Retrospective Clinical Study of Soyangin Chronic Urticaria Patients (少陽人 만성두드러기 환자에 대한 후향적 임상연구)

  • Objectives : This study was designed to assess the effect of Soyangin Medicines on Soyangin chronic urticaria patients by retrospective methods.Methods : We treated 37 Soyangin chronic urticaria patients who visited Haneulmaeum Korean Medicine Clinic from September 1st 2011 to June 30th 2014. We administrated the Soyangin patients with Soyangin Medicines such as Hyeongbangsabaek-san(荊防瀉白散), Yanggyeoksanwha-tang(凉膈散火湯) and so on for 3 months. We measured the effect biweekly by using Urticaria Activity Score(UAS). We analyzed the information and medical history of patients by Frequency analysis, and UAS by Repeated measures, ANOVA.Results & Conclusions : Every items of UAS were improved by Soyangin Medicines. And the improvement was statistically significant(p<0.05).

Follow-up Study on Chronic Idiopathic Urticaria Patients Treated with Pyungweesangamibang (평위산가미방으로 치료한 만성 특발성 두드러기 환자의 추적조사)

  • Objective : The purpose of this study is to investigate the recurrence rate of Chronic Idiopathic Urticaria patients treated with Pyungweesangamibang through long-term follow-up survey.Methods : This tracing study was conducted for sixty-seven Chronic Idiopathic Urticaria patients who received and have finished treatment with Pyungweesangamibang at Woobo Korean Medicine Clinic in 2013 and 2014.Result : 81.4% of the patients, who had spent over 1 year since the finish of treatment with Pyungweesangamibang, did not relapse. In case of more than 2 years elapse, 66.7% of the patients did not suffer relapse.Conclusion : The result of tracing recurrence of the Chronic Idiopathic Urticaria patients treated with Pyungweesangamibang describes that the Korean medical treatment (Pyungweesangamibang prescription) shows lower recurrence rate than other general treatment shows.

  • Urticaria is an erythematous or white non-pitting edematous plague that changes in size and shape by peripheral extention or regression during the few hours or days. Urticaria is classified as acute or chronic type. The etiology of acute urticaria has been determined in many case, but the cause of chronic urticaria has been determined in only $5\%\;to\;20\%$ of cases. This observation was aimed to evaluate the clinical manifestations and clinical indence of urticaria seen between June, 1995 and November, 1996(18months) at the Department of Dermatology, Oriental Medical Hospital, Kyung Hee University. The results were summarized as follows ; 1. In the cases of 106 patients, $43\%\;were\;male\;and\;51\%$ were female. 2. The pick incidence of age group was 20 to 29($32\%$). 3. The most frequent duration of urticaria was from 2 months to 6 months. 4. In this study, the inciting causes could not be found in $61\%\;of\;acute\;patients,\;60\%$ of chronic patients. 5. Among 107 patients, $5\%$ of all patients had personal history of atopic diseases and $9\%$ of all patients had familly history. 6. Angioedema was founded in 7 patients, and the most common involved site were lips, eyelids and tongue. 7. The most frequently treated prescription was Chhunggisan which occupied $51\%$ of all, Gyakhangjunggisan(41\%$) and Gosamhomasan($25\%$), Bangpoongtongsungsan($22\%$) in that order of frequency.

A Case on Combined Korean Medicine of Adult Chronic Urticaria with Papular Form (구진성 만성 두드러기 환자의 한방 단독 치료 보고)

  • Objectives : The purpose of this study is to report the use of Korean medicine on a chronic urticaria patient. Methods : We treated an adult patient had chronic urticaria with herbal medicine, pharmacopuncture, acupuncture, photodynamic therapy. The effects of the treatment were evaluated through urticaria activity score and photo evaluation. Results : The urticaria activity score started at 6 and ended with 0. After 4 months of follow-up, no recurrence was confirmed. Conclusions : This study has demonstrated that Korean medicine could be effective to treat chronic urticaria with papular form.

