노인의 만성 무릎 통증에 대한 전기 근육 자극요법의 적용 효과 (The Effect of Electrical Muscle Stimulation Therapy on Chronic Knee Pain for Aged)

  • Purpose: This study is to examine the effect of electrical muscle stimulation therapy on chronic knee pain for aged. Method: Design was randomized one-group pretest-posttest design. Samples were total 15 elderly on 60 years old and above with chronic knee pain. Measures were S-F McGill Pain Questionnaire and Arthritis Impact Measurement Scale for knee pain. Electrical muscle stimulation therapy, experimental treatment was applied on chronic knee pain for 4 weeks, 3 times/week, 15 min/time. Data were collected from half March 2005 to May 2005. Data were analyzed using SPSS PC+ 12 version. Descriptive statistics was used for analysis of general characteristics in sample, and paired t-test was used to analysis the effect of electrical muscle stimulation therapy. Results: After receiving the electrical muscle stimulation therapy chronic knee pain was significantly decreased (t=-29.163, P=.000 in S-F MPQ; t=-37.005, P=.000 in AIMS). Conclusion: Electrical muscle stimulation therapy can be a better effective primary nursing intervention on chronic knee pain for aged in community.

  • Purpose: This study was to examine the effect of electrical muscle stimulation therapy on chronic knee pain and depression for the aged. Method: Design was one-group pretest-posttest design. Samples were 32 elderly of 60 years old and above with chronic knee pain and depression. Measures were the S-F McGill Pain Questuionnaire for chronic knee pain and the Korean Elderly Depression Scale for depression. Electrical muscle stimulation therapy, experimental treatment, was applied for 12 weeks, 3 times/week, 15 min/time. Data were collected from January 2005 to May 2005. Data were analyzed using SPSS PC+ 12 version. Results: After receiving electrical muscle stimulation therapy, chronic knee pain (t=-88.034, P=.000) and depression (t=-114.659, P=.000) were significantly decreased. Conclusion: Electrical muscle stimulation therapy can be a better effective primary nursing intervention on chronic knee pain and depression for community dwelling elders.

Effects of Lumbar Mobilization and Transcutaneous Electrical Nerve Stimulation on Proprioception and Muscular Strength in Volleyball Players with Chronic Knee Pain

  • Background: The spinal nerves, which start at the lumbar level, are connected to the nerve innervation in the knees. Currently, there is a lack of research on the treatment of knee pain through lumbar mobilization. Objectives: To investigate the effects of lumbar joint mobilization (LJM) and transcutaneous electronic nerve stimulation (TENS) on proprioception and muscular strength in volleyball players with chronic knee pain. Design: Two group pre-posttest. Methods: A total of 26 professional volleyball players with chronic knee pain were allocated to the LJM (n=13) and TENS (n=13) groups. In the LJM group, grade III - IV amplitude was applied 3 times for 1 minute (80 times per minute) at the affected lumbar (L2-3) facet joint in the prone position. In the TENS group, the TENS treatment device was used to directly apply or 15 minutes to the area of chronic knee pain (100 Hz, 150 ㎲). Proprioception was measured by knee flexion and extension angles, and muscle strength was evaluated using an isokinetic test. Measurements were taken before and after interventions. Results: In the eye opened conditiond, proprioception significantly increased during both knee extension and flexion after LJM, while only knee extension was significantly increased in the TENS group. There was also a significant difference in knee extension between the two groups. In the eye close conditiond, proprioception was significantly improved only during knee extension in the LJM group, and the difference in knee extension between the groups was also significant (P<.05). The maximum torque of the affected knee joint was significantly improved at 60°/sec in both groups (P<.05); however, there was no difference between the two groups. There was no significant difference in the maximum flexion torque within or between the groups. Conclusion: This study suggests that LJM improved proprioception and muscular strength in volleyball players with chronic knee pain.

