전정감각, 고유감각 중심의 감각통합중재가 발달장애 아동의 공간 지각력과 시-운동 협응력에 미치는 영향 (The Effect of Vestibular-Proprioceptive Based Sensory Integration Intervention on Spatial Relations and Visual-Motor Coordination in Children with Developmental Disabilities)

  • 목적 : 본 연구를 통하여 발달장애 아동에게 적용한 전정감각, 고유감각 중심의 감각통합 중재가 공간 지각력, 시-운동 협응력에 어떠한 영향을 미치는지 알아보고자 한다. 연구방법 : 본 연구는 발달 장애 아동 만 5~6세 아동 3명을 대상으로 개별실험연구방법(single subject research design) 중 ABA' 설계를 사용하였다. 매주 3회기씩 6주간 30분씩 총 18회기를 진행하였다. 기초선 3회기와 재기초선 3회기는 중재 없이 공간 지각력과 시-운동 협응을 평가하였고 중재기 12회기는 전정감각, 고유감각 위주 감각통합 중재를 실시하였다. 중재의 효과를 알아보기 위한 매회기 측정으로는 전산화 인지 프로그램(CoTras-C)을 이용하였으며, 공간지각력 평가를 위한 퍼즐하기, 시-운동 평가를 위한 미로 찾기를 실시하였다. 연구의 사전, 및 사후 평가는 카우프만 아동용 지능 검사(Kaufman Assessment Battery for Children; K-ABC II)와 한국판 시지각 발달 검사(Korean-Developmental Test of Visual Perception-2: K-DTVP-2)의 하위평가를 이용하였다. 결과 : 발달장애 아동에게 전정감각, 고유감각 중심의 감각통합중재를 적용하기 전보다 후에 공간지각력과 시-운동 협응력 점수에 유의한 차이가 있었고, 세 아동 모두 통계적으로 유의한 차이가 있었다. 결론 : 전정, 고유감각 중심의 감각통합 치료가 발달장애 아동의 공간지각력과 시-운동 협응력에 유의미한 영향을 미쳤다.

Comparison of Trunk Control on Gross Motor Function and Topography in Children with Spastic Cerebral Palsy

  • PURPOSE: This study examined the differences in the trunk impairment scores according to the levels of the gross motor classification system by evaluating trunk control in children with spastic cerebral palsy using the index of trunk impairment. In addition, the characteristics of trunk control disabilities were investigated according to the cerebral palsy type. METHODS: The subjects were 49 children (mean age 8.57±1.83 years, 11 with hemiplegia, 26 with diplegia, and 12 with quadriplegia) with spastic cerebral palsy levels I to IV under the gross motor function classification system (GMFCS). The coordination and balance of the children with cerebral palsy were evaluated using the index for trunk impairment. Statistical analyses were performed using a Kruskal-Wallis test, and Bonferroni analyses were used as a post-hoc comparison for any significant results. RESULTS: The median of the total scores of trunk impairment was 13 (range, 9-17), which was 56% of the maximum score. The total score of trunk impairment and subscales differed significantly according to the disease severity and type of motor disability. The scores for children with quadriplegia were the lowest compared to children with hemiplegia and diplegia. CONCLUSION: Trunk control function in children with spastic cerebral palsy was reduced, and varied according to the disease severity and types of motor disabilities. The degree of trunk impairment differed from the trunk control ability according to the degree of motor disability of children with cerebral palsy.