The Effect of Gamisungmagalguntang(加味升麻葛根湯) on Chronic Urticaria Identified as "Wind Heat" Pattern (풍열형(風熱型) 만성 두드러기에 대한 가미승마갈근탕(加味升麻葛根湯)의 치료효과)

  • Objective : This case series study was designed to assess the effect of Gamisungmagalguntang for chronic urticaria identified as "Wind-Heat" pattern by retrospective methods. Methods : We treated 19 patients for chronic urticaria with Gamisungmagalguntang(加味升麻葛根湯), who visited to care chronic urticaria at the department of Ophthalmology, Otolaryngology and Dermatology of Korean Medicine at Kyunghee University Korean Medical Center from 1st January 2013 to 31st August 2013. We analyzed information and body conditions of 19 patients. And by using Urticaria Activicy Score(UAS), frequency and number of wheals, itch severity and total score on a 4-point(0-3) scale were assessed in 19 patients. The change of each criterion and total scores between baseline and follow up were analyzed using paired t-test and Wilcoxon test(p<0.05). Results : 1. UAS improvement was statistically significant(p<0.001). Effects of symptom improvement were excellent(31.58%) that means symptom remission and good(42.10%) that means symptom improved over 50%. 2. Heat sensitivity as suspected provocation factor could be a marker for the Wind-Heat pattern urticaria. 3. Among the Wind-Heat pattern urticaria patients, normal digestion(52.63%) and no thirst(78.95%) were higher than abnormal conditions. Whereas about sweat item, abnormal conditions(57.89%) were higher than normal. That means abnormal sweat conditions could be a marker for a defense qi(衛氣) dysfunction of the Wind-Heat pattern urticaria. Conclusions : Our findings suggest that Gamisungmagalguntang could be effective on the Wind-Heat pattern urticaria.

A Clinical Case Report Set of Pattern of Soyangin Yin Collapse and Taeumin Dryness-heat Diagnosed with Chronic Urticaria (만성 두드러기를 진단 받은 소양인 망음증 및 태음인 조열증 환자 치험 각 1례)

  • Introduction Chronic idiopathic urticaria in patients diagnosed with an unfavorable pattern in Soyangin and Taeumin based on Sasang medicine showed a significant improvement after herbal medicine treatment. Methods The patients had treatment with both herbal medication and antihistamine. We checked the patients' cutaneous manifestation, original symptoms, and the number of antihistamine doses in each visit. We evaluated the medical records retrospectively. Results After one month of treatment, the patients showed cutaneous manifestation relief. Each patient reduced and withdrew antihistamine at four moths and eight months respectively, and the cutaneous manifestation did not recur. We maintained the herbal medicine prescription until the origin symptoms improved. Throughout the subsequent follow-up, the patients showed both urticaria and the original symptoms in good condition without treatment. Discussion Sasang medicine treatment can be an effective treatment for chronic idiopathic urticaria and long-term management is necessary for an unfavorable pattern which can take more than 6 months.

  • Objectives : The purpose of this study is to report 1 clinical experience of korean medicine treatment on chronic urticaria identified as "Wind Heat" pattern. Methods : We treated a 63-year old woman patient with Gagamtongcheong-san, acupuncture and pharmacopuncture. The improvement of patients evaluated using CU-Q2oL, UAS, and photography. Results : CU-Q2oL score decreased from 95 to 18, UAS decreased from 6 to 0. Conclusions : The complex treatments of korean medicine and was effective to improve of chronic urticaria. It is necessary to have more observations and case reports.

A Clinical Study of Chronic Urticaria (만성 두드러기에 대한 임상 연구)

  • Objective: This study was designed to investigate clinical features of chronic urticaria and assess Korean traditional medical therapy on that. Methods: The ratio of male and female, duration of symptoms, personal history and family history of allergic diseases, serum IgE level, and suspected causes and provocation factors were investigated in 41 patients with chronic urticaria. And frequency of wheals, duration of wheals, number of wheals, itch severity, and antihistamine counts of 6-point(0-5) score was assessed in 26 patients. The change of total scores between baseline and follow up were analyzed using Wilcoxon signed-rank test(p〈0.05). Results: 1. The ratio of male and female was 1:1.28 and the mean duration of symptoms was 26.0 months. 2. The frequency of personal history and family history of allergic diseases was each 53.7$\%$ and 48.8$\%$ and 30.6$\%$ had over 200 IU/mL serum IgE level. 3. The suspected causes were none(43.9$\%$), foods(29.3$\%$), drugs(9.8$\%$) and stress(7.3$\%$). 4. The suspected provocation factors were foods(39.0$\%$), none(19.5$\%$), fatigue(17.0$\%$), heat and sweating(l4.6$\%$), pressure(12.2$\%$) and stress(9.8). 5. The total scores between baseline and follow up of 26 patients was lowered from 14.85${\pm}$3.80 to 7.65${\pm}$4.31 and it was significant statistically. Conclusions: Korean traditional medical therapy was proved to be effective on the chronic urticaria and we need to consider foods as important factor in both treatment and patient education of urticaria.

