A Flexible Modeling Approach for Current Status Survival Data via Pseudo-Observations

  • When modeling event times in biomedical studies, the outcome might be incompletely observed. In this paper, we assume that the outcome is recorded as current status failure time data. Despite well-developed literature the routine practical use of many current status data modeling methods remains infrequent due to the lack of specialized statistical software, the difficulty to assess model goodness-of-fit, as well as the possible loss of information caused by covariate grouping or discretization. We propose a model based on pseudo-observations that is convenient to implement and that allows for flexibility in the choice of the outcome. Parameter estimates are obtained based on generalized estimating equations. Examples from studies in bile duct hyperplasia and breast cancer in conjunction with simulated data illustrate the practical advantages of this model.

Respiratory Motion Correction on PET Images Based on 3D Convolutional Neural Network

  • Motion blur in PET (Positron emission tomography) images induced by respiratory motion will reduce the quality of imaging. Although exiting methods have positive performance for respiratory motion correction in medical practice, there are still many aspects that can be improved. In this paper, an improved 3D unsupervised framework, Res-Voxel based on U-Net network was proposed for the motion correction. The Res-Voxel with multiple residual structure may improve the ability of predicting deformation field, and use a smaller convolution kernel to reduce the parameters of the model and decrease the amount of computation required. The proposed is tested on the simulated PET imaging data and the clinical data. Experimental results demonstrate that the proposed achieved Dice indices 93.81%, 81.75% and 75.10% on the simulated geometric phantom data, voxel phantom data and the clinical data respectively. It is demonstrated that the proposed method can improve the registration and correction performance of PET image.

간호대학생의 임상실습스트레스와 임상수행능력의 관계에 미치는 극복력의 매개효과 (The Mediating Effect of the Resilience on the Relationship between Stress in Clinical Practice and Clinical Competence in Nursing Students)

  • 본 연구는 간호대학생의 임상실습스트레스와 임상수행능력에 대한 극복력의 매개효과를 파악하고자 시도되었다. 연구대상은 C 지역의 3개 대학 간호학과에 재학 중인 4학년 137명이다. 연구기간은 2016년 10월 15일부터 11월 15일까지이고, 구조화된 설문지를 이용하여 자료를 수집하였다. 수집된 자료는 SPSS 22.0 통계프로그램을 이용하여 기술통계, t-test, ANOVA, Pearson's correlation, Multiple regression으로 분석하였다. 대상자들의 임상실습스트레스와 임상수행능력은 총 5점 중 각각 평균 $3.09{\pm}0.43$점, $3.54{\pm}0.45$점이었고, 극복력은 총 7점 중 평균 $4.93{\pm}0.66$점이었다. 임상실습스트레스와 극복력 간(r=-.24, p=.005), 임상실습스트레스와 임상수행능력 간(r=-.21, p=.012)에 음의 상관관계를 보였고, 극복력과 임상수행능력 간(r=.59, p<.001)에 양의 상관관계를 보였다. 임상실습스트레스와 임상수행능력과의 관계에서 극복력의 매개효과를 검정한 결과, 극복력은 두 변수 간 관계를 완전 매개하는 것으로 나타났고, Sobel test을 통해 확인한 결과 매개효과는 통계적으로 유의하였다(Z=-3.02, p=.003). 즉, 임상실습스트레스는 임상수행능력에 미치는 부정적 효과는 극복력이 투입된 후 더 이상 유의한 영향이 없는 것으로 나타났다. 임상실습교육은 간호학과 교과 과정에서 매우 중요한 요소이며 반드시 수료해야 하는 교과과정이다. 따라서 불가피하게 직면하는 스트레스 상황에서 간호대학생이 갖추어야 할 임상수행능력을 향상시킬 수 있도록 다양한 교과 내 외 과정에 극복력 강화 프로그램을 개발하고 그 효과를 검증할 것을 제언한다.

  • 본 연구는 간호대학생이 경험하는 무례함과 임상실습교육환경이 임상실습스트레스에 미치는 영향을 확인하기 위해 시도하였다. 연구대상자는 임상실습 경험이 있는 3, 4학년 간호대학생 204명이며, 자료는 구조화된 설문지를 배부하고 자가보고하는 형식으로 2019년 4월 1일부터 4월 30일까지 수집되었다. 자료 분석을 위해 SPSS 22.0 program을 이용하여 기술적 통계, t-test, ANOVA, Pearson 상관계수, 다중회귀분석을 실시하였다. 본 연구의 결과는 다음과 같다. 간호대학생이 경험한 무례함의 평균은 1.58점, 임상실습교육환경의 평균은 2.79점, 임상실습스트레스의 평균은 3.33점으로 나타났다. 임상실습스트레스는 간호대학생이 경험한 무례함과 정적 상관관계, 임상실습교육환경과는 부적 상관관계가 있는 것으로 나타났다. 간호대학생이 경험한 무례함과 임상실습교육환경은 부적 상관관계가 있는 것으로 확인되었다. 임상실습 중 경험한 무례함과 임상실습교육환경은 임상실습스트레스에 유의한 영향요인으로 나타났으며 22.5%의 설명력을 보였다. 본 연구결과를 바탕으로 간호대학생들의 임상실습스트레스를 효과적으로 관리하기 위해서는 임상현장에서 경험하는 무례함을 감소시키고 임상실습교육환경을 개선하는 노력이 필요 할 것이다.