A cross-sectional survey of clinical factors that influence the use of traditional Korean medicine among children with cerebral palsy

  • Background: Traditional Korean medicine (TKM) is widely used to treat children with cerebral palsy (CP) in Korea; however, studies investigating factors that influence the use of TKM are scarce. Thus, we investigated the clinical factors that might influence the use of TKM. Methods: A population-based, cross-sectional, multicenter survey was performed from August 2014 to May 2016. The history of TKM use, type and severity of CP, current treatment characteristics, presence of accompanying disabilities or other health problems not directly related to CP, and monthly cost for the treatment of CP were surveyed. Results: In total, 182 children were recruited, and 78 children (42.9%) had used TKM. Among these 78 children, 50 (64.1% of the TKM-use group) had used both acupuncture and herbal medication, 15 (19.2%) had used acupuncture only, and 13 (16.7%) had used herbal medication only. Children with non-typical CP, accompanying disabilities and general health problems tended to use TKM. The monthly cost of treatment for CP was significantly higher in the TKM-use group than that in the no-TKM-use group, suggesting that economically disadvantaged children may have difficulty in accessing TKM. Dietary supplements, conventional pharmacological treatments, and rehabilitation therapies did not affect TKM use. Conclusion: Children with non-typical symptoms or those with poor overall health status are likely to use TKM. Additionally, TKM use leads to increased treatment costs. Studies investigating the motivation for starting or ceasing TKM therapy, socioeconomic factors and the attitude of parents towards complementary and alternative medicine should be performed.

일본 장애아 요육시설의 설치 및 이용현황의 특성에 관한 연구 - 일본 동경권의 '아동 데이서비스'를 중심으로 - (A Study on the Characteristics of the Current Situation of the Child Care Centers for the Children with Disability in Japan - Focus on the 'Day Service for Children' in Tokyo -)

  • This paper aims to identify the characteristics and the current situation of the architectural characteristics, children's characters using facilities, and status of management and utilization by surveying 'Day Service for Children' in Tokyo-do. First, About the status of management and utilization, 'Day Service for Children' is operated by a local government, NPO corporate group and social welfare corporate. There are not big differences of the care program or running hours depending on operating principals but there are differences depending on staff organization, number of children and space constitution. Also, infants ratio at 'Day Service for Children' is high for the purpose of detecting disability early and supporting their development early. They have diverse disabilities but developmental disabilities' ratios high and most children are using both this facility and normal nursery centers together. Samples for proper child care based on status of space constitutions were installing lock, raising door knobs and using softer floor materials which are for children's safety. For the problem of space utilization, most comments were about lack of space which is not enough for teaching and training materials. Therefore, as an example, observation rooms for parents and teachers were sometimes used as storages and it changes teachers' moving direction, staying area and behavior zone. The programs of 'Day Service for Children' are divided to group teaching and private one. It will be the best if they have separate spaces such as group class room, private class room and play room. When they were operated by NPO corporate group or Social welfare corporate, there are many cases that their space was not properly zoned for each activity.

발달장애아동의 치료와 사회적응에 활용되는 소셜로봇에 대한 인식 및 기대: 주양육자를 중심으로 한 포커스 그룹 인터뷰 (Awareness and Expectation of Social Robots Used for Treatment and Social Adaptation of Children with Developmental Disabilities: A Focused Group Interview Study Focusing on Parents)

  • 본 연구는 발달장애아동을 대상으로 케어로봇을 치료에 활용하기 위해 발달장애 아동의 부모를 대상으로 소셜로봇이 갖추어야 할 기능과 요구 등을 분석하고자 시행한 질적연구이다. 연구대상자는 자녀가 발달장애 진단을 받았거나 1년 이상 특수교육을 시행하고 있는 만 18세 미만 자녀를 둔 주양육자 11명을 대상으로 진행하였다. 면담질문지는 반구조화된 질문지로 구성하여 소셜로봇에 대한 인식, 필요성, 기대, 염려 및 기타 의견 등으로 구성하였다. 면담은 COVID 19로 인해 비대면 온라인 화상 면담으로 이루어졌다. 연구결과, 4개의 주제가 도출되었다. 4개의 주제는 낯설지만 곧 마주할 현실, 능동적인 기능이 탑재된 소셜로봇, 소셜로봇의 외형과 재질에 대한 바람 및 소셜로봇에 대한 염려로 도출되었다. 본 연구를 통하여 양적연구에서는 파악할 수 없었던 주양육자의 소셜로봇에 대한 기대, 제안 등을 파악할 수 있었고 이를 향후 개발할 소셜로봇에 반영할 수 있다면 보다 수용성이 높은 긍정적인 결과를 도출 할 수 있을 것으로 기대한다.

