목적: 본 연구에서는 CAD/CAM 기술로 제작한 4가지 금속하부구조물의 변연 및 내면 적합도를 비교하여 정확도 및 임상적 효용성을 알아보고자 한다. 재료 및 방법: 상악 중절치 레진모형치아를 삭제한 뒤 복제하여 Ni-Cr 합금 표준 모형을 제작하였다. 이를 공초점 현미경방식의 구강 스캐너를 이용해 12개의 STL 파일을 얻었다. CAD 프로그램 상에서
보훈병원 입원환자의 재원기간과 진료비분석을 위해 일개 보훈병원에서 2001년 1월부터 2003년 12월 31일까지의 입원환자를 대상으로 9,640명의 진료비 대장을 분석하여 다음과 같은 결과를 얻었다. 1. 조사대상자의 성별 분포는 남자 70.9%, 여자 29.1%로 남자의 비율이 여자보다 높았다. 연령별로는 70대이상 42.6%, 50-60대 31.8%, 20대이하 13.4%, 30-40대 12.1%로 나타났으며, 의료보장별로는 의료보험이 78.1%, 의료보호가 14.2%, 일반환자가 4.1%, 자보환자가 2.8%, 산재환자가 0.8%로 나타났다. 진료과목별 분포는 내과가 28.3%, 정형외과 21.3%, 외과 16.6%, 신경외과 7.1%, 소아과 5.9%로 나타났다. 보훈 대상자군의 성별분포는 남자 99.3%, 여자 0.7%이였으며, 연령별로는 70대 이상 51.6%, 50-60대 42.2%, 30-40대 4.7%. 20대이하 1.6%였으며, 지역별로 살펴보면 대전 거주자 1, 550명 43.5%, 충남 거주자 838명 23.5%, 충북 거주자 785명 22.0%순으로 나타났다. 2. 보훈병원 입원환자의 21대분류 상병분포는 손상, 중독 및 외인에 의한 특정기타 결과 17.1%, 소화기계의 질환 16.1%, 근골격계 및 결합조직의 질환 13.9%, 호흡기계 질환 9.4%, 비뇨 생식계의 질환 8.6%로 나타났다. 보훈대상자군을 21대 분류상병별로 보면 근골격계 및 결합조직의 질환 19.4%, 소화기계의 질환 16.8%, 손상, 중독 및 외인에 의한 특정 기타 결과 15.7%, 비뇨 생식계의 질환 9.7%, 순환기계 질환이 8.2%순으로 나타났다. 3. 평균 재원일수는 29.0일, 보훈대상자군 51.8일, 비대상자군은 15.7일이며, 총진료비는 평균 3,669,579원, 보훈대상자군 7,263,877원, 비대상자군 1,560,333원이다. 본인 및 보험자 부담비율은 55.2 : 44.8로 나타났고, 본인부담 비율은 보훈 대상자군의 경우 61.7%, 비대상군의 경우 33.0%였다. 4. 대분류 상병별 총진료비는 순환기계 질환 6,593,662원, 근골격계 및 결합조직의 질환 4,716,317원, 비뇨 생식계 질환 4,487,799원, 손상, 중독 및 외인에 의한 특정 기타 결과가 4,199618원이며, 항목별 진료비 구조는 입원료가 34.7%, 약제비 13.2%, 행위료 부분이 48.6%, 기타 3.4%로 나타났고, 항목별로는 입원료가 34.7%, 물리치료 및 처치료가 26.3%, 수술료 9.7%, 주사재료비 7.8%, 투약재료비 5.4%, 검사료 52%순으로 나타났다. 보훈대상자군의 경우 물리치료비 및 처치료 35.3%, 입원료 35.2%, 주사재료비 62%, 수술료 5.9%로 나타났으며, 비대상자군의 경우 입원료 35.7%, 수술료 16.4%, 주사재료비 11.4%, 검사료 8.3%로 나타났다. 5. 보훈대상자의 거주지와 병원간 거리별로 상병구조를 비교해 보았을 때 21.5Km내의 지역에서는 달리 분류되지 않은 증상, 징후와 임상 및 검사의 이상소견 56.0%, 손상, 중독 및 외인에 의한 특정 기타 결과 55.6%, 눈 및 눈 부속기의 질환 52.9%순으로 나타났고, 21.5km 밖의 지역에서는 신생물 57.4%, 근골격계 및 결합조직의 질환 55.9%, 비뇨생식계의 질환 53.5%순으로 나타났다. 결론적으로 보훈대상자의 70.6%가 60세 이상이고, 평균재원일수가 51.8일 점을 볼 때 보훈병원에 장기요양시설에 대한 대책이 절대적으로 필요하다는 것을 알 수 있으며, 총진료비가 높은 순환기 질환, 근골격계 질환, 신생물 등 만성질환 관리를 위한 노력이 필요하리라 생각된다. 상위 7개 질환군이 77.9%를 차지하고 있으므로 노인연령층에서 지속적으로 발생하고 있는 치사율이 높은 뇌혈관 및 심혈관의 순환기계 질환, 악성신생물, 그리고 불의의 사고를 주요 건강문제로 설정하여 뇌혈관 및 심혈관의 순환기계 질환은 적절한 신체적, 정신적, 사회적 활동유지를, 악성신생물의 경우는 만성질환 조기 발견 및 관리를, 불의의 사고와 관련해서는 장애 및 만성질환에 따른 불편의 최소화를 주요 목표로 하여 다양한 프로그램을 개발하여 시행해야 한다.
