Is Ossiculoplasty Necessary in Canal Wall Down Mastoidectomy? Comparison of Clinical Outcomes Between Type 0 Tympanoplasty and Ossiculoplasty

  • Background and Objectives: To assess whether the audiological and clinical outcomes of type 0 tympanoplasty (T0) performed using cartilage were comparable with those of ossiculoplasty in patients who underwent canal wall down mastoidectomy (CWDM). Subjects and Methods: This study included patients who had chronic otitis media with cholesteatoma and underwent CWDM with ossiculoplasty involving partial ossicular replacement prosthesis (PORP), total ossicular replacement prosthesis (TORP), or T0. Anatomical success rates and hearing outcomes were analyzed. Results: Seventy-two patients were included in this study; 29 of them underwent CWDM with T0, 27 underwent CWDM with PORP, while 16 underwent CWDM with TORP. The difference in mean improvement in the air-bone gap (ABG) between the groups was not significant. The differences in the rates of ABG closure to ≤10 dB HL (p=0.030) and ≤20 dB HL (p=0.029) were significant. There were significant differences in improvements in the ABG at 3 kHz among the PORP, TORP, and T0 groups. Conclusions: The audiological outcomes of CWDM with ossiculoplasty seemed to be better than those of CWDM with T0 with no significant difference in the incidence of complications following ossiculoplasty and T0.

A Case of Hunter Syndrome Presented with Chronic Purulent Rhinorrhea in 24-month-old Boy (24개월에 만성 화농성 비루를 주소로 내원하여 진단된 헌터 증후군 1례)

  • Hunter syndrome(Mucopolysaccharidosis type II, MPS type II) is an X-linked disorder of glycosaminoglycans (GAGs) metabolism caused by an iduronate-2-sulfatase (IDS2) deficiency. A 24-month-old boy visited the department of pediatrics with the chief compliant of chronic purulent rhinorrhea beginning at age one. He had a history of repeated acute otitis media and chronic rhinitis. On physical examination he had a coarse face, enlarged tongue, distended abdomen, joint stiffness, and Mongolian spots at his first visit. The urine GAGs level was elevated at 66.10 mg/mmolCr (reference range, <11.1) and iduronate-2-sulfatase activity in leukocyte was decreased at 0.21 nmol/mg protein/hr (reference range, 18.7-57). Finally with an IDS gene mutational analysis, recombinant known mutation between intron 7 and distal of exon 3 in IDS2 was detected. Recombinant iduronate-2-sulfatase therapy was started without any infusion related reactions. The author highlights the importance of suspecting Hunter syndrome when pediatric patients visit with chronic purulent rhinorrhea which is a common cause of hospital visits for infants and children.

Total Ear Canal Ablation and Lateral Bulla Osteotomy for Chronic Otitis Externa and Media in Dogs: Postoperative Recovery and Long-Term Follow-up (개에서 만성외이염과 중이염에 대한 전이도 적출술과 외측 고포절골술: 수술 후 회복과정과 장기간 예후)

  • The clinical results of 10 consecutive total ear canal ablation combined with lateral bulla osteotomy (TECA-LBO) in six dogs with chronic otitis externa and media were evaluated by the postoperative recovery and long-term follow-up. All dogs were selected for TECA-LBO on the basis of following five clinical symptoms. First, medical treatment couldnt improve clinical signs at least for eve. 2 months. Second, tympanic membrane was completely disappeared. Third, radiopacity was increased in tympanic cavity. Forth, petrous temporal bone was sclerosed. Fifth, ear canal calcification was progressed. And all cases were satisfied all five clinical symptoms. At 14. days after operation, the preoperative symptoms of chronic otitis externa and media which were scratching ear, pain, and hardening ear canal were resolved, and postoperative swelling, erythema, head tilt, and exudate from Penrose drainage were not existed In all cases. Loss of eye blink was happened in 4 cases, but these were disappeared between 14 days a(ter operation except one case on 3 months. All dogs were discharged form hospital at 14 days after operation. Between 3.5 and 6.5 months after discharging from hospital, para-aural abscessation was happened only in all Cocker spaniels. But this complication was solved by ventral bulla osteotomy (VBO). The dogs which didn't show para-aural abscessation after TECA-LBO didn't show scratching, pain, hardening of ear and hearing ability was improved, at 7.5 months after TECA-LBO. And the dogs which showed para-aural abscessation after TECA-LBO also didn't show scratching, pain, hardening of ear and hearing ability was also improved, at 7.5 months after VBO. In conclusion, After TECA-LBO, all dogs were recovered well without complication within 2 weeks except Cocker spaniel. And loss of eye blink can be cured naturally within 2 weeks after surgery. And para-aural abscessation can be happened between 4 and 7 months after TECA-LBO, so surgeon must follow-up until 8 months.

