Purpose: The aim of this study was to investigate the effects of gluteal taping on pelvic alignment, trunk stability, and balance during sitting posture in children with unilateral cerebral palsy (CP). Methods: Thirteen children with unilateral cerebral palsy (six females. seven males; mean age 8.5) participated in this study. All participants were evaluated before and after gluteal taping using an Inclinometer for pelvic lateral inclination, trunk impairment scale (TIS) for trunk stability, and modified functional reaching test for balance during sitting. The collected data were analyzed using a paired t-test. Results: The results of this study were as follows: 1) Statistically significant decreases in the angle of pelvic lateral inclination were observed after gluteal taping in children with unilateral CP (p<.05). 2) Statistically significant increases in TIS score were observed after gluteal taping (p<.05). 3) Statistically significant increases in the range of reaching during sitting were observed after gluteal taping (p<.05). Conclusion: : In conclusion, this study showed that gluteal taping improves pelvic alignment, trunk stability, and balance during sitting in children with unilateral cerebral palsy. Further studies will be required to determine the short- and long-term effects of gluteal taping on improving postural symmetry, trunk stability, and balance.
목 적 : 2세 미만의 소아에서는 임상적으로 확실한 증상이 있거나 알레르기 가족력이 있는 것을 제외하고는 실제적으로 아토피 유무를 판별하는 것은 성인에 비해 제한점이 많다. 본 연구에서는 영유아기에 알레르기 질환과 관련된 사이토카인이 유전자 배열에서 발현하는 지를 말초혈액의 Th2 림프구에서 interleukin-4 유전자의 DNA 탈메틸화를 통하여 알아보고자 하였다. 방 법 : 대상은 한양대학교 구리병원 소아알레르기클리닉을 내원한 3세 미만 소아에 대해 소아의 병력과 가족력을 조사하여 family allergy score와 개인력을 기준으로 알레르기질환에 대한 고위험군 7명, 저위험군 8명, 정상 대조군 7명이었다. 이들 모두에서 총 IgE치와 Der f II 특이 IgE를 측정하였고 이들의 말초혈액에서 T세포를 Der f II로 자극하여 배양한 후 DNA 추출을 하여 IL-4 유전자 두 번째 인트론의 CpG 섬에 대한 메틸화 분석을 실시하였다. 결 과 : 정상군에 비해 알레르기 환아군에서 IL4 유전자의 두번째 인트론에 위치한 CpG 섬의 두 부위 모두에서 메틸화 특이 PCR 신호강도가 약하게 나타나는 반면 정상아에서는 118 bp PCR 산물과 205 bp PCR 산물이 모두 강하게 나타나서 메틸화된 것으로 보이며, 저위험군 알레르기 환아에서 최소한 한 부위에서 탈메틸화가 일어났으며 고위험군 알레르기 환아에서는 두 부위 모두에서 탈메틸화가 일어났다. 결 론 : 정상 대조군의 T 세포에서는 IL-4 유전자의 메틸화가 일어나고 알레르기 환아에서 고위험군과 저위험군 모두 정도의 차이는 있으나 메틸화의 소실이 관찰되었다. 이를 이용하여 향후 영유아기에서 아토피 유무를 판별할 수 있는 가능성을 제시하였으나 더 많은 연구가 필요할 것으로 사료된다.
The purposes of this study were to compare the reliability and validity of an 88-item version of the Gross Motor Function Measure (GMFM-88) and a 66-item version of GMFM (GMFM-66) in children with cerebral palsy (CP). The GMFM was completed in 154 children with CP (age range = 6~12). The internal consistency of the GMFM was calculated by Cronbach's ${\alpha}$ for judging reliability. The reliability of GMFM-88 and GMFM-66 were both above .99. The validity of measurement obtained by the GMFM was assessed by examining the unidimensionality of items and by comparing Gross Motor Function Classification System (GMFCS) levels with tests of the GMFM. Both the GMFM-88 and GMFM-66 were satisfied with unidimensionality. Discriminant validity was demonstrated on significant decreases in scores with increasing GMFCS levels in both measurements. However, GMFM-66 was a more sensitive discriminant in GMFCS level 1 and level 2 and in level 2 and level 4. This study reported a comparison of the reliability and validity of GMFM-88 and GMFM-66. The results of this study have implications for the information on the psychometric properties of two versions of GMFCS. This information will be useful for the selection of tools in clinics.