Alcohol neurolysis of genicular nerve for chronic knee pain

  • Radiofrequency neurolysis (RFN) of the genicular nerves has recently become accepted as an effective technique to alleviate knee pain particularly in patients with knee osteoarthritis (OA) or postoperative pain. However, genicular nerve RFN can produce high procedure and equipment costs, longer procedural times, procedure-related pain, and failure rate of over 25%. We are presenting two cases of alcohol neurolysis of the genicular nerve using fluoroscopy and ultrasonography in patients with knee OA or persistent postsurgical pain of the knee. Alcohol neurolysis of the genicular nerve with dual imaging modality can be a cheap, safe and effective method in patients with chronic knee pain.

  • Purpose: This study was to examine the effects of electrical muscle stimulation therapy on chronic knee pain in the elderly. Method: The research design was a one-group pretest-posttest design. The subjects were 45 (TE: 17, SE: 15, SY: 13) elderly, 65 years old and above with chronic knee pain. Pain was measured by the S-F McGill Pain Questuionnaire (S-F MPQ) and Arthritis Impact Measurement Scale (AIMS). Electrical muscle stimulation therapy experimental treatment was applied for 12 weeks, 3 times/week, 15 min/time. Data was collected from March 2005 to February 2006. Data was analyzed using the SPSS PC+ 12 version. Results: After receiving electrical muscle stimulation therapy, chronic knee pain in TE (S-F MPQ: t=-62.143, p=.000, AIMS: t=-29.155, p=.000), SE (S-F MPQ: t=-76.345, p=.000, AIMS: t=-39.323, p=.000), and SY (S-F MPQ: t=-43.691, p=.000, AIMS: t=-30.306, p=.000) groups were significantly decreased. Conclusion: Electrical muscle stimulation therapy can be a better effective primary nursing intervention for chronic knee pain for community dwelling elderly people with TE, SE, and SY.

전기 자극이 여성 노인의 만성 무릎통증, 운동자기효능 및 삶의 질에 미치는 효과 (Effects of Electrical Stimulation Therapy on Chronic Knee Pain, Exercise Self-Efficacy, and Quality of Life in Korean Elderly Women)

  • Purpose: This study was to examine the effects of electric stimulation therapy on chronic knee pain, exercise self-efficacy, and quality of life in Korean elderly women. Methods: The design was an unequivalent control pretest-posttest study. Samples were total of 60 (experimental: 30, control: 30) elderly women with healthy cognitive and communication abilities aged 65 years old and above. The experimental treatment involved was the electric stimulation of both thigh quadriceps muscles for 15 minutes per treatment, 3times per a week, for a total of 12 weeks. Measurements taken were S-F MPQ and AIMS for chronic knee pain, exercise self-efficacy measurement for exercise self-efficacy, and S-F 36 scale for quality of life. Data were analyzed using SPSS PC+ 12. Results: Chronic knee pain according to the Short-Form McGill Pain Questionnaire (t=43.563, p=.000) and Arthritis Impact Measurement Scale (t=31.364, p=.000) were significantly decreased in the experimental group by the application of electrical stimulation therapy. Exercise self-efficacy (t=107.116, p=.000) and quality of life (t=76.429, p=.000) were significantly increased in the experimental group by the application of electrical stimulation therapy. Conclusion: Electrical stimulation therapy could be a more effective primary nursing intervention in decreasing chronic knee pain, and on increasing exercise self-efficacy and quality of life for Korean elderly women.

  • Purpose: This study was to examine effects of muscle electric stimulation on chronic knee pain, activities of daily living, and living satisfaction for Korean elderly women. Methods: The design was a nonequivalent control group pretest-posttest study. Subjects were 60 (experimental: 30, control: 30) elderly women 65 years old or above with good orientation and communication. The experimental treatment was electric stimulation on both thigh quadriceps muscles for 15 minutes per time, 3 times per week, for a total of 12 weeks. Measures were the S-F McGill Pain Questionnaire and Arthritis Impact Measurement Scale for chronic knee pain, activities measurement of daily living for activities of daily living, and living satisfaction measurement for living satisfaction. Data was analyzed through the SPSS Win 12.0. Results: Chronic knee pain by S-F MPQ (t=43.563, p=.000) and chronic knee pain by AIMS (t=31.364, p=.000) were significantly decreased in the experimental group, and the activities of daily living (t=124.353, p=.000) and living satisfaction (t=71.268, p=.000) were significantly increased in the experimental group for Korean elderly women. Conclusion: Muscle electrical stimulation decreased chronic knee pain, and increased the activities of daily living and living satisfaction for Korean elderly women. Further studies for muscle electric stimulation need to be done.

