Clinical Practice Guideline for Taeeumin and Taeyangin Disease of Sasang Constitutional Medicine: Diagnosis and Algorithm (태음인·태양인체질병증 임상진료지침: 진단 및 알고리즘)

  • Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Taeeumin and Taeyangin Disease of Sasang Constitutional Medicine(SCM): Diagnosis and Algorithm. This CPG was developed by the national-wide experts committee consisting of SCM professors. Methods We searched the literature and articles related to Taeeumin and Taeyangin Symptomatology diagnosis and algorithm. For developing diagnosis and algorithm, we searched the classification, ordinary symptom, present symptom of the Taeeumin and Taeyangin Symptomatology Results and Conclusions We classified the Taeeumin Symptomatology by 4 steps: Exterior-Interior disease, favorable-unfavorable pattern, mild-moderate-severe-critical pattern, initial-intermediate-advanced pattern. And we classified the Taeyangin Symptomatology by 3 steps: Exterior-Interior disease, favorable-unfavorable pattern, mild-moderate-severe-critical pattern. At the unfavorable pattern, ordinary symptom is very important. So doctors are considered to need to focus on the symptom of unfavorable's ordinary symptoms.

A Study on Reliability and Validity of the Yukmijiwhang-tang Patternization Questionnaire (육미지황탕증(六味地黃湯證) 변증설문지(辨證設問紙)의 신뢰도(信賴度) 타당도(妥當度) 연구(硏究))

  • Background and purpose : We previously developed questionnaire of Yukmijiwhang-tang symptom on the Delphi method. But developed a questionnaire was not verified in the clinical. So, to ensure objectivity, quantification and validity, verification is needed for questionnaire items before applying a clinical. On this study, we looked at whether questionnaire items had been validity in the clinical. Methods : The previously developed questionnaire of Yukmijiwhang-tang symptom was applied to 200 patients who visited 12 local oriental medicine clinics. Item data were analyzed by factor analysis and reliability test. Results : The developed questionnaire showed remarkable reliability.(Cronbach's $\alpha$=0.869) In factor analysis, items were clustered to 6 factors. Conclusions : The results of this study indicate that the developed quetionnaire is reliable and valid for Yukmijiwhang-tang symptom. It can be used to evaluate Yukmijiwhang-tang symptom objectively.

A Case Report of Tinnitus Treated by Gyejigagalgeun-tang (계지가갈근탕(桂枝加葛根湯)을 투여한 이명 증례 1례)

  • Objective : This study is aimed to report the outcomes of the treatment of a patient with tinnitus during 6 month. Methods : Basic patient's data was analyzed as a CARE guideline. And related symptom was measured with VAS. Results : The symptom of tinnitus reduced from 10 to 2 on VAS. And the symptom of feeling cold get lower. Conclusions : Gyejigagalgeun-tang improved the symptom of tinnitus on this study.

LYMPHOCYTES POPULATION IN RELATION TO CLINICAL SYMPTOMS IN IRREVERSIBLE PULPITIS (비가역성 치수염의 임상증상에 따른 임파구 분포에 관한 연구)

  • This study was designed to identify the lymphocytes present and to examine the relation between lymphocytes population and clinical symptoms of the pulps clinically diagnosed as normal and irreversible pulpitis. We recorded the history and severity of the pain and performed several clinical tests, before extirpation of vital, irreversibly inflamed pulps in routine endodontic treatment. Then the teeth were divided into two groups. Five teeth, categorized in acute symptom group, had severe spontaneous pain, particularly at night and were extremely sensitive to cold and heat. The other 15 teeth with history of mild to moderate pain and with or without cold or heat responses were categorized as chronic symptom group. Inflamed pulps were also classified into 8 minor groups by presence or absence of signs or symptoms related to the involved teeth, including the presence of pain on percussion, pain on heat and cold stimuli and the periodontal pocket depth. All extirpated pulps were immediately immersed in ultra low-temperature freezer($-74^{\circ}C$), and they were sectioned $6{\mu}m$ in thickness. Specimens were stained using three-stage indirect immunoperoxidase techniques(DAKO, LSAB kit) and monoclonal antibodies for detecting the presence of T lymphocytes(T), B lymphocytes(B) and helper(T4) and suppressor(T8) lymphocytes. Following results were obtained; 1. All the examined normal and inflamed pull) tissues had positive staining for T lymphocytes and T helper and T suppressor cells. But B cells were observed only in inflamed pulp. 2. Statistically more T and B cells were observed in acute symptom group as compared with chronic symptom group(p<0.05). 3. Cell ratio of BIT in acute symptom group were significantly higher than that of chronic symptom group(p<0.05). 4. Only B cells were significantly increased in the percussion positive group than the number of B cells in percussion negative group(p<0.05). 5. No differences were observed in the number of different cell types among other minor groups.

