흉부 컴퓨터단층촬영 영상에서 블라인드 디컨볼루션 알고리즘 최적화 방법에 대한 연구 (Analysis on Optimal Approach of Blind Deconvolution Algorithm in Chest CT Imaging)

  • The main purpose of this work was to restore the blurry chest CT images by applying a blind deconvolution algorithm. In general, image restoration is the procedure of improving the degraded image to get the true or original image. In this regard, we focused on a blind deblurring approach with chest CT imaging by using digital image processing in MATLAB, which the blind deconvolution technique performed without any whole knowledge or information as to the fundamental point spread function (PSF). For our approach, we acquired 30 chest CT images from the public source and applied three type's PSFs for finding the true image and the original PSF. The observed image might be convolved with an isotropic gaussian PSF or motion blurring PSF and the original image. The PSFs are assumed as a black box, hence restoring the image is called blind deconvolution. For the 30 iteration times, we analyzed diverse sizes of the PSF and tried to approximate the true PSF and the original image. For improving the ringing effect, we employed the weighted function by using the sobel filter. The results was compared with the three criteria including mean squared error (MSE), root mean squared error (RMSE) and peak signal-to-noise ratio (PSNR), which all values of the optimal-sized image outperformed those that the other reconstructed two-sized images. Therefore, we improved the blurring chest CT image by using the blind deconvolutin algorithm for optimal approach.

Effect of Posterior-Anterior Mobilization of the Thoracic Spine on Pain, Respiratory Function, and Thoracic Circumference in Patients With Chronic Low Back Pain

  • Background: Posterior-anterior (PA) vertebral mobilization, a manual therapy technique has been used for relieving pain or stiffness treating in spinal segment for in clinical practice, however evidence to gauge efficacy is yet to be synthesised. Objects: This study aimed to investigate the effect of PA mobilization of the thoracic spine on the respiratory function in patients with low back pain (LBP). Methods: The study participants included 30 patients with chronic LBP. They were randomly allocated to the experimental and control groups. The experimental and control groups received PA mobilization of the T1-T8 level of the thoracic spine and placebo mobilization, respectively. All patients received interventions for 35 minutes a day, five times a week, over 2-week period, respectively. Forced vital capacity (FVC), forced expiratory volume in 1 second ($FEV_1$), peak expiratory flow (PEF), forced expiratory flow 25~75% ($FEF_{25{\sim}75%}$), and chest wall expansion were measured before and after the intervention. Statistical analysis was performed using independent t-test and two-way analysis of variance, and Pearson's correlation analysis was used to compare the correlation between respiratory function and chest measurement. Results: The experimental group showed significant improvements in FVC, $FEV_1$, PEF, $FEF_{25{\sim}75%}$ (p<.05), and chest wall expansion (p<.05) compared with the control group. Conclusion: PA mobilization of the upper thoracic spine may be beneficial for improving respiratory function parameters including FVC, $FEV_1$, PEF, $FEF_{25{\sim}75%}$, and chest wall expansion in patients with chronic LBP.

장난감 정리함을 접목한 유아용 옷장 디자인 개발에 관한 연구 (A Study on Closet Design for Infants in Use of a Toy Chest)

  • It suggests a closet design for infants in consideration of space utilization, furniture arrangement, and goods storage by integrating a baby's closet and toy chest, which was used for storage of baby's toys and play instrument. For this, it conducted and analyzed data research on the infants' toy chests in the domestic and overseas market and based on such approach, the colors that helped development of infants' cognition and emotion were selected while suggesting familiar animal characters for infants. Without considering separate arrangement of furniture and toy chest within limited space, it allowed space utilization and provided enough storing space. Also, the attached ball casters enabled easy carry of the closet and with this function, it helps infants arrange and store goods by themselves so that making them feel a sense of accomplishment. For effective spatial utilization as well as security of its genuine storage function, the closet and toy chest were combined in this study. Thus, it is believed that development of such multifunctional furniture will suggest the direction of researches on domestic furniture development that are conducted to maximize utilization of residential space for the people living today.

