Clinical study on Chiropract-Drug treatment, Yongkakkyo-tang which is used for the treatment of arthrosis, vertebra disease (척추 관절 질환 치료에 응용되는 추나.약물요법 중 용각교탕을 중심으로 한 임상적 고찰)

  • The clinical studies were performed on 301 cases who took Yongkakkyo-tang from June 1993 to December 1994 The results were as follows: 1. About 80%(239 cases) patients who took Yongkakkyo-tang were improved. 2. The ratio of male to female patients was 106:133. In the age distribution, it was found that under 20's were 8%(24/301), 30's and 40's were 33%(98/301), and over 50's was 39%(117/301). 3. In the regional distribution, it was found that Cervical region was 22 cases, thoracic region was 12 cases, cervical & lumbar region complex was 50 cases, upper limbs region was 6 cases, lower limbs region was 12 cases, and lumbar region was 153 cases(64%). 4. Among improved cases, the cases treated only with Yongkakkyo-tang were 16(5%), the cases treated with Yongkakkyo-tang, and treated with Chiropractic were 19(6%), the cases treated with Yongkakkyo-tang and Yanggun-tang chiropratic were 133(44%). 5. Among improved cases, the number of Chiropractic treatment, less then 15 teimes were 69 cases, 15 to 30 times were 91cases. Basedon these results, it was shown that Yongkakkyo-tang could be used for the treatment of degenerative disease of vertebra, and the treatment with Yongkakkyo-tang, Yanggun-tang and Chiropratic is more effective.

Perforated Choledochal Cyst: Its Clinical Implications in Pediatric Patient

  • Purpose: Perforation of choledochal cyst (CC) is a relatively rare clinical presentation in pediatric populations and difficult to predict preoperatively. We assess the clinical implications by comparing clinical parameters based on a single-center experience between perforated and nonperforated CC to facilitate the appropriate management for future interventions. Methods: A total of 92 cases of CC in pediatric patients (aged <18 years) who received surgical management between January 2003 and December 2018 at a Pusan National University Children's Hospital were reviewed. After screening the clinical features of perforated cases, we compared the demographic findings, clinical characteristics, and some laboratory results between the perforated and nonperforated groups. Results: Perforated CC was identified in 8 patients (8.7%), and nonperforated CC in 84 patients (91.3%). Perforation can be classified into three categories: free perforation of cyst (3 cases), pinpoint perforation of cyst (2 cases), and necrotic change of cyst (3 cases). CC perforation occurred significantly more commonly in patients aged <24 months. Clinically, the perforated group showed significantly higher frequency of fever and higher C-reactive protein (CRP) level during the initial visit. Conclusion: Perforation is more likely to be suspected in patients aged <24 months presenting together with fever and high CRP level in the initial visit. It is also necessary to keep in mind that it indicates not only a possibility of complicated disease status regardless of its association with stones but also a difficulty of applying a minimal invasive procedure and relatively increased length of hospital stay.

Current Status of Biological Nursing Science Education for Clinical Nurses in General Hospital (종합병원 임상간호사 실무교육에서의 기초간호학 교육현황)

  • Purpose: The purpose of this study was to investigate the extent and the nature of biological nursing science education for clinical nurses in general hospital. Methods: Five advanced general hospitals located in Seoul and Gyeonggi province were conveniently sampled. Contents of education for nurses conducted by the hospital nursing department from January 1 to December 31 of 2015 were collected. Contents of education included biological nursing science and the scope of inclusion and time of assignment were analyzed. Results: A total of 271 cases of nursing education data were collected and 223 cases were analyzed after excluding 48 cases whose contents were not confirmed. Biological nursing science was included in the contents of education for 117 cases (52.5%), but not for 106 cases (47.5%). Regarding the frequency of the biological nursing science education contents, 'pathophysiology'was the most frequently included (n= 286), followed by 'structure and function of the human body' (n= 191), 'mechanisms and effects of drugs' (n= 114) and 'clinical microbiology' (n= 43). Conclusion: Results of this study confirmed that the education for clinical nurses included a lot of biological nursing science related contents. These results can be used as basis for the development of curriculum and training course for nurses.

