Primary Hemangiopericytoma of the Lung -1 Case Report - (폐에 발생한 원발성 혈관주위세포종 -1례 보고-)

  • Primary hemangioperiycytoma is quite rare tumor of the lung and arising from pericyte in external layer of capillaries. Most cases are developed in 4th and 5th decade, are asymptomatic, and have malignant otential. On chest radiography, primary pulmonary hemangiopericytoma shows lobulated, well demarcated, homogeneous soft tissue density. Microscopically, it consisits of numerous vascular spaces of variable size and shape separated by aggregates of tightly packed oval to spindle-shaped cells. Treatment of choice is surgical excision. We report a case of primary pulmonary hemangiopericytoma in a 16-year-old man who had well demarcated homogeneous mass in the superior segment of left lower lobe, but had no symptom. He had undergone left lower lobectomy. He has been followed up for 8 months but has no sign of relapse or metastasis yet.

A Case of Pneumonia Treated with Combined Eastern-Western Medical Therapy and Hwangkibyulgap-tang (한양방협진 및 황기별갑탕으로 호전된 폐염(肺炎)환자 1례 보고)

  • This is clinical report of one patient with pneumonia in stroke who improved with the administration of an herb drug. During treatment for stroke, he broke out with pneumonia, a complication of stroke, which was confirmed by chest radiography. He was administrated Hwangkibyulgap-tang with antibiotics because of old age middle stage pneumonia. After administration, the symptoms improved and chest x-ray film showed no pneumonic finding. This result suggests that oriental medicine is an effective treatment for pneumonia but more clinical case reports are needed.

  • Mediastinal hemangioma is a rare tumor and only few cases have been reported in the literatures. We have experienced one case of cavernous hemangioma occuring at the superior vena cana. The patient was a thrity-five year old female with no specific symtoms except palpable cystic mass in the right cervical area. A routine chest radiography showed an upper mediastinal mass. Computed Tomography showed about 4$\times$5 cm sized cystic mass communicating to the superior vena cava 2 cm above of the veno-atrial junction. After the evaluation, surgical excision was performed and the pathologic diagnosis was confirmed to cavernous hemangioma. Postoperative course was uneventful and the patient has been followed up without any problems.

The Clinical Manifestations of Lipoblastoma in Children (소아 Lipoblastoma의 임상 양상)

  • Lipoblastoma is a rare benign soft tissue tumor occurring in infancy and early childhood. It is characterized by fat lobules with varying degrees of maturity, multivaculoated lipoblasts, fibrocapillary networks and myxoid stroma. Lipoblastoma has a good prognosis with no metastases despite its potential for local invasion. From Jan, 1990 through April, 2007, 12 children underwent the operation for lipoblastoma, 7 boys and 5 girls, diagnosed at median 22 months (5 ~ 43 months). Median follow up was 6 year 7 months. Primary sites included back (n = 5), intraabdominal (n = 2) and one in each of buttock, chest wall, neck, nose and scalp. Tumors presented with a growing mass in 9 patients, abdominal distension in 2, and an incidental finding on chest radiography in one. Complete excisions were done in all patients. There was one recurrence in a patient with a scalp mass. After reoperation, he has been doing well without evidence of recurrence. Lipoblastoma has a favorable prognosis, but recurrence can occur even with complete excision. Regular follow up is necessary to detect recurrences.

The Effectiveness of Ultrasound-Guided Essential Bee Venom Pharmacopuncture Combined with Integrative Korean Medical Treatment for Rib Fracture: A Case Study (분리정제봉약침의 초음파 유도하 시술과 한의복합치료를 병행한 늑골 골절 증례 보고)

  • This study is to report the usefulness of ultrasound in diagnosing rib fracture and the effectiveness of ultrasound-guided essential bee venom pharmacopuncture as a treatment of it. A 46-year-old unable to find a fracture by chest radiography was diagnosed it on ultrasound. The patient was applied integrative Korean medical treatments and ultrasound-guided essential bee venom pharmacopuncture at the fracture site for 4 weeks, 18 times total. Numeral rating scale (NRS) was used to evaluate the amount of pain. The chest pain when turning over the body was reduced from NRS 8 to 2 for 4 weeks. The remarkable aspect is that the pain was immediately relieved after phamacopuncture and its effect lasted for 3 hours. This report suggests ultrasound can be useful for diagnosing rib fracture and also helpful for safety and accuracy of pharmacopuncture. Additionally, essential bee venom can be considered one of the methods of treating rib fracture.

  • Coronavirus disease 2019 (COVID-19) is spreading rapidly around the world. Several articles have so far reported on radiological findings of COVID-19 pneumonia. Herein, we present three cases of COVID-19 pneumonia in South Korea, and provide clinical information as well as chest radiograph and chest CT findings.

