시너(Thinner) 정주로 발생한 급성 폐질환 1례 (Acute Pneumonitis Induced by Intravenous Thinner Injection in a Case of Suicidal Attempt)

  • A 24 year-old man attempted suicide by injection of 1 cc of thinner into his left antecubital vein; 3 hours later, he visited our emergency room because of left chest pain. We suspected a chemical pneumonitis based on the abnormal findings of his chest X-ray and computed tomography. On the 3rd day after admission, a cellulitis also occurred at the injection area. His symptoms were relieved after supportive care for 2 weeks. There is significant experience with intoxication of thinner inhalation, whereas intoxication of intravenous thinner is rare.

상대정맥에 발생한 해면상 혈관종 -1례 보고- (Mediastinal Cavernous Hemangioma)

  • 종격동에 발생하는 혈관종은 매우 드문 질환으로 국내외적으로 발표된 예가 흔하지 않다. 본 인제대학교 상계백병원에서는 상대 정맥의 우심방 유입구 상방에 발생한 해면상 혈관종을 수술적 치험하였기에 보고하는 바이다. 환자는 35세된 여자 환자로써 우측 경부의 낭종성 종괴를 주소로 이비인후과 외래를 방문하여 수술을 위해 촬영한 단순 흉부 방사선상 우측 종격동에 비정상적인 음영이 관찰되어 본과로 전원되어 추가 검사 후 수술적 절제를 시행하였다. 절제한 종괴는 상대 정맥의 우심방 유입구 약 2cm 상방에 약 4x5cm 크기의 해면상 혈관종으로 확진되었으며 수술 후 7일째 퇴원하여 현재 외래 통한 관찰 중인 바, 이상 소견 없이 잘 지내고 있다.

A 45-Year-Old Man With Recurrent Dyspnea and Hemoptysis during Exercise: Exercise-Induced Pulmonary Hemorrhage/Edema

  • A 45-year-old man presented with dyspnea and hemoptysis during exercise. A chest computed tomography (CT) revealed multifocal diffuse patchy ground glass opacity and interlobular septal thickening in both the lungs. Permeability pulmonary edema or pulmonary hemorrhage was suspected. Serologic studies for autoimmune disorders and vasculitis were negative. There was no laboratory evidence of coagulopathy, other hematopoietic disease or infectious disease. Considering correlation with exercise, we diagnosed exercise-induced pulmonary hemorrhage (EIPH) or exercise-induced pulmonary edema (EIPE). The patient was managed with antifibrinolytics, antibiotics, and antitussive agent. After a week, follow-up chest CT revealed completely resolved pulmonary hemorrhage. About 2 months after the first event, he visited again with dyspnea and hemoptysis during running. In the present study, we report a case of recurrent pulmonary hemorrhage after exercise.

  • Pulmonary alveolar microlithiasis is a rare disease of unknown etiology that is characterized by the presence of calcific concentrations in the alveolar spaces. The radiographic appearance is pathognomonic. Plain chest radiographs show a white lung or sandstorm lung consisting of fine sand like microcalcifications diffusely scattered throughout both lungs with a higher density at the lung bases. We now report the case of a 67-year-old male whose diagnosis was based on characteristic findings on a chest X-ray and a high-resolution computed tomography scan.

A Rare Case of Primary Thymic Adenocarcinoma Mimicking Small Cell Lung Cancer

  • Primary thymic adenocarcinoma is a very rare malignancy of the anterior mediastinum with no standardized treatment. A 36-year-old male patient presented with hoarseness over the past 3 months. A chest computed tomography (CT) scan showed an infiltrative mass to the proximal vessels and aortic arch in left upper mediastinum ($4.1{\times}3.1{\times}5.4cm$). Brain magnetic resonance imaging (MRI) showed focal lesions, suggesting metastasis in the left frontal lobe. A thoracoscopic biopsy of the mediastinal mass confirmed a primary thymic adenocarcinoma forming a glandular structure with atypia of tumor cells. The patient received four cycles of systemic chemotherapy, consisting of etoposide and cisplatin, with concurrent radiotherapy (6,000 cGy/30 fractions) to the mediastinal lesion and the metastatic brain lesion (4,200 cGy/12 fractions). A follow-up chest CT scan and brain MRI showed a decrease in the size of the left upper mediastinal mass and brain lesion. We report a rare case of the primary thymic adenocarcinoma with a literature review.

