구개파열 아동의 음음변동에 관한 연구 (Phonological Process of Children with Cleft Palate)

  • Background and Objectives : Children with cleft palate children may be imparied in articulation and resonance. This study examined the phonological process usage of 3-, 4- and 5- year old children with cleft palate. Materials and Method : Twenty seven children with cleft palat participated 3-, 4- and 5-year old children with cleft palate. The authors performed speech evaluation using picture consonants test for children with cleft palate. Percentage of consonants correct(PCC), mean value of each phoneme depends on articulation site and manner were evaluated. Results : In place of articulation, ommission of velar consonants were the most frequent. In manner of articulation, ommission of nasal consonants were the most frequent. Backing, glottal stop, was the most prominent phonological process children with cleft palate. Conclusion : These results may indicate that articulation disorder with cleft palate. and other articulation disorders differences should be considered in the interpretation of speech evaluations.

  • The communicative disorders in cleft palate patients have relationship with the acoustic and He physiological phenomena. Particularily hypernasality is a parameter of cleft palate speech that has been studied by many clinicians and speech pathologists. The degree of hypernasality has been assessed by the listener,s judgement, but perceptual assessements have poor scientific reliability, so objective instruments have been needed to test hypernasality with diagnostics accuracy. This study was analyzed the nasalance score using a Nasometer for cleft palate patients. The simple vowels /a/, /i/, /e/ and the approximants /j/, /w/ were tested for the degree of hypernasality after operation. The phrases containing long and short duration times were used in this study to asses hypeernasality. Fiberopic views shows the open velopharyngeal port that resulted in hypernasality of cleft palate patients. The authors assert the important of the management of cleft palate patients.

Double Opposing Z-Plasty 수술 후의 점막하 구개열 아동의 말소리 개선에 관한 연구 (Speech Outcomes of Submucous Cleft Palate Children With Double Opposing Z-Plasty Operation)

  • Background and Objectives : The operation Double Opposing Z-Plasty, has been used for improving VPI function in the submucous cleft palate. However, few reports on the effects of the speech change were presented. The purpose of this study is to compare the difference of nasality and nasalance, parents satisfaction between before and after this operation and to consider how much improvement in speech. Materials and Methods : Ten submucous cleft palate children who underwent double opposing Z-plasty were analyzed. We retrospectively studied nasalance, auditory perception (nasality) with hypernasality, patients satisfaction, speech evaluation by using charts review, video tape, telephone interview. Results : In 8 patients of 10 submucous cleft palate, hypernasality reduced and speech intelligibility was higher and mean 0.35 point was increased in the velum length after operation. After operation, nasality was improved (2.0 point) and level of nasal emission decreased. Regarding satisfaction of this operation, scale was mean 2.8 (5 point-scale) : 8 parents were satisfied in the resonance, 3 parents were satisfied articulation. The reason of dissatisfaction was mostly compensatory articulation. Conclusion : To improve of speech in the submucous cleft palate, speech therapy afterthis operation as well as successful surgery should be considered.

구개열 언어의 비음화에 관한 공기역학 및 음향학적 연구 (An Aerodynamic and Acoustic Study of Nasalization in Cleft Palate Speakers.)

  • Cleft palate patients have general speech problems with resonance disorders and articulation disorders. The aim of this study is to find the aerodynamic and acoustic characteristics of the nasalization in cleft palate speakers. Thirteen control groups and three cleft palate patients pre- and post operation were selected for these studies. The test words are composed by polysyllabic words: consonants between high vowel /i/ analysis. The cleft palate patients repeated test words pre- and post-operation from one, three and six month periods. The subjects repeated test words on Macquirer and on Nasometer Model 6200-3. The aerodynamic and acoustic results of nasalization show as follows: (1) The nasal rate in overall airflow of aspirated consonant for cleft palate patients shows higher levels than that of the control group. It had decreased since one month after operation. (2) The overall airflow of cleft palate patients is higher than in the control group, however oral air pressure is lower than control group. (3) The nasal airflow and the nasal rate in overall airflow of cleft palate patients has higher than the control group, however its decreased after operation. (4) The nasalance scores of cleft palate patients were 40% higher than that of the control group. The scores did not decrease after operation. The nasalance score of lateral and fricative sounds did not decrease after operation.

  • Submucosal type cleft palate is subdivision of cleft palate. Because of late detection, the treatment - for example, the operation or the speech therapy - for the submucosal type cleft palate patient usually late. In this study, we want to find the objective characteristics of submucosal type cleft palate patient, comparing with the normal and the complete cleft palate patient. Experimental groups are 10 submucosal type cleft palate patients who got the operation in our hospital, 10 complete cleft palate patients. And, 10 normals as control group. The sentence patterns using in this study is simple 5 vowels. Using CSL program we evaluate the Formant, Bandwidth. We analized the spectral characteristics of speech signals of 3 groups, before and after the operation.

Perceptual Speech Assessment after Maxillary Advancement Osteotomy in Patients with a Repaired Cleft Lip and Palate

  • Background : Maxillary hypoplasia refers to a deficiency in the growth of the maxilla commonly seen in patients with a repaired cleft palate. Those who develop maxillary hypoplasia can be offered a repositioning of the maxilla to a functional and esthetic position. Velopharyngeal dysfunction is one of the important problems affecting speech after maxillary advancement surgery. The aim of this study was to investigate the impact of maxillary advancement on repaired cleft palate patients without preoperative deterioration in speech compared with non-cleft palate patients. Methods : Eighteen patients underwent Le Fort I osteotomy between 2005 and 2011. One patient was excluded due to preoperative deterioration in speech. Eight repaired cleft palate patients belonged to group A, and 9 non-cleft palate patients belonged to group B. Speech assessments were performed preoperatively and postoperatively by using a speech screening protocol that consisted of a list of single words designed by Ok-Ran Jung. Wilcoxon signed rank test was used to determine if there were significant differences between the preoperative and postoperative outcomes in each group A and B. And Mann-Whitney U test was used to determine if there were significant differences in the change of score between groups A and B. Results : No patients had any noticeable change in speech production on perceptual assessment after maxillary advancement in our study. Furthermore, there were no significant differences between groups A and B. Conclusions : Repaired cleft palate patients without preoperative velopharyngeal dysfunction would not have greater risk of deterioration of velopharyngeal function after maxillary advancement compared to non-cleft palate patients.

