Autism is characterized with an abnormality of reciprocal social relatedness and of communication development. Also, autistic children show repetitive and stereotypical behavior pattern and deficiency of endurance against change. Dental problems related to autism are self-injury and trauma from accidents. Sometimes self-injury is due to discomfort resulting from restoration and local anesthesia. Also, gingivitis and multiple caries are common because of poor oral hygiene. Autistic children are anxious about visual and auditory stimuli because they don't understand the dental procedure. If the face of any changes in their daily life, their behavior becomes uncontrollable. In a setting such as the outpatient office, an autistic child's uncontrollable behavior makes it difficult to carry out proper treatment. If they need treatment under general anesthesia, it is very difficult to manage them in a ward as they are severly anxious about being admitted to a hospital. Therefore it is necessary to treat these children according to a specific management program under general anesthesia in an outpatient office. This case study reports of a 14 year old autistic teenager who although became uncontrollable because of the pain from multiple caries and from anxiety of the dental treatment eventually received treatment under general anesthesia and under conscious sedation in an outpatient office.
The subjects, under 18 old, 2,694 children who visited ER during 1998, were surveyed by the Triage and other statitical data. The results were as follows 1. The male to female ratio was 1.7 : 1, and the toddler(1-4 old) was the majority. 2. Triage ; critical 1.3%, acute 14.6%, urgent 29.5%, nonemergent 54.6%. 3. The reasons of visiting ER ; 1) The children had diseases(46.5%), injury (36.9%), TA(5.6%) and toxication(1.0%). 2) In diseases, male to female ratio was 1.5 : 1 and in injury, male to female was 2 : 1. 3) Among the children having in TA, 12-18 old groups was the majority(34.9%). 4. The time of visiting : the 20:00 - 22:00 was the majority(16.9%). 5. By monthly and seasonal distribution ; Jan. (9.7%), Mar. and May(9% respectively, Dec. and July(6.7%) respectively. The children who visted ER in spring and autumn showed higher portion than those of summer and winter. 6. Results ; admission(27.4%), discharge(68.4%), operations(2.8%), and DOA and DAA(0.4%), The mortality of the infancy and toddler groups was 83.3%. The infancy group showed the highest rate of admission. 7. The time of staying in ER ; 1-2 hrs was the major group(23.3%) and the average was 4.6 hours. 8. By clinical departments ; Ped. was 34.4%, PS was 20.8%, Dental Surgery was 10.3% and Dermatology was 0.9%. 9. The types of visting ; the group who visited by themselves was 80%, transfer from the primary and secondary clinic was 17% and OPD was 3.0%. 10. The traffic means . by the own cars and taxi were 87.6%, by hospital ambulance was 6.1% and by 119 ambulance was 4.3%.

Objectives : This study investigated factors related to suicide in high school students and focused on perceived academic performance. Methods : We surveyed 293 grade 10 high-school students in Jeju and Gyeonggi provinces and gave them self-assessment questionnaires. To evaluate the suicide risk, the questionnaire included questions about suicidal thoughts, plans, threats, deliberate self-injury, and suicide attempts from the Scale for Suicide Ideation (SSI). Also, it included the Beck Depression Inventory (BDI), Beck Hopelessness Scale (BHS), Locus of Control Scale (LOC), Rosenberg's Self Esteem Scale (RSE) and Perceived Academic Performance (PAP) to identify factors related to suicide in high school students. Results : Depression, hopelessness, and low self-esteem were related with suicidal thoughts, plans, attempts, and severe suicidal ideation (SSI score${geq}$16). In the multivariate analysis, a low level of PAP (OR=3.796, 95%CI 1.280-11.257) was significantly associated with severe suicidal ideation (SSI score${geq}$16). Deliberate self-injury was most prevalent in failing the PAP group and also having a high level of PAP (OR=6.161, 95%CI 1.474-25.760). Conclusion : The authors found that the suicide risk of high school students is related to depression, hopelessness, low self-esteem and a low level of PAP. These results provide significant indicators in planning an adolescent suicide prevention program.

