복합레진 인레이 수복시 와동형태에 따른 치아파절에 관한 유한요소법적 연구 (A THREE DIMENSIONAL FINITE ELEMENT ANALYSIS WITH CAVITY DESIGN ON FRACTURE OF COMPOSITE RESIN INLAY RESTORED TOOTH)

  • Fracture of cusp, on posterior teeth, especially those carious or restored, is major cause of tooth loss. Inappropriate treatments, such as unnecessarily wide cavity preparations, increase the potential of further trauma and possible fracture of the remaining tooth structures. Fracture potential may be directly related to the stresses exerted upon the tooth during masticatory function. The purpose of this study is to evaluate the fracture resistance of tooth, restored with composite resin inlay. In this study, MOD inlay cavity prepared on maxillary first premolar and restored with composite resin inlay. Three dimensional finite element models with eight nodes isoparametric solid element, developed by serial grinding-photographing technique. These models have various occlusal isthmus and depth of cavity, 1/2, 1/3 and 1/4 of isthmus width and 0.7, 0.85 and 1.0 of depth of cavity. The magnitude of load was 474 N and 172 N as presented to maximal biting force and normal chewing force. These loads applied onto ridges of buccal and lingual cusp. These models analyzed with three dimensional finite element method. The results of this study were as follows : 1. There is no difference of displacement between width of occlusal isthmus and depth of cavity. 2. The stress concentrated at bucco-mesial comer, bucco-disal comer, pulpal line angle and the interface area between internal slopes of cusp and resin inlay. 3. The vector of stress direct to buccal and lingual side from center of cavity, to tooth surface going on to enamel. The magnitude of vector increase from occlusal surface to cervix. 4. The crack of tooth start interface area, between internal slop of buccal cusp and resin inlay. It progresses through buccopulpal line angle to cervix at buccomesial and buccodistal comer. 5. The influence with depth of cavity to fracture of tooth was more than width of isthmus. 6. It would be favorable to make the isthmus width narrower than a third of the intercuspal distance and depth of cavity is below 1 : 0.7.

심한 교합면 마모 및 교두 파절 환자에서 유기필러를 함유한 4-META/MMA-TBB 레진을 활용한 직접수복 증례 (A case of direct restore using 4-META/MMA-TBB resin containing organic filler in patients with severe occlusal surface wear and enamel fracture)

  • 교모는 치아 간의 접촉으로 인하여 치아 경조직이 소실되는 것으로, 심한 경우 상아질이 노출되어 선택적인 부식과 치아의 과도한 마모가 동반되는데, 이를 cupping이라고 부른다. 이러한 병소를 치료하지 않고 방치할 경우 병소의 크기가 점차 커지게 되어 지지되지 않는 법랑질이 파절되고, 결과적으로 심미성과 저작 기능이 저하된다. 본 증례보고에서는 심한 교모로 인해 cupping과 법랑질 파절이 동반된 환자에서 유기필러가 함유되어 부드러운 성질을 지닌 레진을 이용하여 직접수복을 실시하였다. 6년 후의 경과관찰에서 교합면의 충전은 대부분 탈락하였으나 협측의 충전은 비교적 온전하게 남아 있음을 관찰할 수 있었고, 이런 형태의 레진이 교합력이 크게 가해지는 부위보다는 교합력이 비교적 약하게 가해지는 부위에 적합한 재료임을 확인할 수 있었다.

구조방정식을 이용한 대도시 일부 노인들의 저작능력 및 구강건강관련 삶의 질과 인지기능 (Association between masticatory ability, oral health-related quality of life and cognitive function in the elderly population using structural equation modeling)

  • Objectives: The aim of this study was to investigate the association between masticatory ability, oral health-related quality of life, and cognitive function in an elderly population using structural equation modeling. Methods: A total of 308 subjects, aged 65 years and over, were recruited from the senior citizen center in Daegu, South Korea and provided consent for inclusion in the study. The Korean version of the Mini-Mental State Examination (K-MMSE) was used to assess the level of cognitive function. Masticatory ability was evaluated through chewing tests, patient-perceived masticatory function and, measurement of relative occlusal forces on molar teeth using the T-Scan $III^{(R)}$ system. All data were analyzed using ${\chi}^2$ tests, t-tests, and one-way ANOVA in SPSS version 23.0 for windows. Structural equation modeling was performed using SPSS AMOS version 22.0. Results: There was a significant association between cognitive function and molar teeth occlusal force. Ability of food chewing score and patient-perceived masticatory function could affect oral health-related quality of life (P>0.05). Higher occlusal forces on molar teeth, compared to anterior teeth, positively correlated with higher cognitive function. Conclusions: These results suggest that an individual's oral health condition could affect molar teeth occlusal forces and may be related to a risk of developing dementia. Therefore, there is a need for implementing nation-wide policies to improve oral health, such as masticatory function, in the elderly population.

