Subungual Squamous Cell Carcinoma of the Toe (A Case Report) (족지에 발생한 조갑하 편평 상피 세포암 (1예 보고))

  • Nail bed malignancies are rare entities. Most nail bed malignancies are squamous cell carcinoma (SCC)s. Less than 10% of subungual SCCs occur in the foot. Fifty percent occurred on the hallux and approximately 25% on both the fourth and fifth digits. The correct diagnosis is often delayed because nail bed malignancies are frequently mistaken for benign or infectious processes. SCC on extremities is hard to distinguish from the benign lesion like chronic ulcer, fistula caused by chronic osteomyelitis, and abscess fistula. Attention should especially be paid in diagnosing the subungual lesion because paronychia is a common disease. SCC is the most common carcinoma second to malignant melanoma as a soft tissue malignant tumor in the West, and it involves mainly the head, neck and upper extremities but rarely involves lower extremities, particularly the toes. The authors emphasize the importance of a biopsy for chronic nonhealing lesions by presenting this case.

The effect of coronavirus disease-19 knowledge, anxiety, and psychosocial well-being on the infection prevention behavior in college students in health care (보건의료계열 대학생의 코로나바이러스감염증-19 지식, 불안, 사회심리적 건강이 감염예방행위에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effect of coronavirus disease-19 (COVID-19) knowledge, anxiety, and psychosocial well-being on the infection prevention behavior of college students in health care. Methods: This study is a descriptive survey research. A total of 301 college students at four health care departments in three regions were surveyed using a structured questionnaire from February 10 to February 16, 2022. The collected data were analyzed using descriptive statistics, independent t-test, and one-way ANOVA. Pearson correlation coefficients and multiple regression analyses were done using SPSS ver. 18.0. Results: Knowledge on COVID-19 was significantly dependent on a family history of chronic disease (p=.049). Anxiety was significantly dependent on test of COVID-19 (p=.040). Gender (p=.049), perceived physical and mental health status (p=.000), and chronic disease (p=.000) had significant effects on infection prevention behavior. When the level of anxiety was higher, the infection prevention behavior was also higher (p=.000) and the psychosocial well-being was improved (p=.017). Stepwise multiple regression analysis revealed that anxiety significantly improved the infection prevention behavior (𝛽=.396, p<.001) and psychosocial well-being (𝛽=-.139, p=.008), which accounts for 18.5% (F=22.444, p<.001) of the variance. Conclusion: In order to prevent infectious diseases such as COVID-19 and its spread, measures to prevent infection and improve the psychosocial well-being should also be sought.

Prevalence of honeybee (Apis mellifera) disease in Daejeon (대전광역시 양봉농가의 꿀벌질병 감염률 조사)

  • This study was conducted to investigate the prevalence of honey bee (Apis mellifera) disease in Daejeon. From May to September in 2014, 63 samples were collected from 63 apiculture farms in the regions and reverse transcriptase-polymerase chain reaction (RT-PCR) and polymerase chain reaction (PCR) was conducted. A total of 11 infectious pathogens, including 6 virus, 2 bacteria, 2 fungi, and 1 parasite, were investigated in honeybee colonies suffering from symptom of sudden collapse, depopulation or paralysis. The infectious pathogens and infection rates among 63 honeybee colonies detected were as follows: sacbrood virus (12.7%), chronic bee paralysis virus (1.6%), stonebrood (11.1%), American foulbrood (19.0%), European foulbrood (6.3%), respectively. The result indicate that foul-brood was most prevalent disease in apiculture farms in Daejeon area.