Impact of an Early Hospital Arrival on Treatment Outcomes in Acute Ischemic Stroke Patients (급성 뇌경색 환자의 증상 발현 후 응급실 도착까지의 시간이 치료 결과에 미치는 영향)

  • Objectives : Recent educational efforts have concentrated on patient's early hospital arrival after symptom onset. The purpose of this study was to evaluate the time interval between symptom onset and hospital arrival and to investigate its relation with clinical outcomes for patients with acute ischemic stroke. Methods : A prospective registry of patients with signs or symptoms of acute ischemic stroke, admitted to the OO Medical Center through emergency room, was established from September 2003 to December 2004. The interval between symptom onset and hospital arrival was recorded for each eligible patient and analyzed together with clinical characteristics, medication type, severity of neurologic deficits, and functional outcomes. Results : Based on the data of 256 patients, the median interval between symptom onset and hospital arrival was 13 hours, and 22% of patients were admitted to the hospital within 3 hours after symptom onset. Patients of not-mild initial severity and functional status showed significant differences between arrival hours of 0-3 and later than 3 in terms of their functional outcomes on discharge. Logistic regression models also showed that arrival within 3 hours was a significant factor influencing functional outcome (OR=5.6; 95% CI=2.1, 15.0), in addition to patient's initial severity, old age, cardioembolism subtype, and referral to another hospital. Conclusions : The time interval between symptom onset and hospital arrival significantly influenced treatment outcome for patients with acute ischemic stroke, even after controlling for other significant clinical characteristics. The findings provided initiatives for early hospital arrival of patients and improvement of emergency medical system.

Treatment for Patients with Acute Ischemic Stroke Presenting beyond Six Hours of Ischemic Symptom Onset : Effectiveness of Intravenous Direct Thrombin Inhibitor, Argatroban

  • Objective : The objectives of this study were to analyze the outcome and hemorrhagic risk of intravenous (IV) argatroban in patients with acute ischemic stroke presenting beyond six hours of ischemic symptom onset. Methods : Eighty patients with acute ischemic stroke who were admitted to the hospital beyond six hours from ischemic symptom onset were retrospectively analyzed. We could not perform IV thrombolysis or intra-arterial thrombolysis because of limited time window. So, IV argatroban was performed to prevent recurrent thrombosis and progression of infarcted area. The outcome was assessed by the National Institute of Health Stroke Scale (NIHSS) score and related hemorrhagic risk was analyzed. Also, each outcome was analyzed according to the initial stroke severity, subtype, and location. Results : The median NIHSS was 8.0 at admission, 4.1 upon discharge, and 3.3 after three months. A good outcome was achieved in 81% of patients upon discharge and 88% after three months. Symptomatic hemorrhage occurred in only two patients (3%). IV argatroban was effective regardless of initial stroke severity, subtype, and location. Conclusion : IV argatroban may be an effective and safe treatment modality for acute ischemic stroke presenting beyond six hours of ischemic symptom onset.

A Study on the Relationship between Climacteric and Depressive Symptoms in Middle Aged Women in an area (일개지역 중년 여성의 갱년기 및 우울증상과의 관계 연구)

  • Purpose. The purpose of the project is to provide basic data to help middle aged women manage their health by identifying their relationships with Climacteric and depression patients and studying ways to properly overcome the changes in the Climacteric Methods. After distributing the questionnaire to women who were approved for middle-aged women in one region from April 14, 2017 to May 1, 2017 a total of 138 sections were collected and 130 copies were used as research and analysis data, excluding eight inappropriate ones Results. The average score for Climacteric was 1.53 out of 2 points on average for Climateric symptoms, and depression was 10.74 out of 60 points. There were three analysis of Climacteric symptoms: physical symptoms, mental symptoms, and sexuality, and a large number of symptoms were reported as physical symptoms with 1.56 points Conclusions. A program of measures to reduce depression in middle-aged women requires active social attention and support to alleviate menopausal symptoms, and further research is needed to determine the relationship between these concerns and support for depression.

