Controlled Inquiry Rates of Clinical Interviews in Telehomecare

  • Conceived to acquire personal information for an electronic medical record, the clinical interview contains probing questions. The number and type of inquiries are assumed to fulfill medical protocols, and therefore are deemed essential for treatment - but the rate can and should be controlled. High rates of inquiry merely intimidate the patient and affect replies. The purpose of this paper is to mathematically formulate permissible rates of clinical interviews held during telehomecare virtual visits and designed to avoid patient anxiety. Mental stress is derived as a function of the weight of importance assigned by the patient, virtual visit duration, and the rate of questioning in the direction of greater sensitivity. Two operations are of interest: Collecting and recording information by the provider, and maintaining synchrony of questions and answers by the patient. The Lorentz transformation yields the patient’s view of the operational rates. Conservation of information momentum is postulated and applied before and after replies are recorded. It is shown that the weight of importance designated by the patient to collecting and recording personal information is driven by a singularity that depends on the rate of questioning. The findings should serve as a guideline in interviewer training programs.

  • 의료 환경이 점차 변화함에 따라 효율적인 건강관리와 의료사고를 룰이기 위한 새로운 병원정보시스템의 개발이 활발히 이루어지고 있다. 이에 본 논문에서는 지속적인 건강관리와 의료정보 공유를 위한 외래환자 Clinical Summary 시스템을 제안하였다. 환자진료에 필요한 core data set을 XML기반의 구조적 문서로 정의하고 이를 웹 서비스(Web Services)를 통해 여러 의료기관에서 활용할 수 있도록 함으로써 진료의 계속성을 높일 수 있도록 설계하였다.

  • This study proposes the actualization of a standard data model for activities through the development of clinical document architecture for medication administration using the health level 7 development frameworks(HDF) process based on object oriented analysis and development method of health level 7 V 3. Medication administration is the most common activity performed by clinical professionals at healthcare settings. A standardized information model and structured hospital information system are necessary to achieve evidence-based clinical activities. We had used HDF and various tools(Rose tree, RMIM designer, V3 generator) to create the clinical document architecture(CDA). This allowed us to illustrate each step of the HDF in the administration of medication. This study generated a information model of the medication administration process, which is one clinical activity. It should become a fundamental conceptual model for understanding international standard methodology by information technology(IT) developers with the objective of modeling healthcare information systems.

  • The development of medical devices based on the theory of tranditional Korean medicine is increasing due to population aging and emerging demand for medical services. Various clinical trials are being conducted to ensure the clinical effectiveness of the developed medical devices, and the importance of systematic management of variously collected information in this process is increasing. In this paper, we designed and constructed the database that can systematically manage the medical information of the patient and the pulse measurement information in conducting clinical trials by using pulse analysis system which is a representative diagnostic method of traditional Korean medicine. The constructed database can be efficiently used for the verification of existing algorithms or developing new algorithms through the quality control of various information and measured data on clinical trials. It also has several advantages in the control of clinical data. The results of this study can contribute to the establishment of Korean medical device data standard for the construction of integrated database management system that can share information with different kind of pulse analysis system and other bio-signal measuring devices.

  • Since its first release in 2007, the National Institute of Food and Drug Safety Evaluation (NIFDS) has provided pharmacogenomic and comparative information specific to Koreans to allow regulatory reviewers and researchers to adapt their working practices to pharmacogenomics. The highlights of this year's additions include "Drug Information", "Gene Information" and "Pharmacogenomic information in the drug labels" sections. These new additions provide information on 737 genes, 719 drugs and pharmacogenomic data of the labels or relabels of 253 approved drugs as of November 2008. The latest version of the Korean Pharmacogenomic Database (KPD, release 2.0) has expanded significantly since its previous release. More SNP and haplotype information has been added to the database with the latest version of the KPD containing approximately four times as many SNPs and haplotypes than the previous version (719 vs. 152, and 30 vs. 7 respectively). Through the "SNP" and "Haplotype" sections, the KPD provides unique Korean SNP and haplotype information as well as comparative information of other populations (Japanese, Chinese, European, African) to offer a range of pharmacogenomic data that can help reviewers and the public understand pharmacogenomic information.   • Purpose: This study sought to analyze perceptions and attitudes regarding patient personal information protection according to the general characteristics of paramedic students and their clinical practice experience. Methods: A total of 215 paramedic students from G university in I metropolitan city and D university in G do were surveyed. Frequency analysis, percentage analysis, and one-way distribution analysis were conducted using the software SPSS version 23.0. Results: First, recognition regarding the operation of laws and systems related to medical law and personal information protection was high among students who had no clinical practice experience, and there were no differences with regard to students' general characteristics. Second, the level of recognition regarding patient personal information protection and exposure did not differ depending on students' general characteristics and clinical practice experience. Conclusion: Based on the results of this study, students' recognition and attitude should be improved by carrying out continuous education on patient personal information protection. Furthermore, more specialized and systematic training related to patient personal information protection should be conducted to nurture appropriately trained paramedic students.

