Evaluation of Organizational Effectiveness as to Types of Maternal-Child Health Management Program in Public Health Centers (보건소 모성과 어린이 건강관리사업의 조직유형별 효과성 평가)

  • Purpose: The purposes of this study are; to classify executive organizations engaged in MCH programs at 23 advanced public health centers according to each characteristic; evaluate the effectiveness of organizations engaged in MCH programs. Method: This study analyzes me data on the present conditions and evaluates the organizational effectiveness of MCH workers at 23 advanced public health centers. The organizational effectiveness of MCH organization is assessed by both MCH workers and clients who have received MCH services at the three health centers selected from me 23 advanced public health centers. Finally, this study seeks to obtain consensus among experts in the field of MCH. Results: The results are as follows. The executive organizational systems for MCH services are classified into three types: Type I (provides MCH and HP services by MCH org.); Type II(provides MCH and HP Services by HP org); and Type III(provides MCH Services by MCH org. and HP services by HP org.) at leading MCH Health Centers. The evaluation of the organizational effectiveness of MCH org. shows that Type II is the most effective in terms of teamwork, autonomy, service quality, and resource utilization. According to clients' evaluation of organizational system in the field of MCH, Type II is also me most effective. Conclusion: This study suggests that the executive organization for the MCH and HP services for maternity and children should be established in accordance with lifetime health programs in order to efficiently utilize the limited MCH and HP resources at public health centers.

An Analysis of Health Education Contents of the 7th Elementary Curriculum (제7차 초등학교 교육과정의 보건교육 내용 분석)

  • In this study we analysed time allotment and the contents of a health textbook and its teacher's guide book of the 7th elementary curriculum. We intended to offer the basic data needed to establish the single health education. So the analysed results are as follows according to the health education model developed by the korean nursing association and health teachers' meeting and the teaching time allotment presented by the teacher's guide book. It's goal is practice in regular class time of the subjects for the time and contents of health education in the 7th elementary curriculum. The total class periods of health education of the 7th elementary curriculum are 229 hours and annual periods of health education per year are an average of 38 hours. The health education of the 7th elementary curriculum is separated into the 9 following subjects: Wise life, Pleasant life, Righteous life, We are 1st grade, Physical education, Science, Social studies, moral education and Practical course. The health education of the 6th elementary curriculum was combined with the units of physical education, but in the 7th curriculum it must be separated by a single, required health subject. The contents of health education of the 7th elementary curriculum is mostly dominated by units of community and environmental health with a total of 55 hours (24%). Therefore, the units of home health and social health, development of physical strength are fairly insignificant. The newly added contents in the 7th curriculum are "pregnancy and child birth, the protection and counterplan from rape, the reasons and prevention of stress, the reasons and the treatment of obesity, the damage of smoking and drinking, etc. According to the result above, we must establish the criteria for each year's health education in the 8th elementary curriculum. The contents of mental health, home health and social health should be revised and added new items. The health education that is separated in some subjects now must be established as a systematically integrated health education.

A Study on Abandoned Children through the Analysis of Medical Records in a Public Hospital (공공기관 의무기록부에서 관찰된 기아(棄兒)에 관한 고찰)

  • It is the purpose of this study to provide the basic data to stimulate the social understanding of an abandoned child from the medical viewpoint and the viewpoint of administrative management. The data are based on the medical dossier of 305 patients which were hospitalized in 'A' hospital (of Seoul municipal hospital) between 1992 and 1996. The method of analysis is SPSS statistical package and the following results can be obtained 1. The abandoned children were frequently found in the Spring by season, in April by month, on Friday by day, and especially in the night and before an individual home and an medical institution in many cases. 2. The children found in an individual home were younger, handicapped smaller, and hospitalized for shorter period. As they were healthy relatively, parents might hope that they were accepted as foundling foster children. Contrary to above case, the children found in an medical institution were worse, handicapped more, and hospitalized for longer period. Accordingly, parents might hope that they were protected in an public institution. 3. The $69.8\%$ of abandoned children had caught a disease at that time and the $53.8\%$ of them had been handicapped. The most serious disease and handicap were the heart-lung trouble and the cerebral palsy, respectively. 4. The only $60.8\%$ of them were adopted and the most of them were healthy relatively. The $10.2\%$ which have been dead had caught four diseases on the average. As the results of above, the followings can be proposed : 1) It is required that the system connected to administrative management, which can provide the abandoned children turned for the better with the family relations to voluntary servants, be developed. 2) It is required that the system of registration management and plan for support economy, which can support the handicapped birth as the congenital malformation in national aspect, be developed. 3) It is required that to protect abandoned children, the health educational program for childbirth-people be prepared in the unit of a public health center.