실험적으로 유발되는 근피로가 근통증 및 교합양상에 미치는 영향 (Effect of Experimental Muscle Fatigue on Muscle Pain and Occlusal Pattern)

  • 치과에 내원하는 주된 이유 중의 하나는 통증이며 이런 통증 치료시에 장시간의 개구는 저작근에 근육문제를 야기할 수 있다. 장시간의 근피로를 유발할 수 있는 치과진료실내에서의 치료로 인하여 두통, 저작근의 통증, 개구 장애, 저작 곤란 등을 유발할 수 있으며, 통증을 치료하기위하여 내원한 환자로서는 예상하지 못한 다른 국면의 통증에 맞부딪치게 된다. 물론 이악물기등의 악습관 그리고 과도한 껌저작등으로 인해서도 임상적으로 근쇠약감이 호소하며 이에 대해서는 실험적 근피로유도후에 근전도등을 이용하여 연구가 이루어져왔다. 근쇠약감 및 근피로로 인하여 교합양상의 변화를 주관적으로 호소하며 이 상태에서의 근육의 근전도 평가가 이루어져 왔던 것에 비하여 교합의 변화에 대해서는 평가가 미진하였다. 과도한 이악물기나 개구 및 껌 저작으로 인한 근피로가 교합양상에 미치는 변화를 평가하고 감각신경 변화로 인하여 근통증에 영향을 미치는 조사하고자 측두하악장애 증상이 없고 정상 범주의 교합을 가졌으며 교정치료나 3개 이상의 보철 치료를 받지 않은 지원자 총 24명(평균나이 25.7세)을 대상으로 하였다. 근 피로를 실험적으로 유발하기 위하여 이악물기, 장시간 개구 상태 유지, 주저작측으로 껌 저작을 시행하여 근피로를 느끼도록 하였다. 본 연구에서는 전기적 자극을 이용하는 뉴로미터를 이용하여 근통증시에 주저작측 삼차신경의 감각신경을 측정하였고, 근통증을 느끼는 시점에 근쇠약감이나 저작근 기능 장애로 인한 교합상태의 변화를 평가하기 위하여 T-scan II system을 이용하여 교합접촉, 교합력 등을 조사하여 다음의 결과를 얻었다. 1. 근피로는 장시간 개구 후에 가장먼저 느꼈으며 근통증을 느끼는 시간은 이악물기 후에 가장 빨리 나타났다. 근피로, 통증을 느끼는 시점에서의 근피로, 통증의 정도는 이악물기 후에 가장 높았다 2. 최초접촉시의 접촉시간 접촉력은 차이가 없었으며 교두간 접촉위에서 껌저작후 접촉시간, 장시간 개구 후와 껌저작후에 접촉력이 감소하였다. 최대접촉위에서는 장시간 개구후와 껌저작후에 접촉력이 감소하였다. 최종접촉위에서는 모든 실험적 근피로후에 접촉시간이 감소하였고 접촉력은 껌저작후에 감소하였다. 3. 실험적 근피로가 유발된 근육에서의 감각 변화는 보이지 않았다. 4. 접촉수는 실험적 근피로 유발후에 최초접촉위 최대 접촉위 그리고 최종접촉위에서 감소되는 양상을 보였다. 5. 접촉수의 주저작측에 따른 비교에서 좌측으로 껌을 저작한 군은 접촉수에 좌우측 차이를 보이지 않았으며, 우측으로 껌을 저작한 군에서는 좌측이 많았다. 결론적으로 구강내외에 발생하는 일련의 사건은 임상적으로 근쇠약감을 호소하는 경우 교합양상에 변화를 초래 할 수 있음을 확인 할 수 있었으며, 장시간의 개구나 이악물기, 과도한 저작등의 악구강계에 영향을 미칠 수 사건은 특히 치과치료 시에 교합 변화에 주의를 요함을 확인할 수 있었다.

