Correlation of Glasgow Prognostic Score or Procalcitonin to Clinical Variables in Patients with Pretreatment Lung Cancer

  • Unfortunately, the five-year survival rate of lung cancer is relatively low compared with other cancers. Therefore, better predictors are need for prognosis, therapeutic strategy, risk stratification and predicting long-term mortality of lung cancer. Recently, increasing data suggest that Glasgow Prognostic Score (GPS) and procalcitonin levels are useful predictor cancer prognosis. In this study, we retrospectively investigated the correlation of GPS or procalcitonin to clinical variables in patients with pretreatment lung cancer. In 135 patients with pretreatment lung cancer, GPS, procalcitonin, demographic characteristics, hematological, coagulation, biochemical, inflammatory and cardiac markers were measured. Monocyte, eosinophil, basophil, neutrophil to lymphocyte ratio, red cell distribution width (RDW), platelet to lymphocyte ratio, mean platelet volume to platecrit ratio, D-dimer and prothrombin time (PT) levels were higher, whereas mean platelet volume was lower than their normal ranges. Glucose and sodium levels were low, whereas gamma glutamyl transferase (GGT), total bilirubin, creatinine and inorganic phosphorus concentrations were increase compared their normal ranges. Procalcitonin, high sensitivity C-reactive protein and troponin-I concentrations were elevated compared with their normal ranges. GPS had significantly positive or negative relations to cancer stage, hematological, coagulation, biochemical, inflammatory and troponin-I. Based on the data, we suggest that GPS may be a potent and useful predictor for prognosis, therapeutic strategy, risk stratification and predicting long-term mortality of lung cancer.

A Comparative Study on Main Role, Professional Self Concept and Job Satisfaction of Public Health Nurses and Clinical Nurses (보건 간호사와 임상간호사의 주된 역할, 전문직 자아개념과 직무만족정도 비교)

  • This study was aimed at comparing and evaluating one main role, the professional self concept and job satisfaction of public health nurses and clinical nurses. Method: The data was collected from 72 nurses in 8 health centers and 86 nurses in 5 hospitals in Daegu city and Kyungpook province from May 5th to June 5th, 2001. This study was interviewed by trained social workers. The origin of questionnaires used in this study were PSCNI(Professional Self-Concept of Nurses Instrument) to measure the professional self concept of nurses and JDI(Job Descriptive Index) to measure the job satisfaction of nurses. The data was analyzed by SAS(version 6.12) and statistical methods used were Chi-square test, t-test, ANOVA(analysis of variables) and ANCOVA(analysis of covariate). Result: The findings of this study were as follows: 1. The most important domain in the main role of public health nurses were as counsellors, managers, mediators and evaluators. In clinical nurses, their main roles were, directors of nursing, researchers, spokesmans, and informers. 2. Public health nurses showed lower scores in the professional self concept of nurses than clinical nurses, what statistically significant tool was used to suggest this finding. 3. Public health nurses showed higher scores in job satisfaction than clinical nurses, but again, explain the tool used in these findings for give a rationals. 4. For public health nurses, general characteristics which affected professional self concept were age, educational level, and duration of job. In clinical nurses, the general characteristics which affected professional self concept were age, religion, marital status, and duration of job. 5. For public health nurses, there was no general characteristic which affected job satisfaction. In clinical nurses, the general characteristics which affected job satisfaction were age, religion, and duration of job. Conclusion: Nursing profession should give a training to competent nurses who can deliver high quality care to meet the needs of health consumers. The role of nurses has been expended and specialized as health conception has been changed in Korea.

Comparison of Psychiatric and Clinical Nurses on Emotional Intelligence, Facilitative communication ability and Communication Conflicts (정신간호사와 일반간호사의 감성지능, 촉진적 의사소통능력 -및 의사소통 갈등 비교)

  • The purpose of this study was to compare emotional intelligence, facilitative communication ability and communication conflict among psychiatric nurse and clinical nurse to improve nurse's emotional intelligence and facilitative communication ability in each clinical field, it was attempted to provide basic data to search for alternatives to reduce communication conflict. Data were analyzed using ANCOVA with SPSS/WIN 20. Finally, emotional intelligence, facilitative communication ability and communication conflicts in clinical nurses higher than psychiatric nurses. These results indicate that need to develope a variety of clinical programs to promote facilitative communication ability and to enhance emotional intelligence of psychiatric nurses and to reduce communication conflicts of clinical nurses.

