Recurrent Contralateral Thoracic Herpes Zoster after Left Thoracic Zoster Sine Herpete -A case report- (좌측 흉부 Zoster Sine Herpete 후 반대측 흉부에 재발한 대상포진 환자의 치험 1예 -증례 보고-)

  • This report is a case of 62-year-old man with anterior chest pain and pin pricking pain with allodynia affecting left T5 sensory dermatome for 3 months without history of vesicular skin eruption. He had a history of diabetes mellitus for 10 years and insulin therapy for recent 1 year. EKG, chest PA and rib series were normal. Serologic evaluation of IgG antibody to varicella-zoster virus was positive and was diagnosed as post herpetic neuralgia after zoster sine herpete. He was treated with left T5 nerve root block followed by thoracic epidural blockade and intercostal nerve block for 2 weeks. His VAS score decreased from 10 to 2 after 2 weeks of treatment. After 3 months, he revisited our clinic complaining right side chest pain followed by vesicular skin eruption 8 days after the onset of pain. He was treated as herpes zoster and tolerates well after 4 months.

Risk Factors of Recurrent Spontaneous Pneumothorax (자연기흉의 개발위험인자)

  • To evaluate the risk factors involving the recurrence of the spontaneous pneumothorax, 125 patients were reviewed. These patients were consecutively diagnosed and treated for the spontaneous pneumothorax at the Department of Thoracic and Cadiovascular Surgery, Yeungnam University Hospital, from Jun. 1986 to Apr. 1991. The patients were divided into two groups, control and recurrent. The control group, consisting of the 125 patients, did not have recurrences of the pneumothorax for a period of 2 years following the first attack. The recurrent group were the remaining 57 patients, who experienced at least one recurrence during the same period. The number of patients over the age of 50 was significantly higher in the recurrent group than the control group. Abnormal findings on chest X-ray[e.g., old tuberculous scar, emphysematous change, visible bullae of blebs] were observed more frepuently in the recurrent group. also, when the pneumothorax size was larger than 50%, and the air-leakage from the chest tube was longer than 3 days during the first attack the incidence of recurrence was significantly increased in the recurrent group. The recurrence occured more frequently in the afternoon, and in the same thoracic cavity. Exertion and smoking were not related to the risk factors in this clinical setting.

Clinical evaluation of thoracic empyema: review of 59 cases (농흉의 임상적 고찰: 59례 보)

  • A Clinical analysis of 59 patients of thoracic empyema was done who were received surgical intervention at dept. of thoracic surgery of the C.A.F.G.H. in the period of 2.5 years from January 1979 to June 1982. Occurrence ratio of Left and Right side pleural cavity of empyema was 1: 1.4. The predisposing factors of empyema were pulmonary Tbc. [49%], Chest pain [25%], Cough [8%], in order. B.P.F. was associated with empyema in 5 cases. The pleural cavity empyema was treated with several surgical procedures and conservative measures. Among of the 59 cases, the 30 cases [50%] were treated with decortication, 12 cases [20%] with closed thoractomy drainage, 9 cases with frequent thoracenteses, 5 cases with partial decortication and thoracoplasty and 3 cases with open thoracostomy tube drainage. Among of the 59 cases thoracic empyema, the full recovery were in 32 cases [54%], partial recovery in 20 cases [34%], not improved in 3 cases [5%] and 3 cases were died. The mortality rate was 5% and the recovery rate was 89%.

Pulmonary Mcosis (폐진균증)

  • It has been known that the pulmonary mycosis generally results from saphrophytic colonization of pre-existing lung cavities, e.g. , due to pulmonary tuberculosis, abscess, bronchiectasis. or congen-ital cysts. Recently, the authors experienced four cases of the pulmonary mycosis which were all treated surgically, and in our opinion, three of them were considered to arise from secondary saphrophytic colonization of pre-existing tuberculous cavities by serial chest roentgenograms. One of them was actinomycosis which was known as relatively rare pulmonary mycosis, and its clinical experience was previously reported. The purpose of this report is mainly to review our clinical experience and some related literatures with three patients with aspergillosis. Many writers have stressed the sputum culture for aspergillus, immunologic study and serial roentgenographic findings were all important or essential in diagnosing aspergillosis. Surgical resection appears to be the treatment of choice for the mycosis of lung, and systemic administration of effective anti-fungal agents such as amphotericin B for aspergillosis and penicillin for actinomycosis respectively following surgical intervention is usually necessary to eradicate completely.