이 연구는 뇌성마비 아동의 기능적 수준 분류 체계의 유용성을 알아보기 위해 실시되었다. 이를 위해 2008년 9월부터 2010년 8월까지의 기간 동안 뇌성마비 아동 217명을 대상으로 대동작 기능 분류체계(GMFCS), 손 기능 분류 체계(MACS), 일상생활 활동을 측정하고 도구 간의 관계를 알아보았다. 그 결과, 대동작 기능 분류체계와 일상 생활 활동은 모든 하위 영역 및 총점과 유의한 상관이 있는 것으로 나타났다(p<.05). 손 기능 분류체계와 일상생활 활동은 이상운동형 아동에서 일상생활 하위 영역 중 의사소통과 상관이 유의하지 않은 것을 제외하고 모든 하위 영역 및 총점과 유의한 상관이 있는 것으로 나타났다(p<.05). GMFCS와 MACS의 관계를 알아본 결과, 가장 많은 분포를 나타낸 것은 GMFCS의 경우 1수준(20.3%)과 5수준(40.6%)이었고, MACS의 경우는 2수준(48.8%)과 5수준(16.6%)이었다. 결론적으로, 뇌성마비 아동의 기능적 수준 분류 체계인 GMFCS와 MACS는 임상적으로 유용한 평가 체계로 사용될 수 있을 것으로 생각된다.
A critical pathway (CP) defines the optimal care process, sequencing and timing of intervention by multi-disciplinary health care teams for a particular diagnosis and procedure. It plays an important role as a cost-effective health care delivery system and a tool for quality control of medical and dental services by means of standardizing medical practices. The aim of this study is to investigate the satisfaction of patients and medical/dental staff after implementation of a critical pathway for dental treatment of disabled children under general anesthesia and its cost effectiveness. Ten patients who underwent dental treatment under general anesthesia were included in the CP group between August and December 2006. The pre-CP group included 20 patients who underwent the same procedure from February 2003. The satisfaction of parent of child patient and medical staff members were compared between two groups. The parents' satisfaction was significantly improved after the implementation of CP and medical/dental staff members were highly satisfied with the usefulness of the critical pathway. In conclusion, the critical pathway for the dental treatment of disabled treatment under general anesthesia can highly improve the satisfaction of parents and medical/ dental staff members.
Purpose : The aim of this study was to determine the reliability of parents and therapists-assessed manual ability using the Manual Ability Classification System (MACS) for children with cerebral palsy Methods : Subjects were 136 children with cerebral palsy using rehabilitation programs in 16 welfare centers. Reliability was determined using the intraclass correlation coefficient (ICC). Parents and therapists assessed manual ability of children using MACS. Result : The 136 children (Male 73, Female 63) mean age was 7y 5mo years [range 3y 11mo - 13y 5mo]. The overall agreement between parents-assessed and therapists- assessed MACS was good (ICC = 0.84, 95% confidence interval 0.77-0.88). Conclusion : The MACS offers a reliable method for population-based research and communicating about the manual ability of children with CP.
Purpose: The aim of the current study was to examine the effects of hinged ankle-foot orthosis (HAFO) on walking function in children with spastic diplegic cerebral palsy (CP). Methods: Thirty-two children (mean age: $6.79{\pm}0.35years$, age range: 5-7 years) who were diagnosed with spastic diplegic cerebral palsy participated in the study. Each subject typically walked through 10 meters of a gait platform with markers on the subject's proper body segments and underwent 3-D motion analysis system with and without hinged ankle-foot orthosis. The HAFOs were all custom-made for individual CP children and had plantarflexion stop at $0^{\circ}C$ with no dorsiflexion stop. The interventions were conducted over three trials in each group, and measurements were performed on each subject by one examiner in three trials. 3-D motion analysis system was used to measure gait parameters such as walking velocity, cadence, step-length, step-width, stride-length, and double support period in two conditions. Results: The walking velocity, cadence, step-length, and stride-length were significantly greater for the HAFO condition as compared to the no HAFO condition (p<0.05). However, no significant difference in step-width and double support period was observed between two conditions. Conclusion: These findings suggest that using the HAFO during walking would suggest positive evidence for improving the spatiotemporal parameters of gait in children with spastic diplegic cerebral palsy.
