USNCAP 정면충돌시험 결과를 이용한 HIC15 예측모델 개발 (A Development on the Prediction Model for the HIC15 using USNCAP Frontal Impact Test Results)

  • This study is to develop the prediction model for the HIC15 in frontal vehicle crash tests. The 28 frontal impact test results of the MY2019 and MY2020 USNCAP are utilized. The metrics for evaluating the crash pulse severity such as moving average acceleration, Restraint Quotient (RQ) and ride-down efficiency are reviewed to find out whether the metrics can predict the HIC15. It is observed that the R2 values based on the linear regression of all pairs between the existing metrics and the occupant injuries such as the HIC15, 3 ms chest g's and chest deflection are very low. In this study, using the vehicle crash pulses, the linear regression model for estimating the HIC15 is developed. The vehicle crash pulse is splitted seven 10 ms intervals in 70 ms after impact for extracting the average accelerations in each intervals. The prediction model can predict effectively not only the HIC15 but also the maximum head g's, chest deflection and 3 ms chest g's of 13 vehicles out of 28 vehicles.

Boerhaave syndrome의 외과적 치험 (Surgical Experiences of Boerhaave`s Syndrome -10 Cases analysis-)

  • All 10 cases of spontaneous rupture of esophagus had violent vomiting as precursor. 9 patients were male, 1 case was female. Chief complaints were chest pain and dyspnea. Chest P \ulcornerA and esophagogram were mainly used as confirm diagnostic tool. Perforation sites of all cases were at distal esophagus near the G-E junction. 6 cases were received primary repair within 24 hrs, other cases were managed with surgical drainage after exclusion and diversion of esophagus. Empyema was the most frequent complication. Other complications were sepsis, pneumonia, leaking etc. Overall mortality rate was about 70.0%.

심폐소생술 구조자의 무릎 높이 정도가 흉부압박의 질에 미치는 효과 (Effects of Knee Height of CPR Rescuer on the Quality of Chest Compression)

  • 본 연구는 심폐소생술 시행 시 구조자의 무릎이 바닥으로부터 10 cm 높은 위치와 바닥 위치 간에서 흉부압박의 질에 차이가 있는지를 알아보고자 시행하였다. G광역시의 G대학 응급구조과 1학년 재학생으로 심폐소생술 교육 과정을 이수한 66명 중, 무작위추출과정을 통해 2011년 11월 8일부터 9일까지 실험군 31명, 대조군 32명으로 선정하였다. 바닥으로부터 10 cm 위치(재질: B4 Copy Paper)와 바닥 위치에는 공통으로 매트리스(재질: PVC, 사이즈: $185{\times}125{\times}0.65cm$)를 깔고 2분 동안 흉부압박 만을 시행하였다. 실험 처치는 Resusci Anne 마네킨 1대를 사용하였으며, Laerdal PC SkillReporting System으로 시행결과를 기록하였다. 수집된 자료는 SPSS 14.0 for Window으로 $x^2$-test와 Fisher's exact probability test, Mann-Whitney U-test, Wilcoxon signed rank test를 사용하였다. 연구결과, 심폐소생술 구조자의 무릎이 바닥으로부터 10 cm 높은 위치가 바닥 위치보다 흉부압박 적절한 깊이(회), 평균 흉부압박 깊이(mm)가 더 효과적이었는데, 신장 170 cm 이하 그룹, 몸무게 65 kg 이하 그룹에서 흉부압박의 적절한 깊이(회), 평균 흉부압박 깊이(mm)가 효과적 이었다.

  • Kawasaki s disease is a multisystemic disorder that is an important cause of cardiovascular disease in children. We experienced a case of coronary artery aneurysm secondary to Kawasaki s disease. The patient was 12-year-old female presented as exertional dyspnea & chest pain[NYHA classII . Her coronary angiogram showed saccular aneurysms at proximal right coronary artery and proximal left anterior descending artery. CABG was performed by use of double internal mammary artery.Postoperative E.K.G. showed a normal pattern.She discharged at postoperative 10th day without problem. 4 months after discharge, she had no dyspnea & chest pain in exertion[NYHA class I .

A Case of Successful Management of Lung Cancer Pain Using Ultrahigh-dose Fentanyl Patch

  • A 55-year old woman with advanced stage non-small cell lung cancer was admitted to hospital for the management of severe chest pain, which measured 7 out of 10 on a numerical rating scale (NRS). Despite palliative radiation and the application of multiple epidural blocks, she continued to experience severe cancer pain. We gradually increased the dose of transdermal fentanyl patches from $500{\mu}g/hr$ to $3,650{\mu}g/hr$, for 3 months without any significant side effects. Concomitantly, adjuvant therapy with antidepressants and anticonvulsants were added, decreasing the patient's pain to NRS 3~4 down from 7. After being transferred to a hospice clinic, her chest pain was well-controlled below NRS 4 by means of strong opioid medications, including the highest dose of transdermal fentanyl $4,050{\mu}g/hr$ for more than 16 months.