The Effects of Acupuncture on the Insomnia, Anxiety and Depression of Liver Cancer Patients: Pilot study

  • Objectives: Most patients who have cancer suffer from anxiety and depression. Because most liver cancer patients have cirrhosis, medication can aggravate their liver function. We need the most effective treatment such that the anxiety and depression can be improved without causing deterioration of liver function. The purpose of this study is to examine the effects of acupuncture on the anxiety and depression of liver cancer patients. Methods: 8 liver cancer patients who complained of distress were treated by acupuncture three times a week for four weeks (12 times in total). Evaluation of the anxiety, depression and insomnia was measured by NCC (National Cancer Center) score. On visits 1, 4, 7, 10, and 13 we had patients check NCC score after acupuncture treatment. To check deterioration of liver cancer, we checked their abdominal CT, Child-Pugh score and blood test. Results: After 12 treatments, NCC of the anxiety, depression and insomnia were significantly decreased in all of the patients without causing deterioration of liver function. In particular, symptoms were almost completely improved in 6 patients. Conclusions: This study suggests that acupuncture treatment will be beneficial for liver cancer patients to reduce anxiety, depression and insomnia. For liver cancer patients with cirrhosis or chronic hepatitis, a large-scale study to confirm efficacy and safety of acupuncture is needed.

A Study on Health Locus of Control and Anxietu of Pregnant Women (임산부의 건강통제위와 불안에 관한 연구)

  • The purpose of this study is to prepare the basic data for improving anxiety relevant to characteristics of pregnant women. The researcher has examined the degree of anxiety of pregnant women and relationship between the characteristics of Health Locus of Control(HLC) and the anxiety. This research was conducted among 202 pregnant women who visited two obstetric outpatient departments for antenatal care at University hospitals in Seoul and Pusan, from December 1, 1998 to January 10, 1999. The tools used for this study were questionnaire and Wallsston and Wallston's HLC Scale was used for measurement of anxiety. The result of this study was as follows; 1. The mean age was 29 years, proportion of women completed high school was 53.5%, the most and low class was the most, 81.2%, necleas family, the most, 92.1% and the most(70.3%)was the buddist. 2. 70.3% had experience of 1-2 pregnancy and 54.0%, the most had 1-2 child, 7-9 month pregnancy is the highest percentage, 66.3% and the most pregnant women(81.2%) was satisfied with their husbands and highest level of 70.8% revealed no change in sexual life compared to before pregnancy. 3. Expectant women showed HLC-Internal and low level of anxiety. 4. The factors of education and economics related to the intensify of the anxiety significantly. 5. The anxiety was affected by relationship with husband, attitude of pregnancy and minor discomfort during pregnancy. It is conducted that the anxiety of pregnant women may be increased due to education, economics, relationship with husband, attitude of pregnancy and minor discomfort during pregnancy.

Early Preterm Infant Feeding Behaviors in Adolescent and Adult Mothers

  • Background: Adolescent mothers may find the transition to motherhood to be overwhelming when compared to adult mothers. The adaptiveness of mother and infant feeding behaviors may greatly influence the quality of adolescent mothers' feeding interactions when compared to adult mothers. Purpose: To examine the adaptiveness of early maternal and infant feeding behavior between adolescent mothers and adult mothers. Method: In this secondary analysis, six adolescent mothers (< 20 years old) and six adult mothers were videotaped while feeding. The mothers were observed 3 times: when the infants were in the hospital, 1 month old, and 4 months old. The videotapes were coded using four scales of the Parent-Child Early Relational Assessment (PCERA). The two maternal scales were Positive Affective Involvement and Sensitivity/Responsiveness (PAISR) and Regulation of Affect and Behavior (RAB). The two infant scales were Infant Positive Affect, Communication, and Social Skills (IPACS) and Infant Regulation of Affect and Behavior (IRAB). The dyads were videotaped just before hospital discharge and at one and four months corrected age. The Wilcoxon signed-rank test was used to examine the difference in the PCERA between the two groups. Results: Adolescent mothers had a significantly lower score on the PAISR before hospital discharge (p-value 0.005) and at one month corrected age (p-value 0.008) than adult mothers. Adolescent mothers also had a significantly lower score on the RAB before hospital discharge, 1 and 4 months corrected age than adult mothers. There were no significant differences in IPACS and IRAB of infants of adolescent and adult mothers. Conclusion: Clinicians need to be aware of the differences in sensitivity that occur.

A Survey of Life Style Habits of Obese School Children (학령기 비만아동의 생활습관에 관한 조사연구)

  • The purpose of this study is to investigate the effect of obese school children's life style habits in order to help school nurse in controlling and preventing the obesity by health education The survey group is extracted from the students of two elementary schools located in Seongnam city Out of 280 children randomly sampled, 102 children were evaluated to be obese and 178 ones to be normal by the standard of the 1985 Korea Pediatric Association Height and Weight Standard Chart The obese children group was compared with the normal one. For this purpose, the surveyors designed a self questionnaire to obtain the description of general background, exercise including play behavior, eating behavior, and family environment of the sampled children. The results are as follows The number of the obese children who took extracurricular physical exercises was significantly (p<0.05) more than that of the normal ones The significantly (p<0.05) more obese children went without a meal to control their weight than the normal ones. In relation to the degree of obesity, the children with the high degree of obesity watched the television significantly (p<0.05) longer than the ones with the mild and the moderate degree of obesity In gender relations, the obese girls participate in outdoorplays and physical exercises significantly (p<0.05) longer and more regularly than the normal ones, while no significant difference was found between the male groups The obese boys had significantly (p<0.05) their mothers occupying in a vocation. Combining television watching habit and mother's vocation of the obese boy, the obesity is resulted from complicated situation It can not be due to the simple absence of the mother, but rather due to the absence of a person responsible for controlling snacking and television watching time of the child Looking at the results, it is necessary for both school and home to actively involve in guiding children and providing an environment to modify life style habits and prevent childhood obesity.

