간호대학생의 임상실습 스트레스와 간호전문직관과의 관계에서 정서조절력의 효과 (The effects of emotional regulation between clinical practice stress and nursing professionalism in nursing students)

  • 본 연구는 간호대학생의 간호전문직관에 영향을 미치는 요인과 임상실습 스트레스가 간호전문직관에 영향을 미치는데 정서조절력이 조절변수로서 효과를 가지는지 규명하고자 실시하였다. 간호대학생 192명의 자료를 사용하였고, 2015년 9월부터 11월까지 자료를 수집하였다. SPSS 21.0 프로그램을 활용하여 기술통계분석, t-test, ANOVA, 상관분석, 위계적 회귀분석을 하였다. 간호전문직관은 임상실습 스트레스와 음의 상관관계를 (r=-.40, p<.001), 정서조절력과는 양의 상관관계 (r=.55, p <.001)가 있었다. 그리고 정서조절력은 임상실습 스트레스 (r=-.20, p <.001)와 음의 상관관계가 있었다. 간호전문직관에 교수지지, 전공만족도, 임상실습 스트레스, 정서조절력이 유의한 영향을 미치는 요인으로 나타났고, 정서조절력은 임상실습 스트레스와 간호전문직관의 관계에서 조절효과는 없는 것으로 나타났다. 본 연구 결과는 간호대학생의 간호전문직관을 높이기 위해 지지체계 구축과 임상실습 스트레스 관리와 정서조절력 훈련 개발의 중요함을 보여준다.

한국과 영국의 건강불평등 국가 데이터베이스 비교: 중독 물질 오남용 관련 정신건강 및 호흡기 질환을 중심으로 (Comparison of National Database of Health Inequality between Korea and the UK: Focusing on Substance Misuse Related Mental Health and Respiratory Diseases)

  • Objective: Taking action on health inequalities starts with the production of information laying out the problems of inequalities, but Korean society has no national database to view related data at a glance. This study aimed to compare Korean national database with the Public Health Profile (PHP), a health inequality database of the UK. Methods: Data were collected from the websites of government and relevant organizations in the both countries between March and August 2020, which was updated in August 2021. Two themes including Co-occurring substance misuse and mental health issues in mental health and INteractive Health Atlas of Lung conditions in England were selected for comparison in terms of data accessibility, data usability and data visualization. Results: The British PHP is being served on a web-based platform, Fingertips. The data collected at the regional level were presented on 31 health inequality themes. The data are displayed at a level that can be compared between comparable communities, and visualized into various tables and figures. Comparable Korean data were scattered in several themes and websites, and mostly provided as a 17 administrative region base, which was too vast to make a meaningful comparisons. Conclusion: The findings proposed several considerations which could be useful for establishing a database of health inequality in the Korean society.

임상적 비판적 사고능력 검사도구 개발 및 평가 (Development and Validation of a Clinical Critical Thinking Skills Scale)

  • This study developed a test entitled Clinical Critical Thinking Skills (CCTS) by using item response theory (IRT) and examined its validity. In the development stage, data obtained from a convenience sample of 627 undergraduate nursing students were analyzed using a discrimination and difficulty index with the TestAn 1.0 program. In the evaluation stage, data from a convenience sample of 284 nursing and non-nursing students were analyzed using a discrimination and difficulty index with the BILOG-MG program. Criterion validity was verified by the group comparison method. Five items received low discrimination index scores according to the IRT results. In terms of the criterion validity, CCTS scores differed according to major (t=2.21, p=0.028), location of high school (F=4.35, p=0.014), clinical experience (t=5.66, p=0.004), and grade point average (F=7.17, p< 0.001). The CCTS can be used to measure critical thinking skills in the clinical setting.