좌골신경가동화기법이 만성 요통 환자의 통증과 슬관절 신전근력에 미치는 영향 (Effects of Sciatic Nerve Mobilization Technique on Perceived Pain and Knee Strength in Patient With Chronic Low Back Pain)

  • The purpose of this study was to determine the effect of sciatic nerve mobilization technique on perceived pain, straight leg raise test (SLR), and strength of knee extensor, location of symptoms (LOS) in patients with chronic low back pain. 22 patients with chronic low back pain were recruited for this study. The subjects were randomly assigned to either the experimental group (EG) or the control group (CG), with 11 patients in each group. All patients received a routine physical therapy (hot pack and transcutaneous electrical nerve stimulation). The mobilization technique of the sciatic nerve was performed for 10 min in the case of the EG subjects. Outcome measurements included the level of the perceived pain, SLR, and strength of the knee extensor, LOS. The measurements were recorded 3 times: before the intervention, after the intervention, and at 1 hour of follow up. The two groups did not significantly differ with regard to the level of perceived pain, SLR, and strength of the knee extensor, LOS before the test (p>.05). In the case of the EG subjects, all the variables measured after the intervention significantly differed from those measured before the intervention (p<.05). However, in the case of the CG subjects, a significant difference was noted only with regard to the level of perceived pain (p<.05). The findings indicate that sciatic nerve mobilization technique exerts a positive effect on the control of subjective symptoms and knee strength in patients with chronic low back pain. Further studies are required to generalize the result of this study.

일선 교사들의 무릎관절 통증에 영향을 미치는 요인 (A study on related factors of knee joint pain in teachers)

  • This study examined on the causes of chronic pain in the knee joint that was related to the aspects of teacher's job and daily lives. The subjects of investigation in this study include 237 of teachers who work at the elementary, middle and high schools. The chronic pain of knee joint of the subjects' is mainly related to their stress, and also to the vehicle that they normally use(p<0.05). This study aims at grasping the frequency of the pain which is caused by a knee joint's stress when teachers teach in a standing-up posture for many hours, looking at the aspects in the pain of knee joint that are related to job, being conducive to the development of the pain-cure, and, in addition, presenting an effective countermeasure to related agencies in order to prevent such diseases as arthritis.

만성 관절 통증 여성 노인환자에 있어 관절염 태극권의 효과에 대한 pilot study (The Effects of Tai Chi for Arthritis on Chronic Arthritic Pain of Senior Female : A Pilot Study)

  • Objectives : Tai Chi for arthritis was developed in 1997 by Paul Lam, M.D. of family medicine in Australia. It is an exercise treatment program and a good approaching method for arthritis. However its application is limited to arthritis of knee joint only. Even there have been no clinical study in korean traditional medical society. The aim of this study is to investigate effect of the Tai Chi for arthritis on chronic pain of body. Methods : We designed this trial as objectives were (1) senior female over 60 years, (2) suffering for osteoarthritis and chronic body pain, (3) have no physical or mental dysfunctions (4) able to walk and exercise alone. Objectives mactched these criteria had been trained the Tai Chi for arthritis 2 hours a day for 21 weeks. Before and after training, pain of 8 joints (neck, shoulder, elbow, wrist, waist, hip, knee, ankle) were estimated with the VAS(Visual Analog Scale). Results : Training the Tai Chi for arthritis relieved multiple joint pain(especially neck, lower back, shoulder, wrist and knee) and these were statistically significant. Conclusions : This study is a pilot study to investigate effect of the Tai Chi for arthritis on chronic pain of whole body. Absence of control group, and other scale to test joint function except VAS were limitations of this study. It is the first clinical approach about Tai Chi in korean medicine field, also the first study of Tai Chi for pain of the whole body.
