In terms of overall expense during 1 cycle of R-CHOP, pegfilgrastim is about 3.43 times more expensive than filgrastim. Conclusion: Pegfilgrastim is more efficient than filgrastim in terms of clinical efficacy. In terms of prices, pegfilgrastim is more expensive than filgrastim for patients, but it is more convenient in clinical use. Therefore, pegfilgrastim should be the preferred choice of G-CSF for neutropenic patients. Further comparative study of pegfilgrastim and filgrastim is needed.

A Study of the Ethical Values of Korean Nurses (임상간호사들의 윤리적 가치관에 관한 연구)

  • The purse of this study is to identify whether ethical values of Korean nurses are deontological or utilitarian. Nurse's ethical value questionnaire was developed from review of the literature and interview of nurses in the clinical settings. Ethical problem are categorized into four areas 1) human life 2) nurse-patient relationship 3) nurse-nursing task relationship 4) nurse-collegue relationship The data were obtained from 250 nurses in the clinical settings from Jun. to July in 1998 using the ethical value questionnaire developed by Lee (1990). The analysis of data was done by Pearson's correlation coefficient, t-test and anova. The results of this study were as follows : 1. The ethical values of human life slightly took up the position of utilitarian. 2. The ethical value of nurse-patient relationship slightly took up the deontological position. 3. The ethical value of nurse-nursing task relationship slightly took up the deontological position. 4. The ethical value of nurse-collegue relationship greatly took up the deontological position. 5. The ethical values of nurses related to demographic characteristics of age, marital status, position. Therefore it is suggested that new strategies and continuing education programs be established to help clinical nurses formlulate higher ethical values.

Types and Causes of Non-Value-Added Activities in Nursing Practice in Korea (비부가가치 간호활동(Non-Value-Added Nursing Activity) 유형과 발생원인 분석)

  • Purpose: The purpose of this study was to identify the types and causes of non-value-added (NVA) activities in nursing practice, and to determine the frequency of each NVA type and causes of NVA in clinical area. Methods: This study was conducted using Delphi technique. First, in order to identify NVA and their causes, 24 nurses with 7 years or more of clinical experience were recruited from medical/surgical units in six general hospitals in Korea. Then the NVA types and causes were tested using a larger sample of 130 nurses with more than 3 years of clinical experience at two general hospitals in Korea. Results: NVA was categorized into 6 different types, which are repeating, duplicating, waiting, reverse-proxy working, reworking, and searching. The most prevalent NVAs were repeating and duplicating works. Reworking and searching were less frequent types than others. The causes of NVA were classified into personnel-related, supporting departments, records, regulations, information, materials or instruments, and others. Among them, personnel-related and supporting departments were reported with the highest scores. Conclusion: NVA leads to waste cost and time. These results demonstrated the situations and causes of NVA occurred in nursing practice. Further studies on the typology and moderation of NVA activities are warranted to improve the efficiency and quality of nursing care in day-to-day practice.

A Study on the Clinical Nurse's Ethical Value (인간생명에 대한 간호사의 윤리의식)

  • Purpose: This descriptive study was designed to explore the clinical nurse's ethical value regarding human life. Method: Data were collected from September to October, 2002. Study subjects were 527 clinical nurses working in General Hospital as tertiary located in Seoul. Ethical value was measured with questionnaire developed by researchers and consisted on items regarding ethical value on human life. Result: Among the items, most nurses highly agree with the item, "When a patient requests his/her health care provider to keep his/her personal secret, the health care provider is obliged to do so." and "When a patient asks for information on his/her medicinal and dietary contents, his/her wish must be granted." Most clinical nurses mainly agree with the item. "Health care providers must always be honest to the patient and/or his/her family". However, most nurses disagree with the item, "When a patient is on the verge of death after an accident, it is justifiable to soothe his/her family by saying 'he/she is OK' instead of telling them the truth, in order to avoid a sudden shock befalling on them". Conclusion: It is intensifying the notion of ethical underpinning for human rights, truthfulness is essential to a trust relationship under what circumstances. Also most clinical nurses agree with that It is essential to trust in the nurse-patient relationship, patients have the right to know and it is the ethical thing to do as health care provider.

