Hemoglobin Level to Facilitate Off-Pump Coronary Artery Bypass without Transfusion

  • Background: Conservation of blood during cardiac surgery is important because of the shortage of donor blood, risks associated with transfusion, and the costs of allogeneic blood products. This retrospective study explored the feasibility of off-pump coronary artery bypass (OPCAB) without transfusion. Methods: One hundred and two consecutive patients underwent OPCAB from January 2007 to June 2012 at Hallym University Sacred Heart Hospital. Excluding 10 chronic renal failures patients, 102 patients were enrolled. Their characteristics, clinical data, and laboratory data were analyzed. We investigated the success rate of OPCAB without transfusion according to preoperative hemoglobin (Hb), and the cutoff point of the Hb level and the risk factors for transfusion. We implemented multidisciplinary blood-saving protocols. Results: The overall operative mortality and the success rate of OPCAB without transfusion were 2.9% (3/102) and 73.5% (75/102). The success rates in patients with Hb<11, 11 70 years, diagnosis of acute myocardial infarction, preoperative Hb and creatinine levels, and operation time. The events precipitating the need for transfusion were low Hb level in 9 patients and hypotension or excessive bleeding in 18 patients. Conclusion: The preoperative Hb level of >11 facilitates OPCAB without transfusion. These results suggest that transfusion-free OPCAB can be performed by modifying the risk factors and correctable causes of transfusion and improving various blood salvage methods.

Measurement and Prediction of Flash Points of Acetic Acid-Formic Acid System using Cleveland Open Cup Apparatus (Cleveland 개방식 장치를 이용한 Acetic acid+Formic acid 계의 인화점 측정과 예측)

  • The flash point is the lowest temperature at which there is enough concentration of flammable vapor to form an ignitable mixture with air. The flash point is a major physical property used to analyse the fire and explosion hazards of a flammable liquid solution. The flash point data for pure components are easily available in several literature. But the flash points of the flammable binary solutions appear to be scarce in the literature. The objective of this study is to measure and estimate the flash point of acetic acid-formic acid system. Cleveland open cup tester was used to measure the flash point. The experimental data were compared with the values estimated by the Raoult's law and the optimization methods based on van Laar and Wilson equations. As a result, the estmated values by optimization methods were found to be better than those based on the Raoult's law.

Selenium arrest G1/S phase of cell cycle in LNCaP human prostate cancer cells (사람 전립선암세포주인 LNCaP에서 셀레늄의 G1/S 세포주기억제에 관한 연구)

  • The trace element nutrient selenium discharges its well-known nutritional anti-tumor activity. Converging data from epidemiological, ecological and clinical studies have shown that selenium can decrease the risk for some types of human cancers, especially those of the prostate, lung, and colon. Mechanistic studies have indicated that selenium has many desirable attributes of chemoprevention targeting cancer cells through DNA single strand breaks, the induction of reactive oxygen species. However, there is no reports about the relationship between methylseleninic acid (MSeA), one of methylselenol metabolites and cell cycle arrest in LNCaP human prostate cancer cells. Our data showed that MSeA arrested G1/S pahse of cell cycle arrest and inhibited DNA synthesis in LNCaP cells and those cellular events by MSeA were due to the induction ofp27 protein which is a well-known cyclin-dependent kinase inhibitor. Taken together, cell cycle arrest occurred by MSeA may contribute to the growth-inhibition of prostate cancer cells.

A Study of Convergence Factors involved in Malignant Tumors in the Breast (유방 악성종양에 관여하는 인자 융합 연구)

  • The purpose of this study was to identify malignant tumor-related factors in breast diseases and to find out how recently reported oral contraceptives affect women in Korea. The research method is data from the 7th National Health and Nutrition Survey conducted by the Korea Centers for Disease Control and Prevention, and was provided according to the procedures for use guided through the website of the Korea Centers for Disease Control and Prevention. The analyzed data covered a total of 9,981 people, 3,341 in 2016, 3,263 in 2017 and 3,377 in 2018. The results from the study showed the significance of age, smoking, drinking, hyperlipidemia and unmarried related variables, especially the significance of oral contraceptives. Based on this study, it can contribute to active prevention of related factors and cancer-causing suppression policies, especially since the use of oral contraceptives is on the rise, it is believed that continuous research of the subjects to be taken, such as the period and dosage, should be carried out.

