Atopic myelitis (AM) is a relatively mild form of myelitis associated with allergic diathesis, and present with predominant sensory manifestations. Lhermitte's sign has been considered as a relatively non-specific clinical sign suggesting demyelinating lesion in cervical cord. Here we report a patient with recurrent AM who presented with isolated Lhermitte's sign, both in first and second attacks. This report suggests that either the diagnosis or recurrence of AM can be frequently underdiagnosed because of its predominant sensory manifestations.

Objectives: The purpose of this study was to investigate the effects of Korean Medicine on postural instability and gait difficulty(PIGD) in patient with parkinsonism. Methods: We reviewed the charts of patients with parkinsonism who were admitted to the department of Korean internal medicine, stroke and neurological disorders center, Kyung Hee University Hospital at Gangdong from October 2009 to May 2017. We analyzed the data of UPDRS (Unified Parkinson's disease rating scale) and calculated PIGD related UPDRS before and after the admission. Results: The average of PIGD score for the 23 patients of Parkinsonism significantly decreased from $8.35{\pm}5.13$ to $4.52{\pm}3.68$ after treatment. The average of PIGD score for the 16 patients of idiopathic parkinson's disease and 7 patients of atypical Parkinsonism decreased from $9.13{\pm}4.46$ to $4.63{\pm}4.15$ and from $6.57{\pm}6.45$ to $4.29{\pm}2.56$ after treatment, respectively. Conclusions: These results provide that Korean Medicine has an effect on the motor function of patients who suffer from Parkinsonism with Postural Instability and Gait Difficulty. Furthermore it could be effective for improving activities of daily life.

■ Objectives 당귀작약산 단회 복용으로 정상인의 총경동맥 탄력도에 미치는 영향을 평가하고자 하였다. ■ Methods 본 연구는 정상인 남성을 대상으로 한 전향적 무작위 대조군 교차시험연구다. 모든 대상자들은 임의로 당귀작약산 복용군과 비복용군인 대조군으로 나누어졌다. 1주일 간격으로 총 2회 방문하며, 첫 번째 방문 시에 당귀작약산 복용군은 당귀작약산 1포(2.5g)과 물을 제공받고, 대조군은 물만 제공받아 복용하였다. 두 번째 방문 시 첫 번째 방문과 반대로 당귀작약산을 복용했던 복용군은 물만 제공받고, 물만 복용했던 피험자들은 당귀작약산 1포와 물을 제공받았다. 물은 100 cc로 매번 동일하게 제공되었다. 모든 대상자들은 복용 직전과 복용 2시간, 4시간 후에 총경동맥 탄력도, 혈압, 총경동맥 내막-중막 두께와 맥박수를 측정하였다. ■ Results 총 20명의 정상인 남성이 모집되었으며, 시간에 따라 당귀작약산 복용 후의 총경동맥 탄력도가 대조군에 비하여 유의하게 상승한 것이 확인되었다. 내막-중막 두께, 혈압 및 맥압, 맥박은 유의한 변화가 나타나지 않았다. ■ Conclusion 당귀작약산의 단회 복용으로 총경동맥 탄력도가 즉시 개선되는 것을 확인하였고, 이로써 당귀작약산이 동맥 경직도 완화 및 탄성의 개선에 영향을 미친다는 것을 알 수 있었다.

■ Objective This is a case report about monocular abducens nerve palsy in a patient with pontine infarction. ■ Method At the time of onset, the patient had eye movement impairment in right eye and diplopia. Brain MRI image showed that there was an infarction in the right pons where the nucleus of abducens nerve is located. There was no obvious improvement in the eye movement before he received Korean medicine treatment. He recieved Korean medicine including acupuncture, electroacupuncture, pharmacoacupuncture and herb medicines for fifty three days. ■ Result After treatment, the movement of right eye was improved to a normal range. The difference in distance from 'center of the pupil' to 'external canthus of the eye' at maximum abducent in both eye changed 0.9cm to 0.1cm at the discharge. In addition, Diplopia was improved at the discharge. ■ Conclusion This case report demonstrated that the Korean medicine treatment is effective to diplopia and eye movement impairment from abducens nerve palsy caused by pontine infarction.

Objectives: This study was performed to review clinical research trends in the treatment of primary headache disorders with pharmacopuncture. Methods: We searched clinical studies on primary headache disorders treated with pharmacopuncture in four electronic databases including OASIS, RISS, CNKI, and Pubmed. The selected studies were analyzed with regard to study design, subject, intervention, evaluation, and result. Results: Five randomized controlled trials, one non-randomized controlled trial, one before-and-after study, and six case series were selected. Most of the studies showed that pharmacopuncture treatment was significantly effective in treating primary headache disorders; however, the quality of the randomized controlled trials was low. Conclusions: According to this study, pharmacopuncture could be a useful treatment option for primary headache disorders. Based on these results, further studies on the effectiveness and safety of pharmacopuncture for primary headache disorders should be performed in the near future.

Septo-optic dysplasia is a congenital anomaly with diverse phenotypes from normal to mixtures of visual abnormality, endocrine dysfunction, psychomotor retardations and epileptic seizures. It is characterized by optic atrophy, pituitary dysfunction and midline structure abnormalities in corpus callosum or septum pellucidum. Diagnosis of septo-optic dysplasia plus is made when cortical malformations accompanied. Here we report a middle-aged woman with septo-optic dysplasia plus having unilateral optic atrophy, agenesis of septum pellucidum and cortical malformations.

Blepharospasm and apraxia of lid opening(ALO) are non-paralytic causes of involuntary eyelid closure. Clinically it is difficult to differentiate blepharospasm and ALO, and these two conditions are sometimes associated. We report a case of pretarsal blepharospasm presenting as apraxia of lid opening. 55-year-old woman was noted to have voluntary eye opening difficulty. We synchronously record the electromyographic(EMG) from the levator palpebrae superioris and the orbicularis oculi muscles in this patient. This results suggested that she has a variant of blepharospasm due to abnormal contraction in the pretarsal orbicularis oculi.

A 69 year-old woman was admitted with sudden left facial weakness. She had no other neurologic deficit, except for left peripheral type facial palsy. She had a presumptive diagnosis of Bell's palsy. The blink test was indicative of left facial neuropathy due to left medullary lesion. Diffusion weighted (DWI) brain MRI demonstrated high signal signal lesion in left dorsolateral ponto-medullary junction. Apperant diffusion coefficient (ADC) brain MRI showed low signal lesion in the same area. We present an unusual case of ipsilateral peripheral facial palsy in dosolateral ponto-medullary infarction without other neurologic deficits.

Subacute sensory neuronopathy and gastrointestinal pseudoobstruction are considered classical paraneoplastic neurological syndromes. We report a 56-year-old male who presented with typical symptoms of subacute sensory neuronopathy and autonomic neuropathy with gastrointestinal pseudoobstruction. The biopsy of the palpable supraclavicular lymph node revealed a small cell lung cancer. To our knowledge, intestinal pseudoobstruction and sensory neuronopathy in a small cell lung cancer have not been reported in Korea.
