심장 질환 진단을 위한 데이터 마이닝 기법 (Data Mining Approach for Diagnosing Heart Disease)

  • 심장의 활동을 기록한 심전도는 심장의 상태에 대한 가치 있는 임상 정보를 제공한다. 지금까지 심전도를 이용한 심장 질환 진단 알고리즘에 대한 많은 연구가 진행되어 왔으나, 심장 질환에 대한 국내 진단 결과의 부정확성 때문에 외국의 진단 알고리즘을 사용하고 있다. 이 논문에서는 원시 심전도 데이터로부터 심장 질환 진단의 파라미터인 ST-segment 추출 방법을 제안한다. ST-segment는 관상동맥 질환 예측에 활용되므로 데이터마이닝의 분류기법을 적용하여 질환을 예측한다. 또한 연관규칙 마이닝을 통해 환자들의 임상 데이터로부터 심장 질환자들의 임상적 특징을 예측한다.

Analysis of Trend of Studies on Microneedle Treatment System (MTS)

  • Objectives: The purpose of this study was to analyze the microneedle therapy system (MTS) and its research methods for the past 10 years in Korea. Methods: Data on microneedle therapy system were collected using NDSL, KISS, RISS, and OASIS electronic databases from January 2010 to August 2021. "microneedle," "derma stamp," "microneedle therapy system" were used as the keywords. The present study, however, excluded data that were 1) unrelated to the microneedle therapy system, 2) from review/meta/protocol studies, and 3) from overseas studies. Data selected through the primary screening process, animal studies, case reports, and clinical data were included in the analysis. However, information data not related to the microneedle therapy system were excluded from the study. Results: Among the MTS-related papers published from January 2010 to August 2021, 7 animal research, 2 clinical trials, and 10 case studies were published. Based on the research topics, there were 8 papers on skin improvement and skin diseases, 7 papers on hair growth and hair loss, 3 papers on stability, and 1 paper on peripheral facial paralysis. Conclusion: Most of the studies related to MTS focused on skin, hair, and stability. The effect of MTS on hair growth and skin improvement has been confirmed, and it has been proven to have significant effects on the treatment of acne, acne scars, and hair loss in clinical practice. No serious side effects were observed during the MTS treatment, and the safety assessment confirmed that it was safe for use.

간호간병통합병동 간호사의 전문직 자율성, 역할갈등이 임상적 의사결정능력에 미치는 영향 (Impacts of Professional Autonomy and Role Conflict Clinical Decision-Making Ability of Nurses on Comprehensive Nursing Care Service Wards)

  • 본 연구는 간호간병통합서비스 병동간호사를 대상으로 전문직 자율성, 역할갈등, 임상적 의사결정능력과의 관계와 함께 임상적 의사결정능력에 영향을 미치는 영향요인 파악을 목적으로 한 서술적 조사연구이다. 본 연구에서는 D시와 G시에 근로간호사 234명을 편의 표집하여 조사하였다. 자료수집은 2017년 10월 12일부터 11월 30일 사이에 D시와 G시에 소재하는 4개의 종합병원의 234명을 대상으로 연구 참여에 서면 동의한 간호사에게 자가 보고식 설문조사지를 사용하여 자료를 수집하였다. t-test, ANOVA, Pearson's coefficients와 단계적 회귀분석으로 자료를 분석하였다. 연구결과에서 임상적 의사결정능력은 평균평점 $3.38{\pm}0.78$점으로 중간수준으로 나타났다. 임상적 의사결정능력은 전문직 자율성과 양의 상관관계(r=.40, p<.001)를 보였으나, 역할갈등과는 음의 상관관계(r=-.21, p<.001)가 있는 것으로 나타났다. 임상적 의사결정능력에 미치는 영향은 근무경력, 교육수준, 전문직 자율성과 역할갈등이었으며, 임상적 의사결정능력에 대한 설명력은 38.7%였다. 임상적 의사결정 능력을 개선하기 위해 중요한 변수인 전문직 자율성과 역할갈등을 통해 간호사의 직무를 결정할 수 있는 임상적 의사결정능력을 향상시킬 필요가 있다. 그러므로, 본 연구 결과가 간호사의 임상적 의사결정능력 향상 프로그램 개발의 근거자료로 활용될 수 있을 것으로 기대하며, 개발된 교육프로그램의 타당성과 효과성을 분석하는 중재연구의 필요성을 제안하는 바이다.

