A Study on multifidus muscle activation by Needle EMG during shoulder flexion in Chronic Low Back Pain Patients (침 근전도로 측정한 만성 요통 환자의 어깨 굴곡시 나타나는 다열근 활성도 비교)

  • Purpose : The purpose of study is activation of lumbar multifidus muscle by needle EMG during shoulder flexion in chronic low back pain patients. The subject were consisted of 10 women patients with chronic low back pain and healthy asymtomatic subject 10 women. Methods : 10 women patients with chronic low back pain and healthy asymptomatic subject 10 women is voluntary participated for the research. Subjects were positioned in standing. The needle EMG were measured activation of multifidus. Needle electrode was used to 28 gauge. The shoulder flexion movement used to activate the multifidus was then measured. Results : Results of the analysis showed that asymptomatic subjects had significantly larger multifidus muscle activation compared with CLBP subjects during shoulder flexion. Conclusion : This study will be used as multifidus measurement method of patient with chronic LBP. The multifidus muscle in chronic LBP patient clinical significance. Most of chronic LBP patients have multifidus contraction pattern. Therefore chronic LBP patients necessary multifidus activation measurement with needle EMG.

A Correlational Study on the Mastery and Depression in Chronic Arthritis Patients (만성 관절염 환자를 대상으로 한 극복력과 우울의 상관성 조사)

  • Purpose: The purpose of this study was to investigate the relationship between the level of mastery and depression in chronic arthritis patients. Method: The subjects for this study were 100 patients registered in S University Hospital, and the period of data collection was from June 20, 2006 to August 30, 2006. Results: The cronbach's ${\alpha}$ of the research instruments were .70-.86. In data analysis, SPSSWIN 12.0 program was used for descriptive statistics. The results were as follows. 1) The range of total mastery scores was from 11 to 28 and the mean score of the depression in chronic arthritis patients was 17.88. 2) The range of total depression scores was from 20 to 72 and the mean score of the depression in chronic arthritis patients was 39.99. 3) The level of mastery was significant correlation with the level of depression in chronic arthritis patients(r=-.466, p<.01). Conclusion: Mastery had significant correlation with depression in patients who have chronic arthritis. Therefore, the strategy of nursing intervention which improve mastery must be developed for patients who have chronic arthritis.

A Study of Pain, Depression and Self-Efficacy According to the Classifications of Pain among Chronic Pain Patients (만성통증환자의 통증분류에 따른 통증, 우울 및 자기효능감 정도)

  • Purpose: this study was to examine the differences of the level of pain, depression and self-efficacy according to the classifications of pain among chronic pain patients. Method: The data were collected by means of self-reported questionnaire from 164 patients with chronic pain visited in one university hospital and one local pain clinic in Busan, from October 7 to November 16, 2002. Analysis was done by ANOVA, and Scheffe test using SPSS program. Result: The subjects were divided into five classifications of chronic pain : 26.2% low back and extremity pain, 23.2% neck, shoulder and upper extremity pain, 19.5% postherpetic neuralgia, 15.9% complex regional pain syndrome and 15.2% peripheral neuralgia. There were significant differences in pain (p=.000), depression (p=.000) and self-efficacy (p=.003) according to the 5 kinds of chronic pain. With the results of the Scheffe test, the patients with peripheral neuralgia experienced pain and depression higher than those with the other kinds of chronic pain. The patients with neck, shoulder and upper extremity pain experienced self-efficacy higher than those with peripheral neuralgia. Conclusion: Chronic pain patients should be provided effective individualized intervention depending on the classifications of chronic pain. Therefore the development of interventions for pain management according to the classifications of pain in chronic pain patients is needed.