Objective: Although suicide is a serious public health concern for adolescents, there is a lack of studies that explore its risk factors in the Republic of Korea. The present study aims to investigate risk factors associated with suicidal behaviors in Korean adolescents. Methods: Participants consisted of 2258 middle and high school students who completed a series of questionnaires regarding suicide ideation or attempts, non-suicidal self-injuries, depression, impulsivity, drinking behaviors, and negative life events, including peer bullying. Results: Among the participants, 8.3% of students reported suicide ideation, while 3.2% reported a history of a suicide attempt in the past 12 months. Depression, peer-victimization, internet-related delinquency, and positive attitudes toward suicide were associated with suicidal ideations and attempts. Adverse life events were also associated with suicide ideation, but not attempts, while not living with both parents and poor family relationships were associated with suicide attempts, but not ideations. Non-suicidal self-injuries were associated with both suicide ideations and attempts. Students with suicidal ideations and attempts can be differentiated depending on the presence of self-injury. Conclusion: In addition to depression or behavioral problems, non-suicidal self-injuries and lack of support from family may also play significant roles in suicide attempts in adolescents. To facilitate the prevention of suicide in adolescents, longitudinal studies should be followed to confirm the risk factors identified in this study.

This study was conducted to analyze factors affecting the length of stay in children with injuries by determining relationships between length of stay and characteristics of children(aged 0 to 12) with injuries. 7,804 patients aged 0 to 12 who participated in the Korea Nation Hospital Discharge In-Depth Injury Surveys, got a diagnosis of sequelae of injuries and of other consequences of external causes(S00-T98), and were discharged between 1 January 2016 and 31 December 2020 were investigated. A frequency analysis, independent samples t-test, and ANOVA were performed. Also, to identify factors affecting the length of stay, a regression analysis was performed. The average length of stay for the patients investigated in this study was 5.5 days. The length of stay for school-age children(aged 7 to 12) and children who had either public or private coverage was higher than that for preschoolers(aged 0 to 6) and children who didn't have public or private coverage, respectively. The length of stay for children admitted to a hospital in a rural area(Jeolla-do or Gyeongsang-do) was higher than that for children admitted to a hospital in a metropolitan area and the length of stay for children admitted to a hospital that had 100-299 hospital beds was relatively long. However, children who first visited a hospital for outpatient care stayed relatively short in hospital and children who had been burned or injured in traffic crashes stayed relatively long in hospital. Children who got a secondary diagnosis and had a principal procedure or who died after being discharged were in hospital for a long time. The findings of this study shall be useful, as they identified characteristics related to the length of stay for Korean children with injuries and factors that determine the length of stay for those children by analyzing the national dataset, or more specifically, the data from the Korea National Hospital Discharge In-Depth Injury Surveys. The risk of child injuries can be easily reduced by taking actions to prevent them and providing safety education programs. The present study has provided essential baseline data for the provision of aggressive care for child injuries and the establishment of a range of policies for child injury prevention.
Objectives: More than 80% of unintentional injury was related to risk-taking behaviors involved in child accidents. Therefore, diverse care and concerns on safety should be provided from teachers as well as parents to build child safety behaviors. The purpose of this study was to identify the effects of safety concerns from students, teachers, and parents on elementary students' safety behaviors in Korea. Methods: The 5th and 6th grade students participated in this study in 14 elementary schools in Korea and the total number of students was 1,033. The study schools were selected by the stratified cluster sampling method throughout 7 metropolitan areas in Korea. Data were collected by the self-administered survey and the questionnaires were delivered to the vice-principals of the designated schools by ground postal service and the vice-principals conducted survey data collection. The questions on students' safety concerns, teachers' safety concerns, parents' safety concerns, safety practices, safety education, and demographics were included in the survey. All survey responses were encoded into SPSS program and t-test, ANOVA, and multiple regression analysis were utilized. Results: The students taking accidents one time more per year were more than 60% and the two thirds of them recognized that their accidents were due to their risk behaviors. Parents' education on safety practices was more frequent than teachers' education; furthermore, teachers' concerns on safety behaviors were poorer than students' or parents' concerns. In terms of safety behaviors, the scores of fire and home-related safety behavior were high but the scores of traffic and bicycle, in particular, helmet wearing practice were low. Three concerns from students, parents, and teachers all were significantly related to children's safety behaviors. Conclusions: In conclusion, safety concerns from the close networks are the important indicator of child safety behaviors and, the safety training programs, therefore, for parents and teachers as well as for students need to be developed for improving children's safe behaviors.