Effect of Grit and Resilience on Clinical Practice Adaptation of Nursing Students (간호대학생의 그릿과 회복탄력성이 임상실습 적응에 미치는 효과)

  • This study is a descriptive study to investigate the effects of grit and resilience on clinical practice adaptation of nursing students. The subjects of this study were 131 nursing students and data collection was from August 19 to September 30, 2019. Data was analyzed using the SPSS 20.0 program by descriptive statistics, t-test, correlation analysis, and multiple regression analysis. Grit and resilience were found to be factors affecting clinical practice adaptation of nursing students and 32.0% of clinical practice adaptation. Therefore, in order to increase the clinical practice adaptation of nursing students it is necessary to make efforts to improve grit and resilience and development of clinical practice curriculum.

The relationship between environmental factors causing stress in clinical practice institutions and the stress level of dental hygiene students (치위생과 학생들의 임상실습기관의 환경요인 스트레스와 스트레스 량의 관련성)

  • Purpose : This study was conducted to provide basic data on the relationship between environmental factors causing stress in clinical practice institutions and stress levels of dental hygiene students. Methods : The research subjects totaled 207 and it was analyzed with structured questionnaires. The collected data were analyzed using an IBM SPSS ver. 20.0. Results : The environmental factors causing stress according to the status of clinical practice institutions was statistically and significantly different in the case of Busan practice areas (p<0.01), dental university hospitals (p<0.001), attendance times prior to 8am (p<0.001), quitting times that surpassed 8pm (p<0.01), and the number of dentists exceeding 4 (p<0.01). It was found to affect the stress level of students when individuals had to stand for a long period of time(p<0.001, ${\beta}=0.254$) and with the use of unfamiliar tools and equipment(p<0.05, ${\beta}=0.178$). Conclusions : As a result of the research conducted, it should be concluded that dental clinics should provide rest areas for individuals to focus on clinical practice. In addition, it is necessary to develop a systematic program that enables students to communicate with students at any time during the training period.

Relationship between Self-Efficacy and Clinical Skill Competence of Nursing Students (4년제 간호대학생의 자기효능감과 간호수기능력과의 관계)

  • Purpose: This study was done to identify the relationship between self-efficacy and clinical skill competence of nursing students. Method: The participants in the study were 375 student nurses enrolled in 3rd & 4th year of a BSN program in Seoul and Gyeongi province, Korea. Data were collected from December 12, 2004 to November 30, 2005 using a structured questionnaire. Data were analyzed using SPSS 11.0 Win program. Results: The average score for self-efficacy was $3.21{\pm}0.60$, and for clinical skill competence $2.80{\pm}0.42$. The self-efficacy score showed a significant difference according to student's year in college (t=2.494, p=0.013), and GPA score (t=4.299, p=0.000). The clinical skill competence score showed a significant difference according to year in college (t=5.351, p=0.000), GPA score (t=2.997, p=0.003) and satisfaction with course (F=5.214, p=0.006). There was a significant positive correlation between self-efficacy and clinical skill competence (r=0.224, p=0.000). Conclusion: These findings show that there is a need to develop strategies to improve self-efficacy for student nurses in Korea. And also, it is recommended that programs to improve clinical skill competence need to be included in nursing curricula.

Study for Clinicians' Perception on the Acupuncture Research and Ways to Symbiosis between Basic Research and Clinical Practice (침 연구에 대한 한의사의 인식과 연구방향에 대한 조사)

  • Objectives : From mechanism studies using animal models to high-level evidence based research such as randomized controlled trials or systematic reviews, various acupuncture studies have been performed. Nevertheless, the current research appears insufficient to satisfy clinicians' needs. We aimed at investigating clinicians' perception on the translational research and suggesting ways of symbiosis between basic research and clinical practice. Methods : Data were collected using a web-based questionnaire made available to Korean medicine doctors who were informed through social networking service or e-mail. The survey participants indicated their awareness of acupuncture research and their thoughts on the applicability of research in practice. Results : Forty-seven clinicians completed the survey. While most respondents agreed that acupuncture research is very important, only 40% of them answered that the results of acupuncture research are useful to their own practice. In more than half of those who responded to the survey, the current acupuncture research is not viewed as reflecting clinical practice very much. Respondents agreed that acupuncture research should be based on clinical practice, and they recognized the need to collect valid clinical data from real-world practice. Also, as many as 77% of clinicians had no scruple to participate in research programs. Conclusions : There is no doubt among practitioners that acupuncture research is necessary. More importantly, practice-based research remains essential in order to feed-back research outcomes to clinical practice. Communications and academic exchanges between clinicians and researchers can mutually benefit research and practice with better information, resulting in symbiosis.