임상간호사의 감정노동 경험 (Clinical Nurses' Experience of Emotional Labor)

  • Purpose: The purpose of this study was to understand the experience of emotional labor of clinical nurses in medical institutes. Methods: A total of 26 nurses from 11 hospitals participated in the study. Six focus groups were organized and 4 to 5 nurses took part in each group. The compositional factors of groups included clinical experience, age, gender, work place and position. Data collection was conducted through focus group interview and it was proceeded by the time of data saturation. In this qualitative study, content analysis was conducted. Results: Five themes, 14 categories, and 33 subcategories, were emerged. The themes were 'Restrain themselves', 'Communion to the patients', 'Working environment provoking emotional tension', 'Respond to emotional events', 'Recovery of emotional energy'. Conclusion: Results indicated that surface acting of emotional labor such as, repression of personal desire and presenting the emotions that the organization ask nurse to express was related to psychosomatic symptoms, depression, burnout, poor job performance, increased mistakes, and low job satisfaction which eventually leads to nurses' turnover. In order to reduce negative influence of emotional labor, it is necessary to build positive organizational culture, to provide support from managers and co-workers. It is also important to improve work environment in order to do more deep acting since sharing emotions with patients can reduce the negative influence of emotional labor.

열다한소탕과 태음조위탕·조위승청탕의 소증 분석을 위한 의사결정나무 구성 (The Decision Tree to Analyze the Cases' Ordinary Symptoms Prescribed Yeoldahanso-tang and Taeeumjowi-tang·Choweseuncheng-tang)

  • Objectives The purpose of this study is to analyze the decision making process of prescribing Yeoldahanso-tang and Taeeumjowi-tang Choweseuncheng-tang using decision tree. Methods We used collected the prospective clinical data of TE type from September 2012 to July 2015. In this study, we used gender, BMI, blood pressure, pulse and clinical symptoms (digestion, sweat, defecation, urination, sleep, physical status, emotion, heat-coldness, water consumption, facial color) as variables. Decision trees were analyzed using open source R version 3.3.2. Results & Conclusions We found that the decision trees differed among institutions. However, in all institutions, it was found that stool type (ordinary symptom), urine frequency (ordinary and present symptom) and anxiety (ordinary symptom) were important in the decision of prescription. Besides, clinical informations such as sex, Body Mass Index and blood pressure affected the prescription decision.

임상간호사의 포커싱적 태도와 감정노동이 간호업무성과에 미치는 영향 (The Influence of Focusing Manner and Emotional Labor on Nursing Performance of Clinical Nurses)

  • Purpose: Focusing manner is very powerful mind-body self-help and self-awareness competence. So focusing manner may affect nurses' emotional labor and nursing performance, but few are known about it to date. The purpose of this study was to identify the degree of focusing manner, emotional labor, nursing performance of clinical nurses, and the factors that affect nursing performance. Methods: In order to collect data, structured questionnaires were administered to 212 nurses who worked at 2 university hospitals located in D city. Data were analyzed by t-test, ANOVA, correlation and multiple regression using SPSS/WIN 20.0. Results: Nursing performance had positive relationship with focusing manner (r=.45, p<.001), and negative relationship with emotional labor (r=-.25, p<.001). And the most prediction factor influencing nursing performance was focusing manner (${\beta}=.41$, p<.001) and the other factors were age (${\beta}=.30$, p=.002), and emotional labor (${\beta}=.14$, p=.012). The total variance was 37.1% by those predictors (F=25.87, p<.001). Conclusion: Based on the results of this study, it is necessary to develop and educational program on focusing manner-oriented psychotherapy in order to improve the nursing performance. Also, the management system for controlling emotional labor needs to be set up for nurses in clinical settings.

종합병원 간호사의 섬망 간호수행 영향요인 (Factors Affecting Nursing Performance of Delirium for General Hospital Clinical Nurses)

  • Purpose: This study was conducted to identify factors affecting nursing performance of delirium among general hospital clinical nurses. Methods: This was a cross-sectional study and conducted from July 20 to July 30 2015, with a sample consisting of 167 registered nurses in three general hospitals. Data were analyzed with SPSS 21.0. Results: Nursing performance of delirium had significant positive correlations with knowledge about delirium (r=.25, p=.001), self-confidence in caring for patients with delirium (r=.45, p<.001) and nursing work environment (r=.29, p<.001). In this study, factors affecting nursing performance of delirium were self-confidence in caring for patients with delirium (${\beta}=.33$, p<.001), nursing work environment (${\beta}=.26$, p<.001), experience in nursing education about delirium(${\beta}=.18$, p=.007) and clinical experience (${\beta}=.18$, p=.007). The factors explained 32% of nursing performance of delirium. Conclusion: This study's results can be used as basic data to develope educational programs related to delirium care through continuing education and to improve work environments in developing strategies to enhance nursing performance for patients with delirium.

