청기산(淸肌散)이 Th2 세포 분화와 염증에 미치는 영향 (Therapeutic Effects of Cheonggisan Extract on Th2 cell differentiation and $NF-kB$ p65 activation)

  • Objectives : Atopic dermatitis is a recurrent or chronic eczematous skin disease with severe pruritus. Although the pathogenic mechanisms of atopic dermatitis are yet unknown, recently hyperresponsive Th2 cells in the acute phase are reported as the important mechanisms. Cheonggisan(CGS) is used in oriental clinics for curing acute skin lesions of eczema, atopic dermatitis or urticaria. There have been no studies on the therapeutic mechanism of CGS for curing atopic dermatitis. We aimed to find out the therapeutic mechanism of CGS on atopic dermatitis, so we observed Th2 cell differentiation in EL 4 cells and $NF-kB$ p65 activation in RAW 264.7 cells. Materials and Methods : EL 4 cells were induced the increase of IL-4 mRNA expression by phorbol-12-myristate-13-acetate(PMA) and 4-tert-Octylphenol(OP) and treated with CGS extract. RAW 264.7 cells were induced the increase of cyclooxygenase(COX)-2 mRNA expression by lipopolysaccharide(LPS) and treated with CGS extract. Results : The PMA and OP induced IL-4 mRNA expression was dose-dependantly decreased in CGS treated EL 4 cells. The LPS-induced COX-2 mRNA expression was dose-dependantly decreased in CGS treated RAW 264.7 cells. Conclusion : The results may suggest that the CGS inhibits Th2 cell differentiation in EL 4 cells and inhibits $NF-kB$ p65 activation in RAW 264.7 cells.

아토피친화학교와 일반학교에 재학 중인 초등학생들의 친환경농산물에 대한 지식도와 식생활 지침 실천도 비교 (Comparison of elementary school students in atopy-friendly and general schools for their knowledge about environmentally-friendly produces and practice levels for dietary guidelines)

  • Atopic dermatitis is a chronic relapsing inflammatory skin disease. In the past years, numbers of children with atopic dermatitis has increased all over the world. Objective of this study was to evaluate awareness of students in atopy-friendly elementary schools about environmentally-friendly grown agricultural products and their practice levels for dietary guidelines. This study was conducted by using a questionnaire on 116 students from three atopy-friendly schools in Jin-an, Jeong-eup, and Gwang-ju and 124 students from general elementary schools. According to the results, the numbers of children who suffer from atopic dermatitis between atopy-friendly schools and general schools were not significantly different. Among, students with atopic dermatitis, more students in atopy-friendly school, compared to general school, restricted foods. Since different school feeding services have been provided, students in atopy-friendly schools generally get more information about the environmentally-friendly agricultural products through nutrition education. However, most results did not show differences in knowledge levels and practice levels for dietary guidelines for children between students in atopy-friendly schools and general schools about environmentally-friendly produces. However, students in atopy-friendly schools less frequently eat cookies, soda drinks, or fast foods. In conclusion, appropriate program for nutrition education should be provided to elementary school students with atopic dermatitis.

우방자(牛蒡子)의 아토피 피부염에 대한 효과 (Effects of Arctium lappa Linne on the DNFB-induced allergic dermatitis)

  • Objectives : Atopic dermatitis is a chronic inflammatory skin disease which is characterized by severe pruritis, erythema, edema, effusion and scabs. The aim of this study was to understand effects of Arctium lappa Linne, which is well known for its efficacy for various skin diseases, on atopic dermatitis Methods : We conducted this experiment using the DNFB-induced NC/Nga mice. After two weeks of applying DNFB to NC/Nga mice, severe symptoms of atopic dermatitis occurred. We divided the mice into three different groups: a control group which was given no treatment at all, a group treated with dexamethasone (1mg/kg), and another group treated with Arctium lappa Linne (300mg/kg). After one week of treatment, results were recorded according to their improvement on skin, itching behavior, IL-4, and INF-$\gamma$ measurement, which is a significant criterion for diagnosing atopic dermatitis. Results : Itching behavior showed significant improvement in the Arctium lappa Linne group. However Arctium lappa Linne failed to reduce IL-4 and INF-$\gamma$ count. Also there was no satisfying improvement on AD-like skin lesions on the rostral back of the NC/Nga mice treated with Arctium lappa Linne. Conclusions : Arctium lappa Linne showed improvement in itching behavior in NC/Nga mice induced by DNFB. Arctium lappa Linne showed neither significant improvement on skin lesions nor in IL-4 and INF-$\gamma$ measurement.