소아의 흉선 과증식증 수술적 치험 1례 - l례보고 - (Giant Thymic Hyperplasia in Children - 1 case report -)

  • 흉선 과증식증은 소아에서 아주 드물게 발생하는 질환으로써 우리나라에서는 아직 보고례가 없다. 환자는 2살된 남아로 잦은 기침과 객담을 주소로 입원 후 시행한 혈액검사상 림프구 수가 55%로 증가돼있는 소견외에는 정상이었으며 흉부 단순촬영 및 흉부 단층 촬영상 우측 전중격등에 5$\times$10cm 크기의 종괴가 발견되었다. 정중 흉골절개를 통한 종괴 적출술을 시행하였다. 적출된 종괴는 180mg이었고 조직검사상 정상 흉선구조를 유지하고 있는 등 흉선 과증식증에 합당한 소견을 보였다. 환자는 술후 별다른 합병증 없이 회복된후 2년이 지난 현재까지 정상생활을 영위하고 있다. 이에 저자들은 소아에서 발생하는 흉선 과증식증으로 문헌고찰과 함께 보고하는 바이다.

Brain Abscesses Associated with Asymptomatic Pulmonary Arteriovenous Fistulas

  • Brain abscess commonly occurs secondary to an adjacent infection (mostly in the middle ear or paranasal sinuses) or due to hematogenous spread from a distant infection or trauma. Pulmonary arteriovenous fistulas (AVFs) are abnormal direct communications between the pulmonary artery and vein. We present two cases of brain abscess associated with asymptomatic pulmonary AVF. A 65-year-old woman was admitted with a headache and cognitive impairment that aggravated 10 days prior. An magnetic resonance (MR) imaging revealed a brain abscess with severe edema in the right frontal lobe. We performed a craniotomy and abscess removal. Bacteriological culture proved negative. Her chest computed tomography (CT) showed multiple AVFs. Therapeutic embolization of multiple pulmonary AVFs was performed and antibiotics were administered for 8 weeks. A 45-year-old woman presented with a 7-day history of progressive left hemiparesis. She had no remarkable past medical history or family history. On admission, blood examination showed a white blood cell count of 6290 cells/uL and a high sensitive C-reactive protein of 2.62 mg/L. CT and MR imaging with MR spectroscopy revealed an enhancing lesion involving the right motor and sensory cortex with marked perilesional edema that suggested a brain abscess. A chest CT revealed a pulmonary AVF in the right upper lung. The pulmonary AVF was obliterated with embolization. There needs to consider pulmonary AVF as an etiology of cerebral abscess when routine investigations fail to detect a source.

  • 코로나바이러스감염증-19는 전 세계적에서 급속도로 확산되고 있다. 몇몇 논문에서 코로나바이러스감염증-19의 영상의학적 소견들이 보고되었다. 우리는 한국에서 코로나바이러스감염증-19로 확진된 세 명의 환자에 대한 임상적 소견과 흉부 일반촬영 및 흉부 전산화단층촬영에서 나타난 영상의학적 소견에 대하여 보고하고자 한다.

  • Candida albicans 감염에 의한 농양 형성은 매우 드물며, 흔하지 않은 위치에 비 전형적인 형태로 관찰되는 경우, 영상의학적 진단을 내리기가 쉽지 않다. 저자들은 산후출혈로 집중치료실에 입원하여 칸디다혈증 진단 후 치료를 받고 퇴원했던 젊은 여성에서, 잠행성으로 발생한 다발성의 Candida albicans 흉벽 및 척추 농양 증례에 대해 보고하고자 한다.

종격동염의 증례보고 (Mediastinitis: a case report)

  • Acute mediastinitis is a fatal infection which occurs related to connective tissue of mediastium, in the thoratic organs. Occurrence of mediastinitis due to craniocervical infection is very rare, and is defined as descending necrotizing mediastinitis. November 8th, 2008, man in his early fifties visited ER due to severe swelling on left neck area and dyspnea. Antibiotic were administered immediately, and vast amount of abscess formation on pneumomediastinum and adjacent tissues were observed via chest computed tomography. With cooperation of thoracic and cardiovascular surgery department, emergency incision and drainage with drain insertion was done to remove abscess, and control the infection. After surgery, everyday saline irrigation through drain was done during hospitalization, with continues antibiotic therapy. Descending necrotising mediastinitis is a most rare and dangerous infection which occurs on oropharyngeal area. In case of descending necrotising mediastinitis, accurate diagnosis, airway maintenance, remove of abscess by incision and drainage, aggressive antibiotic therapy and continuous saline irrigation is necessary to increase patient survivability. Also, computed tomography with contrast media is essential to figure out the size and location of the infection and abscess formation.
