만성 족관절 염좌에 자하거 약침을 병행한 한방 치료로 호전된 2례 보고 (The Case Report of Chronic Ankle Sprain Improved with Hominis Placenta Pharmacopuncture Treatment)

  • Objectives The purpose of this study is to investigate and report the effectiveness of Hominis Placenta pharmacopuncture treatment for chronic ankle sprain. Methods 2 patients are treated at Dept. of Korean Medicine, the Armed Forces Busan Hospital, diagnosed as thinning of anterior talofibular ligament (ATFL), suggestive of partial tear and anterior tibiofibular ligament partial tear. They are treated with Hominis Placenta pharmacopuncture. Each cases are measured and assessed by ankle hindfoot scale (AHS), visual analgue scale (VAS), cumberland ankle instability tool (CAIT) scores. Results 2 patients of chronic disease of ankle sprain have a different kind of diagnoses they have. After treatment of Hominis placenta pharmacopuncture, spontaneous pain is decreased and ankle instability, ankle functions are increased significantly. Conclusions Hominis placenta Pharmacopuncture has a effect on chronic ankle sprain.

저명한 불안정성을 가진 만성 족관절 염좌 환자의 족관절 골성 병변에 대한 분석 (Analysis of Ankle Bony Abnormality in the Patients with Chronic Ankle Sprain and Marked Ankle Instability)

  • Purpose: We analyzed the ankle bony abnormality of patients with marked ankle instability who had chronic ankle sprain more than 3 years. Materials and Methods: We evaluated the chronic ankle sprain (more than 3 years) patients with marked ankle instability tested by varus stress test and anterior draw test from March 2000 to December 2005. Eighty-nine patients (104 ankle) were evaluated and there were 38 males and 51 females. The mean age of patient at the time of diagnosis was 34.5 (range, 18 to 56 years). The average duration of morbidity was 7 years and 3 months (range, 3 years and 3 months to 21 years). The patients who had history of dislocation, fracture, malalignment, operated patients, and rheumatoid ones were excluded. Plain radiographs of AP, lateral, oblique and mortise view were checked. Results: Radilologic abnormalities were found at 74 ankles (71%) among 104 ankles. Frequent sequences of location were anterior talotibial osteophyte, medial malleolar osteophyte, Os subfibulare, lateral malleolar osteophyte. Posteior osteophyte, ankle arthritis, talar articular defect were rarely found. Conclusion: Seventy-one percent among patients with chronic ankle sprain and marked ankle instability showed more than one radiologic abnormalities. Thus, more exclusive and accurate ankle examination should be performed in these patients.

  • Acute ankle sprain is the most common injury in the lower extremities, and approximately 10% to 40% of acute lateral ankle ligament injury causes chronic pain or instability. For chronic symptoms lasting after an acute sprain, the possibility of joint damage, such as bony structures, ligaments, cartilage, and nerves around the ankle joint, should be considered. Patients with chronic lateral ankle instability usually complain of repeated sprains or giving way sensations. There has been steady progress in the treatment options until recently, however new treatments are still being attempted. This paper describes the causes, diagnosis, and recent trends in the conservative and operative treatment of chronic lateral ankle instability.

  • Objectives: The purpose of this study is to investigate the clinical effects of heating-conduction acupuncture therapy on anterior talofibular ligament injury induced by acute and chronic ankle sprain. Methods: From April 19, 2010 to May 30, 2011 the 79 outpatients who had admitted to department of oriental rehabilitation medicine, Dae-Jeon oriental medical hospital, Dae-Jeon university with ankle sprain were performed heating-conduction acupuncture therapy on anterior taIofibular ligament injury. To evaluate the efficiency of heating-conduction acupuncture therapy, pain threshold with pressure algometer and visual analogue scale(VAS) were applied before treatment and after 1st, 2nd treatment. Results: 1. The pain threshold and VAS score showed significant improvement on acute subacute, chronic anterior talofibular ligament injury group. 2. The difference in pain threshold and VAS score between acute, subacute, chronic phase group was not significant. Conclusions: Heating-conduction acupuncture therapy has clinical effects of pan reduction on patient with anterior talofibular ligament injury.

Role of Attentional Focus in Balance Training: Effects on Ankle Kinematics in Patients with Chronic Ankle Instability during Walking - A Double-Blinded Randomized Control Trial

  • Objective: Although balance training has been used as an effective ankle injury rehabilitation program to restore neuromuscular deficits in patients with chronic ankle instability, it is not effectively used in terms of motor learning. Attentional focusing can be an effective method for improving ankle kinematics to prevent recurrent ankle injuries. This study aimed to 1) evaluate the effects of attentional focus, including internal and external focus, and 2) determine a more effective focusing method for patients with chronic ankle instability to learn balance tasks. Method: Twenty-four patients with chronic ankle instability were randomly assigned to three groups (external focus, internal focus, and no feedback) and underwent four weeks of progressive balance training. The three-dimensional ankle kinematics of each patient were measured before and after training as the main outcomes. Ensemble curve analysis, discrete point analysis, and post hoc pairwise comparisons were performed to identify interactions between groups and time. Results: The results showed that (1) the external focus group was more dorsiflexed and everted than the internal focus group; (2) the external focus group was more dorsiflexed than the no feedback group; and (3) the no feedback group was more dorsiflexed than the internal focus group. Conclusion: Because dorsiflexion and eversion are ankle motions that oppose the mechanism of lateral ankle sprain, using the external focus method during balance training may be more effective in modifying these motions, thereby reducing the risk of ankle sprain.