일본감각력개정판(JSI-2)에 의한 발달장애아동과 정상아동의 감각처리능력 비교에 관한 예비연구 (Comparison of Japanese Sensory Inventory-2 Scores of Children With and Without Developmental Disabilities - A Pilot Study)

  • Objective : To describe the sensory-based behaviors of children with developmental disorders as reported on the JSI-2. Methods : The scores of children with disability were compared with those of children without disability. JSI-2 was completed by parents of 38 children with disability 3 through 6 years of age and 36 children without disability 3 through 6 years of age. Results : The scores of children with disability were significantly different from that of children without disability for 6 of 8 factors, including vestibular, tactile, auditory, visual and other. There is no significant differences of age or sex. Conclusion : From this study, it is found that children with disability have deficit in a variety of sensory processing abilities as measured by JSI-2. Further research is needed to replicate and develop these findings.

읽기장애아동과 일반아동의 동시조음 특성 비교 (A comparative study of coarticulation features between children with and without reading disability)

  • 동시조음은 제한된 시간과 공간 안에서 연속적인 조음기관의 움직임으로 인해 주위의 분절음과 다양한 중첩을 통해 서로 영향을 주고받는 것이다. 본 연구는 파열음, 파찰음과 모음(ᅡ, ᅵ, ᅮ)으로 구성된 CVC 음절과 VCV 음절에서 읽기장애아동과 일반아동의 동시조음 특성 차이를 규명하였다. 연구대상자는 초등학교 2-6학년 읽기장애아동 13명과 일반아동 13명이었다. 읽기과제 음성 자료를 이용해 자음이 끝나고 모음이 시작되는 지점과 모음 안정 구간의 중간 지점에서 제 2 포먼트를 측정하였다. 측정된 제 2 포먼트를 이용해 회귀분석을 실시해 집단별 조음위치, 발성유형에 따라 로커스방정식(LE) 기울기를 구하여 삼원분산분석(3-way ANOVA)을 실시하였다. CVC 음절에서 읽기장애아동은 일반아동에 비해 작은 LE 기울기 값을 보였다. 조음위치에서는 연구개음이나 양순음이 치조음이나 경구개음과 비교해 더 가파른 LE 기울기를 보였다. VCV 음절에서는 집단, 발성유형에서 유의미한 차이를 보이지 않았으며 조음위치에서 보인 유의미한 차이도 CVC 음절 결과와 다른 양상을 보였다. 본 연구는 읽기장애아동이 음절구조에 따라 다른 양상의 동시조음 기울기를 보인다는 것과 읽기장애 아동의 높은 휴지 비율은 VCV 구조에서 동시조음에 더 큰 영향을 미친다는 것을 확인하였다.

Teaching Indigenous Students With Developmental Disabilities: Embedding the Cultural Practices of Dance, Movement, and Music in Pedagogy

  • The rich cultural backgrounds and practices of children from native cultures are often absent in classroom instruction, as teachers might feel that they have inadequate knowledge and backgrounds in the unique practices of these populations. Historically, children from native cultures have had challenging educational experiences and poor educational outcomes. To address these challenges, we propose a Family and Culture Based (FCB) framework that draws from family-centered practice, asset-based practice, and culturally responsive pedagogy. This article describes the three steps of the FCB framework, which uses a teacher-as-learner approach to instruct students from native cultures by engaging teachers in reflection, gaining knowledge about the cultural practices of the family and community, and integrating the new knowledge into practice. We use a Pacific Islands case (Native Hawaiian family) and a Pacific Northwest case (First Nations family) to illustrate the potential benefits of using the FCB framework with indigenous students.

