  • Objectives : To determine the effectiveness of Chuna manual therapy for headache Methods : We searched 6 electronic databases(Pubmed, CAJ, Oasis, RISS, DBPIA, KoreanTK) and 2 journals up to Oct 2015. We included randomized controlled trials(RCTs) using Chuna manual therapy for headache. The methodological quality of each RCT was assessed by the Cochrane risk of bias tool. Results : 9 RCT studies were eligible in our review. The meta-analysis of 6 studies showed favorable results for the use of Chuna manual therapy. High risk of bias were observed in all studies. Conclusions : Although there are favorable results with meta-analysis, our systematic review arehighly dependent on the single source of Chinese electrical database, CAJ. Now limited evidence is available tosupport Chuna manual therapy for headache and further well-designed RCTs should be encouraged.

  • This study aims to demonstrate the effectiveness of facial chuna manual therapy in treating the sequelae of Bell's palsy. Four patients were treated with facial chuna manual therapy once a week for 4 weeks, consisting of facial muscle massage, acupoint pressure, contracture chuna, and synkinesis chuna. The changes in symptoms (contracture and sysnkinesis) were measured using the Sunnybrook Facial Grading Scale (SFGS), Synkinesis Assessment Questionnaire (SAQ), Facial Disability Index (FDI), Contracture/Synkinesis scale using a facial scanning system, Numeric Rating scale (NRS) for synkinesis or contracture, and Was It Worth It questionnaire. After treatments, SFGS, Contracture/Synkinesis scale, and NRS for synkinesis or contracture showed significant improvements. SFGS increased in three cases from 39~76 to 52~85 score. SAQ decreased in two cases from 53.33~57.78 to 40.00~55.56. FDI increased in three cases from 120~128 to 138~145. These results suggest that Korean medicine treatment, including facial chuna manual therapy can be effective in improving the sequelae of Bell's palsy.

  • Objectives : This study aimed to report the evidence of difficulty, safety, and frequency of the use of Chuna Manual Therapy. Methods : The survey questions were developed using the consensus from a professor who majored in Rehabilitation Medicine of Korean Medicine (RMKM). November 26th to December 31st 2018, the questionnaire was given to education training members of the Korean Society of Chuna Manual Medicine for Spine & Nerves by paper. Twenty-nine(23%) of the questionnaires were retrieved. Difficulty, safety, and frequency of the use of Chuna Manual Therapy were reported based on the questionnaire. Results : In this study, the more difficult techniques were observed to be less safe. In particular, the adjustment techniques applied to the cervical and lumbar spine was answered with high difficulty. Although these techniques are high difficulty and low safety, the reason for its versatility also suggests that the technique has a high therapeutic effect and necessity. Conclusions : This is the first consensus on experts'opinions on Chuna Manual Therapy for education. We hope that this report is helpful for Korean medicine doctors who operate technique and expected to make clinical evidence of Chuna Manual Therapy.

  • Objectives This study investigated the trends in domestic and international clinical research in craniosacral therapy, classified as a type of Chuna manual therapy, and suggested further directions in Korean medicine. Methods This scoping review was performed using the Arksey and O'Malley methodological framework and preferred reporting items as per the preferred reporting items for systematic reviews and meta-analyses extension for scoping reviews checklist. Eight electronic databases (PubMed, EMBASE, Cochrane Library, Koreanstudies Information Service System [KISS], KMBASE, Oriental Medicine Advanced Searching Integrated System [OASIS], Research Information Sharing Service [RISS], ScienceON) were searched to identify articles with the search terms "craniosacral therapy" and "cranial osteopathy" until December 2021. Results Forty-five studies were eligible as per our inclusion criteria. Most research studies (n=44) were conducted in the field of medicine and pharmacy, especially in rehabilitation medicine (n=16). As a result of the study design, randomized controlled trials (n=20) were the most common, and chronic pain (n=9) was the most frequently targeted disease, followed by headache (n=7). Thirty-two studies suggested interventions and 20 studies used Upledger's 10-step protocol. The average duration of craniosacral therapy was 41 min per session, administered 1.4 times per week. Outcome measurements were analyzed and categorized with the examination procedure for the patient. Conclusions This is the first scoping review of craniosacral therapy in Korea, and we believe that our findings could support its utility as Chuna. In the future, more studies should be conducted to establish the evidence of clinical efficacy of craniosacral therapy and develop standard techniques in Korean medicine.

  • Objectives This study aimed to investigate the status of Chuna manual therapy(CMT) usage before and after COVID-19 social distancing measures. Methods This study utilized Health Insurance data from April 2019 to May 2023. Quarterly data were collected for Simple Chuna (code 40710), Complex Chuna (code 40720;50% patient expense), Complex Chuna (code 40721;80% patient expense), and Special Chuna (code 40730). Results During the period covered by health insurance, the number of CMT patients was 8,315,838, with 19,332,786 instances of CMT, and a total cost of 544,651,407 won. Due to COVID-19, the application of CMT decreased in the first quarter of 2020, but recovered from 2021, showing an increase in usage by the first quarter of 2023. The decline in the use of all medical institutions in the fourth quarter is believed to be due to restrictions on health insurance coverage for CMT. Conclusions Further research on CMT health insurance is necessary. This research should inform discussions on policies aimed at expanding health insurance coverage for CMT.