점막하 구개열 치료에 있어 Furlow 구개성형술 전후 언어 치료의 유용성 (Usefulness of Speech Therapy for Patients with Submucous Cleft Palate Treated with Furlow Palatoplasty)

  • Furlow palatoplasty has been favored by many plastic surgeons as the primary treatment for the velopharyngeal insufficiency associated with submucous cleft palate. The purpose of this article is to introduce an efficacy of Furlow palatoplasty and speech therapy performed on patients who were diagnosed belatedly as having submucous cleft palates. From 2002 to 2004, four submucous cleft palate patients over 5 years of age with velopharyngeal insufficiency received Furlow palatoplasty. The patients were evaluated through the preoperative perceptual speech assessment, nasometry, and videonasopharyngoscopy. Postoperatively, two patients achieved competent velopharyngeal function in running speech. One of the remaining two could achieve competent velopharyngeal function with visual biofeedback speech therapy and the other could not use her new velopharyngeal function in running speech because of her age. Speech therapy can correct the articulation errors and thus improve the velopharyngeal function to a certain extent by eliminating some compensatory articulations that might have an adverse influence on velopharyngeal function. This study shows that Furlow palatoplasty can successfully correct the velopharyngeal insufficiency in submucous cleft palate patients and speech therapy has a role in reinforcing surgical result. But age is still a restrictive factor even though surgery was well done.

포르만트 위치비교를 이용한 구개열 환자의 발음분석 (Sound Analysis of Cleft Platate Patinents Using Formant Position)

  • As one of the main purpose of the physical management of cleft palate is to provide for the anatomic and physiologic requisites for speech, the speech must be as one of the criteria for determining when physical management has been achieved. But there is no objective methods to evaluate the speech of cleft palate patients. The authors tried to analyze the speech of adult cleft palate patients using sound spectrog raphy and compared with normal adults. The results were obtained as follows ; 1. In Vowels, cleft palate patients of both sexes showed reduction of frequency of the first and second formant as compared to normal. There was minimal difference in front vowels (i, e, ae) 2. In consonants, cleft palate patients showed reduction of frequency of the first formant in both sexes but reduction of frequency of the second formant was noticed only in fe- male patients. 3. There was no statistical difference in sound spectrograph between plosive, fricative, africative, nasal, and glide consonants.

구개파열 영유아의 언어발달 특성 연구 (Language Development in Cleft Palate Infants)

  • The purpose of the present study was to investigate the characteristics of receptive and expressive language development of cleft palate infants aged under 3. Twenty-six cleft palate infants and 52 normal infants were grouped into 3 chronological age groups with 1 year intervals: less than 1, 1, and 2 years old. The cleft palate infants were divided into 2 groups: cleft palate only, and cleft lip & palate. Each mother of the infants was asked to complete the questionnaire, Sequenced Language Scale for Infants (SELSI). Receptive and Expressive language scores of SELSI were computed. The scores of receptive and expressive language were respectively analyzed into 4 categories of language: phonology/prosody, semantics, syntax, and pragmatics. The results, concerning the differences of language development between the cleft palate and the normal infants, were as follows: (1) expressive language scores were significantly different at age 2 between the cleft palate and the normal infants; (2) cleft plate groups aged less than 1 and 1 showed lower scores of phonology/prosody of expressive language than the normal groups; (3) cleft palate group aged 2 showed lower scores than the normal group in semantics, syntax, and pragmatics. The results, concerning the characteristics of language development between the cleft palate only and the cleft lip & palate infants, were as follows: (1) the scores of expressive and receptive language were not statistically different between the 2 groups; (2) both groups did not show any difference in .the scores of phonology/ prosody, semantics, syntax, and pragmatics of receptive and expressive language.

Furlow 구개성형술을 시행한 구개열에서 언어발달과 적절한 수술시기 (Speech Outcome and Timing of Furlow Palatoplasty in Cleft Palate)

  • Palatoplasty using Furlow's double-opposing Z-plasty has been performed from June, 1995 to September, 1999 at Seoul National University Children's Hospital. The goal of this study is to determine the optimal timing of repair and cleft severity affecting velopharyngeal function. This is the retrospective study of patients operated by the second author. The mean age of patients was 10.53 months. The patients could be divided into three groups-isolated cleft palate(n=70), unilateral cleft lip and palate(n=88), and bilateral cleft lip and palate(n=42). To evaluate the velopharyngeal function, we used two parameters, speech evaluation and cineofluorography using DSR(digital subtraction radiography). Also, to determine the relevance between cleft severity and speech development, we measured the distance between maxillary tuberosities and cleft margins. Among 200 patients, about 96% had no or minimal hypernasality and 87% had no or mild nasal emission. The cleft width and length of soft palate seemed not to be related with the speech development. Palatoplasty at the age under 12 months resulted in less 'nasal emission' and better 'articulation' of the parameters that were assessed at the age of 7 years. It can be concluded Furlow palatoplasty shows satisfactory results and also it seems that it is better to perform the operation before the age of 12 months.