Long Term Average Spectrum Characteristics of Head and Chest Register Sounds of Western Operatic Singers - Possibility of a Second Singer's Formant-

  • The purpose of this study was to analyze and compare head register with chest register of singers acoustically. Fifteen healthy tenor major students were participated. Fifteen healthy untrained adults were chosen as the control group for this study. Long term average (LTA) power spectrum using the Fast Fourier transform (FFT) algorithm and Linear predictive coding (LPC) filter response were made with /a/ sustained in both head (G4, 392 Hz) and chest registers (C3, 131 Hz). Statistical analysis was performed using the Mann-Whitney test. In the LTA power spectrum, head register of singers increased in the level of energy gain within the frequency of 2.2-3.4 kHz (p<0.01), and 7.5-8.4 kHz (p<0.01, p<0.05). Chest register of singers increased in the frequency of 2.2-3.1 kHz (p<0.01), 7.8-8.4 kHz (p<0.05) and around 9.6 kHz (p<0.01). The LTA power spectrum revealed a peak of acoustic energy around 2,500 Hz, known as the singer's formant and another peak of acoustic energy around 8,000 Hz in the singer's voice.

  • The pitch range of the human voice is variable, extending from chest register to falsetto. Although numerous studies have investigated after laryngeal mechanism description of registers, systematic and objective studies were lack. The purpose of this study was to analyze and compare head register with chest register of singers acoustically. Fifteen healthy tenor major students were selected.   • 본 연구는 119구급대원을 대상으로 심정지 임신부에 시행하는 $30^{\circ}$ 왼쪽 기울기 자세에서 가슴압박 질과 신장 및 체중이 가슴압박 질에 영향을 미치는지 알아보고자 시행하였다. J도 M, N, S, Y시, G광역시에서 근무하는 119구급대원 62명을 대상으로 2015년 6월 15-16일간 실험 전 참여 동의를 받았다. Resusci Anne$^{(R)}$ 마네킨 2대를 사용하여 Laerdal PC SkillReporting System$^{(R)}$으로 가슴압박 시행 결과를 기록하였다. 연구결과, $30^{\circ}$ 왼쪽 기울기 자세에서 바로 누운 자세보다 가슴압박 적절한 깊이 횟수(회), 가슴압박 깊이(mm)는 매우 저조하였고, 또한 170 cm 이하, 65 kg 이하에서 가슴압박 적절한 깊이 횟수(회), 가슴압박 깊이(mm)는 저조하였다. 따라서 신장 및 체중에 따른 가슴압박질을 향상시킬 수 있는 도구의 개발과 실습 교육이 필요하겠다.

Good 증후군 치험 -1예 보고- (Good's Syndrome (Thymoma with Immunodeficiency) -A case report-)

  • Good증후군은 성인에서 발생하는 B세포 와 T세포 면역 결핍증의 드문 원인 질환으로 임상적인 특징은 세군 감염 빈도의 증가 및 바이러스, 진군의 기회 감염이다. 가장 일치하는 면역학적 이상은 저감마글로부린 혈증과 B세포가 감소되거나 소실되는 것이다. 이 증후군의 치료는 흉선종의 절제 및 적절한 면역글로부린 G 역가를 유지하기 위한 면역글로부린의 투여이다. 저자는 1예의 Good증후군을 경험하였는데 환자는 64세 여자로 빈번한 비강 폐 감염의 병력을 가지고 있다. 흉부전산화단층 촬영에서 폐렴과 전 종격동 종괴의 소견을 보였으며 경피적 침 생검에서 흉선종으로 진단되었다. 환자는 흉선절제술을 시행 받았으며 저감마 글로부린혈증을 치료하기 위해서 면역글로부린을 투여 받았다.

심실중격결손, 심방중격결손, 폐동맥협착과 우심증을 동반한 완전대혈관전위증의 치험 (A successful Mustard operation for complete transposition of the great arteries combined with VSD, ASD, dextrocardia and PS: a report of one case)

  • A Complete transposition of the great arteries combined with V.S.D, A.S.D, dextrocardia, and P.S is a rare congenital anomaly. The patient was a 10 year-old female whose complaints were frequent URI, exertional dyspnea, and cyanosis at rest since birth. Cheat X-ray films showed Dextrocardia ; situs inversus, moderate cardiomegaly, and Characteristic egg-shape heart shadow. E.K.G, Echocardiography, Cardiac Catheterization, and Angio-Cardiography were performed. Open heart Surgery was done under diagnosis of d-TGA, Dextrocardia, V.S.D, A.S.D, and P.S. At the time of Operation, Dextroeardia, T.G.A, Secndum type A.S.D, A.S.D, and P.S. At the time of Operation, Dextroeardia, T.G.A, Secndum type A.S.D. ($2.0{\times}2.0cm$. in diameter), V.S.D. type II ($1.5cm{\times}1.5cm$ in diameter), and pulmonary valvular stenosis were noted. Mustard operation using pericardial Baffle in the atrium for T.G.A. was perforsned. Teflon patch graft for closure of V.S.D. through tricuspid orifice and pulmonary Valvulotomy through pulmonary arteriotomy were performed. The post-op, low cardiac output Syndrome and dysrhythmicawere developed till Postoperative day #7, so that was controlled by inotropic and antiarrhythmic agents. After that, patient's conditions were Uneventful.