남자 간호대학생의 임상실습의 경험 (Clinical Practice Experience of Men in Nursing as a Student)

  • 본 연구는 임상실습을 경험한 남자 간호대학생의 임상경험을 심층적으로 살펴봄으로써 임상 실습의 의미와 본질을 이해하고 남자 간호대학생의 임상 실습이 진로설정에 어떠한 영향을 미치는지에 대한 기초자료를 얻고자 함이다. 본 연구의 참여자는 현재 H대학교 간호학과 재학 중인 남학생으로 임상 실습 경험이 있는 3학년 3명과 4학년 남학생 3명을 눈덩이 표집으로 선정하였으며, Colaizzi의 현상학적 연구방법에 따라 의미 있는 진술을 분석하여 남자 간호학생들의 임상 실습 경험의 의미를 파악하는 것이다. 연구결과 3개의 범주로 구분되며, 6개의 주제, 14개의 의미 있는 진술로 도출되었다. 이는 '병동실습에서 경험한 부정적인 간호이미지', '감당해야 할 스스로의 짊', '특수파트 실습경험에서 찾은 간호의 매력'으로 남자 간호학생들은 남자 간호대학생이라는 이유로 차별을 경험했으며, 그로 인해 직업에 대한 갈등과 혼란을 일으키고 있었다. 하지만, 그 속에서도 간호사의 전문성을 확인하고 배움의 즐거움을 찾아가고 있었다. 본 연구결과를 통해 남자 간호학생들의 임상 실습 경험에 대한 총체적인 시각을 제시할 수 있을 것으로 사료된다. 또한 남자 간호대학생의 학과 적응과 임상 실습 적응에 대한 상담과 지도에 유용한 기초자료로 활용 될 수 있으며, 남자 간호학생의 졸업 후 진로결정 상담에서도 유용한 자료로 활용되기를 기대한다.

Machine Learning-Based Prediction of COVID-19 Severity and Progression to Critical Illness Using CT Imaging and Clinical Data

  • Objective: To develop a machine learning (ML) pipeline based on radiomics to predict Coronavirus Disease 2019 (COVID-19) severity and the future deterioration to critical illness using CT and clinical variables. Materials and Methods: Clinical data were collected from 981 patients from a multi-institutional international cohort with real-time polymerase chain reaction-confirmed COVID-19. Radiomics features were extracted from chest CT of the patients. The data of the cohort were randomly divided into training, validation, and test sets using a 7:1:2 ratio. A ML pipeline consisting of a model to predict severity and time-to-event model to predict progression to critical illness were trained on radiomics features and clinical variables. The receiver operating characteristic area under the curve (ROC-AUC), concordance index (C-index), and time-dependent ROC-AUC were calculated to determine model performance, which was compared with consensus CT severity scores obtained by visual interpretation by radiologists. Results: Among 981 patients with confirmed COVID-19, 274 patients developed critical illness. Radiomics features and clinical variables resulted in the best performance for the prediction of disease severity with a highest test ROC-AUC of 0.76 compared with 0.70 (0.76 vs. 0.70, p = 0.023) for visual CT severity score and clinical variables. The progression prediction model achieved a test C-index of 0.868 when it was based on the combination of CT radiomics and clinical variables compared with 0.767 when based on CT radiomics features alone (p < 0.001), 0.847 when based on clinical variables alone (p = 0.110), and 0.860 when based on the combination of visual CT severity scores and clinical variables (p = 0.549). Furthermore, the model based on the combination of CT radiomics and clinical variables achieved time-dependent ROC-AUCs of 0.897, 0.933, and 0.927 for the prediction of progression risks at 3, 5 and 7 days, respectively. Conclusion: CT radiomics features combined with clinical variables were predictive of COVID-19 severity and progression to critical illness with fairly high accuracy.

Analysis of prescription frequency of herbs in traditional Korean medicine hospital using electronic medical records

  • Objectives: To analyze the prescription frequency of various herbs as either individual or major herbs (in terms of dosage) and their usage patterns in the treatment of different diseases for standardization of traditional Korean medicine. Methods: We analyzed the prescription database of patients at the Pusan National University Korean Medicine Hospital from the date of establishment of the hospital to February 2013. The complete prescription data were extracted from the electronic medical records of patients, and the prescription frequencies of individual herbs, particularly, of major herbs, were analyzed in terms of gender, age, and international classification of diseases (ICD) code. Results: The prescription frequency of individual herbs based on age and gender showed a similar pattern. Herbal mixtures were also distributed in a similar manner. The use of some herbs differed according to age and gender (Table 1.). The herbs that were used at high frequencies for a given ICD code had similar usage patterns in different categories. However, some major herbs in the "Jun (King)" category were used uniquely for a given ICD code (Table 2.). There was significant difference between male and female on ICD code E and N, but the other ICD codes had small differences. The ratio of herbal medicine by gender showed different usage patterns in each gender. Conclusions: The findings of our study provide fundamental data that reflect the real clinical conditions in South Korea, and therefore, can contribute to the standardization of TKM.

