scale genomic and postgenomic data means that many of the challenges in biomedical research are now challenges in computational sciences and information technology. The informatics revolutions both in clinical informatics and bioinformatics will change the current paradigm of biomedical sciences and practice of clinical medicine, including diagnostics, therapeutics, and prognostics. Postgenome informatics, powered by high throughput technologies and genomic-scale databases, is likely to transform our biomedical understanding forever much the same way that biochemistry did a generation ago. In this talk, 1 will describe how these technologies will in pact biomedical research and clinical care, emphasizing recent advances in biochip-based functional genomics. Basic data preprocessing with normalization and filtering, primary pattern analysis, and machine teaming algorithms will be presented. Issues of integrated biochip informatics technologies including multivariate data projection, gene-metabolic pathway mapping, automated biomolecular annotation, text mining of factual and literature databases, and integrated management of biomolecular databases will be discussed. Each step will be given with real examples from ongoing research activities in the context of clinical relevance. Issues of linking molecular genotype and clinical phenotype information will be discussed.
Purpose: This study was done to identify the relationship among ego resilience, the problem solving process and clinical competency in nursing students. Methods: Data were collected by questionnaires from 265 nursing students in D city. Measures were ego resilience, problem solving process and clinical competency. Data was analyzed using frequency analysis, t-test, one-way ANOVA, person's correlation coefficients, and multiple regression analysis with SPSS/WIN 19.0 version. Result: The Ego resilience score of subjects was 2.80 out of a perfect score of 4, the problem solving process score of subjects was 3.12, and the clinical competency score of subjects was 3.49 out of a perfect score of 5. There was a significant positive correlation among ego resilience, problem solving process and clinical competency. The significant predictors of problem solving process and clinical competency were ego resilience. Conclusion: These findings indicate that there is a need to increase ego resilience to improve the problem solving process and clinical competency of nursing students. The results should be reflected in the development of effective curricula.
Purpose: In this study, behavioral styles of Dominance, Influence, Steadiness, and Conscientiousness (DISC) were examined and differences in job satisfaction and clinical competence among newly hired nurses were explored. Methods: For this explanatory correlational research, 176 newly hired clinical nurses were recruited from three university hospitals and one general hospital located in Seoul and Gyeonggi province. Data were collected in August, 2013 though self-report questionnaires. Results: The distribution of behavioral styles was 11.4%, 42%, 29% and 17.6% for Dominance, Influence, Steadiness, and Conscientiousness respectively. DISC behavioral styles were associated with sub-areas of job satisfaction such as professional position and doctor-nurse relationship. DISC behavioral styles were significantly associated with the total score for clinical competence and sub-areas of data collection, basic nursing skills, critical thinking, education and leadership, and attitudes toward professional development and practical skills. Multiple linear regression analysis showed that DISC behavioral styles predicted clinical competence (Adj. $R^2$=.14, F=9.42, p<.001). Conclusion: A focus on cultivating influential and steady behavioral styles among newly hired nurses can be helpful in improving job satisfaction and clinical competence. There is a need to improve interpersonal relationships through a deeper understanding of each person's behavioral style based on the analysis of DISC behavioral styles.
Background: Alzheimer's dementia is the most common dementia. However, recently, choline alfoscerate is prescribed for treating Alzheimer's dementia, although it is not a treatment for this disease. Purpose: To analyze the prescription patterns of choline alfoscerate as a dementia treatment for patients with Alzheimer's disease and to analyze, as well as the factors affecting choline alfoscerate prescription. Method: The 2016 HIRA-NPS data was used in this study. The code of Alzheimer's dementia is F00 in the ICD-10 disease classification code. We analyzed the demographic, clinical, and regional characteristics associated with donepezil, rivastigmine, galantamine, memantine, and choline alfoscerate prescriptions. All statistical and data analyse were conducted by SAS 9.4 and Excel. Results: For patients with Alzheimer's disease, choline alfoscerate was the second most prescribed after donepezil. Analysis results showed that choline alfoscerate was more likely to be prescribed to men than to women, and more likely to be prescribed by local health centers than by medical institutions. Moreover, choline alfoscerate was highly likely to be prescribed at neurosurgical departments, among medical departments. Conclusions: This study confirmed that choline alfoscerate was prescribed considerably for patients with Alzheimer's dementia. Further studies valuating its clinical validity should be performed to clarify whether choline alfoscerate prescription is appropriate for treating Alzheimer's dementia.