NC/Nga 아토피 피부염 생쥐 모델에서의 양혈윤부탕(養血潤膚湯) 효과 (Suppression of DNCB-induced Dermatitis in NC/Nga Murine Model by Yanghyulyoonbu-tang)

  • Atopic dermatitis (AD) is a chronic inflammatory skin disease characterized by inflammatory cell infiltration in the skin. This study was performed to assess the therapeutic effects of YHYBT on the DNCB-induced dermatitis in NC/Nga mice, characterized by the onset of AD along with an increase the number of Th2 cells and dysregulation of inflammaroty mediators including cytokines and chemokines. YHYBT administration significantly reduced clinical dermatitis severity including pruritus, edema, eczematous and erythema. Histological findings indicated that the thickening of epidermis/dermis and dermal infiltration of inflammatory cells including mast cells were dramatically reduced. The suppression of dermatitis by YHYBT was accompanied by a decrease in the total number of immune cells in drained lymph node (DLN) and skin. Especially CD3+, CD4+ and CD3+CD69+ T cells in PBMC and DNL were greatly reduced. The level of IL-4 in CD3/CD28 activated splenocyte was downregulated, whereas that of IFN-'처리불가‘ was increased. Furthermore, the expression of eotaxin2 and CCR3 in skin were significanlty decreased. These data suggest that YHTBT may be effective therapeutic agents for the treatment of AD.

비허형 아토피 동물모델 개발 (Development of Atopic Dermatitis Mouse Model with Spleen Deficiency)

  • Atopic dermatitis (AD) is a common skin disease characterized by chronic and relapsing inflammatory dermatitis with immunological disturbances. Spleen deficiency (脾虛) is one of the major causes of AD, so development of animal model is required for AD research that reflects the pattern identification. The groups that we have used in this study included Senna folium extracts (SFE), 2,4-dinitrochlorobenzene (DNCB), and normal mice. Therefore, the present study was developed to atopic dermatitis mouse model with spleen deficiency in 2,4-dinitrochlorobenzene (DNCB) and senna leaves extracts induced AD in NC/Nga mice. The results demonstrated that senna leaves extract treatment significantly increased the dermatitis clinical score and epidermal thickness in AD-like skin lesions. We also proved beyond doubt that there was occurrence of erythema and skin moisture indices in the senna leaves extract groups. Further, we also found that the level of serum immunoglobulin E (IgE) in the senna leaves extract-treated group was increased. The amount of IL-4, IL-13, $TNF-{\alpha}$ and $TGF-{\beta}$ mRNA determined by real-time PCR was increased remarkably when senna leaves extract groups were treated on dorsal skin. Senna leaves extract groups significantly promoted the number of CD11B+/Gr-1 cell in skin, as well as the number of CD4+/CD8+ cell in dorsal skin compared with control. The review summarizes recent process in our understanding of the immunopathophysiology of spleen deficiency AD and the implications for spleen deficiency mouse models of AD on drug discovery from medical plants.

국내 논문 검색을 통한 시호청간탕(柴胡淸肝湯)과 시호청간산(柴胡淸肝散)의 아토피 피부염 치료 가능성에 관한 고찰 (A Review on the Sihocheonggan-tang & Sihocheonggan-san for the treatablity of Atopic Dermatitis)

  • Objectives : Atopic Dermatitis is a recurrent, chronic, allergic eczema with severe itching sensation. This study designed to research Sihocheonggan-tang & Sihocheonggan-san to find out treatability for atopic dermatitis. Methods : We got 14 search results with searching word '시호청간탕', '시호청간산' in 'OASIS', 'NDSL', 'RISS'. We classified these studies according to the type of study, diseases, treatment methods and treatment outcomes. Results : In the analysis of the diseases, Sihocheonggan-tang & Sihocheonggan-san was given for Eczema nummulare, Ermatitis seborrheica, Erythema multiforme, Eczema and Atopic dermatitis. In the analysis of the treatment methodes, the dosage form of Sihocheonggan-tang & Sihocheonggan-san was herbal medication, pill, granules and herbal medication & pill. In the analysis of the treatment outcomes, the symptoms of all patients were improved. In the experimental researches, it was proved that Sihocheonggan-tang & Sihocheonggan-san has effects that affect anti-inflammatory, skin protection and recovery of keratinocyte. Conclusions : For this study, we can consider Sihocheonggan-tang & Sihocheonggan-san is effective for atopic deramatitis. So, it is necessary to carry out clinical trial for Sihocheonggan-tang & Sihocheonggan-san in atopic dermatitis to evaluate safety and efficacy.

비허온습방(脾虛蘊濕方)이 NC/Nga 아토피피부염 생쥐 모델의 피부 및 면역학적 변화에 미치는 영향 (Suppression of DNCB-induced Dermatitis in NC/Nga Murine Model by Biheoonsupbang)

  • Atopic dermatitis (AD) is a chronic inflammatory skin disease characterized by inflammatory cell infiltration in the skin. This study was performed to assess the therapeutic effects of BHOSB on the DNCB-induced dermatitis in NC/Nga mice, characterized by the onset of AD along with an increase the number of inflammatory cells and dysregulation of inflammatory mediators including cytokines and chemokines. BHOSB administration significantly reduced clinical dermatitis severity including pruritus, edema, eczematous and erythema. Histological findings indicated that the thickening of epidermis/dermis and dermal infiltration of inflammatory cells including mast cells were dramatically reduced. The suppression of dermatitis by BHOSB was accompanied by a decrease in the number of CD11b$^+$/Gr-1$^+$ immune cells in skin but not CD3$^+$/CCR3$^+$ cells. However, the number of CD3$^+$ cells was increased in BHOSB administrated NC/Nga mice. Oral administration of BHOSB significantly reduced the level of IL-6, TNF-a and eotaxin 2 mRNA in skin. These data suggest that BHOSB may be effective therapeutic agents for the treatment of AD.

