서울 일부 지역 노인의 체지방률, 허리둘레와 체질량지수에 의한 비만 분류 및 만성질환 유병율과의 연관성 (The Prevalence of Obesity by Percentage of Body Fat, Waist Circumference, and Body Mass Index and Their Association with Prevalence of Chronic Diseases of Elderly in Seoul Area)

  • The purpose of this study was to compare the validity of obesity indices among the body mass index (BMI), waist circumference (WC), and body fat percentage (BF%), and to determine which is the most useful index to predict the risk of chronic diseases of elderly people. This study was conducted as a cross-sectional study at welfare centers in Seoul. The total number of subjects was 261 (68 men and 193 women) with age ${\geq}60$ years. The distribution of obesity using 3 obesity indices in the subjects with hypertension, diabetes, or arthritis was BF%>WC>BMI in elderly men and WC>BF%>BMI in elderly women. In elderly women, odds ratios (ORs) for hypertension in BMI and WC quartiles were significantly increased in quartile 2 and 3 (p<0.05). The ORs for hypertension, hyperlipidemia, and arthritis in BF% quartiles were significantly increased in quartile 3 and 4 (p<0.05). The BF% was sensitive obesity index for predicting the occurrence of chronic disease in men, and the WC was sensitive index in women. Our results suggested maintaining BMI less than $23.5kg/m^2$, WC less than 82 cm, and BF less than 35% in order to prevent chronic diseases in elderly women.

류마티스 관절염 환자에서 지각된 질병의거 변화(illness intrusiveness)가 심리사회적 상태에 미치는 영향 (Illness Intrusiveness and Psychosocial Impact in Rheumatoid Arthritis Patients)

  • Chronic disease such as rheumatoid arthritis is believed to induce a significant psychosocial stressors. The concept of illness intrusiveness-illness induced life style disruptions-was hypothesized to affect psychosocial status. And demographic, socioeconomic, disease and social characteristics were hypotehsized to affect illness intrusiveness. Hierarchial multiple regression analyses were used. As a result, among the demographic factors gender was identified as contributor to directly increase the psychosocial stress and education level was identified to affect the psychosocial stress through illness intrusiveness. Among socioeconomic factors, the burden about the cost of medical treatment was found to indirectly affect the psychosocial stress through the illness intrusiveness. Also income and job were found to affect directly the psychosocial status. Among the disease characteristics, only the pain level was identified to increase the psychosocial stress through the illness intrusiveness. Among the social characteristics, perceived social support is identified to increase the psychosocial stress through the illness intrusiveness. Based upon these results, some suggestions were made for minimizing illness intrusiveness in rheumatoid arthritis and future research.

  • The systematic approach to pharmacologic treatment is typically to begin with the safest, simplest, and most conservative measures. It has been realized that the more rapidly inflammation is under control, the less likely it is that there will be permanent sequelae. Nonsteroidal anti-inflammatory drugs (NSAIDs) are the mainstay of initial treatment for inflammation. In addition, the slow-acting antirheumatic drugs (SAARDs) and disease-modifying antirheumatic drugs (DMARDs) have efficacy of anti-inflammatory action in children with chronic arthritis. New therapeutic modalities for inflammation, such as etanercept and infliximab, promise even further improvements in the risk/benefit ratio of treatment. It is not typically possible at the onset of the disease to predict which children will recover and which will go on to have unremitting disease with lingering disability or enter adulthood with serious functional impairment. Therefore, the initial therapeutic approach must be vigorous in all children.

근전도상 근긴장증이 관찰되는 콜치신에 의한 근병증 1예 (Colchicine Induced Myopathy Presenting Electrophysiologic Myotonia)

  • Colchicine is widely used in the treatment of gouty arthritis, and chronic use of colchicine can cause vacuolar myopathy. We report colchicine-induced myopathy with electrophysiological myotonia in a patient with gouty arthritis. A 86-year-old man with gouty arthritis presented with progressive proximal weakness and gait disturbance. Electrophysiological finding showed myotonic myopathy and muscle biopsy revealed a vacuolar myopathy. His symptoms soon resolved with the discontinuation of colchicine.