Clinical Durability and Deflation of Saline-Filled Breast Implant in Breast Reconstruction (유방재건술에 사용된 식염수 보형물의 임상적 내구성과 누출)

  • Purpose: Despite wide clinical use of breast implants, there is continued concern about the lifespan of these devices. The causes of explantation were infection, deflation of implant and patient's want. The deflation of saline-filled breast implant was related to strength and durability of implant shell. The purpose of this study is to evaluate the clinical durability of saline-filled breast implant through the analysis of duration until deflation occurred, causes, incidence and influencing factors. Methods: Retrospective analyses were conducted on clinical records for 19 cases of deflation of saline-filled breast implant from 201 cases of breast reconstruction with saline-filled implant between May 1995 and June 2011. The authors had been analyzed the causes of deflation, survival duration, symptom, sign, nipple excision, volume of implant, saline filling, method of reoperation, breast cancer stage and combined capsular contracture. Results: The causes of deflation were attributed to the cases that cannot be evaluated the causes in 15 cases, fall down in 1 case, mammography in 2 cases, accidental needle injury in 1 case. Mean survival duration was 4 years and 5 months. The duration of survival was less than 1 year for 5 cases, 1 year to 10 years for 10 cases, more than 10 years for 4 cases. The volume between 201 and 250 cc of deflated breast implant was rated as high by 14.0 percent. The deflation rate of underfilled implants was 11.4 percent, adequate filled implants was 9.3 percent. None of overfilled implant was deflated. The deflation of smooth surface implant was 5 of 152 cases. Textured implant was 14 of 49 cases. The capsular contracture of non-deflated breast implant was 28 of 182 cases and that of deflated breast implant was 6 of 19 cases. Conclusion: The patients who underwent saline-filled breast implant implantation should be informed that their implant could deflate. The analysis of clinical durability and causes of deflation in breast implant was important for the prediction and prevention of reopeation. The authors could suppose the causes of deflation of saline-filled breast implant through history, duration of survival, inspection of the shell of implant.

Clinical Analysis of Spinal Cord Tumor (척수종양의 임상적 분석)

  • Objective : This study was undertaken to evaluate operative results and prognosis according to preoperative clinical status and histopathological finding of spinal cord tumor. Methods : We analyzed of clinical feature, tumor location, histopathologic finding, operative results and prognosis in 55 patients with spinal cord tumor during last 10 years. Results : 1) The incidence of spinal cord tumors varies with the age of affected patients who are 2 to 75 years of age. Peak incidences were in the 5th & 7th decade of life, and the ratio of male to female was 1.2:1. 2) The most common histopathologic type was neurinoma(41.9%). 3) The tumors were located most frequently in the thoracic area(22 cases, 40.0%) and in the intradural extramedullary space(30 cases, 54.5%). 4) The most common initial clinical feature was pain in 20 cases(36.4%). For neurologic status on admission, 30 cases(54.5%) showed motor disturbance. 5) In radiologic studies, there were abnormal finding in 21 cases from plain X-rays among 37 cases. The entire 20 cases in when myelography was done showed subarachnoid blockade, either complete or incomplete. The magnetic resonance imaging, regard as the most accurate diagnostic method, revealed the exact location of the tumor and the relationship of the tumor with the adjacent anatomical structure. 6) The total removal was possible in 36 cases(65.5%), subtotal removal in 17 cases(30.9%) and biopsy in 2 cases (3.6%). Nineteen cases(90.5%) among 21 cases with preoperative radiculopathy group showed recovery or improvement, where as only 11 cases(36.7%) among 30 cases with preoperative motor weakness group showed recovery or improvement, with statistically significant difference(p<0.01). Pathologically, 26 cases(83.9%) among 31 cases of neurinoma and meningioma showed postoperative recovery or improved, but only 1 case(6.3%) among 16 cases of metastatic tumor, astrocytoma and ependymoma recovered. Postoperative complication noted in 5 cases(9.1%), and were noted postoperative hematoma, pneumonia, pulmonary edema and spinal cord infarction. Conclusion : Preoperative neurologic status and histopathologic finding are considered important factors of Postoperative outcome in patients with spinal cord tumor.

