Asymmetry in mesial root number and morphology in mandibular second molars: a case report

  • Ambiguity in the root morphology of the mandibular second molars is quite common. The most common root canal configuration is 2 roots and 3 canals, nonetheless other possibilities may still exist. The presence of accessory roots is an interesting example of anatomic root variation. While the presence of radix entomolaris or radix paramolaris is regarded as a typical clinical finding of a three-rooted mandibular second permanent molar, the occurrence of an additional mesial root is rather uncommon and represents a possibility of deviation from the regular norms. This case report describes successful endodontic management of a three-rooted mandibular second molar presenting with an unusual accessory mesial root, which was identified with the aid of multiangled radiographs and cone-beam computed tomography imaging. This article also discusses the prevalence, etiology, morphological variations, clinical approach to diagnosis, and significance of supernumerary roots in contemporary clinical dentistry.

  • Tram-track and doughnut-shaped enhancements of the optic nerve sheath in axial and coronal magnetic resonance imaging (MRI) views, respectively, play crucial roles in the diagnosis of optic nerve sheath meningioma (ONSM). However, this finding is not specific to ONSM since it can also be observed in optic perineuritis (OPN). Here we report a 42-year-old female with ONSM who presented with clinical and MRI findings similar to those of OPN.

  • In this issue of Kidney Research and Clinical Practice, Cabral et al. [1] further reported that 8-iso-PGF2α could override NO's natriuretic effects in the cTAL. This finding suggests that sodium retention may prevail over sodium excretion in the renal tubule in clinical conditions, particularly when they are associated with an increased 8-iso-PGF2α level and blunted NO production. Further studies are warranted to elucidate the effects of 8-iso-PGF2α on the phosphorylation and intracellular trafficking of NKCC2 and the expression of other ion transporters expressed in TAL and different renal tubular segments.

A STUDY ON THE DENTAL RADIOGRAPHIC MANIFESTATIONS OF END-STAGE RENAL DISEASE (말기신장질환에서의 치과방사선학적 양상에 관한 연구)

  • For the assessment of changes in the bone architecture of the maxilla and mandible in renal osteodystrophy, 31 chronic renal failure patients who were undergoing hemodialysis therapy were selected. They were evaluated through clinical oral examination, radiographic and biochemical examination. The results were as follows: 1. In 17 cases (54.8%), there were evidences of bony change in jawbone. 2. The most common dental radiographic finding was decreased bone density (14 cases, 45.2%). 3. The second most common dental radiographic finding was total or partial loss of lamina alveolar dura (11 cases, 35.5%). 4. The third most common dental radiographic finding was total or partial loss of inferior canal wall (8 cases, 25.8%). 5 cases showed evidences of bony change only in jawbone, and 5 cases only in hand, and 12 cases in both. 6. Serum creatinine, urea nitrogen and alkaline phosphatase values in hemodialysis group were much higher than in control group. 7. There were statistically significant correlation between bone density and lamina dura, and inferior alveolar canal wall.

Clinical Characteristics of Subacute Necrotizing Lymphadenitis in Pediatrics (소아에서의 아급성 괴사성 림프절염의 임상적 고찰)

  • Background:Subacute necrotizing lymphadenitis or Kikuchi's disease is unknown ethiology and self-limiting process. This disease predominantly affects young women age but rarely affects pediatrics, and usually manifests as lymphadenopathy and fever. Even though this disease is self-limited, benign process, many cases are misidentified as malignant lymphoma. The purpose of this study is to report the clinicopathologic finding, radiological finding and many labolatory test and to compare with characteristics of adult patients in this disease. Meterial and Methods:We reviewed 27 pediarics patients with subacute necrotizing lymphadenitis by excision biopsy or fineneedle aspiration cytology.Result:The most common symptomes were palpation of cervical lymh node(88.9%) and fever(66.7%). The common site of the involvement was cervical lymph node. The multiple involvement was 93% and bilateral involvement was 59%. Leukopenia(52%) and elevated erythrocyte sedimentation rates(93%) appeared in abnormal laboratory data. Microscopically, the characteristic finding was the wide area of florid nuclear dusts engulfed by histiocytes and well-circumscrbed area with eosinophilic fibrinoid material. There was a striking degree ofkaryorrhexis and an absence of granulocyte with paucity of plasma cell. All patients recovered with the conservative treatment and there was no specific complication and recurrence. Conclusion : We reviewed pediatric patients with this disease. Characteristics of this disease inpediatric patients were similar to adult patients.