  • Purpose : The purpose of this study is change of lumbar multifidus muslce recorded simultaneously by ultrasound imaging during upper extremity functional movement in chronic low back pain patients. The subject were consisted of 10 women patients with chronic low back pain and healthy asymptomatic subject 10 women. Methods : 10 women patients with chronic low back pain and healthy asymptomatic subject 10 women is voluntary participated for the research. Subjects were positioned in standing. Multifidus size were measured from L4 vertebral segement. The ultrasound imaging apparatus(Sonoace 6000, Medison, Korea) was epuipped with a 5-MHz convex array transducer. The upper extremity lifting movement used to activate the multifidus was then measured. Results : Results of the analysis showed that at the L4 vertebral leves, healthy asymptomatic subjects had significantly larger multifidus muscle compared with chronic LBP subjects. Conclusion : This study will be used as treatment method of patient with chronic LBP. The multifidus muscle in chronic LBP patients clinical significance. Most of chronic LBP patients have multifidus contraction pattern. Especially multifidus contraction in L4 vertebral segement. So chronic LBP patients necessary multifidus muscle release treatment.

Estimate over the Number of Chronic Disease Patients and Medical Care Expenditure at the Time of Transition of Baby Boomer into 65 Years Old Aging Population (베이비붐세대가 65세 노인인구로 전환 시의 만성질환 환자수와 진료비 예측)

  • Purpose: The purpose of study is to estimate the number of chronic disease patients and medical care expenditure at the time baby-boomers belong to 65 years old aging population, and compare with current 65 year-old aging population. Methods: Analysis method used an estimating formula devised by the researcher and estimated the number of chronic disease patients and medical care expenditure of each generation. Results: When comparing the estimated number of chronic diseases patients of each generation, 40.6% of the first generation, 76.4% of the second generation, 95.2% of third generation are expected to get chronic disease. When comparing each generation's total medical care expenditure, based on the estimated number of chronic diseases patients of each generation, the second generation( 1,206,251,224 thousand won) showed higher than other generation. This study compared the number of chronic disease patients and medical care expenditure between the second generation of the elderly and current elder generation. As a result, the second generation patients was higher than the fourth generation in high blood pressure, diabetes, psychological and behavioral disorder, and neurological diseases whereas the fourth generation is only high the number of patients in heart disease. As for total medical care expenditure, the second generation paid more in high blood pressure, psychological and behavioral disorder while the fourth generation in neurological disease and heart disease. Conclusion: It is desired that considering the number of chronic disease patients and medical care expenditure of baby boomers accounting for 14.6% of total population, in-depth follow-up study is carried out that inquires into what are issues with a current chronic disease management project, what business is needed in order to manage these issues, and how to fund to cover increasing medical care expenditure.

  • With the medical progress that has given spinal cord injured(SCI) individuals greater longevity and better overall health, chronic pain is emerged as major challenge in treating this population. According to past reports, estimates of prevalance of severe/disabling chronic pain in SCI patients have ranged from 18% to 63%. In etiologies of chronic pain in SCI patients, psychic or psychogenic pain categories should be included and more recent data have demonstrated that the persistant pain is directly related to higher levels of psychosocial distress and impairment. Recently, neurophysiological classification of the SCI pain syndrome into three etiologic groups(a; mechanical pain, b; radicular pain, c; deafferentation pain) is more frequently adopted for the classification of chronic SCI pain syndrome. The deafferentation pain is most common of the pain syndromes associated with SCI. After cervical epidural anesthesia for the surgical intervention of decubitus ulcer on the hip of two SCI patients, there were much reduction of existing chronic deafferentation character pain.