만성 스트레스를 동반한 성인형 아토피 피부염 환자의 한의 단독 치료 3례 (Three Treatment Cases on Combined Korean Medicine of Adult Atopic Dermatitis with Chronic Stress)

  • Objectives : The purpose of this study is to report 3 cases of Korean medical treatment on adult atopic dermatitis. Methods : Three patients with adult atopic dermatitits were treated with herbal medicine (Soyo-san hap Samul-tang), acupuncture, and moxibustion. Psychosomatic symptoms were considered the main criteria for treatment. The effects of treatment were evaluated through photographs of the lesions, using EASI and DLQI scores. Results : After treatment, EASI and DLQI scores on three atopic dermatitis patients improved significantly. Conclusions : This clinical case study has demonstrated that Korean medicine may be an effective option for treating adult atopic dermatitis. In addition, considering psychosomatic symptoms when treating atopic dermatitis may also lead to effective treatment.

청기산(淸肌散) 변방(變方)을 이용한 아토피 피부염 치험 1례 (A clinical study on a patient with atopic dermatitis)

  • Objectives : Atopic dermatitis is a skin problem that causes dry skin, intense itching, and then a red, raised rash. It may be a long-lasting (chronic) skin problem that requires more than one treatment. The cause of atopic dermatitis isn't clear, but it affects the skin's ability to hold moisture. The skin becomes dry, itchy, and easily irritated. The purpose of this study was to report the clinical effects of herbal medicine on atopic dermatitis. Methods : We employed oriental medical treatments; herbal-medication (Cheonggisanbyeonbang), acupuncture, moxibustion and cupping therapy. At the same time, the patient sweated out the skin poison. We treated him two times a week with oriental therapy method. He took medicine three times a day after a meal. During taking medicine, we let him avoid fatty food, flour based food, and alcohol. We took a picture the patient's body parts and compared the symptom with previous symptom. With the picture, we evaluated the patient's condition. Results : After taking treatment - 17 times' acupuncture, moxibustion and cupping therapy during 4 months- and taking herbal-medicine, the skin condition of the patient was much improved. At first, he was troubled with itching and hot feeling at reddish area, time after time the itching and hot feeling was weakened. The reddish area was changed to dry condition and changed to normal state. In advance, the symptom of arms was improved, and later the symptom of legs was started to improve. Conclusion : Herbal medicine (Cheonggisanbyeonbang) was an effective in the treatment of atopic dermatitis and it helped to improve regenerating the skin in the body.

개 아토피 피부염에서 3종 프로바이오틱스 복합제의 임상 효능 평가 (Clinical efficacy of L. plantarum, L. reuteri, and Ped. acidilactici probiotic combination in canine atopic dermatitis)

  • Canine atopic dermatitis (CAD) is an inflammatory and pruritic skin disease with a genetic predisposition, characterized by allergic sensitivity. It is known for its distinctive clinical features, including a high recurrence rate and chronic progression. To manage CAD, medications such as steroids and immunosuppressants are commonly used, but consideration should be given to the potential resistance and side effects associated with long-term use. In order to reduce these risks, various adjunctive factors are currently under consideration. One of these adjunctive agents, probiotics have shown effectiveness in regulating atopic dermatitis by modulating immune responses, as demonstrated in several recent studies. In this study, a substance combining three probiotics-L. plantarum, L. reuteri, and Ped. Acidilactici-was used in patients diagnosed with CAD, and its clinical effects and safety were evaluated. The trial involved four groups: a group receiving conventional treatment for atopic dermatitis (A), a group prescribed low-dose probiotics (B), a group prescribed high-dose probiotics (C), and a group prescribed topical probiotics (D). For assessment, the Canine Atopic Dermatitis Extent and Severity Index (CADESI), Trans-Epidermal Water Loss (TEWL) test, gut microbiome, and serum IgE test were conducted. As a result, the CAD severity index (CADESI-4) significantly decreased in the probiotics groups (B & C). In the serum total IgE test, the groups consuming probiotics showed a significant difference, while the group using topical probiotics (D) did not exhibit a significant change. Also, the TEWL test showed improved scores in the probiotics groups (B & C). Therefore, L. plantarum, L. reuteri, and Ped. Acidilactici probiotic combination could be considered as an effective adjunctive treatment, especially for atopic patients with moderate to severe skin lesions.
