Purpose: This study aimed to investigate student nurses' satisfaction by type of clinical practicum and to determine predictors of clinical competence in pediatric nursing. Methods: A total of 189 Junior and Senior student nurses across seven colleges in the Busan Metropolitan City were enrolled in the study. The participants completed a structured questionnaire containing items about their learning satisfaction with different types of pediatric nursing practicums and their clinical competence. Data were analyzed using the mean, standard deviation, independent t-test, ANOVA, and multiple regression analysis. Results: Regarding satisfaction with each type of clinical practicum, the mean satisfaction score (out of 10) was 8.18±2.26 for on-site clinical rotations and 7.35±2.20 for alternative practicums. Among the different types of alternative practicum approaches, those with a satisfaction score of 7 or higher included fundamental nursing skills, watching videos, simulation etc., while those with a satisfaction score of less than 6 were virtual simulation and problem-based learning. The predictors of clinical competence in pediatric nursing were learning satisfaction with practice, school year, and alternative practicum, accounting for 35.0% of the variance in clinical competency. Conclusion: It would be helpful to combine on-site clinical rotations with alternative practicum approaches and to develop various alternative practice programs using simulation practice, virtual reality, immersive interactive systems, and standardized patients to enhance students' clinical competency.
Purpose: To evaluate the clinical outcomes and radiographic results of open reduction and internal fixation for intraarticular calcaneal fractures. Materials and Methods: We reviewed 20 cases of calcaneal fractures managed with open reduction and internal fixation from March 2003 to January 2005. We used the computed tomographic classification system proposed by Sanders et al to classify these fractures. Preoperative and postoperative Bohler's angle, heel height (calcaneal facet height) and calcaneal length, calcaneal width were measured. The Creighton-Nebraska Health Foundation Assessment score was used for clinical evaluation. Results: There were 12 cases of type II fractures, 5 of type III fractures and 3 of type IV fractures. The mean clinical score was 84.3 for type II, 82.6 for type III and 56.1 for type IV. The mean preoperative $B{\ddot{o}}hler$ angle was $6.1^{\circ}$ and final was $22.8^{\circ}$. The mean preoperative calcaneal facet height was 76.6 mm and final was 80.3 mm (The mean calcaneal facet height was changed from preop 76.6 mm to postop 80.3 mm). The mean preoperative calcaneal length was 88.2 mm and final was 92.6 mm. The mean preoperative width was 38.1 mm and final was 35.6 mm. Conclusion: Open reduction and internal fixation showed good results for type II and III fractures, but for type IV fractures the clinical result was significantly worse than the other types. However, type IV fractures still had restoration of (should be restored in) $B{\ddot{o}}hler's$ angle, calcaneal facet height, calcaneal length and width which may be helpful in later subtalar fusion.
Neurofibromatosis type 1 (NF1), a prevalent genetic disease that is transmitted in an autosomal dominant manner, is characterized by multiple cutaneous cafe-au-lait spots and neurofibromas as well as various degrees of neurological, skeletal, and neoplastic manifestations. The clinical features of NF1 increase in frequency with age, while the clinical diagnosis can remain undetermined in some pediatric patients. Importantly, affected patients are at risk for developing tumors of the central and peripheral nervous system. Therefore, adequate counseling for genetic testing, age-appropriate surveillance, and management are important. This review suggests several issues that should be considered to help general pediatricians provide adequate clinical care and genetic counseling to patients with NF1 and their families.
Kniest syndrome (OMIM #156550) is a rare autosomal dominant disorder caused by a dysfunction of type II collagen, which is encoded by the COL2A1 gene (OMIM +120140) mapped to chromosome 12q13.11. Type II collagen, a molecule found mostly in the cartilage and vitreous tissues, is essential for the normal development of bones and other connective tissues. Kniest syndrome is a type II collagenopathy that presents as skeletal abnormality associated with disproportionate dwarfism, kyphoscoliosis, enlarged joints, visual loss, hearing loss, and cleft palate. This report describes a Korean patient with Kniest syndrome who was diagnosed with typical clinical features and radiologic findings. The patient presented with disproportionately short stature and kyphoscoliosis from birth. A skeletal survey revealed fused lamina in the thoracic spine, hemivertebrae, flexion deformities in multiple joints, and plagiocephaly.
Objectives : We investigated the incidence rate and type of adverse events associated with RCTs(Randomized Controlled Trials) of acupuncture and moxibustion. Methods : This study included 949 patients who received acupuncture or moxibustion or usual care from 8 RCTs. We collected data including gender and age of patients, intervention, treatment frequency and type of adverse events in clinical trials from their case report forms. Results : Among the 949 patients, 83 patients(8.7%) suffered at least one adverse event throughout the clinical trials. Types of adverse event in acupuncture & moxibustion clinical trials are common cold, skin changes, pain, dizziness, bruise, gastrointestinal diseases, changes of blood chemistry, burn. Adverse events were significantly correlated to patients' age, intervention, body mass index and treatment group. Conclusions : A Guide-line for collecting and managing adverse events of acupuncture & moxibustion clinical trials are needed.
The purpose of this study is to explore perception types and to understand the nature of experience of nursing students' clinical education by using the Q methodology. A Q sample was developed through a review of the literature and descriptions about nursing students' experience in clinical practice. Thirty-six statements made up the finalized Q sample. The P sample consisted of 33 third grade nursing students in K university. Q statements were written on separate cards and were given to the 33 subjects to sort according to degree of agreement or disagreement. The Q-sorts by each subject were coded and analysed with the Quanl PC program. A a result, three major perception types, namely, 'alienation of ideal and reality', 'active participation', and 'perception of limitation of ability' were identified. By identifying the nature of the three types, this study suggests efficient strategies for developing clinical educational programs according to the perception types of nursing students. Clinical education would thereby be more valuable.