  • The flash point the lowest temperature at which the concentration of vapor of the substance in the air reaches the lower flammability limit(LFL), and is one of the most important physical properties used to determine the potential for fire and explosion hazards of industrial materials. The most published flash point data was for pure components and the flash points of the binary solutions that have flammable components, appear to be scarce in the literature. In the present study, the flash points of tert-pentanol+n-decane system were measured by Tag open-cup tester. The measured data were compared with the values calculated by the Raoult's law and the optimization methods based on the Wilson and NRTL equations. The calculated values by optimization methods were found to be better than those based on the Raoult's law.

A nationwide study of children and adolescents with pneumonia who visited Emergency De­partment in South Korea in 2012

  • Purpose: Acute respiratory infection, particularly pneumonia, is the most common cause of hospitalization and death among children in developing nations. This study aimed to investigate the characteristics of children and adolescents with pneumonia who visited Emergency Department (ED) in South Korea in 2012. Methods: We analyzed National Emergency Department Information System (NEDIS) records from 146 EDs in South Korea for all pediatric patients aged ${\leq}18years$ who were diagnosed with pneumonia between January and December 2012. Results: Among 38,415 subjects, the male-to-female ratio was 1:0.8. Patients aged <12 months comprised 18.0% of the study population; those aged 1 to 3 years, 54.4%; those aged 4 to 6 years, 16.8%; those aged 7 to 12 years, 7.4%; and those aged 13 to 18 years, 3.4%. Presentation rates were highest in April, followed by January, March, and May. The hospital admission rate was 43.5%, of which 2.6% were in intensive care units. The mortality rate was 0.02%. Based on the International Classification of Diseases, 10th Revision, diagnostic codes, the types of pneumonia according to cause were viral pneumonia (29.0%), bacterial pneumonia (5.3%), Mycoplasmal pneumonia (4.5%), aspiration pneumonia (1.3%), and pneumonia of unknown origin (59.3%). Conclusion: Despite the limited data due to the ED data from the NEDIS lacking laboratory results and treatment information, this study reflects well the outbreak patterns among children and adolescents with pneumonia. Our results provide a basis for future studies regarding ED treatment for children and adolescents with pneumonia.

Experimental In-Vivo Models Used in Fat Grafting Research for Volume Augmentation in Soft Tissue Reconstruction

  • As the popularity of fat grafting research increases, animal models are being used as the source of pre-clinical experimental information for discovery and to enhance techniques. To date, animal models used in this research have not been compared to provide a standardized model. We analyzed publications from 1968-2015 to compare published accounts of animal models in fat grafting research. Data collected included: species used, graft characteristics (donor tissue, recipient area, amount injected, injection technique), time of sacrifice and quantification methods. Mice were most commonly used (56% of studies), with the "athymic nude" strain utilized most frequently (44%). Autologous fat was the most common source of grafted tissue (52%). Subcutaneous dorsum was the most common recipient site (51%). On average, $0.80{\pm}0.60mL$ of fat was grafted. A single bolus technique was used in 57% of studies. Fat volume assessment was typically completed at the end of the study, occurring at less than 1 week to one year. Graft volume was quantified by weight (63%), usually in conjunction with another analysis. The results demonstrate the current heterogeneity of animal models in this research. We propose that the research community reach a consensus to allow better comparison of techniques and results. One example is the model used in our laboratory and others; this model is described in detail. Eventually, larger animal models may better translate to the human condition but, given increased financial costs and animal facility capability, should be explored when data obtained from small animal studies is exhausted or inconclusive.

