Effect of Field Size on the Clinical Exposure Index for Lumbar Spine X-ray Examination (노출지수를 이용한 요추 X선 촬영의 조사야 유효성 평가)

  • The field size of the lumbar spine X-ray examination, which belongs to the most frequent examination in general radiography, is 5 times wider than the width of the lumbar spine. Exposure index (EI) as per International Electrotechnical Commission has a proportional relationship with the dose incident on the image receptor for clinical protocols in addition to RQA5, which is a calibration beam quality. In this study, the effectiveness of the set field size was evaluated through the change of EI according to the size of field during lumbar spine X-ray examinations. Lumbar anterior-posterior and lateral examinations was performed using a whole-body phantom, and the national average exposure conditions of Korea investigated in 2017 were introduced for the X-ray exposure. As a result of comparing the EI displayed on the console of digital radiography system for the three field size in ① 18 × 36 cm2 ② 25 × 36 cm2 ③ 36 × 36 cm2, the EI values showed a tendency to increase as the field size increased. Since the patient dose, such as organ dose around the lumbar spine, increases as the field size becomes larger, thus, if the EI obtained from the field size at a level that does not interfere with diagnosis is set as a reference, the effectiveness of the field size can be evaluated through the EI displayed on the console when the lumbar spine X-ray examination is conducted.

  • Purpose: The objectives of this study is to collect the opinions of experts in the field of dental lab technology, and to present a plan for reforming the curriculum for producing clinical-based dental technicians. Methods: 71 experts were selected for this study. Experts were grouped into clinicians, educators, and policy-makers. First of all, the purpose and method of this study were explained to experts. After receiving the consent to participate in the research, their opinions were investigated. The survey was conducted through a questionnaire created based on their opinions and opinions collected. Results: The results showed that they were relatively satisfied with the curriculum. In addition, it was found that they hope to increase the proportion of mandatory education of digital dentistry and clinical practice. Emphasis was placed on expanding opportunities for on-the-job training as well as on time for hands-on training. As for admission capacity, the highest opinion was to keep it as it is, followed by the need for reduction. Conclusion: It seems necessary to minimize the difference between the curriculum and the clinical field in order to produce field-oriented dental technicians. To this end, it is necessary to reorganize the operation of field-oriented subjects and to increase the practice time to improve practical skills.

  • Botulinum toxin (BTX) has various therapeutic indications: bruxism, square jaw, facial wrinkle, oral ulcer and maxillofacial pain, etc. In this paper, we will discuss the effectiveness of using BTX in dental implant surgery and orthognathic and orthodontic treatment. We summarized the clinical application of botulinum toxin in the maxillofacial field at the finale.

Genomic polymorphism in clinical mycobacterial strains analyzed by pulsed-field gel electrophoresis

  • The Mycobacterium tuberculosis clinical isolates in Korea, showing different drug resistances, were analyzed by comparing large restriction fragment (LRF) patterns produced y digestion of genomic DNA with infrequent-cutting endonucleases of SpeI, AsnI and pulsed-field gel electrophoresis (PFGE). SpeI and AsnI allowed with AsnI and SpeI, strains yielded an absolutely identical pattern for Korean type's mycobacteria even though they showed different drug resisstance. However, when three M. tuberculosis strains, showing drug resistance, were digested with XbaI, patterns were different from those of the other M. tuberculosis strians which are susceptible to drugs. This stuyd reveals that the comparison of chromosomal restriction patterns is very useful as an additional aid for the differentiation and identification of M. tuberculosis strains showing drug resistances.

  • Compared to the present clinical field strengths, MR at 47 and above promises to improve anatomic imaging quality by factors, and to bring metabolic and functional imaging to the forefront of research and diagnostic modalities. While human bore sized magnets as high as 9.4T are now installed, realization of the potential benefit of these magnets will require more of the MR system than a simple field, frequency or power scaling from technologies used at lower fields. New constraints on the high field MR studies, both physical and physiological, will require new technical developments to be considered for the highest field systems.