A Comparison Study of Single with Double Intrauterine Insemination with Mild Ovarian Hyperstimulation for Infertility Patients (불임 환자의 치료에서 Mild Ovarian Hyperstimulation을 이용한 Single IUI와 Double IUI의 비교)

  • Objective: To compare the clinical efficacy of double intrauterine insemination with single intrauterine insemination in GnRH antagonist combined ovarian hyperstimulation (Mild ovarian hyperstimulation) Materials and Methods: From Jan. 2001 to Jul. 2004, a retrospective clinical analysis was done of a total of 295 cycles in 170 patients who underwent ovarian hyperstimulation for ART (assisted reproductive technique). Subjects were divided into three groups; only clomiphene citrate ovarian hyperstimulation (n=55, 95cycles), GnRH antagonist combined ovarian hyperstimulation (soft ovarian hyperstimulation) (n=66 99cycles), and GnRH agonist combined ovarian hyperstimulation (short protocol) (n=49, 101cycles) Each group were randomly devided into two subgroups. One group underwent single IUI and the other group underwent double IUI. Results: GnRH antagonist group and GnRH agonist group had similar pregnancy rate. In GnRH antagonist Group, pregnancy rate was 36.1% in single IUI subgroup and was 36.6% in double IUI subgroup. These finding were not statistically significant. And Pregnancy rate was 20.8% in single IUI subgroup and was 19.3% in double IUI subgroup in single clomiphene citrate group, and 36.3% in single IUI subgroup and was 33.3% in double IUI subgroup in GnRH agonist group. These finding were not statistically significant, too. Conclusion: Pregnancy rate of GnRH antagonist was high and complication rate such as OHSS and multiple pregnancy was lower. In GnRH antagonist group, to compare with single IUI and double IUI, the result do not statistically differ. So GnRH antagonist single injection with single IUI was relatively comparable ART in infertiliry patient.

Extraskeletal Osteosarcoma of the Mammary Gland in a Dog (개의 유선에 발생한 골격외 골육종 1예)

  • A 16-year-old female Poodle with multiple mammary masses was referred to the Veterinary Medical Teaching Hospital of Seoul National University. Radiographic finding indicated a round mass of soft tissue density at the ventral portion of the 4th-7th sternum, which was 7 cm in diameter and presented multiple mineralization within the mass. Cytologically, aspirate of the mammary mass was moderately cellular and revealed well-differentiated osteoblasts with moderately basophilic cytoplasm, various sized and eccentrically placed nuclei and distinct nucleoli. Osteoblasts interspersed with osteoid-suspected pink-staining intercellular matrix. Some cells displayed malignant features such as macronuclei, macronucleoli, binucleation, abnormal mitotic figures, anisocytosis and anisokaryosis. On which the mass was diagnosed as osteosarcoma. Histopathologic examinations of the mass were compatible with a diagnosis of osteosarcoma. The patient was treated with surgery alone. The patient died 45 days later from surgery.

Live birth in a woman with recurrent implantation failure and adenomyosis following transfer of refrozen-warmed embryos

  • The aim was to report a healthy live birth using re-vitrified-warmed cleavage-stage embryos derived from supernumerary warmed embryos after frozen embryo transfer (ET) in a patient with recurrent implantation failure (RIF). The case was a 39-year-old female with a history of polycystic ovarian syndrome and adenomyosis, along with RIF. After ovarian hyperstimulation, 33 cumulus-oocyte complexes were retrieved and fertilized with conventional in vitro fertilization and intracytoplasmic sperm injection. Because of the risk of ovarian hyperstimulation syndrome, 16 grade B and C embryos were vitrified. After 3 and 6 months, 3 and 4 B-C warmed embryos were transferred to the uterus, respectively. However, implantation did not take place. Ten months later, four embryos were warmed, two grade B 8-cell embryos were transferred, and two embryos were re-vitrified. One year later, the two re-vitrified cleavage-stage embryos were warmed, which resulted in a successful live birth. This finding showed that following first warming, it is feasible to refreeze supernumerary warmed embryos for subsequent ET in patients with a history of RIF.

  • The cytologic finding of proliferating trichilemmal tumor is not well documented and have difficulties in the cytological analysis of the fine needle aspirates. This rare dermatopathologic entity may be encountered during aspiration biopsy of subcutaneous masses, and is, occasionally, confused with other neoplasm, particulary squamous carcinoma owing to its close cytological resemblance. We report a case of proliferating trichilemmal tumor in the left breast. The patient is a woman and had a lump in the breast for 20 years. Fine needle aspiration revealed a few small clusters of squamoid cells in the background of amorphous and calcified material. After excisional biopsy, the mass was confirmed as proliferating trichilemmal tumor. The main cytologic features discriminating squamous cell carcinoma were bland nature of epithelial cells, absence of atypical dyskeratotic cells, and rich amount of amorphous material.

A Case of Pure Red Cell Aplasia (순수적혈구형성부전증 1 례)

  • Pure red cell aplasia is uncommon disorder characterized by finding of anemia, absence of nucleated red blood cell in the marrow, absence of reticulocytes in the peripheral blood and normal peripheral platelet and leukocytes counts. We experienced one case of pure red cell aplasia associated with hemolytic anemia characterized by hemoglobinuria, reticulocytopenia, and erythroid hypoplasia of the bone marrow. The cause of the illness was not definitely identified, but we concluded that this patient had simultaneous occurence of PRCA and hemolytic anemia following administration of diphenylhydan. toin after craniotomy rather than virus or bacteria induced. The simultaneous occurance of PRCA and hemolytic anemia is uncommon and the mechanism for diphenylhydantoin induced PRCA and hemolytic anemia is unclear.
