Our medical care system is trying to diversify in order to meet the client's needs, and to adjust to a medical environment which is changing very rapidly. Because current nursing theory and practice focus on holistic care, health care management, education, and research, contrary to the traditional emphasis on only assisting a physician, more autonomy and specialization for the implementation of nursing are required. Considering these trends and actual needs, the category of clinical nurse specialist should be established as soon as possible. In order to develop strategies for implementing this new professional specialty, the authors conducted a field survey and literature review of the current system in Korea. As a result, various obstacles and constraints were discovered as follows : 1) There are few accredited educational programs for the training of CNS's. 2) Several hospitals already have staff designated as clinical nurse specialist (CNS) even though the term CNS is not yet standardized or adopted in nationwide. 3) The role of the CNS is not clearly understood by the medical societies, or even nursing societies. A nurse who works in specific nursing areas such as central supply, kidney dialysis, intensive care, coronary care, etc. for a long time, considers herself /himself a CNS. Based upon the above findings, the following alternatives are recommended. 1) The role of the CNS should be defined according to specified functions and authority : professional autonomy ; counselling and educating patients and their familes, nurses, and even other medical personnel ; research on improvement of nursing ; and management of the nursing environment including medical resources, information, and cases. 2) the qualification of CNS should be attained only by a nurse who has an RN license and clinical experience of more than 3 years in a specific nursing field: passes a qualifying examination; and contributes to the professional development of peers, colleagues, and others. A master's degree should only be optional, because of the insufficient of graduate programs which are well designed for the CNS. 3) The CNS should initially be a head nurse rather than line staff in order to deal with as wide an experience base as possible. 4) The nursing specialty could be divided into two areas such as a clinical field and a community field. The clinical field could then be categorized by the Styles' classification such as diseases and pathogenics, systems, ages, acuity, skills/techniques, and function/role ; the community field could be classified according to work site.
In the 18th century, Xu Lingtai (徐靈胎) and Yoshimasu Todo (吉益東洞) were medical revolutionaries. They emphasized researches about synthesis of formulae, efficacy of medication and observation and then classification of clinical phenomena, so they assumed a modern scientific character. But, there were clear differences between their academic thoughts. In this paper, we examine the causes of difference in three fields, i.e. traditional culture, viewpoints of talented people and academic personality. The first, difference was due to traditional culture. Chinese medicine has a long history and heavy traditional culture. Yin-Yang (陰陽) theory, Five Phase(五行) theory, Viscera and Bowels (臟腑) theory and Meridian and Collateral (經絡) theory stemmed from everyday practice, and Chinese people learn these theories from experience and observation. From the standpoint of Chinese people, particularly scholarly doctors [儒醫] such as Xu Lingtai, it was easy to debate medical theories. In contrast, Japanese traditional culture didn't have as long a history as China. Thus as a necessity, it was harder to disseminate traditional Chinese medicine theories in Japan. Yoshimasu Todo simplified it by cutting out the superfluous traditional Chinese medicine theory, so at that time it must have been shocking to the Japanese medical world's trends. The second, difference was due to viewpoints of talented experts. From the standpoint of Xu Lingtai, above all, medicine is just a learning, only a kind of technique, even more not a means of living. Xu Lingtai was concerned with the appearance of very talented experts such as 'great man' (偉人), and 'exceptional man' (奇士) who carried out medical research. Instead of cultivating a few talented people, Yoshimasu Todo tried to produce a large number of clinicians quickly who could treat ordinary people. The third was due to personality difference. As Xu Lingtai threw away Confucianism and studied medicine in his youth, although he had a critical attitude, he was always mild-mannered. Yoshimasu Todo always had a clearly critical and rebellious nature. Personality influenced their literary spirit and learning style, so although both advocated reactionism, the academic thought of Xu Lingtai was reformative and mild, while that of Yoshimasu Todo was revolutionary and fierce. Xu Lingtai and Yoshimasu Todo had considerably similar research domains and academic thought, so it is proper for them both to serve as examples for making a comparative study of medical history in China and Japan in 18th century.