Evaluation of Necessity of Clinical Nutrition Education in the Medical School Curriculum (의과대학 교육과정중 임상영양교육의 필요성 평가)

  • This study was conducted to evaluate the necessity of clinical nutrition education in the medical school curriculum. One hundred and eighty five medical school students, residents, nurses, and forty undergraduate students majoring in food science and nutrition were involved in this study. The survey included questions about nutritional knowledge, necessity of nutrition counseling, necessity of clinical nutrition and nutritional attitudes. The nutritional knowledge scores of the subjects in medical school and residents were significantly lower than those of the students majoring in food science and nutrition. Subjects of 91.3% strongly agreed to the necessity of nutrition counseling for patients ; 78.9% agreed that the present nutrition education in medical schools was inadequate. Most subjects (94.1%) agreed that clinical nutrition was an important field in the treatment of diseases. The medical profession is facing the challenges of the 1990s with the shift to outpatient care and preventative services. Clinical nutrition is an essential element in the process of curriculum change. The nutrition educator will be expected to take a leading role in integrating clinical nutrition into medical school curriculum.

Evaluation of Clinical Competence in Plastic Surgery using OSCE(Objective Structured Clinical Examination): 3-Year Experience (객관구조화진료시험을 이용한 성형외과학 진료능력평가: 3년간의 경험)

  • This is the evaluation report of clinical competence of undergraduate medical students in Plastic Surgery field using OSCE(Objective Structured Clinical Examination) executed in three years. OSCE comprises of assessment of subject, choice of clinical stations sampling, identification of components of clinical competence to be evaluated, the level of performance required, development of specification table, editing of OSCE presentation page and assessment of practicability and results exploitation. About fifty students were examined annually. Seven station stimuli with simulated patient participation were carried out. The mean OSCE score was $82.3{\pm}6.19$. The reliability of the total station was 0.72. The examination shows a positive response to the OSCE. Our experience shows OSCE's feasibility for Plastic Surgery during the initial course of education. Referring to our experiences, the Korean Society of Plastic and Reconstructive Surgeons (KPRS) applied the OSCE test for Korean Plastic Surgery Board Certification

  • Bioinformatics is a rapidly emerging field of biomedical research. A flood of large-scale genomic and postgenomic data means that many of the challenges in biomedical research are now challenges in computational sciences. Clinical informatics has long developed methodologies to improve biomedical research and clinical care by integrating experimental and clinical information systems. The informatics revolutions both in bioinformatics and clinical informatics will eventually change the current practice of medicine, including diagnostics, therapeutics, and prognostics. Postgenome informatics, powered by high throughput technologies and genomic-scale databases, is likely to transform our biomedical understanding forever much the same way that biochemistry did a generation ago. The paper describes how these technologies will impact biomedical research and clinical care, emphasizing recent advances in biochip-based functional genomics and proteomics. Basic data preprocessing with normalization, primary pattern analysis, and machine learning algorithms will be presented. Use of integrated biochip informatics technologies, text mining of factual and literature databases, and integrated management of biomolecular databases will be discussed. Each step will be given with real examples in the context of clinical relevance. Issues of linking molecular genotype and clinical phenotype information will be discussed.

Constructing a Standard Clinical Big Database for Kidney Cancer and Development of Machine Learning Based Treatment Decision Support Systems (신장암 표준임상빅데이터 구축 및 머신러닝 기반 치료결정지원시스템 개발)

  • Since renal cell carcinoma(RCC) has various examination and treatment methods according to clinical stage and histopathological characteristics, it is required to determine accurate and efficient treatment methods in the clinical field. However, the process of collecting and processing RCC medical data is difficult and complex, so there is currently no AI-based clinical decision support system for RCC treatments worldwide. In this study, we propose a clinical decision support system that helps clinicians decide on a precision treatment to each patient. RCC standard big database is built by collecting structured and unstructured data from the standard common data model and electronic medical information system. Based on this, various machine learning classification algorithms are applied to support a better clinical decision making.
