Anti-inflammatory Effects of Extracts of Duchesnea chrysantha in Human Monocytic THP-1 Cells and Human Eosinophilic EoL-1 Cells

  • Atopic dermatitis is a recurrent or chronic eczematous skin disease with severe pruritus and has annually increased in Korea. In this study, we investigated whether Duchesnea chrysantha (Dc) extracts have an anti-inflammatory effect in human monocytic THP-1 cells and human eosinophilic EoL-1 cells. The dried and powdered whole plants of Dc were extracted with 80% EtOH (Dc-1). The residue was diluted with water, and then successively partitioned with n-hexane, EtOAc, and BuOH to produce the n-hexane (Dc-2), EtOAc (Dc-3), BuOH (Dc-4), and the water-soluble fractions (Dc-5), respectively. The mite extract and LPS increased the production of IL-6, IL-8 and MCP-1 in THP-1 cells and the increase was strongly suppressed by Dc-3 extract, as compare with other extracts. Dc-3 also inhibited the release of IL-6 increased by mite extract and LPS in EoL-1 cells. However, Dc-3 extract increased IL-8 production induced by the mite extract and LPS in EoL-1 cells. These results suggest that Dc extract may be used as anti-inflammatory agents in treating allergic disorders such as asthma and atopic dermatitis.

Impact of perinatal environmental tobacco smoke on the development of childhood allergic diseases

  • Allergic diseases such as asthma, allergic rhinitis, atopic dermatitis, and food allergy, are most common chronic, noncommunicable diseases in childhood. In the past few decades, the prevalence has increased abruptly worldwide. There are 2 possible explanations for the rising prevalence of allergic diseases worldwide, that an increased disease-awareness of physician, patient, or caregivers, and an abrupt exposure to unknown hazards. Unfortunately, the underlying mechanisms remain largely unknown. Despite the continuing efforts worldwide, the etiologies and rising prevalence remain unclear. Thus, it is important to identify and control risk factors in the susceptible individual for the best prevention and management. Genetic susceptibility or environments may be a potential background for the development of allergic disease, however they alone cannot explain the rising prevalence worldwide. There is growing evidence that epigenetic change depends on the gene, environment, and their interactions, may induce a long-lasting altered gene expression and the consequent development of allergic diseases. In epigenetic mechanisms, environmental tobacco smoke (ETS) exposure during critical period (i.e., during pregnancy and early life) are considered as a potential cause of the development of childhood allergic diseases. However, the causal relationship is still unclear. This review aimed to highlight the impact of ETS exposure during the perinatal period on the development of childhood allergic diseases and to propose a future research direction.

中醫皮膚疾患의 現況과 處置에 對한 分析 (A Clinical Study on Dermatic Disease in the Dongzhimen Hospital, Beijing.)

  • The study was clinically performed with the 128 out patients of dermatic disease who had visited the Dept. of dermatology, Dongzhimen Hospital, Beijing traditional medical college from May 2nd to February 14th 1996. The results were as follows. 1. The dermatologic findings which showed higher incidence in this study were Acne of Melasma 27 cases($21.0\%$), Psoriasis 25($19.5\%$), Eczema including Contact dermatitis 21($16.4\%$), Seborrheic dermatitis including Alopecia of hair 12($9.3\%$) and Urticaria 11($8.5\%$). 2. The sex ratio of male to female was 4.5 : 5.5. The peak incidence of age group was 30-39($21.8\%$). 3. In the duration of disease, 37 cases($28.9\%$) were between 1 year and 5 years. 27($21.1\%$) were over 10 years. 4. About the single herb drugs, they are Angelicas gigantis Radix, Paeoniae Radix and Salviae Radix in order of frequencies. In the case of pill type, Danggui gosam hwan. Chunggan hwan. Whaeo hwan and Shinnongjiyang hwan are most frequently used. In the case of topica, Whangyeongo, Juachanggo and Soeungo are used in high frequencies. 5. For the test and surgical treatment KOH surgical treatment of Molluscum contagiosum in 8 cases and the bloody treatment of Chronic psoriasis in 3 cases.