The Objectives of this study were to identify family nursing phenomena at the community in Korea and to contribute to build up family domain of International Classification for Nursing Practice. The method of this study was used retrospective one among three methods to develop ICNP during the period from April 1997 to June 1999. The procedure was to choose nursing phenomena using preliminary terms(stepl) from the reports on family nursing care of the nursing students of 5 junior colleges of nursing and 5 colleges of nursing. The study group members identified 3 common family nursing phenomena with 5 characteristics related to each phenomenon. In order to consensus the appropriate characteristics of a phenomenon(step2), 17 study group members had regrouped nursing phenomena and scored its characteristics 5 times. The essential characteristics of each family phenomenon were selected above 3.5 mean score from related characteristics(step 3). Finally, 17 phenomena were named preferred terms such as following, that was selected after investigated preliminary terms(step4). Family nursing phenomena in Korea are named as Lack of family interaction in community. Social isolation. Lack of social support system in community. Disturbance in parent role, Disturbance in marital role, Dissatisfaction of sexual life, Disturbance in family communication, Inappropriate family coping, Lack of family intimacy, Inappropriate family power structure, Family violence. Unhealthy life style. Deficit of financial management skill and support. Inadequate care a sick member. Insecure safety and hygiene in neighborhood, Inadequate home-sanitation. Inadequate home-making. Family nursing phenomena in Korea were partially confirmed family architecture of ICNP, Beta version. by this study. Further study on Family nursing phenomena in Korea will be required to support evidence through literature review of nursing classifications or field studies.

Objective : The purpose of this study was to evaluate radiographic/clinical outcomes of adolescent idiopathic scoliosis (AIS) patients treated by a Korean neurosurgeon. Methods : Ten AIS patients were treated by a single neurosurgeon between January 2011 and September 2013 utilizing segmental instrumentation with pedicle screws. Basic demographic information, curve pattern by Lenke classification, number of levels treated, amount of correction achieved, radiographic/clinical outcomes [by Scolisis Resarch Society (SRS-22r) questionnaire] and complications were evaluated to determine the surgical results. Pulmonary function test was utilized to assess forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) before and after surgery. Results : The average percentage of correction of the major structural curve was 73.6% (ranged from 64% to 81.5%). Preoperative and final postoperative absolute FVC averaged 3.03 L and 3.76 L (0.73 L increase, p=0.046), and absolute FEV1 averaged 2.63 L and 3.49 L (0.86 L increase, p=0.021). Preoperative and final postoperative average self-image and function scores of SRS-22r were, $2.6{\pm}0.5$, $3.3{\pm}0.1$, $4.0{\pm}0.5$, and $4.6{\pm}0.0$, respectively. There was a significant improvement of the self-image and function scores of SRS-22r questionnaires before and after surgery (p<0.05). There was no case of neurological deficit, infection and revision for screw malposition. One patient underwent a fusion extension surgery for shoulder asymmetry. Conclusion : Radiographic/clinical outcomes of AIS patients treated by a Korean neurosurgeon were acceptable. Fundamental understanding of pediatric spinal deformity is essential for the practice of AIS surgery.