  • During the last 10 years 8 months, clinical observation has been done on 43 cases of cryptorchisms. 1. The cryptorchism has relatively high incidence rate among the anomalies of genitourinary tract(27.2%). 2. The age group, most frequently seen, was between 6 to 10 and the average age visited the hospital at the first time is 11.4 years. It means 4 to 6 years later than age for the adequate treatment required. 3. Atrophy of the seminiferous tubules, interstitial fibrosis and poor or absent spermatogenesis were noted on the 8 cases of cryptorchid biopsies. 4. Hormonal therapy(Puberogen) was done on 16 cases and 8 cases were responded. But there was no complete descent of testis through this hormonal therapy.

Research about application of Shanghanlun disease pattern identification diagnostic system to 2 cases treated by Oreong-san (오령산(五苓散)을 활용한 2개 증례 분석을 통한 『상한론(傷寒論)』 변병진단체계(辨病診斷體系) 응용에 관한 연구)

  • Objective : The objective of this study is to define the clinical meaning of Shanghanlun sentence by applying Shanghanlun disease pattern identification diagnostic system. Methods : 2 clinical cases, which were treated by Oreong-san according to Shanghanlun provisions dualized with Je-Gang(提綱) and Jo-Moon(條文), were analyzed. Results : According to the results of analysis of 2 cases, the patients' diseases were improved. The pain was decreased, and general weakness and anorexia were improved in 2 cases. Conclusions : These results suggests that the medicines from Shanghanlun are clinically effective when Shanghanlun is interpreted considering the origins of Chinese characters in the text, and Shanghanlun disease pattern identification diagnostic system is applied.

Polyposis of gastrointestinal tract after COVID-19 mRNA vaccination: a report of two cases

  • Cronkhite-Canada syndrome is a rare gastrointestinal polyposis syndrome with distinctive clinical features and endoscopic findings. Diagnosis can be challenging without suspicion, and the disease carries high mortality due to complications such as infection, gastrointestinal bleeding, and malignancies. This paper presents two cases of Cronkhite-Canada syndrome occurring after coronavirus disease 2019 (COVID-19) mRNA vaccination. Both cases exhibited typical clinical findings, including hypogeusia, onychodystrophy, alopecia, and weight loss. Typical polyposis in the gastrointestinal tract was confirmed through endoscopies. As symptomatic treatment did not improve the symptoms, corticosteroids were administered, and symptoms and laboratory test results improved immediately. The patients improved upon corticosteroids tapering. These cases illustrate typical presentations of Cronkhite-Canada syndrome and the course of the disease following corticosteroid treatment. Additionally, they suggest the possibility that Cronkhite-Canada syndrome may be triggered by COVID-19 mRNA vaccination.

Clinical Ewperiences of Rib Fixation Using Judet's Strut (Judet's strut를 사용한 늑골 고정술의 임상적 고찰)

  • For the purpose of evaluation of clinical characteristics in multiple rib fracture due to accident , 24 cases treated by surgical rib fixation using Judet`s strut for multiple rib fracture and flail chestduring the period from June 1993 to October 1994 were reviewed. There were 17 males and 7 females.They ranged in age from 19 years old to 56 years old. The causes of rib fracture were traffic accident in 18 cases, fall down in 3 cases, compression in 2 cases stab wound in 1 case. The number of rib fracture were five in 7 cases, six in 5 cases, four in 5 cases, three in 3 cases. Associated intrathoracic injuries were hemopneumothorax in 12 cases, hemothorax in 10 cases, lung laceration or hemorrhagic contusion in 7 cases. Associated extrathoracic injuries were abdominal injuries in 21 cases, orthopedic problem in 7 cases, head trauma in 4 cases. The most common fractured site was posterolateral portion of the ribs. The causes for operation were flail chest, severe rib displacement and pain, hemothorax or hemopneumothorax with continuous air leakage and stab wound. There were 6 postoperative complications ; one with hydrothorax, two with fibrothorax, two with wound infection and one case of death due to multiful organ failure. Postoperatively, all patients became comfortable and complained less painful. Twenty patients restored spontaneous breathing without ventilator support, three patients were ventilated during a day and one patient expired after 2 days. There were no morbidity and mortality related to operation.