The effect of intrinsic foot muscle training on medial longitudinal arch and ankle stability in patients with chronic ankle sprain accompanied by foot pronation

  • Objective: The purpose of this study was to investigate whether the intrinsic foot muscle training method can improve the medial longitudinal arch in patients with chronic ankle injury and with pronated feet, as well as to investigate for the most effective exercise method for these patients. Design: Randomized controlled trial. Methods: Thirty men and women with pronated feet had participated in this study and were allocated to either the short foot exercise group (SFEG) or the towel curl exercise group (TCEG) randomly. SFEG and TCEG underwent exercises three times a week for 8 weeks, with three sets per day, totalling up to 5 minutes per day. The navicular drop test (NDT) was used in order to assess for changes in the medial longitudinal arch and the Cumberland ankle instability tool (CAIT) was used to assess for ankle instability of the chronic ankle sprain patients. Results: There was a significant increase in CAIT scores in the SFEG (p<0.05) and a significant difference between groups was presented (p<0.05). The NDT scores were significantly decreased in both groups (p<0.05). In the SFEG, the NDT scores were more decreased than in the TCEG (p<0.05). Conclusions: These results suggest that short foot exercises are more effective in providing intrinsic foot muscle training for patients with pronated feet among chronic ankle sprain patients. Furthermore, short foot exercises may be used to provide ankle stability.

족근관절 염좌시 동반된 신경 손상 (Neural Injuries in Ankle Sprain)

  • Purpose: The neural injuries by the sprain around the ankle joint may contribute the chronic pain. Authors analyzed the incidence and the contributing factor of the neural injuries in ankle sprain. Materials and Methods: 52 patients (54 cases) were involved in this retrospective study. Patient with diabetes or spinal disease were excluded. Plain radiograph and MR image were evaluated. Treatments were consisted of cast immobilization for 4 weeks with weight bearing ambulation following bracing for 8 weeks. Neurologic evaluation were performed at 3 months, 6 months, 12 months post-injury period and each neural injury were confirmed by electromyography or lidocaine block test. Results: The average age was 39 years old and 34 cases were male and 20 cases were female. Rupture of the anterior talofibular ligament was observed in 48 cases, distal anterior tibiofibular ligament in 37 cases, calcaneofibular ligament in 6 cases. One cases revealed no injury of the ligament. Neural injuries around ankle was observed in 13 cases ; superficial peroneal nerve in 9 cases, sural nerve in 5 cases, saphanous nerve in 1 case. Neural injury was not influenced by the degree of ligament injuries but by the incidence numbers of ankle sprain. All cases were treated conservatively and symptom was subsided in all but 2 cases. Conclusion : Although the incidence is relatively low, the neural injuries in ankle sprain may occur in the recurrent ankle sprain and conservative treatment for neural injuries is satisfactory.

만성 족관절 불안정성의 진단 및 동반 질환 (Diagnosis and Comorbidity of Chronic Ankle Instability)

  • Ankle sprains are among the most common injuries sustained during athletic activities and daily life. Acute ankle sprain is usually managed conservatively with functional rehabilitation but the failure of conservative treatment leads to the development of chronic ankle instability. The development of repetitive ankle sprains and persistent symptoms after injury has been termed chronic ankle instability. Acute ankle sprains and chronic ankle instability require a careful evaluation to detect other comorbidities, such as subtalar instability, osteochondral defect, peroneal tendinopathy, tarsal coalition, os trigonum, flexor hallucis longus tendinitis, calcaneus anterior process fracture, and neural injuries. For the successful treatment of acute ankle sprains and chronic ankle instability, the treatment of comorbidity lesions should be performed first.

  • 만성 외측 발목 불안정(chronic lateral ankle instability; 이하 CLAI)은 급성 발목 염좌의 주요 합병증으로 일상생활과 스포츠 활동에 불편을 줄 수 있으며, 장기 후유증으로 발목 관절에 퇴행성 변화를 가져오기도 한다. CLAI의 정확한 원인은 여전히 논쟁 중이며 다인자적으로 알려져 있으나, 특히 만성적인 외측 발목 인대 손상 여부를 확인하는 것이 환자의 원발 병소를 결정하고 적절한 치료 계획을 수립하는 데 필수적이다. 본 종설에서는 CLAI에서 인대 손상을 진단하는데 이용되는 여러 검사들의 특징과 유용성에 대해 기술해 보았다.

  • Chronic lateral ankle instability occurs in 10% to 20% of individuals after acute ankle sprain. The management of chronic lateral ankle instability is traditionally conservative treatment in the acute phase. On the other hand, surgical intervention is considered if conservative treatment fails and the symptoms are ongoing. This review focuses on the surgical approaches to treatment of chronic lateral ankle instability, including $Brostr{\ddot{o}}m$ surgical techniques, with a review of the traditional procedure and newer techniques.