BACKGROUND/OBJECTIVES: This study was conducted to examine relationships between dietary habits and intakes of antioxidants and B vitamins and the risk of ischemic stroke, and to compare dietary factors according to the presence of cerebral artery atherosclerosis and stroke subtypes. SUBJECTS/METHODS: A total of 147 patients and 144 control subjects were recruited consecutively in the metropolitan area of Seoul, Korea. Sixty participants each in the case and control groups were included in analyses after 1:1 frequency matching. In addition, 117 acute ischemic stroke patients were classified into subtypes according to the Trial of Org 10172 in Acute Stroke Treatment (TOAST) guidelines. Dietary intake was measured using a semi-quantitative food frequency questionnaire composed of 111 food items and plasma lipid and homocysteine levels were analyzed. RESULTS: When compared with control subjects, stroke patients had unfavorable dietary behaviors and lower intakes of fruits ($73.1{\pm}83.2g$ vs. $230.9{\pm}202.1g$, P < 0.001), vegetables ($221.1{\pm}209.0g$ vs. $561.7{\pm}306.6g$, P < 0.001), and antioxidants, including vitamins C, E, $B_6$, ${\beta}$-carotene, and folate. The intakes of fruits, vegetables, vitamin C, and folate were inversely associated with the risk of ischemic stroke after adjusting for confounding factors. Intakes of vegetables, vitamins C, $B_6$, $B_{12}$, and folate per 1,000 kcal were lower in ischemic stroke with cerebral atherosclerosis than in those without. Overall vitamin $B_{12}$ intake per 1,000 kcal differed according to the TOAST classification (P = 0.004), but no differences among groups existed based on the post-hoc test. CONCLUSIONS: When compared with control subjects, ischemic stroke patients, particularly those with cerebral atherosclerosis, had unfavorable dietary intake, which may have contributed to the development of ischemic stroke. These results indicate that proper dietary recommendations are important for the prevention of ischemic stroke.

BACKGROUND/OBJECTIVES: In Oriental medicine, certain foods may be beneficial or detrimental based on an individual's constitution; however, the scientific basis for this theory is insufficient. The purpose of this study was to investigate the effect of body constitution, based on the Sasang type of Korean traditional medical classification system, on the bioavailability of soy isoflavones of Cheonggukjang, a quick-fermented soybean paste. SUBJECTS/METHODS: A pilot study was conducted on 48 healthy Korean men to evaluate the bioavailability of isoflavone after ingestion of food based on constitution types classified by the Sasang typology. The participants were classified into the Taeeumin (TE; n = 15), Soyangin (SY; n = 15), and Soeumin (SE; n = 18) groups. Each participant ingested 50 g of Cheonggukjang per 60 kg body weight. Thereafter, blood was collected, and the soy isoflavone metabolites were analyzed by ultra-performance liquid chromatography/quadrupole time-of-flight mass spectrometry. Ntrikinetic analysis of individual isoflavone-derived metabolites was performed. RESULTS: Our nutrikinetic analysis identified 21 metabolites derived from isoflavones in the blood samples from 48 healthy Korean men (age range, 21-29 years). Significant differences were observed in the time to maximum concentration (Tmax) and elimination half-life (t1/2) for nine metabolites among the three groups. The Tmax and t1/2 of the nine metabolites were higher in the SE group than in the other groups. Moreover, the absorption rates, as determined by the area under the plasma-level curve (AUC) values of intact isoflavone, were 5.3 and 9.4 times higher in the TE group than in the SY and SE groups, respectively. Additionally, the highest AUC values for phase I and II metabolites were observed in the TE group. CONCLUSIONS: These findings indicate that isoflavone bioavailability, following Cheonggukjang insgestion, is high in individuals with the TE constitution, and relatively lower in those with the SE and SY constitutions.
Purpose: Fatty liver is one of the most commonly found disease by abdominal ultrasonography. The status of fatty liver is classified into mild, moderate and severe degrees. The study was conducted to investigate the clinical significance of fatty liver using ultrasonography. Materials and method: Test set consisted of 2,185 patients who visited D healthcare center in Daejeon to receive an abdominal ultrasonic test from January to December 2007. Out of the 2185 patients, 524 patients was diagnosed as fatty liver (290 male and 234 female patients). They were divided into three groups, group I for mild degree. II for moderate degree, and III for severe degree, depending on the echo of liver parenchyma, the sound attenuation, and the visibility of intrahepatic blood vessels and diaphragm. Then the correlation of obesity indices, liver function tests and metabolic syndrome was analyzed for males and females separately. Results : As for the degree of fatty liver, 350 cases (66.8%) were classified as group I, 153 cases (29.2%) as group II, and 21 cases (4.1%) as group III. In addition, severe degree of fatty liver was more frequently found in males than in females. The mean ages of three groups for males were 46.1, 44.5, and 39.1, and those for females were 48.8, 50.2, 52.4, respectively. Males with lower mean ages have severely of fatty liver for both males and females. Conclusion: The results in this study show that the classification into three degrees of fatty liver in ultrasonography practice is helpful to treat and observe the progress of fatty liver. In addition, careful examination is required to measure the severity of fatty liver as well as detection of it. A standardized method to classify the degree of fatty liver is also needed for more objective measurement.