Learning Outcomes and Teaching Methods in Fundamentals of Nursing (기본간호학 학습성과와 교육방법)

  • Purpose: The purpose of this study was to provide a basis for improvement and enhancement of nursing education by investigating learning outcomes that apply to fundamentals of nursing and teaching methods used in classes. Methods: Data were collected from 111 professors of fundamentals of nursing who responded to the self-report questionnaire. Data were analyzed using chi-square test. Results: For learning outcomes in fundamentals of nursing the most frequent number of outcomes was two (35.2%), or three (32.4%). For learning outcomes in fundamentals of nursing practicum, the most frequent number of outcomes was two (32.4%), or three (31.6%). In fundamental nursing classes, teaching methods used most frequently were lectures (98.2%) and videos (60.4%), and in practice classes, demonstration (98.2) and open laboratory (90.9%). Constructivist teaching methods that were utilized in fundamental nursing were team-based learning (19.8%) and case-based learning (19.8%), and for practice classes, objective structured clinical examination (29.7%). In the cross analysis, 28.8% of the nursing professor used the constructivist teaching methods in fundamental nursing and in practice classes. Conclusion: There is a need to continue to improve teaching methods for new nurse-educators and professors and to discuss learning outcomes of fundamental nursing.

Etiologies, Prognostic Factors, and Outcomes of Pediatric Acute Liver Failure in Thailand

  • Purpose: Pediatric acute liver failure (PALF) is a serious condition; however, data on PALF in developing countries are sparse, particularly concerning molecular diagnosis and liver transplantation (LT). This study aimed to determine the causes, outcomes, and prognostic factors of PALF. Methods: We retrospectively reviewed the medical records of children (age <15 years) with PALF diagnosed using the American Association for the Study of Liver Diseases criteria at our center from 2011 to 2016. The collected data included laboratory results, complications, outcomes, and potential factors associated with death and LT. Results: We included a total of 27 patients, with a median age of 2 years (interquartile range, 3 months to 4 years). Viral infection was the most common etiology (n=8, 30%), predominantly dengue infection (n=4). A total of 16 patients (59%) died and 11 patients survived (3 patients with LT). The prognostic factors associated with death or LT requirement were grade IV hepatic encephalopathy (p<0.01), hypotension (p=0.02), gastrointestinal bleeding (p=0.03), increased intracranial pressure (p=0.04), and higher peak serum lactate level (p=0.01). Peak serum lactate ≥6 mmoL/L had a sensitivity of 79% and a specificity of 88% for predicting mortality or the necessity of LT. Conclusion: Viral infection was the most common cause of PALF. The mortality rate remained high, and a considerable number of patients required LT. In addition to several clinical factors, peak serum lactate could be a potential marker for predicting poor outcomes in PALF.

Efficacy of Herbal Medicines for the Treatment of Psoriasis : Systematic Review and Meta-analysis (건선의 한약치료 효과에 대한 체계적 문헌 고찰과 메타 분석)

  • Objectives : This study aimed to validate the effect of herbal medicine intervention to relieve the symptoms of psoriasis using systematic review and meta-analysis and provide the newest reason of effectiveness of Korean medicine to psoriasis. Methods : Data were collected through electronic database including Pubmed, Cochrane CENTRAL, NDSL OASIS, Koreantk. Two experts assessed risk of bias of randomized controlled trials by Cochrane group's Risk of Bias tool after searching, reviewing and selecting papers. Data were analyzed using Review Manager(RevMan) 5.3 and Comprehensive Meta Analysis 2.0. Results : Total number of selected trials was 16 randomized controlled trials. This study evaluated the risk of bias and effectiveness of herbal medicine to psoriasis. There were high frequency uncertain in selection bias, performance bias and detection bias. In this meta-analysis, Korean medicine treatment was more effective than western medicine (ES:0.507, 95%CI:0.147-0.867) and placebo (ES:0.955, 95%CI:0.598-1.312). Conclusions : Herbal medicine intervention can be an effective for treatment in psoriasis. But enhancing levels of evidence, we must try to accumulate clinical researches of herbal medicine to psoriasis in Korea.