Nursing Need of Patients with Chronic Illness - A Primary Study for Development for the Nursing Center of Chronic Illness - (만성질환자의 간호요구 사정 : 만성질환자 간호 센터 모형 개발을 위한 1차 연구)

  • The purpose of this descriptive study was to identify the nursing needs of patients with chronic illness. Method: The subjects of this study were 636 patients with chronic illness, 323 general nurses, and 106 public health nurses. The instruments used for this study were questionnaires including perceived functions of nursing from the center for chronic illness, preference to placement, intention to use, needs to receive services from the center for chronic illness, strategies management for nursing, and nursing needs of chronically ill patients. Results: The mean of perceived functions for nursing from the center were 3.1(0.5) in public health nurses, 2.9 (0.59) in general nurses, and 2.4(1.33) in chronically ill patients. Regards of needs to receive on services of the nursing centers were, the regularly physical examination, for health educational services which was perceived highest request amongst chronically ill patients. We found the means of each specific need as 2.2(0.6), for physical health, 2.1(0.7), for psychosocial health, and 1.8(0.6) for spiritual health. Conclusion: From the results of this study, it is suggested that establishing a nursing center for chronically ill patients consider physical, psychosocial, for spiritual health needs of chronically ill patients. It is also a consideration that direct care for symptom management and health education in the nursing center be implemented.

The Evaluation of Permanent Impairment in Chronic Pain Patients (통증질환의 장애인정 여부에 대하여)

  • Pain is one of the most common causes of disability and work loss. However, patients with chronic pain have not been regarded as impaired persons in the Republic of Korea because of a lack of objective findings in those patients and difficulties in validating their genuine suffering and quantitative evaluation of pain. Here, the difficulties in the evaluation of impairment in chronic pain patients are discussed. In addition, guidelines for rating impairment in chronic pain patients, which are under development by the Korean Academy of Medical Sciences, are introduced.

Avidity of serum immunoglobulin G antibodies to Porphyromonas gingivalis in the elderly with chronic periodontitis

  • The present study aimed at evaluating serum immunoglobulin G (IgG) avidity to Porphyromonas gingivalis in elderly patients with mild and severe chronic periodontitis. The avidity of antibodies against P. gingivalis present in the sera of 18 patients with mild chronic periodontitis and 18 patients with severe chronic periodontitis was evaluated using an ammonium thiocyanate-dissociated enzyme-linked immunosorbent assay (ELISA). The results showed that the mean absorbance value in serum IgG antibody titers was significantly higher in the severe chronic periodontitis group than in the mild chronic periodontitis group ($198{\pm}35ELISA$ unit [EU] vs. $142{\pm}32EU$, p < 0.01). However, there was no significant difference between the two groups in antibody avidity ($65{\pm}57EU$ vs. $54{\pm}27EU$). These findings suggest that humoral immune responses to P. gingivalis between mild and severe chronic periodontitis in elderly patients are characterized by the differences in the quantity rather than the quality of the antibodies.

The Effect of Cigarette Smoking on Physical Fitness and Depression of Patients with Chronic Low Back Pain

  • Purpose: This study was conducted to investigate the effect of cigarette smoking on physical fitness (dominant hand grip power, dominant isokinetic leg muscle strength, abdominal muscle endurance, flexibility, cardiopulmonary endurance) and depression in patients with chronic low back pain. Methods: This study was a cross sectional study and subjects consisted of 60 young males with chronic low back pain. The subjects were allocated to two groups following a self-report survey: cigarette smoking group (n=25) or non-smoking group (n=35). Physical fitness (dominant hand grip power, dominant isokinetic leg muscle strength, abdominal muscle endurance, flexibility, cardiopulmonary endurance) were measured using objective methods and depression in patients with chronic low back pain was measured using the Korean version of center for epidemiologic studies depression (CES-D) scale. Results: The results of this study were as follows: In physical fitness, the cigarette smoking group showed a significant decrease in abdominal muscle endurance, flexibility, and cardiopulmonary endurance compared with the non-smoking group. Depression index (CES-D scale score) was significantly higher in the cigarette smoking group than in the non-smoking group. Conclusion: These results suggest that cigarette smoking had a negative effect on abdominal muscle endurance, flexibility, and cardiopulmonary endurance in patients with chronic low back pain. In addition, depression of patients with chronic low back pain was affected by cigarette smoking. Thus, we suggested that cigarette smoking may play a significant role in the deterioration of physical fitness and depression of chronic low back pain patients.