Convergence Study of Influence of Nursing Information Literacy Competency on Problem Solving Ability, Self-Directed Learning Ability and Clinical Performance Ability in Nursing Students (간호대학생의 간호정보활용역량이 문제해결능력, 자기주도적 학습능력 및 임상수행능력에 미치는 영향에 대한 융합적 연구)

  • This study was done to develop and validate a path model for the causal relationship of nursing information literacy competency and problem solving, self-direct learning, and clinical performance ability of nursing students. Subject were 208 nursing students in the fourth grade of nursing school. Data were collected from June 4 to June 18, 2018. The nursing information literacy competency of nursing students showed direct effects on problem solving ability (${\gamma}=.70$, p<.001) and direct effects on self-directed learning ability (${\gamma}=.53$, p<.001). Nursing information literacy competency showed direct (${\gamma}=.31$, p<.001) and indirect effects (${\gamma}=.31$, p=.007) on clinical performance ability. These variables explained for 46.2% of clinical performance ability. Therefore, it is recommended to develop an education program that can enhance the nursing information literacy competency for improving the problem solving, self-directed learning, and clinical performance ability of nursing students.

  • We have developed MITS(Medical Information Transmission System) that captures, stores and manages the digitalized medical images and transmit them via LAN, and also have developed DSCW(Dedicated System for Clinical Ward) that enables to review the medical images and clinical laboratory test results transmitted via LAN at clinical ward. They were implemented on the 386 platforms, and interconnected via 10 Mbps LAN between department of radiology and clinical ward.

Exploring the Use of Information Technology in Dietetics Practice among Clinical Dietitians

  • This exploratory study investigated the software programs utilized by clinical dietitians and their perceptions on the use of information technology. The study focused on the use of specialized software for dietetics, general software for any office use and the Internet. A self-administered survey was used to collect data from clinical dietitians identified in the Kentucky Dietetic Association Directory 2003-2004. The survey was conducted in March through April, 2004. A total of 22 responses were collected, which yielded a response rate of $68\%$. Findings suggest that computer use has become a routine for optimal clinical practice ; major specialized software programs are used for nutrition assessment, nutrition analysis and menu development. Use of specialized software appears to be unassociated with dietitians' age and years of experience ; however, the associations are indicated between use of specialized software and education level as well as their specialty ; word processing and spreadsheet are the major general software used. Internet is used mainly for information search and communication. Respondents perceived that overall work efficiency and productivity significantly increased with computer use. The complexity of software is the major barrier encountered so training is the most needed support by the department. Strategic promotion on the use of specialized software should be targeted to the management team of the hospitals/facilities.

Comparison among Algorithms for Decision Tree based on Sasang Constitutional Clinical Data (사상체질 임상자료 기반 의사결정나무 생성 알고리즘 비교)

  • Objectives : In the clinical field, it is important to understand the factors that have effects on a certain disease or symptom. For this, many researchers apply Data Mining method to the clinical data that they have collected. One of the efficient methods for Data Mining is decision tree induction. Many researchers have studied to find the best split criteria of decision tree; however, various split criteria coexist. Methods : In this paper, we applied several split criteria(Information Gain, Gini Index, Chi-Square) to Sasang constitutional clinical information and compared each decision tree in order to find optimal split criteria. Results & Conclusion : We found BMI and body measurement factors are important factors to Sasang constitution by analyzing produced decision trees with different split measures. And the decision tree using information gain had the highest accuracy. However, the decision tree that produced highest accuracy is changed depending on given data. So, researcher have to try to find proper split criteria for given data by understanding attribute of the given data.
