Adequacy of Source to Image Receptor Distance with Chest Postero-Anterior Projection in Digital Radiology System (디지털방사선 환경에서 흉부 후-전 방향 검사 시 초점과 영상수용체간 거리의 적절성)

  • The purpose of this study is to evaluate propriety of using SID 180cm at Chest PA examination and to find effect of geometrical cause to the image. XGEO-GC80, INNOVISION-SH, CXDI-40EG detector and a chest phantom designed self-production was used for this study. Images were acquired at SID 180cm with changing the factor OID as 0, 75 and 83mm and were analyzed by Centricity Radiography RA1000 PACS system. Statistical program was used the SPSS (Version 22.0, SPSS, Chicago, IL, USA), p-value(under 0.05) was considered to be statistically significant. In OID 0 mm was enlarged about 2.7~3.5 mm than the actual degree of the HS, BS of phantom in all equipments. Compared with the calculated magnification has been expanded 1.6~2.8% when viewed. The OID 75 mm with OID 83 mm was extended from the CS and BS 6~8 mm range. Compared to the calculated values, the measured values are expanded from 6.1 to 7.9%. CS and BS according to the OID change showed a statistically significant difference (p<0.05) among each group, the post-analysis only OID 0 mm group appeared as an independent group, 75 mm and 83 mm are separated in the same group It was. But had no statistically significant difference could change depending on the OID (p>0.05), post-mortem analysis showed, both in the same group. Heart sizes appears larger than actual size 6~8 mm at chest PA examination which is enlarged 6.1~7.9% more than the actual theoretical value. We can find magnification of the image because of the increase of the OID due to technical limitations between cover of standing detector and the image plate. so we suggest to have occurred between them when considering the need to adjust the equipment installed by the SID to match the characteristics of the equipment.

Effect of Human Implantable Medical Devices on Dose and Image Quality during Chest Radiography using Automatic Exposure Control (자동노출제어를 적용한 흉부 방사선 검사 시 인체 이식형 의료기기가 선량과 화질에 미치는 영향)

  • In this study, we applied AEC(Auto Exposure Control), which is used in many chest examinations, to evaluate whether medical devices inserted into the body affect the dose and image quality of chest images. After attaching three HIMD(Human implantable medical devices) to the ion chamber, the Monte Carlo methodology-based program PCXMC(PC Program for X-ray Monte Carlo) 2.0 was applied to measure the effective dose by inputting the DAP(Dose Ares Product) value derived from the Pacemaker and CRT and Chemoport Additionally, to evaluate image quality, we set three regions of interest and one noise region on the chest and measured SNR and CNR. The final study results showed significant differences in DAP and Effective dose. There was a significant difference between Pacemaker and CRT when AEC was applied and not applied. (p<0.05) When applied, the dose increased by 37% for Pacemaekr and 52% for CRT. Chemoport showed a 10% increase in effective dose depending on whether AEC was applied, but there was no significant difference. (p>0.05) In the image quality evaluation, there was no significant difference in image quality between all HIMD insertions and AEC applied or not. (p>0.05) Therefore, when the HIMD was inserted into the chest during a chest x ray and overlapped with the ion chamber sensor, the effective dose increased, and there was no difference in image quality even at a low dose without AEC. Therefore, when performing a chest X-ray examination of a patient with a HIMD inserted, it is considered that performing the examination without applying AEC is a method that can be considered to reduce the patient's radiation exposure.

Analysis of High-Resolution CT Findings in Patients with Spontaneous Pneumothorax (자연기흉환자의 고해상 CT소견의 분석)

