Future Directions of Pharmacovigilance Studies Using Electronic Medical Recording and Human Genetic Databases

  • Adverse drug reactions (ADRs) constitute key factors in determining successful medication therapy in clinical situations. Integrative analysis of electronic medical record (EMR) data and use of proper analytical tools are requisite to conduct retrospective surveillance of clinical decisions on medications. Thus, we suggest that electronic medical recording and human genetic databases are considered together in future directions of pharmacovigilance. We analyzed EMR-based ADR studies indexed on PubMed during the period from 2005 to 2017 and retrospectively acquired 1161 (29.6%) articles describing drug-induced adverse reactions (e.g., liver, kidney, nervous system, immune system, and inflammatory responses). Of them, only 102 (8.79%) articles contained useful information to detect or predict ADRs in the context of clinical medication alerts. Since insufficiency of EMR datasets and their improper analyses may provide false warnings on clinical decision, efforts should be made to overcome possible problems on data-mining, analysis, statistics, and standardization. Thus, we address the characteristics and limitations on retrospective EMR database studies in hospital settings. Since gene expression and genetic variations among individuals impact ADRs, pharmacokinetics, and pharmacodynamics, appropriate paths for pharmacovigilance may be optimized using suitable databases available in public domain (e.g., genome-wide association studies (GWAS), non-coding RNAs, microRNAs, proteomics, and genetic variations), novel targets, and biomarkers. These efforts with new validated biomarker analyses would be of help to repurpose clinical and translational research infrastructure and ultimately future personalized therapy considering ADRs.

A within-subject comparison of short implants in the posterior region: retrospective study of up to 10 years

  • Purpose. This intra-patient retrospective study of up to 10 years evaluated the clinical success and risk factors of 6- and 8-mm long implants and their respective prostheses. Materials and Methods. The sample consisted of patients treated at a Military Polyclinic dental service, who received both 6- and 8-mm long tissue level implants in the posterior region of the same arch. Data were collected from the dental charts, clinical and radiographic exams, self-report of sleep bruxism, measurement of maximum occlusal force, and clinical crown-to-implant (C/I) ratio. Data were analyzed by descriptive and inferential statistics with univariate and hierarchical multivariate models, at the 0.05 significance level. Results. The 30 patients (27 women) had 85 implants and 83 prostheses. Two implants were lost before prosthesis installation (implant survival: 97.6%). Ten events of prosthetic complication (screw tightening loss) occurred in five patients (success rate: 87.9%) in a single moment. Only the variable C/I ratio had a significant effect for repairable prosthesis complication (P<.05). Conclusion. The results suggest that 6- and 8-mm long implants have similar long-term clinical success for implants and prostheses.

월경통 한의표준임상경로 적용 가능성 및 유효성 평가 연구 프로토콜 (A Study on the Application of Standard Korean Medicine Clinical Pathway for Dysmenorrhea)

  • Objectives: The purpose of this study protocol is firstly to investigate the status of Korean medical treatment of dysmenorrhea, and secondly to investigate effectiveness of the Clinical Pathway (CP) of dysmenorrhea and review the applicability. Methods: This is a multi-center observational study. The data will be prospectively collected from 14 Korean medicine clinics. 45 patients for each of the CP-applied and non-applied groups will be recruited to compare the degree of improvement in menstrual pain. All the diagnosis records, treatment methods, treatment results, adverse events, and medical expenses of patients with dysmenorrhea will be collected. We will investigate the proportion of patients who could be applied with CP, and the actual number of CP applications, and CP completion rate to evaluate the applicability. Additionally, the satisfaction survey will be used to collect feedback from medical staff members and patients. Results: The results of this study will be disseminated through peer-reviewed publications and scientific presentations. Conclusions: This study is expected to provide valuable data for application of standard Korean medicine clinical pathway for dysmenorrhea.