Relationships between Descriptive Sensory Attributes and Physicochemical Analysis of Broiler and Taiwan Native Chicken Breast Meat

  • Unique organoleptic characteristics such as rich flavors and chewy texture contribute to the higher popularity of native chicken in many Asian areas, while the commercial broilers are well-accepted due to their fast-growing and higher yields of meat. Sensory attributes of foods are often used to evaluate food eating quality and serve as references during the selection of foods. In this study, a three-phase descriptive sensory study was conducted to evaluate the sensory attributes of commercial broiler (BR) and Taiwan native chicken (TNC) breast meat, and investigate correlations between these sensory attributes and instrumental measurements. The results showed that for the first bite (phase 1), TNC meat had significantly higher moisture release, hardness, springiness, and cohesiveness than BR meat. After chewing for 10 to 12 bites (phase 2), TNC meat presented significantly higher chewdown hardness and meat particle size, whereas BR meat had significantly higher cohesiveness of mass. After swallowing (phase 3), TNC meat had higher chewiness and oily mouthcoat and lower residual loose particles than BR meat. TNC meat also provided more intense chicken flavors. This study clearly demonstrates that descriptive sensory analysis provides more detailed and more objectively information about the sensory attributes of meats from various chicken breeds. Additionally, sensory textural attributes vary between BR and TNC meat, and are highly correlated to the shear force value and collagen content which influence meat eating qualities greatly. The poultry industry and scientists should be able to recognize the sensory characteristics of different chicken meats more clearly. Accordingly, based on the meat's unique sensory and physicochemical characteristics, future work might address how meat from various breeds could best satisfy consumer needs using various cooking methods.

In vitro performance and fracture resistance of novel CAD/CAM ceramic molar crowns loaded on implants and human teeth

  • PURPOSE. To investigate the fatigue and fracture resistance of computer-aided design and computer-aided manufacturing (CAD/CAM) ceramic molar crowns on dental implants and human teeth. MATERIALS AND METHODS. Molar crowns (n=48; n=8/group) were fabricated of a lithium-disilicate-strengthened lithium aluminosilicate glass ceramic (N). Surfaces were polished (P) or glazed (G). Crowns were tested on human teeth (T) and implant-abutment analogues (I) simulating a chairside (C, crown bonded to abutment) or labside (L, screw channel) procedure for implant groups. Polished/glazed lithium disilicate (E) crowns (n=16) served as reference. Combined thermal cycling and mechanical loading (TC: $3000{\times}5^{\circ}C/3000{\times}55^{\circ}C$; ML: $1.2{\time}10^6$ cycles, 50 N) with antagonistic human molars (groups T) and steatite spheres (groups I) was performed under a chewing simulator. TCML crowns were then analyzed for failures (optical microscopy, SEM) and fracture force was determined. Data were statistically analyzed (Kolmogorow-Smirnov, one-way-ANOVA, post-hoc Bonferroni, ${\alpha}=.05$). RESULTS. All crowns survived TCML and showed small traces of wear. In human teeth groups, fracture forces of N crowns varied between $1214{\pm}293N$ (NPT) and $1324{\pm}498N$ (NGT), differing significantly ($P{\leq}.003$) from the polished reference EPT ($2044{\pm}302N$). Fracture forces in implant groups varied between $934{\pm}154N$ (NGI_L) and $1782{\pm}153N$ (NPI_C), providing higher values for the respective chairside crowns. Differences between polishing and glazing were not significant ($P{\geq}.066$) between crowns of identical materials and abutment support. CONCLUSION. Fracture resistance was influenced by the ceramic material, and partly by the tooth or implant situation and the clinical procedure (chairside/labside). Type of surface finish (polishing/glazing) had no significant influence. Clinical survival of the new glass ceramic may be comparable to lithium disilicate.

유한요소법을 이용한 치아 임플랜트 피로수명 예측 (FATIGUE LIFE ESTIMATION OF IMPLANT USING A FINITE ELEMENT METHOD)

  • Purpose : The purpose of this study is to use finite element analysis to predict the fatigue life of an implant system subjected to fatigue load by mastication (chewing force). The reliability and the stability of implant system can be defined in terms of the fatigue strength. Not only an implant is expensive but also it is almost impossible to correct after it is inserted. From a bio-engineering standpoint, the fatigue strength of the dental implant system must be evaluated by simulation (FEA). Material and Methods Finite element analysis and fatigue test are performed to estimate the fatigue strength of the implant system. Mesh of implant is generated with the actual shape and size. In this paper, the fatigue strength of implant system is estimated. U-fit (T. Strong, Korea, internal type). The stress field in implant is calculated by elastic-plastic finite element analysis. The equivalent fatigue stress, considering the contact and preload stretching of a screw by torque for tightening an abutment, is obtained by means of Sine's method. To evaluate the reliability of the calculated fatigue strength, fatigue test is performed. Results: A comparison of the calculated fatigue strength with experimental data showed the validity and accuracy of the proposed method. The initiation points of the fatigue failure in the implant system exist in the region of high equivalent fatigue stress values. Conclusion: The above proposed method for fatigue life estimation tan be applied to other configurations of the differently designed and improved implant. In order to prove reliability of prototype implant, fatigue test should be executed. The proposed method is economical for the prediction of fatigue life because fatigue testing, which is time consuming and precision-dependent, is not required.