  • Imatinib mesylate is a targeted therapy that acts by inhibiting tyrosine kinase of the bcr-abl fusion oncoprotein, which is specific to chronic myeloid leukemia (CML), and the c-transmembrane receptor, which is specific to gastrointestinal stromal tumors. Interstitial pneumonitis is a rare adverse event of imatinib therapy. It is clinically difficult to distinguish from infectious pneumonia, which can frequently occur due to the underlying disease. The standard treatment for imatinib-induced pneumonitis is to discontinue the medication and optionally administer corticosteroids. However, there are a few cases of successful retrial with imatinib. We describe a case of successful rechallenge of imatinib in a patient with imatinib-induced interstitial pneumonitis and CML without a recurrence of the underlying disease after 3 months of follow-up.

  • Myocarditis refers to inflammation, necrosis, or myocytolysis that may be due to many infectious, connective tissue and many other causes affecting the myocardium or involvement of the endocardium or pericardium. The most common manifestation is congestive heart failure, although arrhythmias and sudden death may be the first sign of myocarditis. Viral myocarditis is typically a sporadic but occasionally epidemic illness, noted as an acute potentially fulminant disease of 1-to 4-wk-old infants, as an acute but more benign myopericarditis of toddlers and young children. The most common casuative agent in viral myocarditis is Coxsackievirus and the outcome of the biopsy-proven chronic dilated cardiomyopathy associated with Coxsackievirus is poor without therapy. Myocarditis may be confirmed by percutaneous endomyocardial biopsy and the viral myocarditis may be diagnosed by the serological viral study with the clinical manifestations. He was admitted for the management of tachyarrhythmias occurred suddenly without prodromal symptoms and signs and diagnosed as viral pancarditis by serological Coxsackievirus study, echocardiogram, chest x-ray, EKG and other clinical manifestations.

Identification of Actinobacillus pleuropneumoniae Genes Preferentially Expressed During Infection Using In Vivo-Induced Antigen Technology (IVIAT)

  • Porcine pleuropneumonia is an infectious disease caused by Actinobacillus pleuropneumoniae. The identification of A. pleuropneumoniae genes, specially expressed in vivo, is a useful tool to reveal the mechanism of infection. IVIAT was used in this work to identify antigens expressed in vivo during A. pleuropneumoniae infection, using sera from individuals with chronic porcine pleuropneumonia. Sequencing of DNA inserts from positive clones showed 11 open reading frames with high homology to A. pleuropneumoniae genes. Based on sequence analysis, proteins encoded by these genes were involved in metabolism, replication, transcription regulation, and signal transduction. Moreover, three function-unknown proteins were also indentified in this work. Expression analysis using quantitative real-time PCR showed that most of the genes tested were up-regulated in vivo relative to their expression levels in vitro. IVI (in vivo-induced) genes that were amplified by PCR in different A. pleuropneumoniae strains showed that these genes could be detected in almost all of the strains. It is demonstrated that the identified IVI antigen may have important roles in the infection of A. pleuropneumoniae.

A Systematic Review of Spatial and Spatio-temporal Analyses in Public Health Research in Korea

  • Objectives: Despite its advantages, it is not yet common practice in Korea for researchers to investigate disease associations using spatio-temporal analyses. In this study, we aimed to review health-related epidemiological research using spatio-temporal analyses and to observe methodological trends. Methods: Health-related studies that applied spatial or spatio-temporal methods were identified using 2 international databases (PubMed and Embase) and 4 Korean academic databases (KoreaMed, NDSL, DBpia, and RISS). Two reviewers extracted data to review the included studies. A search for relevant keywords yielded 5919 studies. Results: Of the studies that were initially found, 150 were ultimately included based on the eligibility criteria. In terms of the research topic, 5 categories with 11 subcategories were identified: chronic diseases (n=31, 20.7%), infectious diseases (n=27, 18.0%), health-related topics (including service utilization, equity, and behavior) (n=47, 31.3%), mental health (n=15, 10.0%), and cancer (n=7, 4.7%). Compared to the period between 2000 and 2010, more studies published between 2011 and 2020 were found to use 2 or more spatial analysis techniques (35.6% of included studies), and the number of studies on mapping increased 6-fold. Conclusions: Further spatio-temporal analysis-related studies with point data are needed to provide insights and evidence to support policy decision-making for the prevention and control of infectious and chronic diseases using advances in spatial techniques.