Purpose: Despite the established role of imatinib (IM) in chronic myelogenous leukemia (CML) in adults, there are few reports on its efficacy in children. In this study, we compared the outcomes of children with CML before and after the advent of IM-based treatment. Methods: The study cohort consisted of 52 patients treated for CML at the Department of Pediatrics, The Catholic University of Korea from January 1995 to October 2010. Patients were divided and analyzed according to the preImatinib group (pre-IMG) and imatinib group (IMG). Results: Median age at diagnosis for the overall cohort (pre-IMG, n=27; IMG, n=25) was 9 years, with a median follow-up duration of survivors of 84 months. Except for 5 patients in the IMG, all were diagnosed in chronic phase (CP). The overall survival (OS) of patients diagnosed in CP was 45.7% and 89.7% for pre-IMG and IMG, respectively (P=0.025). The OS of hematopoietic stem cell transplantation (HSCT) recipients in the 2 groups was similar, but the OS of patients diagnosed in CP who did not receive HSCT was superior in IMG (91.7% vs. 16.7%, P=0.014). Of the 12 patients in IMG who remained on IM without HSCT, 2 showed disease progression, compared to 11 of 12 in pre-IMG. No difference was observed in the progression free survival (PFS) of matched donor HSCT recipients and IM-based treatment recipients. Conclusion: Similar PFS of patients treated with IM and those who received matched donor HSCT underscore the potential of IM as effective first-line treatment in childhood CML.
The purpose of this study was to find out the acoustic variation on the pre-and post respiration and oral motor for children with cerebral palsy. Five children with spastic CP at the age of 6 in average were practiced by a caregiver at home each for 25 minutes, in total, 45 times. The sustained of vowel /a/ and vowels /a/, /i/, /u/, /e/, /o/ were recorded on CSL and MDVP and analyzed by acoustic parameters. As a result, the maximum phonation time(MPT) was increased from 2.06 to 6.31 and the formant of vowels(F1, F2, F3) had significant differences in F1(/a, i/), F2(/i.u.o/), and F3(/a/) between the controls and the children with CP in pre-treatment. The total average value of vowels had significant differences between the pre-and post-treatment (p< .05). The energy of vowels had significant differences in the vowels /i, u, e, o/ and the total average value between the pre-and post-treatment(p< .001). The jitter percent, shimmer percent, and noise to harmonic ratio had significant differences between the pre-and post-treatment(p< .05). As the respiration and the oral motor improved MPT, voice quality, and articulation of vowel, and the variation of the formant(F1, F2, F3) showed the changes in the shape of lips, the place and the height of the tongue, the various development of therapy programs and the consistent intervention of treatment is needed for the children with cerebral palsy.
목적 : 본 연구는 한글판 PDMS-2(Peabody Developmental Motor Scale-2)를 뇌성마비 아동과 일반아동을 대상으로 평가하여 구성타당도를 검증하고 뇌성마비 아동 10명을 대상으로 재평가하여 검사-재검사 신뢰도를 알아보고자 하였다. 연구방법 : 구성타당도 검증은 뇌성마비 아동과 일반아동 84명을 대상으로 하였다. 일반아동은 뇌성마비 아동과 연령이 6개월 이내로 비슷하고 성별이 일치하는 아동으로 매칭하여 선별하였고, 한글판 PDMS-2평가를 실시하여 두 집단의 표준점수 비교를 통해 구성타당도 검증을 하였다. 이 후 뇌성마비 아동 중 10명을 선정하여 2주 뒤 재평가를 실시하여 검사-재검사 신뢰도를 알아보았다. 결과 : 구성타당도 검증 결과 대근육, 소근육 영역별 점수와 하위 다섯 항목의 점수가 모두 통계적으로 유의미하게 나타났다(p=.000). 검사-재검사 신뢰도는 Pearson 상관계수 .989로 매우 높은 수준의 시간적 안정성을 나타냈다. 결론 : 본 연구를 통해 한글판 PDMS-2는 아동의 운동 발달 수준을 알아보기에 높은 신뢰도와 타당도를 가진 평가 도구임을 확인하였다. 추후 한글판 PDMS-2는 국내 임상 현장과 연구 도구로 유용하게 사용될 수 있을 것이다.