Purpose: In general, X-ray examinations are not recommended for radial head subluxation (pulled elbow) patients. The purpose of this study was to determine the frequency of X-ray examinations and to investigate the factors associated with the decision to perform an X-ray examination on a patient with a pulled elbow. Methods: Patients who visited the pediatric emergency department (ED) of one tertiary hospital from January 1, 2011, to December 31, 2012, with a diagnosis of radial head subluxation at discharge were enrolled in this study. Through retrospective chart reviews, factors that could have influenced the decision to perform an X-ray examination and their statistical relevance were analyzed. Results: A total 308 patients were enrolled, and 101 patients (32.8%) underwent X-ray examinations. Among the 252 patients with a typical pulled elbow, 65 underwent X-ray examination. This result showed statistical significance compared to atypical pull-elbow group (25.8% vs. 64.3%, p<0.001). Factors associated with the decision to perform an X-ray examination were analyzed using the Chi-square test and the Fisher's exact test. The mechanism of injury and consultation with an orthopedic surgeon (p=0.001) showed statistical significance. In the multivariable logistic regression, the odds ratio (OR) for the injury mechanism was 4.7 (95% CI: 1.8~8.8, p<0.001) and that for consultation with an orthopedic surgeon was 8.0 (95% CI: 1.6~40.7, p=0.004). Conclusion: One third of patients with a pulled elbow underwent X-ray examination, and patients with an atypical mechanism of injury underwent more frequent X-ray examinations than did patients with a typical mechanism of injury. The factors that could have influenced the decision to perform an X-ray examination were mechanism of injury and consultation with an orthopedic surgeon.
Baby walkers have been a major cause of injuries in young children. The main purpose of this study is to identify the pattern of injuries associated with baby walker. The data ore collected from May 13 to June 15, 1998 from 438 mothers who have used or are using baby walkers for their children aged average 6 month old(range 1-33 month). It was founded that 19.2%(84 infants) of these children had walker-related accidents. The types of injuries included ‘falling down’(52.4%), ‘tiping over’(21.4%), ‘being crashed into the wall’(17.9%), and burns(1.2%). These injuries predominantly involved the head and neck region(88%). The majority of injuries were minor, and most injuries occured at home with the mother present. The most common reason to use the baby walker was to keep the infant happy and occupied. Although many parents used walker to promote walking, there was no supportive evidence that walkers helped babies learn to walk sooner. In conclusion, injuries among infants who use walkers are minor, but common. Also, baby walkers may cause a fatal min to some infants. Therefore, child safety warning label policies, anticipatory safety guidance and quality control of infant walkers are needed to prevent injuries associated with a infant walker.

Purpose: The evaluation and management of a completely transected tendon of the hand is relatively clear. In most cases, the integrity is assessed by physical examination(eg, broken normal cascade, motion loss) but occasionally, It happens in troubles. because of pain, limited patient cooperation(eg, child, unconscious patient), and other associated injuries(eg, fractures, foreign bodies). Methods: From september 2006 to August 2007, 28 patients were evaluated with real time ultrasonography preoperatively and postoperatively. Twenty eight patients with injured tendon were evaluated using an HDI 5000 Sono CT(Philips Medical Systems, Bothell, WA) machine with a high resolution, 7.5MHz hockey stick linear probe. Dynamic evaluation was performed in real time. Results: Surgery was performed after the ultrasonographic evaluation. Sonographic diagnosis and intraoperative findings were correlated. Ultrasonographic findings show tendon discontinuity, gaps, and fluid collection. Conclusion: We conclude that ultrasonography is helpful in evaluating and managing acute tendon injury. Especially, in cases of completely lacerated tendon, ultrasonography can identify the location of the proximal tendon stump and partial lacerated tendon, ultrasonographic diagnostic tool can decrease misdiagnosis.