Objective: This study was conducted to investigate the differences in several serum adipokines in perinatal dairy cows with type I and II ketosis, and the correlations between these adipokines and the two types of ketosis. Methods: Serum adiponectin (ADP), leptin (LEP), resistin, tumor necrosis factor-α (TNF-α), and interleukin-6 (IL-6) levels, and energy balance indicators related to ketosis were measured. Type I and II ketosis were distinguished by serum glucose (Glu) and Y values and the correlations between adipokines in the two types of ketosis were analyzed. Results: β-Hydroxybutyric acid of type I ketosis cows was significantly negatively correlated with insulin (INS) and LEP and had a significant positive correlation with serum ADP. In type II ketosis cows, ADP and LEP were significantly negatively correlated, and INS and resistin were significantly positively correlated. Revised quantitative INS sensitivity check index (RQUICKI) values had a significantly positive correlation with ADP and had a very significant and significant negative correlation with resistin, TNF-α, and IL-6. ADP was significantly negatively correlated with resistin and TNF-α, LEP had a significantly positive correlation with TNF-α, and a significantly positive correlation was shown among resistin, IL-6, and TNF-α. There was also a significant positive correlation between IL-6 and TNF-α. Conclusion: INS, ADP, and LEP might exert biological influences to help the body recover from negative energy balance, whereas resistin, TNF-α, and IL-6 in type II ketosis cows exacerbated INS resistance and inhibited the production and secretion of ADP, weakened INS sensitivity, and liver protection function, and aggravated ketosis.
Objectives The purpose of this study was to revise Sasang Digestive Function Inventory (SDFI) and improve its validity and reliability using clinical data. Methods The Sasang type and Cold-Heat pattern differentiation by certified clinical specialist and responses to SDFI items were acquired from 419 hospital patients. The revision of SDFI was performed using item analysis and Explorative Factor Analysis (EFA). Furthermore, the validity and reliability of the revised SDFI (rSDFI) were investigated using Confirmatory Factor Analysis (CFA), Internal Consistency and Item Response Theory (IRT). And, clinical significance of the rSDFI was examined for the differentiation of Sasang types and Cold-Heat patterns. Results The number of the SDFI items were modified from 21 to 15. And, the validity and reliability of the rSDFI subscale structure were found to be acceptable. The scores of rSDFI-total and rSDFI-E significantly decreased in the order of Tae-Eum (TE), So-Yang (SY), So-Eum (SE) types, and the rSDFI score of SE type was significantly lower than that of TE and SY types. The rSDFI-total score could differentiate Cold-Heat pattern in both SY and SE types. And the difference of digestive function between Cold and Heat pattern of SE type could be explained with the rSDFI-D score. The rSDFI-total score in Cold pattern significantly decreased in the order of TE, SY, and SE types, and the rSDFI-total score of TE type is significantly higher than that of SE type in Heat pattern. Conclusions Current results demonstrated the reliability, validity and clinical usefulness of the rSDFI in clinical patients. Therefore, rSDFI can be utilized as an objective clinical measure supporting the differential diagnosis of Sasang typology.
본 연구에서는 2017년 6월부터 7월까지 국내 충남 소재 2차 의료기관의 임상검체에서 분리 된 20개 균주와 ICU환경으로부터 분리 한 4개를 포함해 총 24개의 MRSA 균주에 대한 분자유전학적 특성을 분석하기 위해 SCCmec typing과 mec complex typing을 수행하였다. 임상검체에서 분리된 MRSA 20균주의 SCCmec typing 분석 결과 SCCmec type II가 8건, type IVa가 1건, Not-typeable이 11건으로 나타났다. 의료기관 중환자실 환경에서 분리 된 MRSA 중 한 균주는 mecA 유전자를 가지고 있지 않은 것으로 확인되었으며, 나머지 세 균주는 SCCmec type II, III, IVa가 각각 한 건으로 나타났다. 임상검체와 중환자실 환경검체에서 분리된 MRSA는 SCCmec typing 결과 그 기원이 서로 다른 것으로 나타났으나, 의료기관 환경에서 분리된 1건의 경우 그 기원이 서로 일치하여 의료종사자의 손, 가운 등을 매개체로 하여 환경내의 MRSA가 환자에게 전파되었을 가능성이 있는 것으로 사료된다. 이후 추가적인 연구를 통해 좀 더 많은 수의 병원감염 관련 균주를 대상으로 추적조사를 실시 한다면 임상 현장에 감염관리와 관련한 더욱 유용한 정보를 제공할 수 있을 것으로 사료된다.
본 연구의 목적은 종합병원 신규간호사의 실무적응에 대한 주관성을 유형을 파악하고자 Q방법론을 적용하였다. 연구 대상자는 종합병원 신규간호사 40명을 대상으로 개별 면담을 실시하고 Q방법론을 적용하였다. 진술문은 자신의 의견에 따라 +4점(가장 긍정하는 진술문)에서부터 0(중립) -4(가장 부정하는 진술문)을 선택하여 9점 척도상에 강제 분포하도록 하였다. 자료 분석은 PC QUANL program을 이용하였으며, 분석결과 eigenvalue 1.0이상을 가진 값을 가지고 5가지 유형이 나타났으며 변량은 62.38%이었다. : (1) 새로운 업무 부담형, (2) 흥미, 책임감형, (3) 상황개선형, (4) 지식, 기술 부족형, 그리고 (5) 과도한 업무 부담형의 5가지 유형으로 분류되었다. 이 차이점으로 보았을 때 종합병원 신규간호사의 실무적응유형을 볼 수 있었으며, 이 연구 결과 효과적인 교육과 개별화된 임상실무전략을 개발해야 할 것이다.