An Investigational Study of Nursing College Student's Clinical Practice Satisfaction (3년제 간호대학생의 임상실습 만족도에 관한 조사 연구)

  • The purpose of this study was to investigate the factors and the degree of nursing college student's clinical practice satisfaction. The sample consisted of 488 nursing students who had been in the clinical practice experience from 6 junior nursing colleges in Korea. Data for this study was collected from 20 May to 20 June, 2000. Data were analysed by SPSS/PC using frequency, percentage, mean, standard deviation, T-test, one-way ANOVA, and regression. The results of this study are as follows : 1. The highest motive of selecting a nursing science came to be 'having a good prospect for getting a job' (29.3%), major place of the clinical nursing practice was university hospital(46.5%), and the degree of satisfaction about the nursing science was 'relatively satisfied' (43%). 2.The fields that the highest satisfaction of the nursing student's clinical practice were DR(3.25), NR(3.18), ICU(3.11), and OR(3.10). The highest rank of the nursing student's clinical practice satisfaction was 'feels more continuous nursing studying is required after getting a job or pass the RN exam'(4.24). 3. There was significant value between the Grade(2yr, 3yr) and the satisfaction of the clinical practice environment(p<0.001), the clinical practice interpersonal relationship (p<0.001), the clinical practice evaluation (p<0.05), and there was significant value between the satisfaction about the nursing science and the satisfaction of the clinical practice environment(p<0.001), the clinical practice interpersonal relation ship (p<0.05), the satisfation after the clinical practice(p<0.001). 4. There was significant value between 'the opinion of the clinical practice periods' and the satisfaction of the clinical practice place(p<0.05), the clinical practice environment(p<0.001), the clinical practice interpersonal relationship(p<0.001), the satisfation after the clinical practice(p<0.001). 5.The satisfaction of the clinical practice interpersonal relationship influenced on the nursing student's clinical practice satisfaction($R^{2}=.188$). From these results, it is suggested that the higher the satisfaction of the clinical practice interpersonal relationship be like to improve the nursing college student's clinical practice satisfaction.

Effectiveness of Temporomandibular Joint Disorder Follow-Up Using Bone Scans

  • Purpose: To evaluate the effectiveness of temporomandibular joint (TMJ) disorder follow-up and determine the factors that affect the TMJ bone scan hot spot numerical value (bone scan value), and to compare this value to the diagnosis of patients with temporomandibular joint disorders (TMD), their treatment options, and the resolution of their symptoms. Materials and Methods: A retrospective cohort study was performed on 24 patients (four males, 20 females) who received TMD treatment in the Section of Dentistry, Seoul National University Bundang Hospital (Seongnam, Korea) from 2007 to 2014. An analysis of the significance test and correlation between TMD diagnosis, treatment options, a baseline the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) questionnaire, treatment before and after the clinical examination and subjective progress, and TMJ bone scan value change were completed by using SPSS version 12.0. Result: Although only 14 patients had bony factors that caused TMD, the average pre-treatment bone scan value of the all patients was $4.29{\pm}0.31$, which is higher than the finding for osteoarthritis (3.88), and reduced post-treatment bone scan value was found to be without a statistically significant difference (P=0.056). After the treatments, clinical symptoms in 18 patients disappeared, and six patients did not require additional treatment, although they still displayed subjective symptoms. It was observed that the higher the pre-treatment bone scan value, nonspecific physical symptoms, chronic pain index, characteristic pain intensity, disability score, were, the lower the post-treatment bone scan value was. And this reduced post-treatment bone scan value tendency was not shown with the pre-treatment depression index, but there was not a statistical difference. Conclusion: The post-treatment TMJ bone scan value tended to be insignificantly reduced in the 24 patients whose clinical symptoms were improved (P=0.056). Moreover, the TMJ bone scan value showed no relation to the TMD type or its related symptoms.

Is Transcranial Doppler Ultrasonography Old-fashioned?: One Institutional Validity Study

  • Objective : The purpose of this study is to investigate the correlation between various transcranial Doppler (TCD) ultrasonography parameters and clinical vasospasm after aneurysmal subarachnoid hemorrhage (SAH). Methods : This study enrolled 40 patients presented with aneurysmal SAH between September 2006 and August 2007. We measured differences of mean blood flow velocity (BFVm), highest systolic blood flow velocity (BFVh), and Lindegaard ratio (LR) in the middle cerebral artery on TCD examination. These parameters were evaluated for correlation with clinical vasospasm by univariate analysis and the receiver operating characteristic analysis. Results : Twelve patients (30%) developed clinical vasospasm. The best TCD parameters for the detection of clinical vasospasm were revealed to be differences of BFVm, BFVh, and LR values between $1^{st}$ TCD test and $3^{rd}$ TCD (7 cm/s. 11.5 cm/s, 0.45 respectively). The positive predictive value of anyone of three parameters was 60% and the negative predictive value was 100%. Conclusion : TCD is still considered a useful tool for screening clinical vasospasm. To confirm the predictive value of the above parameters. further prospective study will be needed.
