뇌성마비 아동의 대동작운동기능 수준에 따른 흉곽발달 양상 (Development of the Chest Wall in Children with Cerebral Palsy according to GMFCS Levels)

  • Purpose: The purpose of this study was to provide quantitative data regarding development of the chest wall in children with cerebral palsy (CP) according to Gross Motor Function Classification System (GMFCS) levels and age using the radiological image diameter measurement method. Methods: Subjects included 112 children with CP and 110 healthy children, All of the children underwent simple chest x-ray. The diameters of the upper chest ($D_{apex}$) and lower chest ($D_{base}$) were measured on the anteroposterior (AP) view of a chest x-ray, and the $D_{apex}$ to $D_{base}$ ratio was calculated. Chest wall ratios were compared among children with CP at GMFCS levels I ~ III, GMFCS levels IV and V, and healthy children. Results: The results showed significant differences between the upper and lower chest wall diameters of children with CP at GMFCS levels IV and V, and healthy children (F=4.54, p=0.01; F=3.20, p=0.04). Results of comparison between the chest wall ratios of children with CP and healthy children, showed that the upper chest walls of healthy children were significantly larger in children younger than 48 months (p<0.05), and both the upper and lower chest walls of healthy children were significantly larger compared to children with CP in children older than 48 months (p<0.05). Conclusion: Radiographic measurement for examination of chest wall development is relatively simple, and the results yield quantitative data on development of the chest wall for children with CP. In addition, therapeutic interventions may be considered based on the results.

  • Purpose : The purpose of this study was to analysis on diaphragm thickness and lung function of stroke patients by walking ability. Methods : We recruited thirty-five adults after stroke(20 male, 15 female) for our study. The subjects were divided into two groups; independent walking group(11 male, 9 female) and non-independent walking group(9 male, 6 female). Assessment of diaphragm thickness was performed using ultrasound in B-mode with a 7.5 MHz linea probe. During the experiment, the subject was seated in the chair. All subjects performed maximal expiratory flow maneuvers using a spirometer in order to determine the forced expiratory volume in 1 second ($FEV_1$), forced vital capacity(FVC), peak expiratory flow(PEF) and $FEV_1$/FVC. Chest expansion was measured with a tape-measure placed circumferentially around the chest wall at the xiphoid process. The collected data analyzed by independent t-test. Results : The diaphragm thickness were significant differences between the independent walking and nonindependent walking group. Values of forced vital capacity, forced expiratory volume at one second, peak expiratory flow in pulmonary function tests were significant differences between the independent walking and non-independent walking group. However, chest expansion were not significant differences in both of the group. Conclusion : This study showed that walking ability of stroke patients have influenced on diaphragm thickness and pulmonary function.

건강한 성인에게 가슴가동운동과 스트레칭운동이 최대들숨압 및 최대날숨압에 미치는 효과 (The Effects of Thoracic Mobilizing and Stretching Exercise on Maximal Inspiratory Pressure and Maximal Expiratory Pressure in Healthy Adults)

  • Purpose: The purpose of this study was to investigate the effect of chest mobilization and stretching exercises on maximal inspiratory pressure and maximal expiratory pressure in healthy adults who use computers for extended periods of time each day due to coronavirus disease 2019. Methods: Twenty-five healthy adults in their 20s and without respiratory disease (15 female, 10 male) took part in this study. Two types of thoracic mobilizing exercises using a Theraband and three types of stretching exercises using a foam roller were performed. Maximum inspiratory pressure and maximum expiratory pressure were measured three times each before and after the interventions. In terms of statistical methods, the maximum inspiratory pressure due to chest mobility and stretching was compared with the maximum expiratory pressure using parametric paired t-test and non-parametric Wilcoxon signed-rank test. Results: Maximum inspiratory pressure (p = .012) and maximum expiratory pressure (p = .006) showed significant differences before and after chest mobilization exercise and stretching among the participants. Conclusion: The results of this study suggest that chest mobilization and stretching exercises are effective exercise methods for improving maximal inspiratory and expiratory pressure. They suggest that these exercises can prevent respiratory muscle weakness and improve aerobic fitness in healthy people as well as those in need of cardiorespiratory physiotherapy.