The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. 연구배경 : 소세포폐암은 전체 폐암의 약 20%를 차지하며, 비소세포암에 비해 예후가 불량한 것으로 알려져 있다. 하지만, 최근 치료 효과가 점차 좋아지면서 생존율이 향상되고 있으며, 환자들의 적절한 치료를 위해 소세포폐암 환자의 생존 기간과 연관된 예후인자를 조사하였다. 방법 : 1993년에서 2001년까지 고신대학교 복음병원에서 소세포폐암으로 진단 및 치료받은 입원 환자 중에서 추적 경과 관찰이 가능한 394명의 의무기록을 바탕으로 예후 인자들의 생존기간을 후향적조사 분석하였다. 결과 : 남자가 314명(79.7%), 여자 80명(20.3%), 제한기 177명(44.9%), 확장기가 217명(55.1%)이었다. 이들 중 사망이 확인된 경우는 366명이었으며 중앙 생존기간은 215일이었다. 예후인자 중에서 병기, 전신 활동도, 최근 3개월간의 5% 체중감소, 항암 치료 여부, 항암 화학 약제 종류, 부가적 방사선 치료여부가 생존 기간의 연장과 통계적으로 유의하였다. 치료 여부에 따른 분석에서 대중 요법만 받은 환자의 중앙 생존 기간은 71일, 한가지의 암 치료만 받은 경우는 211일, 두 가지 이상 항암 치료를 받은 경우는 419일로 치료에 따라서 의미 있게 생존 기간이 연장되었다(p<0.000 각각). 항암화학요법과 방사선 치료를 같이 시행받은 환자들 중에서 조기동시 항암 화학 방사선 치료 군 (30명)은 지연교대 항암 화학 방사선 치료 군 (55명)보다 중앙생존기간이 유의하게 길었다(528일 대 373일, p=0.0237). 검사실 지표로는 백혈구
The writers have conducted the investigation to assess the nutritional status of young rural Korean children aged from 0 to 4 years old in August 1971. The survey areas were Kaejong-myon. Daeya-myon, Okku-gun, Jeonra-bukdo, Korea. These survey areas were typical agricultural plain areas. The total numbers of children examined were 2,706 comprising 1,394 male and 1,312 female. The weight, height, and chest circumference of children were measured and means and standard deviations. were calculated for each measurement. In addition, the nutritional status of each child was classified by the four levels of malnutrition and the Gomez classification, The examination of red blood cell count, haematocrit value, and intestinal parasite infection were carried out at the same time. In general, recent work tend to suggest that environmental influences, especially nutrition, are of great importance than genetic background or other biological factors for physical growth and development. Certainly the physical dimensions of the body are much influenced by nutrition, particularly in the rapidly growing period of early childhood. Selected body measurements can therefore give valuable information concerning protein-calory malnutrition. Growth can also be affected by bacterial, viral, and parasitic infection. For the field workers in a developing country, therefore, nutritional anthropometry appears to be of greatest value in the assessment of growth failure and undernutrition, principally from lack of protein and calories. In order to compare and evaluate the data obtained, the optimal data of growth from the off-spring of the true well-fed, medically and socially protected are needed. So-called 'Standards' that have been compiled for preschool children in Korea, however, are based on measurement of children from middle or lower socio-economic groups, who are, in fact, usually undernourished from six months of age onwards and continuously exposed to a succession of infective and parasitic diseases. So that, the Harvard Standards which is one of the international reference standards was used as the reference standards in this study. Findings of the survey were as follows: A. Anthropometric data: 1) Comparing the mean values for body weight obtained with the Korean standard weight of the same age, the rural Korean children were slightly haevier than the Korean standard values in both sexes. Comparing with the Japanese children values, the rural Korean children were slightly haevier in male and in the infant period of female but lighter in female of the period of 1 to 4 years old than Japanese children. 2) Comparing the mean values for height obtained with the Korean standard height of the same age, the rural Korean children were taller than the Korean standard values except the second half of infatn period in both sexes. Comparing with the Japanese children, the rural Korean children were slightly smaller than Japanese children except the first half of infant peroid in both sexes. 3) Mear values of chest circumference of rural Korean children obtained were less than the Korean standard values of the same ages in both sexes. B. Prevalence of Protein-Calory Malnutrition: Children examined were devided into two groups, i. e., infant(up to the first birthday) and toddler (1 to 4 fears old). 