외상성 기관지 파열 -1례 보고- (Tracheobronchial Rupture following Blunt Chest Trauma -1 case report-)

  • The rupture of tracheobronchial tree caused by non penetrating blunt trauma is being increased in incidence though it is still rarely occurred on now Because this type of injury is uncommon, a high index of suspicion should be maintained in all crush injuries involving the chest. Early diagnosis and primary repair not only restore normal lung function but also avoid the difficulties and complications associated with delayed diagnosis and repair. We experienced a case of tracheobronchial rupture caused by non penetrating blunt chest trauma without rib fracture. The patient was a 16 year old male who was a high school student. He was compressed on anterior chest by hand ball goal post being failed down on the morning of admission day. After this accident, he was suffered from progressively developing dyspnea and subcutaneous emphysema on face, neck and anterior chest. The diagnosis, tracheal rupture, was made by chest CT and bronchoscopy. After right thoracotomy, the ruptured site was directly closed by using interrupted suture. Post-operative course was uneventful. Thus we report this case of traumatic tracheal rupture with review of literature.

흉곽확장을 병행한 흡기근훈련이 뇌졸중 환자의 폐기능과 최대흡기압력 및 보행지구력에 미치는 영향 (The Effects of Inspiratory Muscle Training with Chest Expansion on Pulmonary Function, Maximal Inspiratory Pressure, and Gait in Individuals with Stroke)

  • Purpose: The purpose of this study was to evaluate and compare the effects of inspiratory muscle training with chest expansion exercises on pulmonary function, maximal inspiratory pressure, and gait in individuals with stroke. Methods: The participants in this study included 36 stroke patients. These patients were randomly divided into three groups: an inspiratory muscle training (IMT) with chest expansion (CE) group (n=12), an IMT group (n=12), and a control group (n=12). Participants in the IMT with CE group underwent IMT and CE exercises 5 times per week for 30 minutes over 4 weeks, whereas those in the CE group and the control group received IMT and conventional physical therapy, respectively, for the same duration. The investigator measured the patients' pulmonary function, maximal inspiratory pressure, and gait endurance. Results: After the intervention, the change values for the forced vital capacity (FVC), forced expiratory volume in one second (FEV1), peak expiratory flow (PEF), maximal inspiratory pressure (MIP), and six-minute walk test (6MWT) in the IMT with CE group and the control group were significantly greater than those of the control group (p<0.05). Similarly, after the intervention, the change values of the FVC, FEV1, PEF, MIP, and 6MWT in the IMT with CE group were significantly greater than those in the IMT group (p<0.05). Conclusion: These findings suggest that IMT with CE could be used to increase pulmonary function, maximal inspiratory pressure, and gait endurance in stroke patients.

Annuloaortic Ectasia 의 치험 1례 보고 (Annuloaortic Ectasia Associated with Aortic Regurgitation (One case report))

  • The incidence of annuloaortic ectasia has known rare, and approximately 5-10% of aortic regurgitation. The patient was 44 years old male who complained exertional dyspnea and left anterior chest pain. He had done Lt. side 2 stage thoracoplasty for pulmonary tuberculosis about 20 years ago at Dept.of Chest surgery of National Medical Center. At that time, there was no abnormal findings in cardiovascular system. The preoperative aortic cineangiogram showed pear shaped dilatation [7.3 cm x 6.8 cm] of aortic mot with aortic valve regurgitation but left ventricular ejection function was fair. Preop. ventilatory function test showed mixed type pulmonary insufficiency. Recently, we corrected surgically, by AVR with Carpentier-Edwards Bioprosthesis [29mm] & supracoronary Woven Dacron graft [29mm x 5cm] replacement, with good clinical result for follow up 6 months.