Purpose: The purpose of this study are to understand the meaning and essential structure of clinical nurses' experiences of verbal violence and find nursing intervention for them by applying phenomenological research method. Methods: The participants in this study were seven clinical nurses with experience of verbal violence. Data collection were conducted from August 2016 through January 2017 until data saturation. Data analysis was performed through a cyclical process of data collection and analysis by applying Giorgi's phenomenological method. Results: Four categories were identified: A weapon of the mind: the shock of verbal abuse; negative emotions that shake my heart; skepticism about the value and meaning of a job; an unforgettable wound. Conclusions: The results of this study provide a deeper understanding of the pain caused by verbal violence experienced by nurses from their care recipients. These results suggest that nursing interventions for nurses with experience of verbal violence will contribute to resolving the problem.
Background: The present study aimed to provide basic data to establish a foundation for efficient operation of the organization by identifying the effects of factors related to moral sensitivity and psychological well-being on job engagement of clinical dental hygienists. Methods: This cross-sectional survey consisted of self-administration questionnaires. After the approval by the Institutional Review Board, a survey was conducted from September 2019 to November 2019, targeting dental hygienists with clinical experience of 1 year or more. A total of 189 questionnaires were analyzed using SPSS 18.0 for Windows. The data were analyzed using average standard deviation, t-test, one-way analysis of variance, Scheffe test, Pearson's correlation analysis, and multiple regression analysis. Results: According to job-related characteristics, higher job engagement was associated with more amount of clinical experience, less turnover intention, higher positions, and occasional job fatigue. Job engagement was improved by increasing moral sensitivity and psychological well-being of clinical dental hygienists. The factors affecting job engagement revealed that along with clinical career, the sub-areas of job engagement, namely turnover intention, moral sensitivity, and psychological well-being had statistically significant influence on job engagement and the explanatory power of the model was approximately 44.8%. Conclusion: Joy of working as a clinical dental hygienist, clinical career, turnover intention, moral sensitivity, and psychological well-being were confirmed as influencing factors that affected job engagement of clinical dental hygienists. It is necessary to establish an effective plan to improve job engagement by increasing moral sensitivity and psychological well-being of clinical dental hygienists.
Bioinformatics is a rapidly emerging field of biomedical research. A flood of large-scale genomic and postgenomic data means that many of the challenges in biomedical research are now challenges in computational sciences. Background: Cancer survival analysis is an essential indicator for effective early detection and improvements in cancer treatment. This study was undertaken to document colorectal cancer survival and associated prognostic factors in Malaysians. Materials and Methods: All data were retrieved from the National Cancer Patient Registry-Colorectal Cancer. Only cases with confirmed diagnosis through histology between the year 2008 and 2009 were included. Retrieved data include socio-demographic information, pathological features and treatment received. Survival curves were plotted using the Kaplan-Meier method. Univariate analysis of all variables was then made using the Log-rank test. All significant factors that influenced survival of patients were further analysed in a multivariate analysis using Cox' regression. Results: Total of 1,214 patients were included in the study. The overall 3- and 5-year survival rates were 59.1% and 48.7%, respectively. Patients with localized tumours had better prognosis compared to those with advanced stage cancer. In univariate analysis, staging at diagnosis (p<0.001), primary tumour size (p<0.001), involvement of lymph nodes (p<0.001) and treatment modalities (p=0.001) were found to be predictors of survival. None of the socio-demographic characteristics were found to exert any influence. In Cox regression analysis, staging at diagnosis (p<0.001), primary tumour size (p<0.001), involvement of lymph nodes (p<0.001) and treatment modalities (p<0.001) were determined as independent prognostic factors of survival after adjusted for age, gender and ethnicity. Conclusions: The overall survival rate for colorectal cancer patients in Malaysia is similar to those in other Asian countries, with staging at diagnosis, primary tumor size, involvement of lymph node and treatment modalities having significant effects. More efforts are needed to improve national survival rates in future.