  • Recently, onset of bacterial disease, especially infiltration of chronic respiratory diseases have been increasing at high rate. The main cause of these diseases are originated from inappropriate sanitary management and slow progress in introducing system and herd free system, which are the base of productivity improvement in bigger pig management, Methods for the prevention and removal of these diseases, are divided into four categories. The first category includes prevention and removal of infectious diseases by organizing strict animal quarantine, enforcing vaccination, and legal regulation and disposal at the time of outbreak. The second category includes improvement of production systems. This purpose can be achieved by discontinuing the open herd system, which brings on the invasion of foreign pathogens and replacing the system by the closed type of farm. To Continue eradication process step-wisely by performing the all-in all-out system at every pig pen or herd level is also effective for this purpose. The third category includes introduction of herd free system can be achieved by repopulating conventional pigs with SPF pigs. If these means are unrealizable from economic or technical viewpoint, medicated early weaning system is perfomed. This system consists of disinfection of pigsty at pre-postpartum stage, application of drugs to pregnant sows and newborn piglets in accordance with the medication program, and early weaning and rearing of newborn piglets in isolation. The fourth category is sanital leading by veterinarians, aimed to eliminating incentives for diseases through diagnoses. Their main activities include periodical monitoring and improvement leading on each responsible subject.

Isolated aortic valvular heart disease : analysis of etiology and surgical experience (대동맥판막질환의 임상적 고찰 -판막병리 및 임상성적에 대하여-)

  • Clinical and pathologic data were reviewed in 20 patients who had have surgery for isolated aortic valvular heart disease between April 1978 and April, 1987. Hospital mortality was 10%, with no additional late mortality during a mean follow-up period of 24.1 months. Prosthetic valve failure developed in 3 patients and two had reoperation. Niety four percent of the survivors who were in NYHA Funtional class III or IV before operation are now in class I or II. Ninety percent of all patients are still alive at a maximum follow up of 9 years. The clinical histories, gross and histologic examination of valves estabilished the causes for isolated aortic valve disease: 3 rheumatic, 2 congenital bicuspid, 2 hypertention, 2 aortitis and each one case of floppy valve, medial cystic necrosis of aorta, bacterial endocarditis. But etiology was unknown in 8 cases. Sixteen patietns had myxoid degeneration, defined as significant disruption of the valve fibrosa and its replacement by acid mucosaccharides and cystic changes. Myxoid degeneration was also the primary pathologic abnormality in the patients with 2 hypertention, 2 rheumatic, 1 aortitis, 1 bacterial endocarditis, 1 floppy valve, 1 congenital bicuspid. The patients with myxoid degeneration of uncertain origin were 8. Histologic finding of all of them revealed nonspecific patients with myxoid degeneration of uncertain orgin were 8. Histologic finding of all of them revealed nonspecific chronic valvulitis with myxoid degeneration. This finding may indicate that the etiology w uld be infectious.

Actinomycotic Osteomyelitis of the Mandible: A Case Report

  • Actinomycosis is rare, chronic, slowly progressive disease caused by gram-positive anaerobic organisms from the Actinomycosis family that normally colonizes the oral cavity. Actinomycotic osteomyelitis is even more rare and refractory disease because diagnosis by bacterial culture is not easy. In our case, 80-year-old man visited our clinic with a complaint of swelling and severe sinus tracts without teeth evolvement on anterior mandible. Computed tomography (CT) scan demonstrated erosive bone destruction on anterior mandible, clinically suspected actinomycotic osteomyelitis. The patient also had thoracic actinomycosis on Lt. lower lung. We could diagnosis actinomycosis by histopathologic examination. He treated by conservative surgery and long term antibiotics. After 2 year, no recurrence was seen in CT scan.