A new proposed repository has a final capacity of 800,000 drums radioactive waste. Most of foreign repositories have a general practice of segregating control zones which mainly contributes to classification of degree of control, whether it is called buffer zone or not. Domestic regulatory requirements of establishment of buffer zone in a repository are not much different from those of nuclear power plants for operation period, in which satisfactory design objective or performance objective is the most important factor in determination of the buffer zone. The meaning of buffer zone after closure is a minimum requested area which can prevent inadvertant intruders from leading to non-allowable exposure during institutional control period. Safety assessment with drinking well scenario giving rise to the highest probability of exposure among the intruder's actions can verify fulfillment of the buffer zone which is determined by operational safety of the repository. At present. for the repository to be constructed in a few years, the same procedure and concept as described in this paper are applied that can satisfy regulatory requirements and radiological safety as well. However, the capacity of the repository will be stepwise extended upto 800,000 drums, consequently its layout will be varied too. Timely considerations will be necessary for current boundary of the buffer zone which has been established on the basis of 100,000 drums disposal.
Monovision is defined as correcting one eye for distance vision and the other eye for near vision. Single-vision contact lenses are used for each eye. The patient selectively suppresses one eye while using the other eye. This study was performed to measure binocular function and patient satisfaction with monovision induced by adding +3.00D to spectacle prescription for distance vision in nonpresbyopic students. This study comprised 144 nonpresbyopic students with monovision induced by adding +3.00D to spectacle prescription. Monovision was induced by undercorrecting the nondominant eye by adding 3.00 diopters for near vision and correcting the dominant eye with emmetropia for distance vision. For research of the binocular function on Monovision. This study was tested the obstacle course, water pouring test, catching and throwing, bead classification including the stereopsis test. These tests were comparative to the normal binocular state. As a results, the stereopsis have a significant reductions in Monovision. There was no significantly difference between monovision group and control group in monovision exercise (obstacle course, water pouring test, catching and throwing, and bead sorting) at near and distance. This study indicates that monovision is an effective and reasonable therapeutic modality for correcting presbyopia. By the application of above methods, this study could be used the student for the practical practice and understand on the Monovison.
Purpose: The purpose of this study was to evaluate students and optometrist opinion of the usefulness of different subjects of optometry curriculum.This date will then be used as base-line data during a process of curriculum development Methods: 292 of ophthalmic optic's students and 123 of optometrists were surveyed in the usefulness of curriculum (five-point scale) followed by statistical analysis of the results. Results: Optometrists gave a higher average score for curriculum usefulness (3.89) than the students (3.71). The subject which showed the highest usefulness for the student was optometry (4.55) but clinical practice was the highest for the optometrist (4.48). There was significant differences (p<.05) on optics, ophthalmology, basic science, optometrist related subject and foreign language but optometry and contact lenses showed high usefulness in common without significant difference. The usefulness of subject for the student showed significant difference according to gender, grade, age, and desired working place and has demonstrated significant difference depend on gender, age, scholarship, total working experience as an optometrist, workplace, and position for optometrists as well. Conclusions: It is necessary to continue research for the development of standard and bench-marked curriculum for ophthalmic optics and also the curriculum, which reflecting the usefulness and needs obtained from ophthalmic optics' students and optometrists need to be developed.