  • Background: We analysed simple chest PA and high-resolution CT findings in patients with spontaneous pneumothorax in order to help selecting the kind of treatment, provide a guidline during surgical treatment, and to recognize the bulla which may not be detected by simple radiographs or may be a potential cause of recurrence. Material and Method: We retrospectively analysed the presence and number of bulla in each side, combined pulmonary disease on simple chest films and high-resolution CT, and methods and frequency of the treatment in 70 patients with spontaneous pneumothorax excluing traumatic origin. Result: 45 patients were revealed primary spontaneous pneumothorax, and the remaining 25 patients were revealed secondary spontaneous pneumothorax. All secondary spontaneous pneumothorax were from the longstanding sequelle of pulmonary tuberculosis. The patients with primary spontaneous pneumothorax group was younger(mean:26.0 years old) than secondary group (mean: 44.1 years old). On simple radiography, bulla was detected in 16 patients(30.2%). On HRCT, the bulla was detected in 53 patients(75.7%) of the total 70 patients. In 48 patients(68.6%), the bulla or bleb was noted in ipsilateral side to the pneumothorax, and 34 patients(48.6%) of them showed bulla or bleb bilaterally. 39 patients(55.7%) showed bulla or bleb in contralateral side. The number of bulla or bleb was variable. In secondary spontaneous pneumothorax group, the incidence of multiple(more than 10) bulla or bleb was higher than primary type. Most of the patients were treated by thoracostomy(36 patients) or bullectomy( 7 patients). Conclusion: HRCT was superior to detect bulla and analyse the combined pulmonary disease than simple radiography. Therefore, HRCT can help to determine the mothod of treatment, provide a guidline during surgical treatment, and notify the bulla as a possible cause of recurrent pneumothorax.

Evaluation of Effective Dose with National Diagnostic Reference Level using Monte-Carlo Simulation (몬테카를로 시뮬레이션을 이용한 국내 일반엑스선검사 진단참고수준의 유효선량 평가)

  • In this study, the effective dose for frequently general radiography among the diagnostic reference level (DRL) for examinations provided by the government in Korea was evaluated using the Monte Carlo N-Particle eXtended (MCNPX) simulation tool. We were selected to evaluate for a total of 5 examination sites which included head anterior-posterior, chest (posterior-anterior, lateral), abdomen anterior-posterior and pelvis anterior-posterior. Physical conditions such as tube voltage and tube current used in MCNPX simulation were used in domestic conditions of the Korea Disease Control and Prevention Agency (KDCA). To evaluate domestic medical radiation exposure, we used the HDRK-Man computerized human phantom manufactured based on the international standard ICRP 103 that was applied to the MCNPX simulation. The phantom could represent the standard body shape of Koreans. As a results, the effective dose corresponding to the DRL based on adult males of head anterior-posterior position was 0.086 mSv, chest posterior-anterior position was 0.05 mSv, chest lateral was 0.354 mSv, abdomen anterior-posterior position was 0.548 mSv, and pelvis anterior-posterior position was 0.451 mSv.

The reduction methods of operator's radiation dose for portable dental X-ray machines

  • Objectives: This study was aimed to investigate the methods to reduce operator's radiation dose when taking intraoral radiographs with portable dental X-ray machines. Materials and Methods: Two kinds of portable dental X-ray machines (DX3000, Dexcowin and Rextar, Posdion) were used. Operator's radiation dose was measured with an 1,800 cc ionization chamber (RadCal Corp.) at the hand level of X-ray tubehead and at the operator's chest and waist levels with and without the backscatter shield. The operator's radiation dose at the hand level was measured with and without lead gloves and with long and short cones. Results: The backscatter shield reduced operator's radiation dose at the hand level of X-ray tubehead to 23 - 32%, the lead gloves to 26 - 31%, and long cone to 48 - 52%. And the backscatter shield reduced operator's radiation dose at the operator's chest and waist levels to 0.1 - 37%. Conclusions: When portable dental X-ray systems are used, it is recommended to select X-ray machine attached with a backscatter shield and a long cone and to wear the lead gloves.

  • Percutaneous dilatational tracheostomy (PDT) has become an increasingly popular method of establishing an airway for patients in need of chronic ventilator assistance. We report a rare case of a 42-year-old female who developed extensive subcutaneous emphysema, bilateral pneumothoraces, pneumomediastinum, and pneumoperitoneum after percutaneous dilatational tracheostomy. The patient suffered from amyotrophic lateral sclerosis, and underwent PDT after a period of mechanical ventilation. During PDT, tracheostomy tube was inserted into the paratracheal space. Follow-up chest radiography and computed tomography of chest and abdomen revealed extensive subcutaneous emphysema, bilateral pneumothoraces, pneumomediastinum, and pneumoperitoneum. The patient was treated successfully with insertion of the thoracostomy tube and conservative care.