Treatment and retention of relapsed anterior open-bite with low tongue posture and tongue-tie: A 10-year follow-up

  • The purpose of the current report is to present 6-year long-term stability and 10-year follow-up data for an adult patient who was treated with a tongue elevator for relapsed anterior open-bite. The 19-year-old male patient presented with the chief complaint of difficulty in chewing his food. Collectively, clinical and radiographic examinations revealed an anterior open-bite, low tongue posture, and tongue-tie. The patient opted for orthodontic treatment alone, without any surgical procedure. A lingual frenectomy was recommended to avoid the risk of relapse, but the patient declined because he was not experiencing tongue discomfort. Initial treatment of the anterior open-bite with molar intrusion and tongue exercises was successful, but relapse occurred during the retention period. A tongue elevator was used for retreatment, because the approach was minimally invasive and suited the patient's requirements regarding discomfort, cost, and time. The appliance changed the tongue posture and generated an altered tongue force, which ultimately resulted in intrusive dentoalveolar effects, and a subsequent counterclockwise rotation of the mandible. The results showed long-term stability and were maintained for six years through continual use of the tongue elevator. The results of this case indicated that a tongue elevator could be used not only as an alternative treatment for open-bite, but also as an active retainer.

In-vitro development of a temporal abutment screw to protect osseointegration in immediate loaded implants

  • PURPOSE. In this study, a temporal abutment fixation screw, designed to fracture in a controlled way upon application of an occlusal force sufficient to produce critical micromotion was developed. The purpose of the screw was to protect the osseointegration of immediate loaded single implants. MATERIALS AND METHODS. Seven different screw prototypes were examined by fixing titanium abutments to 112 Mozo-Grau external hexagon implants (MG Osseous$^{(R)}$; Mozo-Grau, S.A., Valladolid, Spain). Fracture strength was tested at $30^{\circ}$ in two subgroups per screw: one under dynamic loading and the other without prior dynamic loading. Dynamic loading was performed in a single-axis chewing simulator using 150,000 load cycles at 50 N. After normal distribution of obtained data was verified by Kolmogorov-Smirnov test, fracture resistance between samples submitted and not submitted to dynamic loading was compared by the use of Student's t-test. Comparison of fracture resistance among different screw designs was performed by the use of one-way analysis of variance. Confidence interval was set at 95%. RESULTS. Fractures occurred in all screws, allowing easy retrieval. Screw Prototypes 2, 5 and 6 failed during dynamic loading and exhibited statistically significant differences from the other prototypes. CONCLUSION. Prototypes 2, 5 and 6 may offer a useful protective mechanism during occlusal overload in immediate loaded implants.

임플란트 국소의치 금나사의 3차원 유한요소법 접촉응력 분석 (3D Dimensional Finite Element Analysis of Contact Stress of Gold Screws in Implant Partial Denture)

  • Purpose: In this research, non-linear three dimensional finite element models with contact elements were constructed. For the investigations of the distributions of contact stresses, 3 units fixed partial dentures model were studied, especially on the interface of the gold screw and cylinder, abutment screw. Methods: 3 types of models were constructed ; the basic fixed partial denture in molar region with 3 units and 3 implants, the intermediate pontic fixed partial denture model with 3 units and 2 implants, and the extension pontic fixed partial denture model with 3 units and 2 implants. For all types, the external loading due to chewing was simulated by applying $45^{\circ}$ linguo-buccal loading of 300 N to the medial crown. For the simulation of the clamping force which clinically occurs due to the torque, thermal expansion was provided to the cylinder as a preload. Results: Under 300 N concentrated loading to the medial crown, the maximum contact stress between abutment screw and gold screw was 86.85~175.86MPa without preload, while the maximum contact stress on the same area was 25.59~57.84MPa with preload. Conclusion: The preloading affected the outcomes of the finite element stress analysis. Reflecting the clinical conditions, the preloading conditions should be considered for other practical study utilizing FEA. For the study of the contact stresses and related motions, various conditions, such as frictional coefficient changes, gap between contact surfaces, were also varied and analyzed.