1) Percentages of four levels of malnutrition: a) When the nutrtional status of each child was classified (1) by body weight value, the percentages for male and female of children attained standard growth were 52.8%(infant 83.3%, toddler 44.4%) and 39.7% (infant 74.5%. toddler 30.5%), the first level of malnutrition were 31.9%(infant 13.7%, toddler 36.9%) and 31.7%(infant 15.3%, toddler 36,0%), the second level of malnutrition were 12.3%(infant 1.7%, toddler 15.3%) and 23.3% (infant 7.7%, fodder 27.5%), the third level of malnutrition were 2.7%(infant 0.7%, toddler 3.2%) and 4.6%(infant 1.8%, toddler 5.3%) the fourth level of malnutrition were 0.3% (infant 0.7%, toddler 0.2%) and 0.7% (0.7% for infant and toddler) respectively. (2) by height value, the percentages for male and female of children attained standard growth were 80.3% (infant 97.3%, toddler 75.6%) and 75.1% (infant 96.4%, toddler 69.5), the first level of malnutrition were 17.9% (infant 2.0%, toddler 22.3%) and 23.6% (infants 3.6%, toddler 28.8%), the second level of malnutrition were 1.2% (infant 0.3%, toddler 1.5%) and 1.1% (infant 0%, toddler 1.4%), the third level of malnutrition were 0.4%(infant 0.3%, toddler 0.5%) and 0.2%(infant 0%, toddler 0.3%), the fourth level of malnutrition were 0.1%(infant 0%, toddler 0.1%) and 0% respectively. (3) by body weight in relation to height, the percentages for male and female of children attained standard growth were 87.9% (infant 77.6%, toddler 87.9%) and 78.2% (infant 77.4%, toddler 78.2%), the first level of malnutrition were 12.2% (infant 18.4%, toddler 10.6%) and 18.2% (infant 17.9%, toddler 18.3%), the second level of malnutrition were 1.9%(infant 3.3%, toddler 1.5%) and 3.0%(infant 3.3%, toddler 2.9%), the third level of malnutrition were 0.1%(infant 0%. toddler 0.1%) and 0.5% (infant 0%, toddler 0.6%), the fourth level of malnutrition were 0.1%(infant 0.7%, toddler 0%) and 0.3% (infant 1.5%, toddler 0%) respectively. b) When the nutritional status of each child according to the mother's age at perturition, i. e., young aged mother (up to 30 years old), middle aged mother (31 to 40 years old) and old aged mother (41 years or above) was classified (1) by body weight, among infants and toddlers, at each year of age, with increasing the mother's age, there was an increase in percentage of subjects underweight. This tendency of increasing percentage of underweight was more significant in the infant period than the toddler period. (2) by height value, no significant differences between each mother's age group were found. c) When the nutritional status of each child according to the birth rank, i. e., lower birth rank (first to third) and higher birth rank (fourth or above) was classified (1) by weight value, children of higher birth rank were slightly more often underweight than those of lower birth rank, but not significant. (2) by height value, no differences were found between children of lower and higher birth rank. 2) Gomez Classification: When the nutritional status of each child was classified a) by body weight value, the percentages for male and female of children. attained standard growth were 53.1% (infant 82.6%, toddler 44.9%) and 39.2% (infant 73.4%, toddler 30.1%), the first degree of malnutrition were 39.4% (infant 14.7%, toddler 46.2%) and 47.1% (infant 21.9%, toddler 53.8%), the second degree of malnutrition were 7.3%(infant 2.3%, toddler 8.6%) and 12.9% (infant 4.0%, toddler 15.2%). and the third degree of malnutrition were 0.2%. 서론: 최근 경제수준의 향상과 사회구조의 변화 등으로 신생활이 서구화되고 노령화 사회가 되어가면서 만성질환 및 성인병 등의 박병률이 급격히 증가하고 있다. 특히 당뇨병은 전 세계적으로 및 유병율이 계속 증가하고 있으면, 만성질환 중 주요사망 원인인 뇌혈관 질환과 심혈관계 질환의 중요한 위험요인으로 잘 알려져 있다. 그러나 당뇨병은 고혈당 자체보다는 여러 장기에 복합적으로 침범하는 합병증으로 인하여 환자자신은 물론 가족과 사회에 심각한 건강 문제와 사회, 경제적 문제를 초래하게 되는 질환이다. 그러므로 당뇨병의 치료에 있어 엄격한 혈당의 조절과 함께 정서적 스트레스에 대한 배려가 필요할 것이다. 이에 저자들은 당뇨병 환자의 과거 3개월간의 혈당조절의 지표로 알려진 당화혈색소를 기준으로 당뇨조절과 관련된 정신병리를 확인해보고자 하였다. 방법: 2004년 3월부터 2004년 8월까지 가톨릭대학교 성빈센트병원 내분비내과에서 치료 중인 환자 55명을 대상으로 하였으며, 이 중 당화혈색소가 7%이하인군을 혈당조절 양호군으로 하였다. 이들에 대하여 사회 인구학적인 변인에 대한 문항, Beck의 Beck Depression Inventory(BDI), Spielberger의 State-Trait Anxiety Inventory (STAI), Taylor의 Tronto Alexithymia Scale (TAS), 고경봉의 스트레스반응척도(SRI)를 사용하여, 혈당조절 양호군과 불량군간의 차이를 비교하였다. 결과: 1) 대상군 55명의 평균나이는