  • The purpose of this study was to evaluate respiratory functions in relation to the gross motor functions(total value of GMFM), the difference of chest girth, and the changing position in spastic children. The respiratory functions(FVC, FEV1, $FEV1\%$, and PEF) were measured in the supine, the $45^{\circ}$semi-sitting, and the $45^{\circ}$sitting in 9 subjects. In the supine position, the mean difference of chest girth was $1.56{\pm}0.80cm$, the total value of GMFM was $45.41{\pm}17.79\%$. In the supine position, there was significant positive relationship in FVC-FEV1, FVC-PEF, and FEV1-PEF, but there was no significant relationship in GMFM and all respiratory functions. In the $45^{\circ}$semi-sitting, there was significant positive relationship in GMFM-FVC, FVC-FEV1, FVC-PEF, FEV1-PEF, and $FEV1\%-PEF$. In the $90^{\circ}$sitting, there was significant positive relationship in GMFM-FEV1, $GMFM-FEV1\%$, FVC-FEV1, FVC-PEF, and FEV1-PEF. In results of measured respiratory functions according to the postures, the supine position had highest value in all respiratory functions, but there were no significant (p<0.05).

The changes of rectus abdominis muscle thickness according to the angle during active straight leg raise

  • Objective: The purpose of this study was to investigate changes of abdominal muscles thickness according to the angle during the active straight leg raise (ASLR) in young healthy subjects. Design: Cross sectional study. Methods: Twenty-three healthy university students (13 men and 10 women) voluntary participated to the study in S University. The ASLR was performed with the subject lying supine with lower extremities straight on a standard plinth, hands resting on the chest, and elbows on the plinth. When one subject performed ASLR from each angles ($30^{\circ}$, $45^{\circ}$, $60^{\circ}$, $90^{\circ}$), compared changes in the thickness of rectus abdominis muscle. Changes in muscle thickness during ASLR test were assessed with ultrasonography. All subjects were to provide enough time of rest after performed ASLR. Rectus abdominis thickness were measured using rehabilitative ultrasound image. Results: Good quality rectus abdominal muscle activation data were recorded during ASLR. The length changes of linea alba showed significantly shorter in between $0^{\circ}$ and $30^{\circ}$ (p<0.05). The thickness of rectus abdominis muscle were significantly different between $0^{\circ}$ and $30^{\circ}$, $0^{\circ}$ and $45^{\circ}$, $0^{\circ}$ and $60^{\circ}$, $0^{\circ}$ and $90^{\circ}$. According to increase of pelvic angle, the thickness of rectus abdominis muscle were more thickening (p<0.05). Conclusions: This result is changes of abdominal muscles thickness according to the angle during the ASLR.

Effect of the High Frequency Chest Wall Oscillation (HFCWO) on Pulmonary Function and Walking Ability in Stroke Patients

  • Purpose: This investigation aimed to determine the effects of treadmill training (TT) and high frequency chest wall oscillation (HFCWO) on pulmonary function and walking ability in stroke patients as well as propose an exercise program to improve cardiovascular function. Methods: Twenty hemiplegic stroke patients were randomized to either the control group (CG) (n=10) or the experimental group (EG), which received TT and HFCWO (n=10). Pulmonary function was quantitated using patient forced vital capacity (FVC) and forced expiratory volume at one second (FEV1) while walking speed was assessed by the 10m walking test (10MWT). Further, walking endurance was determined utilizing the 6-minute walk test (6MWT). Subjects of the EG performed the study protocol for 60 minutes, five times a week for six weeks; CG patients did not participate in regular exercise. To determine significance for the differences observed before and after exercise, within-group and between-group comparisons were conducted utilizing paired and independent t-tests, respectively, with the level of significance set at ${\alpha}=0.05$. Results: Within-groups, significant differences were observed in both FVC and FEV1 (p<0.01) following completion of the study protocol. Further, between-group comparisons demonstrated significant differences in both FVC (p<0.05) and FEV1 (p<0.01). Post-exercise, significant changes in the 10MWT and 6MWT score were observed between the EG and CG (p<0.01). Further, statistically significant differences were observed in 6MWT scores between-groups (p<0.05). Conclusion: The TT and HFCWO effectively improved pulmonary function and walking ability in subjects with stroke. The proposed program can be applied to stroke patients as a useful therapy.

심호흡 운동과 흉추가동성 운동이 뇌졸중 환자의 폐기능에 미치는 영향 (The Effect of Pulmonary Function with Thoracic Mobility Exercise and Deep Breathing Exercise in Stroke Patients)