2016년 전북대학교 치과병원 소아치과를 방문한 장애인환자에 관한 조사연구 (A SURVEY OF PATIENTS WITH DISABILITIES VISITING THE DEPARTMENT OF PEDIATRIC DENTISTRY OF CHONBUK NATIONAL UNIVERSITY DENTAL HOSPITAL IN 2016)

  • According to the Ministry of Health and Welfare, the portion of physical disability was 51.4% which was the highest among the registered disabled in December 2015, followed by visual disability(10.1%), disability of the brain lesions(10.0%), hearing disability(10.0%). The aim of this study is to investigate the types of disability in patients with disabilities visiting the Pediatric Dentistry, Chonbuk National University Dental Hospital in 2016. Of the patients who visited the pediatric dentistry, 196 patients (7.5%) had disabilities, and 19 patients had two or more disabilities. Twelve patients (6.1%) had both disabilities of brain lesion and intellectual disorder. Of the 177 patients with only one disability, 54 patients (30.5%) had physical disorders and 123 patients (69.4%) had psychiatric disorders. In psychiatric disorders, intellectual disorder was the most with 98 patients (55.3%), followed by autistic disorder with 23 patients (12.9%). In physical disorders, disability of brain lesion was the most with 37 patients (20.9%), followed by speech disability with 8 patients (4.5%). In this study, as a result of analyzing the types of disability with the patients who visited the Pediatric Dentistry of Chonbuk National University Dental Hospital, most of them had either intellectual disorder, disability of brain lesion, or autistic disorder. Dentists need to understand their general and dental characteristics to treat children and adolescents with disabilities. Since the most chief complaint of the patients was dental caries, early interventions should be made by dentist to reduce the incidence of the disease.

장애아동의 학교준비도 프로그램(School Readiness Program)에 대한 체계적 문헌 고찰 (A Systematic Literature Review of School Readiness Programs for Children With Disabilities)

  • 목적 : 본 연구는 장애아동의 학교준비도 프로그램을 다룬 문헌을 분석함으로써 연구의 대상자, 프로그램의 제공자, 기간, 요소 및 결과 등을 알아보고자 한다. 연구방법 : PubMed, Embase, Web of Science, 학술연구정보서비스(Research Information Sharing Service)를 사용하여 문헌을 수집하였다. 주요 검색어는 "School readiness" AND ("Occupational Therapy" OR "Rehabilitation"), "학교준비도" AND ("작업치료" OR "재활")을 사용하였다. 선정기준과 배제기준을 통하여 최종 8편의 연구가 선정되었다. 결과 : 학교준비도 프로그램 유형에는 복합적 기능 훈련, 운동 기술 훈련, 부모교육 프로그램, 애플리케이션을 이용한 프로그램들이 있었고 가장 높은 비중을 차지한 유형은 복합적 기능 훈련이었다. 프로그램의 제공자는 작업치료사, 심리학자, 물리치료사, 언어치료사, 사회복지사, 교육자가 있었고 심리학자가 가장 많은 연구를 진행하였다. 중재 프로그램의 구성요소는 학업 기능, 운동 기능, 사회적 기능, 부모교육, 기타로 분류할 수 있었으며 학업 기능과 사회적 기능이 큰 비중을 차지하였다. 프로그램 적용 결과 다기능적 기술, 문해력, 양육 기술, 대동작·소동작 기능의 향상을 나타냈다. 결론 : 본 연구는 장애아동의 학교준비도 프로그램 유형, 제공자, 중재 요소, 결과 등을 분석하여 학교 기반 작업치료의 기초 자료를 마련하고자 하였다. 최근 학교준비도 프로그램에 관한 관심과 연구는 증가하고 있으며 작업치료사들도 학교 관련 재활 분야에서 역할을 정립하여 다양한 학교 기반 작업치료가 이루어져야 할 것이다.