간호학생의 임상실습 만족도에 관한 조사연구 (Level Satisfaction on Clinical Practice of Baccalaureate Nursing Students)

  • As it Provides nursing students the opportunity for correlating principles and practice, clinical practice h s been considered as on of the most important part of nursing education. This study was desinged to measure the level of satisfaction in according with the conten. guidance environment. hours and the evaluation of clinical pracice. and to investigate the extent of influence of the variables have on the level of satisfaction. Two hundred and fifty-two nursing students from 9 baccalaureat programs in Seoule were randomly sampled. Instrument consists of forty questionaires. developed by the researcher, was used to gather data data from September 7 through 22, 1978. The level of satisfaction was measured by 5 Point rating scale(Likert-type). and level of significance were(t-test. F-test & X$^2$-test). -Results are as follow; 1. Level of satisfation according to the four variables ( class health. academic achievement, motives) revealed no significant difference. (P>.05). Hypothesis 1,2,3,4, are rejected. 2. Level of satisfaction according to the two variables (clinical instructor interpersonal relationship) revealed significant difference. (p<.01, p<.05), Hypothesis 5,6 are accepted. 3. Level of satisfaction of contents (mean score=3.02) revealed to be high.. 4. Level of satisfaction on guidance (mean Score=2.37), environment (mean score=2.59), hours (mean score=2.72) and evaluation (mean score=2.50) revealed to be low. 5. Level of satisfaction revealed to be low. (total mean satisfaction score =2.64).

임상실습 오리엔테이션 프로그램이 간호학생의 첫 임상실습에 대한 불안, 자아존중감 및 자기효능감에 미치는 효과 (Effect of Clinical Practice Orientation Program on Anxiety, Self-Esteem and Self-Efficacy in College Students in Nursing)

  • Purpose: This study is done to identify how a clinical practice orientation program would affect anxiety, self-esteem and self-efficacy in college students in nursing. Method: Data was collected from 127 nursing students in February, 2008. The collected data were analyzed with the SPSS/Win 12.0 program, using t-test, ANOVA, paired t-test and Pearson's correlation coefficients. Results: The total mean scores were; anxiety 53.22/80, self-esteem 29.51/40 and self-efficacy in 59.76/85. Self-esteem and self-efficacy level were significantly different according to the level of Fundamentals practice and satisfaction with nursing as a major satisfaction(p=.001). The effect of the orientation program on clinical practice were a significantly increased in self-esteem and self-efficacy (p=.000). But in terms of anxiety, the effect was the opposite. So study to identify other factors causing anxiety and strategies to reduce the anxiety need to be considered. Conclusion: It is important to improve self-esteem and self-efficacy and the study results indicate that an orientation program is one strategy that needs to be developed to meet these needs for college students in nursing.

임상간호사가 경험한 긍정적인 간호조직문화 (Clinical Nurses' Experience of Positive Organizational Culture)

  • Purpose: This study was conducted to explore clinical nurses' experience of positive organizational culture in order to provide data for effective strategies of acquisition and retention of competent nurses. Methods: In this qualitative study, interviews with four focus groups of four to six nurses, 19 in total, were held. Compositional factors in groups included clinical experience, age, work place, and position. Interviews proceeded until data were saturated. Results: Fifteen sub-themes, categorized into six themes, emerged. Positive organizational culture themes included "Helping nurses to be organization members", "Allowing nurses to communicate with one another", "Helping nurses take an initiative to lead organization", "Having competent leader take charge of organization", "Enabling nurses to achieve organizational changes", and "Leading nurses to accomplish organizational performance." Conclusion: Results indicate that positive organizational culture is related to increases in occupational satisfaction and decreases in turnover through supportive organizational culture which makes it possible to reinvest expenses required for training new members to promoting quality growth in the organization and the prestige of professional nurses. In order to improve occupational satisfaction and sustained growth in nurses, it is necessary to provide nurses with positive work environments and require members to make active efforts leading to strategic changes.