A Study on Findings from Simple Chest Radiographes without Any Clinical Symptoms (임상적 증상이 없는 흉부 단순X선영상 소견에 대한 분석)

  • In this study, the analysis on findings from simple chest radiography(CXR) test with total 1,669 subjects without any special clinical symptom came to the following conclusions : 1. In terms of the general characteristics of subjects hereof, male and female group accounted for 55.2% and 44.8% respectively out of all 1,669 people. 2. Pulmonary disease cases amounted to 249 persons(14.9%) out of all subjects. 3. In the analysis on prevalence rate by age distribution, it was noted that the older age led to the more number of diseases, which was demonstrated by age 34 or younger(6.1%), age $35{\sim}39(9.7%)$, age $40{\sim}49(13.3\;%)$, and age 50 or older(30.8%). 4. In regard of pulmonary disease alone, the region of onset was represented primarily by right upper lobe, which was followed by both upper lobe and left upper lobe, respectively. 5. In terms of disease types, it was found that most cases were represented by pulmonary nodule(55.0%), which was followed by cardiomegaly(24.5%), CP angle blunting(4.8%), scoliosis(4.6%), tortuous aorta(2.8%), bronchial luminal dilatation(2.4%), and pleural thickening(2.0%). However, dextrocardia, cystic dilation of bronchus, cavitary lesion, and lung collapse accounted for relatively low rate(0.4% respectively). 6. In terms of disease types by sex, it was found that male group accounted for higher percentage of having pulmonary nodule than female group, while the latter accounted for higher percentage of having cardiomegaly, tortuous aorta and scoliosis than the former. 7. In terms of disease types by age distribution, it was noted that age 34 or younger group accounted for higher percentage of scoliosis than any other age groups, while age $40{\sim}49$ group, age $35{\sim}39$ group, and age 50 or older group represented the case of CP angle blunting, pulmonary nodule, and cardiomegaly/tortuous aorta, respectively.

The Review of Exposure Index in Digital Radiography and Image Quality (디지털 영상에서 화질관리에 관한 노출지수(EI)의 유용성 연구)

  • The aim of this study was to determine the correlation between exposure index (EI) and dose factors related to radiation dose optimization in digital radiography (DR) system. Two phantoms with built-in regional test object for quantitative assessment of images were used to produce image signals that acquired in chest radiography background. EI and entrane surface dose (ESD) increased proportionally with rise of radiation dose (kVp, mAs) in both DR and CR systems. Especially, DR detector was effective to form good contrast and hence, reached easily to improvement of image quality with minimal dose changes. It made operators possible to expect the accuracy of EI values deeply related to absorbed dose of the detector. The evaluation of images was obtained specially employed calculation of noise to signal ratio (NSR) and contrast to noise ratio (CNR). These measurements were performed for how exposure factors affect image quality. NSR was inversely proportional to kVp and mAs and low NSR represented high signal detection efficiency. Consequently, EI values was the measure of the amount of exposure received by the image receptor and it was proportional to exposure factors. Therefore the EI in a recommended range from manufacturer can offer optimal image quality. Also, continuous monitoring of EI values in the digital radiography can reduce the unnecessary patient dose and help the quality control of the system.

A Convergence Study on Evaluation of Usefulness of Copper Additional Filter in the Digital Radiography System (디지털 방사선장치에서 구리 부가필터의 유용성 평가에 관한 융복합 연구)

  • This convergence study analyzed the effectiveness of digital radiography system of copper(Cu) filter in the added filtration for the removal of lower energy radiation through dose and image evaluation. We were analyzed from April to June 2015 result of the examination. Cu filter was applied to each non, 0.1, 0.2, 0.3 mm according to change of kV and mAs and doses were evaluated. Image quality was evaluated by PSNR, MAE, MSE, CNR, SNR and qualitative analysis was performed by seven items for resolution and contrast from chest x-ray criteria of national cancer checkup. The absorbed doses with Cu were lowered by 16-88 % than non-filter but the gaps decreased as kV increased. PSNR were over 30 dB and all significant and CNR and SNR were superior with non-filter but in the qualitative analysis, there were different statistical significant according to each item. The score of 0.1 mm filter was high at pulmonary blood vessel observation and in the 0.3 mm Cu, there were no statistical signigicant except high density and full of air portion. Cu filter can improve image quality with lower radiation dose using better radiation quality and correction power at digital radiography system.

Diagnosis of Location and Size of Lesions using Chest X-ray Image (X-선 영상을 이용한 암의 위치 및 크기 진단)

  • X-ray general radiography is the simplest and most important one to get a lot of information. Nevertheless, current x-ray general radiography does not observation in-depth observation. Information about the anatomy of the human body and changes in disease in x-ray general radiography can be obtained but it is difficult to determine the size and shape of the actual lesion due to the disadvantage of expanding the image. In this study, PA and LAT images were acquired and cancer magnification was calculated in the images by measuring the distance of cancer samples. By adjusting the magnification the actual cancer length and thickness were measured and compared with the CT image and the actual cancer sample size. After the PA and LAT images of the inserted 6.0 mm cancer sample were obtained and the magnification was corrected, the length was 5.9 mm and the thickness was 6.1 mm. This value was measured similarly to the actual. The problem of obtaining the magnification that needs to know the actual length from the detector to the cancer sample was secured by obtaining the magnification through PA and LAT images and it is possible to accurately measure the cancer sample size. X-ray general radiography may provide useful information in situations where CT imaging is difficult.
