Retrospective study of changes in pharyngeal airway space and position of hyoid bone after mandibular setback surgery by cephalometric analysis

  • Background: The posterior movement of mandible was known as the main cause of the changes in the pharyngeal airway space (PAS) and the postoperative obstructive sleep apnea (OSA). The purpose of this study was to know the changes of PAS and position of hyoid bone. Methods: Lateral cephalographies of 13 patients who had undergone sagittal split ramus osteotomy (SSRO) setback surgery were taken preoperatively (T1), postoperatively within 2 months (T2), and follow-up after 6 months or more (T3). On the basis of F-H plane, diameters of nasopharynx, oropharynx, and hypopharynx were measured. The movements of the soft palate, tongue, and hyoid bone were also measured. Results: The amount of mandible setback was $7.5{\pm}3.8mm$. In the measurements of PAS, there was a statistically significant decrease of $2.8{\pm}2.5mm$ in nasopharynx (P < 0.01), and $1.7{\pm}2.4mm$ in oropharynx (P < 0.01) were observed after surgery. The hypopharynx decreased $1.0{\pm}2.1mm$ after surgery and continuously decreased $1.0{\pm}2.8mm$ at follow-up. The changes in hyoid bone position showed the posterior movement only after surgery and posteroinferior movement at follow-up. Conclusions: The PAS such as nasopharynx, oropharynx, and hypopharynx showed relatively high correlation with the amount of mandibular setback. The change of resistance in upper airway may be important for the prevention of OSA after mandibular setback surgery.

  • Nontuberculous mycobacterial infections are a rare, but clinically important cause of infections in continuous ambulatory peritoneal dialysis (CAPD) patients. This is typically suspected when a patient does not respond to treatment with the usual antibiotics. We describe here a case of $Mycobacterium$ $abscessus$ exit site infection with abdominal wall abscess formation that was associated with CAPD, which required peritoneal catheter removal, surgical debridement of the abscess and long term antibiotic therapy.

Odontogenic carcinosarcoma of the mandible: a case report and review

  • Odontogenic carcinosarcoma is an extremely rare malignant odontogenic tumor with only a few reported cases. It is characterized by a true mixed tumor showing malignant cytology of both epithelial and mesenchymal components. It has been assumed to arise from pre-existing lesions such as ameloblastoma, ameloblastic fibroma, and ameloblastic fibrosarcoma. To date, the reported cases have exhibited considerably aggressive clinical behavior. The case of an odontogenic carcinosarcoma in the mandible of a 61-year-old male is described herein. The tumor destroyed the cortex of the mandible and invaded the adjacent tissues. Treatment was performed by surgical resection and reconstruction. The purposes of this article are to introduce odontogenic carcinosarcoma through this case study, to distinguish it from related diseases and to discuss features of the tumor in the existing literature.

  • It has been found that the colony size of bacteria grown on an agar plate decreases with increasing agar gel concentration. Evidenc from recent studies suggests that the bacterial colony dynamics is closely related with the mechanical properties of the substrate. We investigate whether bacterial growth on the agar substrate is controlled mostly by the nutrients' diffusion which is hindered more in porous medium than in solution. The number of bacterial cells in single colonies is found to be inversely correlated with agar concentration. High-resolution live cell imaging at the single bacterium level confirms that the bacterial growth rate is reduced with increasing agar concentration. There is a strong correlation between the slowed diffusion and the reduced number of cells in a high concentration of agar medium.

  • Mandibular condylar hyperplasia is an uncommon condition of excessive unilateral growth of the condyle causing facial asymmetry and occlusal alterations. The etiology of condylar hyperplasia is unclear, but several factors are suspected, including previous trauma, hormonal disturbances, and abnormal functional loadings. Acute or chronic recurrent dislocation of temporomandibular joint (TMJ) is common, but long-standing dislocation is rare. We present two cases of the exophytic condylar hyperplasia that lasted for over 20 years with TMJ dislocation. In both cases, we performed a condyloplasty to restore normal occlusion and facial symmetry, with satisfactory results.

  • This paper describes the design and implementation of the DMI display based on ERTMS specification for the Integrated Onboard Signaling System (IOSS). IOSS is consists of ATS, ATC, and ATP based on ERTMS/ETCS Level 1. We propose an improved DMI display design which has single layout concept for the IOSS. When a train is in the ATS, ATC, or ATP railway section, it is protected by the ATS, ATC, or ATP equipment, respectively. Therefore, DMI should display the different information according to the each signaling system. We design DMI display implemented as single layout for ATP, ATS and ATC information. This single layout display has a well integrated look and feel using the ERTMS specification and layout. This is for the driver to perceive the information easily and not to experience inconvenience resulting from the sense of difference when the train crossed into the different signaling section. Finally, we show the implementation result following this concept.

  • As Cyber threats are rising, the scope of information Security (IS) is extending from technical protection of a single information system to organizational comprehensive IS capability. The ministry of National Defense (MND) has established the IS evaluation for defense organization in 'the Directive for Defense Informatization Affairs.' However, no information about an evaluation method, process and organization is provided. We surveyed information security management system (ISMS) and related best practices in public sector and other countries, and analysed the military information security affairs. Thus, this paper recommends the IS evaluation method and process. The trial IS evaluation is in progress this year and the MND will expand this IS evaluation to the entire organization.

  • Clinicians must be able to recognize post-extraction complications and treat them in a timely manner; complications that may potentially be life-threatening require special attention. Although arteriovenous malformation (AVM) is a very rare disorder, it may induce life-threatening hemorrhage during surgical intervention in the pertinent site. The present article examines the diagnosis and treatment modalities of AVM based on the case of a patient who was diagnosed with AVM with continuous bleeding after tooth extraction and who was successfully treated.

  • This paper proposes a point of common coupling (PCC) voltage compensation algorithm using a current limitation strategy for use in distributed generation (DG). The proposed strategy maintains the PCC voltage by prioritizing currents when an output current reference is larger than the current capacity of the power condition system (PCS) of the DG. With this strategy, the DG outputs the active current, reactive current, and the negative sequence current. The DG uses the reactive current for maintaining the PCC voltage within a normal range; the negative sequence current is used for reducing the PCC voltage unbalance. The proposed method was verified using PSIM simulation and experimental results.