IgA 疱疹狀 皮膚炎의 治驗 1例 報告 (A Case Report Of IgA Dermatitis Herpetiformis)

  • Dermatitis herpetiformis (DH) is a chronic disease of the skin marked by groups of watery, itch blisters. It is characterized by urticarial plaques and blisters on the elbows, buttocks, and knees, although other sites may also be involved. The ingestion of gluten (the proteins gliadin and prolamin contained in wheat, rye, oats, and barley) triggers an immune system reponse that deposits a substance, IgA (immonuglobin A), under the top layer of skin. IgA is present in affected as well as unaffected skin. DH is a hereditary autoimmune disease linked with celiac disease. Treatment for DH is twofold. (1) Remove the cause: gluten. (2) Suppress the skin response with drugs such as Dapsone or some other sulphones. The latter is the most common treatment used as it is rapidly relieves the itch. However there are some side effects associated with these medications and they need to be taken under medical monitoring with blood tests to detect side effects. Recently, we experienced a DH and that was successfully treated by the herbal medication and external therapy. The medications taken by the patient were yongdamsagantanggami and external therapy were gosam and gumunhwa. So we report this case with a bief review of the oriental medical and medical literatures.

Immunopathology and Immunotherapy of Inflammatory Skin Diseases

  • Recently, there have been impressive advancements in understanding of the immune mechanisms underlying cutaneous inflammatory diseases. To understand these diseases on a deeper level and clarify the therapeutic targets more precisely, numerous studies including in vitro experiments, animal models, and clinical trials have been conducted. This has resulted in a paradigm shift from non-specific suppression of the immune system to selective, targeted immunotherapies. These approaches target the molecular pathways and cytokines responsible for generating inflammatory conditions and reinforcing feedback mechanisms to aggravate inflammation. Among the numerous types of skin inflammation, psoriasis and atopic dermatitis (AD) are common chronic cutaneous inflammatory diseases. Psoriasis is a IL-17-mediated disease driven by IL-23, while AD is predominantly mediated by Th2 immunity. Autoimmune bullous diseases are autoantibody-mediated blistering disorders, including pemphigus and bullous pemphigoid. Alopecia areata is an organ-specific autoimmune disease mediated by CD8+ T-cells that targets hair follicles. This review will give an updated, comprehensive summary of the pathophysiology and immune mechanisms of inflammatory skin diseases. Moreover, the therapeutic potential of current and upcoming immunotherapies will be discussed.

어머니의 임신기 동안 식품섭취와 영유아의 아토피피부염 유무와의 관계 (Comparison of Maternal Food Intakes during Pregnancy in Children with and without Atopic Dermatitis)

  • The prevalence of atopic dermatitis (AD), chronic cutaneous disease, has increased rapidly worldwide. Since AD is the most common disease affecting infants, maternal food intake during pregnancy as well as lactation is important. The purpose of this research was to examine the difference in maternal food intake during pregnancy between mothers having children with or without AD. One hundred forty-eight mothers with children aged under 2 years with AD (AD group, n=74) or without AD (non-AD group, n=74) were included in the study. Diet during pregnancy was examined using a food frequency questionnaire. There were no differences in the status of mothers' food consumption during pregnancy between the AD and non-AD groups. Mothers in the AD group consumed significantly less white fish (P<0.05), carrot & pumpkin (P<0.01), walnut & pine nut (P<0.05), mandarin & orange (P<0.05), and peach (P<0.05) than those in the non-AD group. As food frequencies were categorized according to <1/month, 1~3/month, 1~6/week, and 1~3/day, lower consumption of carrot & pumpkin (Odds Ratio (OR)=6.67, 95% Confidence Interval (CI)=1.98~22.44), tomato (OR=5.11, 95% CI=1.62~16.08), mandarin & orange (OR=8.64, 95% CI=1.59~46.81), and walnut & pine nut (OR=3.85, 95% CI=1.26~11.77) increased the risk of childhood AD. According to the results, maternal food intakes during pregnancy were significantly different between the AD and non-AD groups. Therefore, further studies are necessary to examine the causal relationship between maternal food intake and prevalence of AD in children.

Effect of Dendritic Cells Treated with CpG ODN on Atopic Dermatitis of Nc/Nga mice

  • Atopic dermatitis (AD) is a chronic inflammatory skin disease and the pathogenesis of AD is associated with the release of various cytokines/chemokines due to activated $Th_2$ immune responses. Synthetic oligodeoxynucleotides (ODNs) containing unmethylated CpG dinucleotide in the context of particular base sequence (CpG motifs) are known to have the immunostimulatory activities in mice and to convert from Th2 to Th1 immune responses in AD. We aimed to investigate that CpG ODN, especially phosphodiester form, can stimulate the protective immunity in NC/Nga mice with AD. We isolated BMDCs from NC/Nga mice and then, cultured with GM-CSF and IL-4 for 6 days, and treated for 2 days by either phosphorothioate ODN or phosphodiester ODN. CpG ODN-treated DCs resulted in more production of IL-12. When CpG ODN-treated DCs were intravenously injected into the NC/Nga mice, the NC/Nga mice with CpG ODN-treated DCs showed significant improvement of AD symptoms and decrease of IgE level. Histopathologically, the NC/Nga mice skin with CpG ODN-treated DCs showed the decreased IL-4 and TARC expression comparing with non-injected mice. These results may suggest that phosphodiester CpG ODN-treated DCs might function as a potent adjuvant for AD in a mouse model.