Severe chest pain with mid-ventricular obstruction in a patient with hyperthyroidism

  • Mid-ventricular obstruction (MVO) rarely occurs in patients without hypertrophic cardiomyopathy. Increased cardiac contractility may play an important role in causing MVO. We experienced a case of severe chest pain and MVO in a 50-year-old female patient. She had hypertension, diabetes, stroke and peripheral artery disease. Her blood pressure was very high (222/122 mmHg) with severe fluctuation. The transthoracic echocardiography revealed MVO accompanied by hyper-dynamic left ventricular systolic function. We regarded her chest pain and MVO as secondary findings related to other diseases. Coronary angiography and several tests for uncontrolled hypertension were performed, and those evaluations revealed that she had coronary artery disease and hyperthyroidism. We considered that the increase in the myocardial oxygen demand in response to the increase in cardiac contractility and workload associated with hyperthyroidism aggravated her symptoms and MVO. She was treated with methimazole and beta blockers and her symptoms dramatically improved.

횡격막 성형술: 5례 보고 (Phrenoplasty [Diaphragmatic Thoracoplasty]: Report of 5 Cases)

  • Since the turn of the century there has been a constant search for a satisfactory method of controlling a large intrathoracic space following lobectomy. Primarily these methods consist of thoracoplasty, plombage, and phrenic nerve paralysis which are not completely satisfactory for they may result in loss of chest wall motility or diaphragmatic function. Incising the diaphragm at its periphery and resuturing to the chest wall at a level several rib spaces higher is an effective method of reducing intrathoracic space with minimal interference with pulmonary function. It is of particular value when the anticipated space problem is in the lower part of the thoracic cavity. Five cases are presented in which the diaphragm was peripherally detached and advanced to higher levels. Two cases were following lower lobectomy and three cases were following decortication for chronic empyema in which expansion was not good enough to adequately fill the space. Results in these cases were satisfactory.

학령후기 여아의 상반신 체형 연구 (A Study on the Upper Body Shapes of Late Elementary Schoolgirls)

  • This study is done to classify the upper body shapes for late elementary schoolgirls. The sampling was done for 11~12 years-old-girls resident in Busan and Kyungnam. Based on the somatometric charateristics of them, 33 anthropometic and 7 photogrphic measurment data were acquired from every girl. These data are statistically analyzed with the following methods; Factor Analysis, Cluster Analysis, and Discriminant Analysis. Resulting from the factor analysis, it is shown that 79.95% of the whole variances can be explained with 8 factors. Through the cluster analysis, 3 types of upper body shapes can be categorized as follows: Type I has average horizontal size, big vertical size and lots of protruded chest ; Type III has big horizontal size, the mean vertical size, and big upper angle of the back ; Type II has small horizontal and vertical size and long surface length of the upper body. Through the discriminant analysis, the high discriminative items in discriminant function are follows: Upper chest circumference, arm length and waist front length of discriminant function I and waist depth, front length, back breadth, nipple to nipple breadth and upper chest circumference of discriminant function II have large coefficient values.

Chest Wall Reconstruction for Chronic Intrathoracic Wounds Using Various Flaps

  • The treatment of chronic chest wounds should be focused on eradicating the infection and obliterating the dead space thus providing improved pulmonary function. Chronic chest wounds, although the incidence has decreased over the years, is still associated with high morbidity and prolong hospitalization. In cases where the disease is advanced and conventional measures fail, aggressive approaches achieve adequate resolution or significant improvement. This paper reports four cases of chronic chest wound including bronchopleural fistula and osteomyelitis managed by debridement followed by muscle coverage using latissimus dorsi, rectus abdominis, and omental flap. The intrathoracic reconstruction entails thorough debridement of empyema cavities, bronchpleural fistulas and infection focus. The infection must be completely eradicated prior to or at the time of flap transposition. The flaps used for obliteration of dead spaces provided adequate bulk, abundant blood supply, and minimal donor morbidity. The results were satisfactory with improved respiratory function without complications.