Clinical aspects of chronic urticaria in children (소아 만성 두드러기의 임상 양상 및 경과)

  • Purpose : Chronic urticaria is a disorder characterized by the appearance of wheals for more than 6 weeks; in most cases, the etiology is unknown. This study was aimed to discover the clinical aspects, the etiologic factors, and the course of chronic urticaria. Methods : 51 children who were diagnosed with chronic urticaria in the past 4 years, and who had had follow-ups more than 6 months after diagnosis in the pediatric department of Soonchunhyang University Hospital in Bucheon, were enrolled in the study. The laboratory findings, clinical aspects, and courses were retrospectively investigated by medical record review and telephone interview. Results : The median age of children with chronic urticaria was 4 years (8 months to 16 years) and the ratio of male to female was 1.4:1. Of the total, 39.2% of patients had a history of atopy. Angioedema occurred concurrently with urticaria in 11.8% of patients, and dermographism was seen in 41.2%. Results of thyroid function tests were normal and thyroid autoantibodies were absent in all cases. Regarding etiology, most cases (74.5%) were forms of idiopathic urticaria. Urticaria was induced by physical factors in 19.6% of patients. Open challenge tests revealed that 3 patients were allergic to food additives (glutamate 2, glutamate, and sulfite 1). In this study, most of the patients reported good response after medication of 1st- or 2nd-generation antihistamines alone. Follow-up at 6 months revealed that 70.6% of patients had experienced remission, and 84.8% of children who had follow-up at 1 year presented remission. Conclusion : Chronic urticaria in most patients was idiopathic. Remission occurred within 1 year of diagnosis, in most cases so chronic urticaria in children seems to have good prognosis.

A Clinical Report on 39 Urticaria Patients Treated With The Ko-Bang(古方) (고방(古方)으로 치료한 두드러기 환자 39례의 임상보고(臨床報告))

  • Objectives : This study was designed to investigate the chronical features of urticaria and the effects of urticaria treatment with Ko-Bang(古方). Methods : We treated 39 patients for urticaria with Ko-Bang(古方), who visited to care urticaria at the Dept. of Dermatology Oriental medical hospital Dong-eui university from Jan, 2008 to Sep, 2009. This study was assessed using the chart analysis and the follow-up survey by telephone. Results & Conclusions : 1. 39 outpatients were surveyed, who were 20 males and 19 females. And patients who were ages 21-30 were the largest group, 11(28.21%). Among the 39 patients, acute urticaria patients were 8(20.5%), chronic urticaria patients were 31(79.5%), and contraction of a disease period between 6 weeks with 3 months was 17.9%. 2. The causes of urticaria were measured unknown 37.5%, foods 37.5%, drugs 25% in acute and unknown 35.5%, a change of temperature 29%, foods 16% in chronic. And suspected provocation factors were measured unknown and foods each 12 cases. The most of accompanied symptoms was itching sign(34 cases), the second most was self-conscious fever(10 cases). 3. Out of treat prescription(Ko-Bang,古方), Injinho-Tang(茵蔯蒿湯) was used most in 5 cases, Daehwanghwangryunsasim-Tang(大黃黃連瀉心湯), Hwangryun-Tang(黃連湯), Daesiho-Tang(大柴胡湯) were used in each 4 cases, Galgeun-Tang(葛根湯), Chijadaehwangsi-Tang(梔子大黃豉湯), Sosiho-Tang(小柴胡湯) were used in each 3 cases, Gaemagakban-Tang(桂麻各半湯), Chijasi-Tang(梔子豉湯),Oryeongsan(五笭散) were used in each 2 cases. 4. When the processes of treatment were classified by one poison(一毒), there were the 19 cases (48.7%) which was caused by Bun(煩), the 12 cases(30.8%) by Water(水) and the 4 cases(10.3%) by Gyur(結).