간호대학생의 팀워크와 간호전문직관이 임상 실습 적응에 미치는 영향 (The Effects of Teamwork and Nursing professionalism on Clinical Practicum Adaptation of the Nursing Students)

  • 본 연구는 간호대학생의 팀워크와 간호전문직관이 임상 실습 적응에 미치는 영향을 알아보기 위한 연구이다. 2019년 8월 5일부터 8월 30일까지 J 대학교 간호학과 4학년 학생을 대상으로 자료를 수집하였고, 115부를 최종 분석에 이용하였다. 수집된 자료는 SPSS 23.0 프로그램을 이용하여 Independent t-test와 One way ANOVA, Pearson's correlation coefficient, Multiple regression을 실시하였다. 연구 결과, 임상 실습 적응에 영향을 미치는 요인은 팀워크, 임상 실습 시 임시 주거 형태, 임상 실습에 대한 만족도였으며 이들 변수는 전체 변량의 67%의 설명력을 보였고 임상 실습 적응에 가장 큰 영향요인은 팀워크로 나타났다. 따라서 간호대학생의 임상 실습 적응을 위해 팀워크의 중요성을 인식하고 팀워크를 강화시킬 수 있는 프로그램 개발 및 적용이 필요하다.

병임상간호사의 간호조직몰입과 직무만족이 이직의도에 미치는 영향 (The influence of nursing organizational commitment and job satisfaction on intention of resignation of clinical nurses)

  • The purpose of this study was to identify the influence of nursing organizational commitment and job satisfaction on intention of resignation of clinical nurses. The subjects of this study were clinical nurses who were working at general hospitals in Pusan and Taegu in Korea. The questionnaires were sent to 850 clinical nurses at 8 general hospitals and 790(92%) answers were replied. Data were collected from Oct. 4 to Oct. 30. The instruments of this study were scales for the measurement of nursing orgainzational commitment, job satisfaction and intention of resignation of clinical nurses. The Cronbach ${\alpha}s$ of the scales were .9385, .9232, .6847. After screening of data, 769 cases were analyzed with SPSS/WIN 10.0 program. The results of the study were as followings: 1. Average score of the nursing organizational commitment, job satisfaction and intention of resignation of the subjects were 48.50, 50.23 and 12.20. 2. There were significant differences in nursing organizational commitment, job satisfaction and intention of resignation by age, marital status, religions, education level, position, career, and work place. 3. 치과위생사의 임상치과 핵심역량 개발 (Development of clinical dental competencies in dental hygienists)

  • Objectives: This study was conducted to identify the requirements for the clinical dental tasks in the dental hygienists using the frequency of dental hygienists' tasks in the Korean dental clinics, and to provide them with the core competencies for achieving these. Methods: This study was based on evaluation of a self-reported survey. The dental hygienists were investigated upon classification of their tasks into direct and assistant performances, and the dentists were investigated by the competency level of the dental hygienists. The data from 481 clinical dental hygienists and 67 dentists were used for final analysis in this study, excluding the subjects who provided incomplete or inappropriate responses to the survey. The collected data were analyzed by frequency analysis. Results: The clinical dental core competency tasks of the dental hygienists were divided into 4 tasks for common dentistry, 4 for periodontics, 5 for conservative dentistry, 5 for pediatric dentistry, 5 for dental prosthodontics, and 3 for dental orthodontics. Clinical dental core competency tasks of the dental hygienists were performed more frequently of tasks performance than 8.0, and dentists required competence level was similar to that of most dental hygienists clinical dental core competencies. Conclusions: It is necessary to check whether the curricula of the universities include the competencies for the students enough to perform the corresponding tasks and the core competencies need to be reflected in the curricula. The clinical dental core competencies need to be agreed by dental hygienists, and it is necessary to be organized as the evaluation guide of the institute of dental hygiene education and evaluation and utilized as the national examination.

매개모델을 이용한 간호대학생의 임상수행능력 영향요인 (Affecting Factors of Clinical Competence in the Nursing students Using a Mediation Model)

  • 본 연구는 간호대학생의 감성지능, 자기효능감과 임상수행능력 간에 인과관계를 알아보고, 감성지능이 자기효능감을 매개로 하여 임상수행능력에 미치는 영향을 살펴보았다. 연구대상자는 3개 대학교의 간호대학생 182명을 대상으로 실시하였으며, 자료수집 기간은 2014년 10월 14일부터 11월 10일까지 실시하였다. 자료분석은 SPSS 21.0 프로그램과 AMOS 18.0 programs를 이용하여 기술통계, 상관분석, 매개효과를 파악하였다. 연구결과는 첫째, 대상자의 감성지능, 자기효능감과 임상수행능력 간의 상관관계가 나타났다. 둘째. 대상자의 감성지능은 자기효능감에 미치는 영향은 직접효과가 나타났으며, 감성지능은 임상수행능력에 미치는 영향은 직접효과와 간접효과가 나타났다. 따라서 대상자의 감성지능과 임상수행능력의 관계에서 자기효능감의 부분 매개효과가 확인되었다.