In oriental medicine, the case of sinusitis can be explained, not only external invasion such as PUNG HAN(wind-cold), PUNG YUL(wind-hot), SHUP YUL(damp-heat), but also functional disorder of internal organ such as spleen(脾), lung(肺), kidney(腎) The western medicine classified the cause of sinusitis as two factors. That is bacterial infection factor-Haemophilus influenza, streptococcus pyogeues, streptococcus pneumonia- and viral infection factor-Rhinovirus, parainfluenza, Echo28, Coxsacki21, Sinusitis is complicated to allergic rhinitis, chronic otitis media purulent, chronic tonsilitis, sinubronchitis. On the condition of nasal septum deformity, turbinates deformity, nasal septum deviation, sinusitis can be developed. the predisposing factors of sinusitis is swimming, air pollution, malnutrition, shortage of immunity.2. According to survey, sinusitis occurred that children from 4 to 12 years old and from 5 to 7 years old occupied 70% 3. From the past history data, they experienced chronic tonsillar hypertropy(20%), otitis media, atopic dermatitis, allergic rhinitis, bronchial asthma, pneumonia, bronchiolitis, chronic sore throat, urticaria, milk allergy in sequence. 4. the symptoms of sinusitis is nasal obstruction, postnasal dripping, purulunt(yellow)or white discharge, cough, nose bleeding in sequence. nasal obstruction take the portion of 95%, postnasal dripping 65%, night time or early morning cough 60%. 5. The suffering period of sinusitis is 6 month minimum, 4 years maximum, most cases are included in a year. The suffering period of children was shorter than adult. 6. Diagnosis depend on inspection of nasal cavity, postnasal dripping, X-ray finding. 75% of patient(15case) showed both maxillary sinusitis, 25%(5cases) showed left or right maxillary sinusitis. 7. Treatment of oriental medicine, consist of Herb-medicine, acupuncture and exposing of Lazer beam. Kamihyunggyeyungyotang(加味荊芥蓮翹湯) is administered mainly as the medical therapy, Kamigwaghyangjeungkisan(加味藿香正氣散) Kimizwakwieum(加味左歸飮), Kamihyangsosan(加味香蘇散) is administered for a additional symptoms which occurred by influenza recurrence. Kamijeonxibackchulsan(加味錢氏白朮散) is administered to treat gastro-intestine trouble patients who have sinusitis. 8. The period of treatment is varied with patient conditions and X-ray finding. The minimal period is 35days, maximal period is 202days. So it took about 86days in average and about 50% of patient(10cases) is recovered in one or two month.

  • Stellate ganglion block(SGB) is frequently performed in pain clinics. Facial palsy produces asymmetry of the face, lagophthalmos and incomplete eyelid closure. Exposure keratitis and eyeball pain can be induced by prolonged lagophthalmos. SGB was performed on a 51-year-old female patient who had exposure of keratitis and eyeball pain due to facial nerve palsy after an operation for chronic otitis media. After 31 stellate ganglion blocks, eyeball pain and keratitis nearly subsided. SGB is an effective method to treat lagophthalmos caused by facial nerve palsy.

A case of improved Wegener's granulomatosis with Oriental-Western Medicine Treatment (한 양방 동시 치료를 통해 호전된 베게너 육아종증 1예)

  • Objectives : The aim of this study is to report the improved case of Wegener's granulomatosis with Oriental-Western Medicine Treatment. Methods : The patient was treated by herbal medicine(Manhyeongja-san, Banhabaekchulchenma-tang, Jaeumgeonbi-tang), acupuncture and moxa constantly. And she was also treated by glucocorticoids treatment and immunosuppressive therapy provided by department of Rheumatology. The otorhinolaryngologic and the ophthalmologic Western medication treatment were also supplied. The information was collected retrospectively. Results & Conclusions : Chronic otitis media of both ear and hearing loss were improved by Oriental-Western medicine treatment. Ophthalmagia was controlled as less. Dizziness and facial palsy were also disappeared. Oriental-Western medicine treatment may be effective on Wegener's granulomatosis patient when pain controling and fast improvement of symptoms are needed.

Pathogensis of Benign Paroxysmal Positional Vertigo (BPPV) (양성돌발성 두위현훈의 역학)

  • Benign paroxysmal positional vertigo (BPPV) is one of the most common clinical entities encountered in a dizzness clinic. Retrosepective review was performed for the patient's diagnosed as BPPV at Sunchon "S" hospital dizzness center. Variables for statistical analysis included age, sex, involved canal presence of recent head tramua, presence of chronic otitis media, history of middle ear surgery, underlying disease such as hypertension or diabetics, headache, central lesion. From July 2004 to May 2005, we sampled the 600 dizzness patient's who visited the dizzness center. Dizzness could be classified into BPPV, vestibulopathy. Among these patients, 256 patients had BPPV, 222 patients had vesibulopathy and 97 patients had Normal. Among these BPPV patients, 80 patients had lateral canalolithiasis BPPV (31.3%), 28 patients had lateral cupulolithiasis BPPV (10.9%), 90 patients had posterior canalolithiasis BPPV (35.2%), 7 patients had multicanalolithiasis BPPV (2.3%), 52 patients simultaneously had BPPV, other vestibular disease, and central lesion (20.3%).

  • The cholesteatoma is usually associated with chronic purulent otitis media and destroys the surrounding structures and may cause dangerous complications. The cholesteatoma with a positive fistula sign may be observed occasionally, if a softened area is formed in the bony capsule over a semicircular canal which exposed the membranous structure of the canal. Recently, the authors have experienced a case of cholesteatoma combined with labyrin thine fistula in a 22-year old male patient. We present this case with a brief review of the literatures.

A Clinical Observation of the Nasal Acupunture Therapy on Nasal Obstruction (비침이 비색증에 대한 영향)

  • We studied 32 patents, who had visited Kyung Hee downtown oriental hospital with nasal obstruction. They has been observed their nasal cavity through anterior rhinoscopy. We had paten't middle and inferior turbinte bleeding by acupuncture. This symptom had been disappeared after this treatment. The result were as follows. 1. Sex and age distribution: The males were 18($56.2\%) and Female were 14($43.8\%$). The most common occurrence was found between 1-5 years old. 2. The most common duration of disease was between 2-6 months. 3. In decreasing order, the common complication & past history of otolaryngologic or allergic disease were allergic rhinitis, atopic dermatitis, chronic sinusitis, adenoid or tonsil hypertrophy, otitis media with effusion and asthma. 4. In descending order, distribution of symptoms were rhinorrhea, sneezing, dry sensation, sputum, frontal headach, postnasal drip and cough. 5. The improvement rate in symptom were $31.3\%\;excellent\;and\;53.1\%$ good.