A Probiotic Preparation Alleviates Atopic Dermatitis-Like Skin Lesions in Murine Models

  • Atopic dermatitis (AD) is a chronic inflammatory skin disease with a complex etiology that encompasses immunologic responses. AD is frequently associated with elevated immunoglobulin (Ig) E levels, and common environmental factors contribute to its pathogenesis. Several recent studies have documented the role of specific lactic acid bacteria in the treatment and prevention of AD in humans and mice. In this study, the efficacy of Duolac ATP, a probiotic preparation, was determined in a mouse model with AD-like skin lesions. Alterations in the cytokine levels and histological staining suggested the alleviation of AD. The in vivo test showed that T helper (Th)2 cytokines, IgE, interleukin (IL)-4, and IL-5, were significantly downregulated, whereas Th1 cytokines, IL-12p40 and interferon (IFN)-${\gamma}$, were upregulated in all groups of mice treated with Duolac ATP compared to that observed in the group of mice treated with 1-chloro-2,4-dinitrobenzene (DNCB) alone. Moreover, the scratch score decreased in all mice treated with Duolac ATP. Staining of the dorsal area of the mice in each group with hematoxylin and eosin and toluidine blue further confirmed the alleviation of AD in mice orally treated with Duolac ATP. These results suggest that Duolac ATP inhibits the development of AD-like skin lesions in NC/Nga mice by suppressing the Th2 cell response and increasing the Th1 cell response. Thus, Duolac ATP is beneficial and effective for the treatment of AD-like skin lesions.

삼황사심탕가미방(三黃瀉心湯加味方)이 생쥐에 유발된 아토피 피부염에 미치는 영향 (Effects of Samhwangsasim-tang Gamibang on Atopic Dermatitis in Mice)

  • Objectives : Atopic Dermatitis (AD) is chronic skin disease characterized by allergic hypersensitivity reactions. Samhwangsasim-tang Gamibang(SHSST) can treat skin disease by cooling down blood heat, clearing away congenital fever and detoxicating. This study was conducted to investigate the effects of SHSST on AD induced by oxazolone in mice. Methods : We investigated the effects of SHSST on skin tickness, skin lesions, erythema and melanin index, water content and histopathological changes in vivo. The effects on body weights and spleen body weight ratio were also investigated in mice with AD. Results : In our results, oral administration of SHSST ameliorated skin lesion such as flare, erythema, exudate and petechia and lowered skin thickness induced by oxazolone. In addition, SHSST lowered erythema index and elevated water content compared to AD control respectively. In the histopathological observation, oral administration of SHSST prevented epidermal hyperplasia and spongiotic changes which is a hallmark of skin inflammation respectively. Finally, SHSST did not affect spleen/body weight ratio. Conclusions : These data imply that SHSST can ameliorate skin lesion of AD such as flare, erythema, exudate and petechia, and be used to treat AD patients with relative safety.

The Inhibitory Effect of Premature Citrus unshiu Extract on Atopic Dermatitis In Vitro and In Vivo

  • Atopic dermatitis (AD) is a chronic, recurrent inflammatory skin disease that is associated with Th2 cell-mediated allergy. The process that leads to infiltration of inflammatory cells into an AD lesion is remarkably dependent on various chemokines, especially TARC (thymus and activation-regulated chemokine/CCL17) and MDC (macrophage-derived chemokine/CCL22). Serum levels of these chemokines are over-expressed in AD patients. Citrus unshiu, which is known as Satsuma mandarin, has anti-oxidative, anti-inflammation, and anti-microviral activity. Therefore, we investigated the effect of EtOH extract of premature C. unshiu on AD. We did this using a DNCB-induced AD mouse model. We also tried to confirm an inhibitory effect for premature C. unshiu on the expression of inflammatory chemokines in IFN-${\gamma}$ and TNF-${\alpha}$ stimulated HaCaT human keratinocytes. We found that extract of premature C. unshiu reduced DNCB-induced symptoms such as hyperkeratosis, increased skin thickness, and infiltrated mast cells, in our AD-like animal model. The extract decreased levels of IFN-${\gamma}$ and IL-4 in ConA-stimulated splenocytes isolated from DNCB-treated mice. Also, extract of premature C. unshiu inhibited mRNA expression and protein production of TARC and MDC through the inhibition of STAT1 phosphorylation. These results suggest that C. unshiu has anti-atopic activity by regulating inflammatory chemokines such as TARC and MDC.