Relationships among Activity Status, Anxiety, Depression, Social Support, Symptom Experience, and Functional Status in Lung Cancer Patients based on the Theory of Unpleasant Symptoms (폐암 환자의 활동상태, 불안, 우울, 사회적 지지, 증상경험과 기능적 상태의 관계: 불쾌감이론 기반으로)

  • Purpose: The purpose of this study was to investigate the relationships of activity status, anxiety, depression, social support, symptom experience, and functional status in patients with lung cancer based on the theory of unpleasant symptoms. Methods: The participants for this study were 101 lung cancer patients who visited the out-patient department for treatment or follow-up at one hospital in Seoul. Data were collected from January 1 to February 8, 2013 using self-reported questionnaires and clinical records. To measure variables, the functional scale and symptom scale of the European Organization for Research and Treatment of Cancer (EORTC) Quality of Life Questionnaire-Core 30, EORTC QLQ-Lung Cancer 13, Eastern Cooperative Oncology Group Performance Status, Hospital Anxiety and Depression Scale, and Multidimensional Scale of Perceived Social Support were used in this study. The data were analyzed using SPSS 19.0 software for Windows. Results: The symptom experience showed more severity in patients with lower activity status, higher anxiety and depression. With lower activity status and social support, functional status was lower. When anxiety, depression, and symptom experience were higher, functional status was also lower. The significant factors predicting symptom experience were depression, anxiety, activity status, and social support, which explained 57.8% of the variance. Conclusion: These results suggest that psychological factors such as anxiety and depression had a negative influence on the symptom experience of lung cancer patients. Therefore, providing emotional support based on the patients' needs prior to providing symptom management could be a useful strategy for improving symptom experience and functional status.

Clinical features of COVID-19 as presented in journal articles : A Korean Medical Approach (COVID-19 임상표현에 대한 한의학적 접근 -국내외 논문을 중심으로-)

  • Objectives : This paper examines major symptoms representation in COVID-19 patients as groundwork for development of an effective clinical data collection format in Korean Medicine. Methods : Major symptoms representation in COVID-19 related papers published worldwide were collected. Corresponding symptoms in Korean Medicine were then examined, followed by discussion of symptomatic features that require further consideration in regards to a more systematic clinical data collection. Results : Of 256 papers, most papers listed fever and cough while symptoms such as difficulty breathing, diarrhea, muscle pain, headache, nausea, fatigue, chest pain, phlegm, nasal discharge were also mostly listed. Clinical representations could be categorized into general symptoms, throat symptoms, chest symptoms, head and facial symptoms, gastrointestinal symptoms, musculo-skeletal and cutaneous symptoms, psychiatric symptoms and sensory problems. Conclusions : Although each clinical representation could be likened to certain clinical representations of Korean Medicine, the variety of symptoms were too limiting and lacking in detail to be applied in the pattern identification[辨證] of Korean Medicine. For effective clinical data collection and analysis in the future, symptom change according to time, comparison between location, climate and ethnicity, existence of interior symptoms when diagnosing exterior symptoms, deficiency-excessiveness of blood patterns, consciousness levels, etc., need to be considered in establishing criteria for symptom evaluation.

The Decision Tree to Analyze the Cases' Ordinary Symptoms Prescribed Yeoldahanso-tang and Taeeumjowi-tang·Choweseuncheng-tang (열다한소탕과 태음조위탕·조위승청탕의 소증 분석을 위한 의사결정나무 구성)

  • Objectives The purpose of this study is to analyze the decision making process of prescribing Yeoldahanso-tang and Taeeumjowi-tang Choweseuncheng-tang using decision tree. Methods We used collected the prospective clinical data of TE type from September 2012 to July 2015. In this study, we used gender, BMI, blood pressure, pulse and clinical symptoms (digestion, sweat, defecation, urination, sleep, physical status, emotion, heat-coldness, water consumption, facial color) as variables. Decision trees were analyzed using open source R version 3.3.2. Results & Conclusions We found that the decision trees differed among institutions. However, in all institutions, it was found that stool type (ordinary symptom), urine frequency (ordinary and present symptom) and anxiety (ordinary symptom) were important in the decision of prescription. Besides, clinical informations such as sex, Body Mass Index and blood pressure affected the prescription decision.