간호대학생의 임상실습교육에 대한 자기효능감, 전문직 자아개념 및 임상실습만족도 (Self-Efficacy, Self-Concept of Professional Nursing, and Clinical Practice Satisfaction to Clinical Practice Education in Nursing Students)

  • The purpose of this study was to identify self-efficacy and self-concept of professional nursing which might influence clinical practice satisfaction in nursing students. The participants were 278 nursing students in U city. Date were collected from November 29 to December 17, 2010. The collected data was analyzed with the PASW 18.0 statistics program. Self-efficacy had significant positive correlation with self-concept of professional nursing(r=.595, p<.001) and self-concept of professional nursing had significant positive correlation with clinical practice satisfaction(r=.396, p<.001). In multiple regression analysis, self-concept of professional nursing is most influential factor explained 21% of variance of clinical practice satisfaction. The results indicate that to develop a more reasonable and effective curriculum and teaching program for enhancement of students' self-concept of professional nursing.

전문대 간호학생들의 첫 임상실습 체험 연구 (A Qualitative Study of Nursing Students' first Clinical Experience)

  • This qualitative nursing research used a hermeneutic phenomenological approach to discovery of meaning of the nursing students' clinical experience in junior colleges. Data collecting was utilized by in- depth face-to-face interviews of 10 sophomores who were participating in their first clinical experience. The following themes of the experience emerged. 1) The first step of fear (fear, being embarrassed) 2) Rhythmical patterning of theory and practice(disappointment of nurses, recognition of the gap between the theory and practice, the conflict between medical doctors and nurses, trouble with the students from other colleges, confusion from nurses' educational and noneducational attitudes, burden of responsibility for nurses' jobs, and the tiresome nature of repeated practice) 3) Discovery of changing self, awareness of self-expansion (awareness of professional nurses' roles, formation of self-confidence, changing of one's state of mind) From this study essential theme of clinical experience of nursing student's was identified, and continuous efforts to establish better adaptation of nursing students to clinical practice are required.

임상간호사의 직무스트레스와 우울 - 한국인 직무스트레스 측정도구를 이용하여 - (Occupational Stress and Depression in Clinical Nurses - Using Korean Occupational Stress Scales -)

  • Purpose: The purpose of this study was to examine the level of occupational stress and depression in clinical nurses and to investigate the effects of subscales of occupational stress on depression. Method: A sample consisted of 202 nurses from 6 hospitals located in Busan and GyeongNam province. The data were collected from November 2008 to march 2009. A self-administered questionnaire was used to assess socio-demographics, job characteristics, depression measured by Korean CES-D, and occupational stress assessed by KOSS. Result: The total score of occupational stress in clinical nurses was within bottom 25%, but the scores of 'Difficult physical environment', 'High job demand', 'Inadequate social support', 'Organizational injustice' were within top 50%. The 8 subscales of occupational stress had significantly effects on clinical nurses' depression after controlling for age, marital status, type of hospital, current position. Conclusion: The administration of employee assistance program for management occupational stress is important for decreasing depression in clinical nurses.

간호대학생의 비판적 사고성향, 전문직 자아개념 및 임상수행능력 (Critical Thinking Disposition, Professional Self-Concept and Clinical Competence in Nursing Students)

  • The purpose of this study was to identify the relationships among critical thinking disposition, professional self-concept and clinical competence in nursing students. The participants were 291 nursing students in Gwangju Province. A questionnaire survey was conducted from March 02 to 16, 2013. Data were analyzed using descriptive statistics, t-test, ANOVA, Scheffe test, pearson's correlation coefficients, and stepwise multiple regression analysis with SPSS/WIN 20.0 version. There was a significant positive correlations among critical thinking disposition, professional self-concept, and clinical competence. The variable predicting clinical competency were professional self-concept and critical thinking disposition. These factors accounted for 44.1% of the variance. The findings of this study suggest that the most influential factor for clinical competence was professional self-concept. Therefore it is necessary to improve professional self-concept through effective education program.

임상치료사를 위한 신경학적 검사의 이해 (Understanding of Neurological Examination for Clinical Therapist)

  • Clinical therapist use neurological examination to acquire the necessary information from the patients who is neurological damaged. It is necessary to have enough neurological knowledge and clinical experience to collect useful data. Neurological disease of symptom is well correspond with anatomical location and function, therefore neurological examination is one of the powerful tool to diagnosis. These tools will be a great help to clinical therapist to evaluate the patients and helps to select most pertinent treatment approach to patients. Neurological examination can classified and evaluate with Mental Status Examination, Cranial Nerves Examination, Motor and Sensory System Examination, Reflexes, Gait and Station Evaluation, Special Maneuver. Generally, various neurological examination tools are used by therapist in clinical field. Understanding of method of Neurological examination tools and understanding of result of examination from patients's response is very important. Therefore, this research will help to understand clinical meaning by neurological examination.

  • To promote the inclusion of quality of life (QOL) end-points In clinical research on cancer, the National Cancer Institute (USA) sponsored a workshop on QOL assessment In cancer clinical trials In July, 1990. Experts In clinical trials and QOL research formed four working groups to Identify current areas of cancer treatment In which QOL end-points are most Important; to discuss methodologic problems In QOL assessment; to address common problems In Implementing clinical studies with QOL end-points; and to consider statistical Issues In design, Implementation, and data analysis. Recommendations made by the working groups are summarized in this paper.