Evaluation of anti-inflammatory activity of Solanum trilobatum roots

  • This study evaluated the anti-inflammatory potential of the crude alkaloidal fraction (CAF) of methanol extract of Solanum trilobatum Linn. (Solanacea) root in animal models of inflammation. Crude alkaloidal fraction at doses of 25, 50 and 100 mg/kg significantly (p < 0.01) reduced carrageenan induced rat paw volume at 3 h after carrageenan challenge as compared to control group of animals. CAF (25, 50 and 100 mg/kg) significantly (p < 0.01) and dose dependently suppressed cotton pellet induced granuloma formation. Topical application of CAF (1, 5 and 10 mg/ear) markedly inhibited multiple application of TPA in mice. CAF elicited pronounced inhibitory effects on formaldehyde and adjuvant induced arthritis in rats. These results indicate that CAF of methanol extract of the Solanum trilobatum has anti-inflammatory activity in acute and chronic inflammation.

  • Head of Panax ginseng indicates its growth number of years and it has been widely used for supplying energy to weak person. However, the underlying mechanisms are not sufficiently reported. Thus, we inclined to study head of Panax ginseng in rheumatoid arthritis and inflammation. Rheumatoid arthritis (RA), an organ-specific inflammatory disease of human, is characterized by a chronic and destructive inflammatory reaction and possibly autoimmune in etiology. It is occurred when the synobial membranes of joints and many other tissues of the body is attacked which induces significant health problem in terms of numbers of sufferers (U.S. incidence 23.7/100,000), and the synobial inflammatory is dominated by activated macrophage.

  • The fruiting body of Ganoderma lucidum (Polyporaceae) is one of the valuable crude drugs, which has been used clinically in Korea, China, and Japan for a long time as a tonic and sedative, and for the treatment of hepatopathy, chronic hepatitis, nephritis, gastric ulcer, hypertension, arthritis, neurasthenia, insomnia, asthma, and poisoning and chronic bronchitis. 고려홍삼의 콜라겐 유도 관절염의 예방과 억제효과 (Preventive and Inhibitory Effect of Korean Red Ginseng on Collagen-Induced Arthritis in Mice)

  • Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by joint inflammation and progressive cartilage and bone erosion. Korean red ginseng (KRG) has been shown to have an anti-inflammatory effect by inhibiting the secretion of inflammatory cytokines like $TNF-{\alpha}$, IL-1, -6, and -8, and $IPN-{\gamma}$. In this study, whether KRG extract has an inhibitory effect on the collagen-inducible development of arthritis in DBA/1J mice was investigated. To induce arthritis, type II collagen emulsified in Complete Freund's Adjuvant was intradermally injected into the base of the tails of mice. Three weeks after the initial injection, a booster injection of type II collagen emulsified in Incomplete Freund's Adjuvant was administered. The oral administration of KRG extract for 8${\sim}$10 weeks at the dose of 300 mg/kg (three days a week) inhibited the development of arthritis in the experimental group, compared to the control group which was given saline. While the administration of KRG extract three times a week demonstrated both preventive and inhibitory effects, the administration of KRG extract once a week had little inhibitory effect. In other studies, the regimen of KRG administration has been shown to decrease the plasma level of inflammatory cytokines like IL-8 and TNF-${\alpha}$, but the plasma levels of these cytokines were not decreased in the present study. The results of the present study suggest that KRG has preventive and inhibitory effects on collagen-induced arthritis.

  • Subcutaneous tissue calcification in rheumatic diseases usually occurs in connective tissue diseases, such as systemic lupus erythematosus, scleroderma, and dermatomyositis. Domestic cases of calcification in rheumatoid arthritis have not been reported. The mechanism of subcutaneous tissue calcification may differ depending on the cause and it can develop on all parts of the body. Calcification occurring in rheumatic diseases is a major mechanism of tissue damage caused by chronic inflammation. No standard therapy for calcification has been established; however, many studies have reported on medical and surgical treatment. We report on subcutaneous tissue calcification in a rheumatoid arthritis patient tissue calcification on both sides of the buttocks, the upper limbs, and the lower limbs.