간호학생의 영성과 임상실습 스트레스 (Spirituality and Stress of Clinical Practice among Nursing Students)

  • 본 연구는 간호학생의 영성과 임상실습 스트레스 정도를 평가하고 이들 간의 관계를 파악하여 간호학생들의 영성증진과 임상실습 스트레스 중재방안으로 영성의 활용가능성을 확인하고자 시도하였다. 2개 대학의 간호학생 203명을 편의 추출하여 자가 보고식 설문지를 사용하여 자료를 수집하였다. 연구결과, 본 연구대상자들의 영성은 5점 만점에 3.18점, 임상실습스트레스는 5점 만점에 3.42점으로 중간이상의 수준이었다. 하위영역별로는 영성의 경우 '삶의 의미와 목적'이 가장 높았고, 임상실습 스트레스에서는 '실습교육환경'에 대한 스트레스가 가장 높게 나타났다. 일반적 특성에 따른 차이에서는 영성은 종교와 전공만족도에 따라, 임상실습 스트레스는 성별과 전공만족도에 따라 유의한 차이를 보였다. 영성과 임상실습 스트레스는 유의한 부적상관관계가 있었다. 결론적으로, 간호학생들의 임상실습 스트레스를 감소시키기 위해서는 영성증진을 위한 교육프로그램 개발과 이를 효과적으로 교육과정에 접목시킬 수 있는 방안을 구축할 필요가 있다.

간호대학생의 가상 현실 시뮬레이션 교육과 교외 임상실습을 통합한 교육의 효과 (The Effect of Education Integrating Virtual Reality Simulation Training and Outside School Clinical Practice for Nursing Students)

  • 본 연구의 목적은 간호대학생에게 가상 현실 시뮬레이션 교육과 교외 임상실습을 통합한 교육을 적용한 후의 자기효능감, 임상수행능력 및 비판적 사고 성향의 차이를 확인하여 효과적인 실습교육의 기초자료로 제공하기 위함이다. 본 연구는 2020년 6월 8일부터 7월 3일까지 G시 소재 대학 3학년에 재학 중이며, 가상 현실 시뮬레이션 교육과 교외 임상실습을 받은 경험이 없는 간호대학생 30명을 대상으로 하였다. 자료분석은 Paired t-test로 분석하였다. 연구결과, 가상 현실 시뮬레이션 교육과 교외 임상실습을 통합한 교육을 적용한 후 자기효능감, 임상수행능력 및 비판적 사고 성향정도가 교육 전보다 통계적으로 유의하게 높아진 것으로 나타났다. 본 연구에서 간호대학생의 가상 현실 시뮬레이션 교육과 교외 임상실습을 통합한 교육은 임상수행능력 향상과 더불어 비판적 사고 성향, 자기효능감을 향상시키는 것으로 확인되어 교외 임상실습 전 임상상황에 맞는 다양한 가상 현실 시뮬레이션 교육이 필요할 것으로 사료된다.

수술실 간호사의 경력개발제도에 대한 인식이 직무만족, 이직의도에 미치는 영향 (Effect of Perception of Career Ladder System on Job Satisfaction, Intention to Leave among Perioperative Nurses)

  • Purpose: This was a correlational study to identify effects of perception of clinical ladder system on job satisfaction and intention to leave in perioperative nurses. Methods: Participants were 154 of perioperative nurses from larger general hospitals in Seoul. Data were collected from April, 16 to 22, 2013 using self-report questionnaires which included items on perception of clinical ladder system, job satisfaction, and intention to leave. Data were analyzed using frequency, t-test, ANOVA, $Scheff{{\acute{e}}$ test, Pearson correlation coefficients, and multiple stepwise regression. Results: The average mean score for perception of clinical ladder system was midline at 2.69 point out of 4 point. Perception of clinical ladder system correlated positively with job satisfaction (r=.38, p<.01) and negatively with intention to leave (r=-.88, p<.01). Perception of clinical ladder system was the factor which most influenced job satisfaction explaining 17.1% of the variance, also perception of clinical ladder system was the factor which most influenced intention to leave, explaining 12.7% of the variance. Conclusion: Results of this study suggest that there is a need to enhance the perception of the clinical ladder system and to find ways to fulfill the expected effects for improving perioperative nurses' job satisfaction and reducing the intention to leave.