An Analysis of Relations between Perceived Family Characteristics, Experienced Abuse and Mental Health in Childhood (학령기 아동이 지각한 가족 특성, 경험한 아동 학대와 정신 건강과의 관계)

  • This study analyzes the relations between perceived family characteristics, experienced abuse and mental health in childhood. For this, this study used row data by questionnaire, analysis, and frequency, ANOVA, t - Test, Pearson' correlation analysis. The sample was 118 children 10-12 years old in primary school. The findings are as follows. First, children perceived family cohesion and family adaptability highly, family adaptability showed a significant difference from the relations with a parent's job, a parent's academic level, and type of residence. Second, it appeared that some children experienced physical, emotional, and sexual abuse. Third, in general the mental health of children was good. Their mental health showed a significant relation to economic level of family, and type of residence, creating problems such as depression, anxiety, phobic anxiety, psychoticism. By family size, their mental health showed a significant relation to somatization, interpersonal sensitivity, depression, hostility, phobic anxiety, paranoid ideation, and psychoticism, Fourth, family cohesion and mental health perceived by children supported a linear relation to phobic anxiety, and family adaptability and mental health perceived by children supported the reverse -linear relation to somatization, anxiety, paranoid ideation, etc. Fifth, connections with perceived abuse and mental health as well as emotional abuse and mental health were also supported. Further more, on somatization, interpersonal sensitivity, depression, hostility, phobic anxiety, paranoid ideation, psychoticism, etc, a reverse-linear re lation existed. Physical abuse supported a reverse-linear relation with interpersonal sensitivity, depression, paranoid ideation, etc. and sexual abuse supported a reverse-linear relation with depression. These findings suggest that school and family have to concern themselves with the mental health of children because experienced abuse and family characteristics do indeed affect the mental health of children.

Review of Prenatal Care on Clinical and Statistical Observation (산전관호 결과에 대한 고찰(임상 통계적 관찰에 의한))

  • Anions a total of 1,410 women who were taken in th the Ilshin Maternity Hospital in Pusan for child-bearing front January 1 to Hay 31, 1970, review and observation were made on 653 mothers and 661 babies delivered (including twin) who had received parental care, and on 757 mothers and 773 babies delivered (including twin) who had not received parental care. The results are: 1. Out of 1,410 women 653 received parental care, The number is 46.4% 2. Most of them are between 25 and 29 years of age, and it shows the highest percentage. 3. Twenty five % of them visited the hospital care for the first time in the tenth month of pregnancy It was the highest percentage. 4. Those who visited the clinic only once for parental care during the pregnancy were 25.7%, the highest percentage. 5. Those who got parental care among primiparae were 43.7%, and among multiphase were 48.5% The rates are similar to each other. 6. Maturity rate of the babies delivered after receiving prenatal care: mature 91%, Immature- 1% and premature- 8%. 7. It was found that among primiparae were the higher frequency of prematurity. 8. Among babies delivered by mothers after receiving parental care, 8.1% were premature, while 16.1% of babies delivered by those who han't received the care were premature. 9. Mothers between 25 and 29 years of age brought birth to premature. babies most. 10. Frequency of prematurity based on the times of parental care received was: once or twice -8.7% 3 or 4 time-8.9%, 5 or times-10.3%, 7 or 8 times-4.5%, 11 or 12 times-2.7%, 13 or 14 times-3.8%. 11. Percental death rate shows 2.9% among the mothers received prenatal care, and 7.3% among those without parental care. 12. Average weight of a baby boy was 3,114 gm, and that of a baby girl was 3,021 gm. 13. Obstetric complication rate was 63,2% in the babies delivered by mothers with prenatal care, and 85.6% in those delivered by mothers without parental care.

A multi-cultural family assistance program is provided these infants and children to the objects. Conclusions: Infant vaccination execution in Primary Health Care Posts showed to high vaccination rate because the prevention of vaccination delay or omission of the interior child who raised geographical accessibility to object sons nowadays it difficult to management of transportation of vaccine and storage management for a little shroud objects limited and management of inoculation business in quality. A main problem is as follows. There are a little vaccine transportation, trouble of proper temperature maintenance of a storage process, and having a lot of vaccine abolition as a little objects, educational training shortage of health care practitioner regarding to execute an inoculation. Therefore, vaccination business needs measures for management in quality in Primary Health Care Posts.