  • Pulmonary artery aneurysm is Behcet`s disease is rare and can be fatal due to rupture. We experienced a case of pulmonary artery aneurysm in Behcet`s disease. The patient was 21 year old woman who was adimitted with three month history of dyspnea, fever and cough. On examination, she had aphthous ulcer in the mouth and erythema nodosum on the left popletial fossa and forearm, but didn`t have any lesion at eyes and genitalia. The latex fixation test for rheumatoid factor, VDRL test for syphillis, antinuclear antibody and LE cell test were all negatives. The third and fourth components of complement in the serum, serum immunoglobulin concentrations[IgG, IgM, IgA] were within normal range. The chest radiography revealed a 5x6cm sized radiopaque mass density in the left hilar region. Two months later, the mass was enlarged to 6x7cm. The IV-DSA showed a single aneurysm at the proximal part of left lower lobe artery with lingular segment artery and no distal perfusion by thombotic obstruction. The steroid therapy was done for a month, but symptoms not improved. We performed resection of lingular segment and lower lobe including the aneurysm. The microscopic findings of the operative specimen were intimal hyperplasia and fragmentation of the internal elastic fibers. She was improved without remarkable event, except infection of the operative wound.

  • Pulmonary embolism is one of the moot common acute pulmonary disease in the adult general hospital population However, the disease is still frequently unsuspected and underdiagnosed due to the nonspecificity of both clinical findings and laboratory tests. The chest radiography in a patient suspected acute pulmonary embolism do not provide adequate information to establish or exclude the diagnosis of pulmonary embolism. Even in the case of infarction, there is no pathognomonic clues on the chest film. Rarely infarction presents unusual roentgenologic manifestation such as lobar consolidation, coin lesion, multinodular opacity, or massive pleural effusion Especially, lobar consolidation in pulmonary embolism might mislead into the diagnosis of pneumonia. We experienced a case of pulmonary embolism presenting lobar consolidation in a 62 years old woman, originated from deep vein thrombosis. She took a compression stocking and underwent anticoagulant therapy with excellent outcome.

  • Von Recklinghausen's disease is an autosomal dominant hereditary disease associated with characteristic cafe-au-lait spots of skin and multiple neurofibromatosis. It is complicated by malignancies, which in most cases is neurofibrosarcoma. The development of lung cancer in von Recklinghausen's disease is rare. A 61-year-old male was admitted for cough and sputum for 20 days. He had multiple cafe-au-lait spots and subcutaneous neurofibromas in whole body area and Lisch nodules in both iris and he had been diagnosed von Recklinghausen's disease 35 years ago. Chest radiography showed emphysematous bullae in both upper lung field and mass in right upper lung field. Chest CT scan revealed subcarinal lymph node enlargement. Bronchoscopic biopsy was done in mass in superior segment of right lower lobe and the results showed squamous cell carcinoma. The presence of von Recklinghausen's disease and lung cancer are noteworthy.

  • X-ray quality is identified numerically by half value layer(H.V.L) and the HVL is affected by the kVp and the amount of filtration in the useful beam. X-ray quality evaluated by H.L.D is influenced by kVp and filtration. Author had several experiments with phantom in diameter of 8 cm normal adult chest, for reduction of radiation dose of the patients in diagnostic radiology and got some results. 1. H.V.L is increased the thicker the filter and the higher the kVp. 2. If the kVp is increased from 60 to 120, the skin dose can be reduced as 34%(Skin dose of 60 kVp with 4 mmAl filter : 100%). 3. If the 4 mmAl filter with 60 kVp is added to x-ray tube, skin dose can be reduced as 23% than no filter. 4. Therefore high kVp and filtration can increase output to input dose ratio and 120 kVp and 4 mmAl filter were most effective for reduction of patient dose in chest radiography.