  • Background: To evaluate the effect of thoracic mobility exercise and deep breathing exercise applied to stroke patients on pulmonary function. Methods: The subjects were divided into two group. Twenty-five patients with stroke were randomly assigned to DB (deep breathing exercise) group (n=13) and TM (combination of deep breathing exercise and thoracic mobility exercise) group (n=12). During four weeks, DB group were carried out deep breathing exercises for 5~10 minutes twice a day and TM Group were carried out deep breathing exercises for 5~10 minutes and thoracic mobility exercise for 20~30 minutes twice a day. All tests were completed before and after experiment. The pulmonary functions were measured by PowerBreathe K5 (Hab International Ltd, England) and tape measure respectively. For each case, the experimental data were obtained in 4 items; average of inspiratory load, inspiratory flow speed, inspiratory flow volume and chest expansion. Results: The results of this study were as follows: 1. In DB group, the statistically significants were shown on average of inspiratory flow speed, inspiratory flow volume and chest expansion (p<.05). 2. In TM group, the statistically significants were shown on all items (p<.05). 3. There was a statistically significant difference on all items between DB group and TM group (p<.05). Conclusions: The above results revealed that DB and TM group can be used to improve pulmonary function in stroke patients. In comparison of DB and TM group, TM group was more improved. In conclude, thoracic mobility exercise helped improving function of vital capacity and chest expansion in stroke patients.

The Effects of Circuit Training and Circuit Training with Whole Body Vibration on Pulmonary Function in Adolescent

  • The purpose of this study was to parallel circuit training and circuit training with sonic systemic mechanism was to compare the differences in pulmonary function and chest expansion in adult men. This study was performed on 20 subjects. 20 subjects were divided into two groups; Circuit training group(n=10), Circuit training with sonic systemic mechanism(n=10). Both of the group performed the exercise 3 times a week for 5 weeks. The data was analyzed by the Repeated t-test for comparing before, during and after changes of factors in each group and the Independent t-test for comparing the between groups. The result are as follows. Circuit training group was statistically significant difference FVC, FEV1/FVC(p<.05), Circuit training with sonic systemic mechanism group was statistically significant difference PEF, VC in pulmonary function(p<.05). Circuit training group was statistically significant difference FEV1/FVC of between the two group in pulmonary function(p<.05). Circuit training group and circuit training with sonic systemic mechanism group was statistically significant difference in chest expansion(p<0.05) and there was no statistically significant difference of between the two group in chest expansion(p>.05).

노래 심리 치료를 활용한 지언고론요법(至言高論療法)을 적용하여 호전된 심신증(心身證)환자 1예(例) 보고 (A Clinical Report of a Psychosomatic patient who has treated by Giungoroen-therapy based on Song Psychotherapy)

  • Objectives : This study was performed to report the effect of Giungoroen-therapy based on song psychotherapy in the treatment resistant patient with psychosomatic disease. Methods : A 43 year-old female patient suffered from headache, facial numbness, right upper and lower limb's numbness, ptosis, chest discomfort, dysarthria without identifiable physical cause. We diagnosed her as psychosomatic disease, and tried to apply Giungoroen-therapy with Korean traditional medical treatment. But she was resistant to Giungoroen-therapy. So we applied Giungoroen-therapy based on song psychotherapy. Results : After the Giungoroen-therapy based on song psychotherapy, the patient revealed her unrecognized hidden issues and got some insights about herself. Then the symptoms, such as headache, facial numbness, right upper and lower limb's numbness, ptosis, chest discomfort and dysarthria were reduced considerably. Conclusions : These results suggest that Giungoroen-therapy based on song psychotherapy can be effective on improvement of resistant psychosomatic disease.

건강한 20대 성인여성의 브래지어 착용 여부에 따른 흉곽 가동성과 가로막 움직임 및 폐활량에 대한 비교 (Comparison of Chest Mobility, Diaphragm Movement, and Lung Capacity Between With and Without Bras in a Healthy 20s Adult Women)

  • 본 연구는 여성의 브래지어 착용 여부와 가슴 크기에 따른 흉곽 가동성과 가로막의 움직임 및 폐활량을 비교해보고자 하였다. 충남 B 대학에 재학 중인 10명을 대상으로 실험하였으며 속옷의 사이즈가 75A인 5명을 A그룹, 85B인 5명을 B그룹으로 나누어 실험을 진행하였다. 브래지어 착용 전후 원형 줄자를 이용하여 흉곽 가동성을, 초음파를 이용하여 가로막의 움직임을 측정하였으며 폐활량 측정을 위하여 Pony FX를 이용하였다. 브래지어 착용 여부를 비교한 결과, A그룹과 B그룹 모두에서 브래지어를 착용하였을 때 흉곽 가동성과 가로막 움직임이 유의하게 감소하였지만. 폐활량은 A 그룹에서만 유의하게 감소하였다. A그룹과 B그룹을 비교한 결과, 모든 종속변수에서 유의한 차이는 없었다. 그러므로 본 연구는 가슴 크기에 상관없이 브래지어 착용 자체만으로도 흉곽 가동성과 가로막의 움직임에 부정적 영향을 끼칠 수 있음을 시사한다.