Clinical Significance and Prognostic Value of Pentraxin-3 as Serologic Biomarker for Lung Cancer

  • Purposes: Lung cancer is prevalent worldwide and improvements in timely and effective diagnosis are need. Pentraxin-3 as a novel serum marker for lung cancer (LC) has not been validated in large cohort studies. The aim of the study was to assess its clinical value in diagnosis and prognosis. Methods: We analyzed serum PTX-3 levels in a total of 1,605 patients with LC, benign lung diseases and healthy controls, as well as 493 non-lung cancer patients including 12 different types of cancers. Preoperative and postoperative data were further assessed in patients undergoing LC resection. The diagnostic performance of PTX-3 for LC and early-stage LC was assessed using receiver operating characteristics (ROC) by comparing with serum carcinoembryonic antigen (CEA), cytokeratin 19 fragments (CYFRA 21-1). Results: Levels of PTX-3 in serum were significantly higher in patients with LC than all controls. ROC curves showed the optimum diagnostic cutoff was 8.03ng/mL (AUC 0.823, [95%CI 0.789-0.856], sensitivity 72.8%, and specificity 77.3% in the test cohort; 0.802, [95%CI 0.762-0.843], sensitivity 69.7%, and specificity 76.4% in the validate cohort). Similar diagnostic performance of PTX-3 was observed for early-stage LC. PTX-3 decreased following surgical resection of LC and increased with tumor recurrence. Significantly elevated PTX-3 levels were also seen in patients with non-lung cancers. Conclusions: The present data revealed that PTX-3 was significantly increased in both tissue and serum samples in LC patients. PTX-3 is a valuable biomarker for LC and improved identification of patients with LC and early-stage LC from those with non-malignant lung diseases.

  • Purpose: The aim of this study was to investigate distribution of the admitted patients with disease of oral cavity, salivary glands and jaws, current clinical treatment condition and clinical dental practice. Methods: The subject were 4,564 patients with disease of oral cavity, salivary glands and jaws of the Korean National Hospital Discharge Injury Survey 2004-2008 data. This study was carried out using the administrative database including patients' characteristics and comorbidity. The statistical analysis for ratio, gender, age, region, primary diagnosis, comorbidity, operation, hospital location and bed size of inpatients in Korean National Hospital Discharge Injury Survey was conducted by frequency analysis. Results: Among the total discharge injury patients in 2004-2008, the trend showed decrease of ratio of patients with disease of oral cavity, salivary glands and jaws. The portion of male was higher than female, and 20~29 age group was the highest portion compared with other age groups. Seoul-Gyunggi region was the highest among the other residences. patients with Dentofacial anomalies[including malocclusion] as primary diagnosis, digestive system as comorbidity and operations on facial bones and joins showed the highest portion respectively. Seoul-Gyunggi region was the highest portion compared with other residences. 500~999 bed size showed the highest portion. Conclusion: In this study showed that distribution of patients with disease of oral cavity, salivary glands and jaws, current dental clinical treatment condition using the Korean National Hospital data.

A Study of the Clinical Experience of Nursing Students (간호학생의 임상실습경험)

  • Nursing students have identified the clinical experience as one of the hardest and most anxiety producing components of their nursing program. Nurse educators play a critical role in assisting students to apply their knowledge skillfully in the clinical area. In order to help students through their clinical experience, faculty must first understand more fully the meaning of this experience for nursing students. A phenomenological study was conducted to explore the actual reality of the nursing students' clinical experience. The research was done through indepth interviews. Fifteen Pusan senior nursing students who had completed their clinical practice described their experiences. The data were collected during the period from October 1994 to January 1995. The data were analyzed by Colaizzi's method of phenomenological analysis. Six theme clusters emerged : the experience of emotional difficulties ; the experince of physical diffuculties ; the perception of deficiencies of knowledge and skill ; the disappointments and regrets perceived during the experience ; the awareness of various sources of joy and gratitude ; the experience of personal growth. The results of this study enable us to understand nursing students better. Also through these results, we should be better placed to help nursing students during their clinical experience and to help their gain more from this experience.