Clinical application of anti-M$\ddot{u}$llerian hormone as a predictor of controlled ovarian hyperstimulation outcome

  • Objective: In 2009 anti-M$\ddot{u}$llerian hormone (AMH) assay was approved for clinical use in Korea. This study was performed to determine the reference values of AMH for predicting ovarian response to controlled ovarian hyperstimulation (COH) using the clinical assay data. Methods: One hundred sixty-two women who underwent COH cycles were included in this study. We collected data on age, basal AMH and FSH levels, total dose of gonadotropins, stimulation duration, and numbers of oocytes retrieved and fertilized. Blood samples were obtained on cycle day 3 before gonadotropin administration started. Serum AMH levels were measured at a centralized clinical laboratory center. The correlation between the AMH level and COH outcomes and cut-off values for poor and high response after COH was analyzed. Results: Concentration of AMH was significantly correlated with the number of oocytes retrieved (OPU; r=0.700, p<0.001). The mean${\pm}$SE serum AMH levels for poor ($OPU{\leq}3$), normal ($4{\leq}OPU{\leq}19$), and high ($OPU{\geq}20$) response were $0.94{\pm}0.15$ ng/mL, $2.79{\pm}0.21$ ng/mL, and $6.94{\pm}0.90$ ng/mL, respectively. The cut-off level, sensitivity and specificity for poor and high response were 1.08 ng/mL, 85.8%, and 78.6%; and 3.57 ng/mL, 94.4%, and 83.3%, respectively. Conclusion: Our data present clinical reference values of the serum AMH level for ovarian response in Korean women. The serum AMH level could be a clinically useful predictor of ovarian response to COH.

The Intelligent Clinical Laboratory as a Tool to Increase Cancer Care Management Productivity

  • Studies of the causes of cancer, early detection, prevention or treatment need accurate, comprehensive, and timely cancer data. The clinical laboratory provides important cancer information needed for physicians which influence clinical decisions regarding treatment, diagnosis and patient monitoring. Poor communication between health care providers and clinical laboratory personnel can lead to medical errors and wrong decisions in providing cancer care. Because of the key impact of laboratory information on cancer diagnosis and treatment the quality of the tests, lab reports, and appropriate lab management are very important. A laboratory information management system (LIMS) can have an important role in diagnosis, fast and effective access to cancer data, decrease redundancy and costs, and facilitate the integration and collection of data from different types of instruments and systems. In spite of significant advantages LIMS is limited by factors such as problems in adaption to new instruments that may change existing work processes. Applications of intelligent software simultaneously with existing information systems, in addition to remove these restrictions, have important benefits including adding additional non-laboratory-generated information to the reports, facilitating decision making, and improving quality and productivity of cancer care services. Laboratory systems must have flexibility to change and have the capability to develop and benefit from intelligent devices. Intelligent laboratory information management systems need to benefit from informatics tools and latest technologies like open sources. The aim of this commentary is to survey application, opportunities and necessity of intelligent clinical laboratory as a tool to increase cancer care management productivity.

임상실습 중 간호대학생이 경함하는 무례함, 소진, 전공만족도 및 자기효능감과의 관계 (Relationship Between Rudeness, Burnout, Major Satisfaction and Self-efficacy which Nursing Students Experience During Clinical Practice)

  • 본 연구는 간호대학생을 대상으로 임상실습 중 경험하는 무례함, 전공만족도, 자기효능감 및 소진간의 관계를 알아보고, 질적인 임상실습을 위한 기초자료를 제공하기 위하여 시도되었다. 본 연구는 상관성 조사연구로서, 간호학과에 재학 중인 임상실습 경험이 있는 간호대학생 4학년 210명을 대상으로 실시하였으며, 자료수집기간은 2016년 10월 1부터 10월 15일까지였다. 자료는 SPSS Program20.0을 이용하여 Correlation coefficient, regression으로 분석하였다. 간호대학생이 경험한 무례함은 소진과 정적 상관관계를, 전공만족도 및 자기효능감은 소진과는 부적 상관관계를 나타냈다. 간호대학생의 소진에는 무례함, 전공만족도, 자기효능감이 영향력을 나타냈다. 결론적으로 무례함을 감소하기 위한 병원과 학생, 임상실습지도자 및 병원의 유기적인 관계형성 및 대처 능력 향상을 위한 방안이 필요하다.

RN-BSN 과정 학생의 간호실무 수행능력 분석 (Analysis of RN-BSN Students' Clinical Nursing Competency)

  • Purpose: The purpose of this study was to investigate RN-BSN students' clinical nursing competency in order to establish baseline data for developing nursing competency based clinical education for RN-BSN students. Method: A survey of 1,453 RN-BSN students from 21 nursing schools was conducted using a self administered questionnaire. Result: The mean score of the clinical nursing competency was 2.93. The scores for competency were shown as 2.91 for nursing management, 2.94 for developing professionalism & legal implementation, 2.95 for critical thinking, 2.96 for teaching & leadership, and data collection, basic nursing care, and communication were above 3.00. The items perceived as insufficient competency were physical examination and observation & monitoring in data collection, cardiopulmonary resuscitation, psycho-social care, spiritual care, hospice in basic nursing care, application of knowledge and theory, formulating nursing diagnosis, nursing care planning in critical thinking, education material development, leadership, delegation in teaching and leadership, analysis of organization, planning, infection control, role & job description, evaluation of nursing activities in nursing management, quality improvement, and research in developing professionalism and legal implementation. Conclusion: This study will contribute to developing a nursing competency based on clinical education for RN-BSN students who have various education needs and clinical backgrounds.