A clinical study of patent ductus arteriosus (개방성 동맥관의 임상적 고찰)

  • 24 cases of patent ductus arteriosus were experienced from Aug. 1977 to October 1983, at the department of thoracic surgery. Chungnam National University Hospital. Patent ductus arteriosus is one of the common congenital heart disease and the diagnosis can be made easily and simply. Interruption is done by thoracic surgeons and it is considered on of the simple separation. 1. Age ranged from 10 month old to 19 year old. 2. Sex ratio [male to female] was 1:2. 3. Diagnosis was made by the finding of the physical examination, chest X-ray, E.K.G., and Cardiac catheterization. 4. Operation was performed with double ligation and transfixion suture ligation at 23 cases, division was one case. 5. Postoperative recanalization was occurred in one patient, who had subacute bacterial endocarditis.

Mitral Valve Replacement: A Case Report (승모판막 이식수술 1례 보고)

  • 37 years old female was admitted with chief complaints of dyspnea on exertion and hemoptysis. Past history and family history were non-contributory. Physical examination showed Grade III systolic murmur at the apex, which transmitted to the back. E. K. G. and X-ray findings were compatible with the mitral insufficiency. With small size of Beall mitral valve, mitral valve replacement was done under the cardia-pulmonary bypass using hemodilution technic. Patient was tracheotomized after operation and assisted respiration was done for four weeks. Postoperatively, all signs were fine and patient walked around the ward without any difficulty, but she was in psychotic state. On postoperative 60th day, she complained of sudden dyspnea and on chest film, tracheal stenosis was found and recannulation of the tracheal tube was made. Thereafter, she was quite fine until postoperative 110th day when she, by berself, removed the tracheal cannula and died of asphyxia. Autopsy findings of the valve showed no thrombosis, no variance of the valve, and good endothelization of the valve cuffs. Asphyxia, due to removal of the tracheal connula by herself under psychotic state, was considered to be the cause of death in this patient who had tracheal stenosis after tracheostomy.

  • The child restraint system is blown to be excellent to reduce child occupant injury in frontal collisions. The effects of the child restraint system are experimently investigated according to FMVSS 213. A sled simulator is utilized with varying restraint types such as 2point, 3point seat belts, forward-facing types and booster types of child restraint systems. The head and chest injuries for various cases are evaluated based on industrial standards. Also, the maximum displacements of the head and the knees are measured by film analysis. Using the results of the test, the effects of the child restraint system is discussed and reduction of child occupant injury is pursued.

A Case Report of IgG4-Related Disease Clinically Mimicking Pleural Mesothelioma

  • An immunoglobulin G4 (IgG4)-related disease is a recently emerging entity, and a few cases of IgG4-related disease in lung and pleura have been reported. Herein, we report the case of a 74-year-old man with IgG4-related disease of lung and pleura, clinically suspicious of malignant mesothelioma. Chest computed tomography showed diffuse nodular pleural thickening, and microscopic finding disclosed diffuse thickening of visceral pleura with infiltrations of many lymphoplasma cells with increased number of IgG4-positive plasma cells and a few multinucleated giant cells. It is important for pathologists and clinicians to recognize this rare entity and its histologic finding, because it can be confused with malignant tumors on the radiologic examination although it can be treated with steroid therapy.

  • In emergency rooms, patients with acute chest pain should be diagnosed as quickly as possible with higher diagnostic accuracy for an appropriate therapy to the patients with acute coronary syndrome or for avoiding unnecessary hospital admissions. At present, electrocardiography(ECG) and biochemical markers are generally used to detect myocardial infarction and coronary angiography is used as a gold standard to reveal the degree of narrowing of coronary artery. Magnetocardiography(MCG) has been proposed as a novel and non-invasive diagnostic tool fur the detection of cardiac electrical abnormality associated with myocardial ischemia. In this study, we examined whether the MCG can be used fur the detection of coronary artery disease(CAD) in patients, who were admitted to the emergency room with acute chest pain. MCG was recorded from 36 patients admitted to the emergency room with suspected acute coronary syndrome. The MCG recordings were obtained using a 64-channel SQUID MCG system in a magnetically shielded room. In result, presence of CAD could be found with a sensitivity of 88.2 % in patients with acute chest pain without 57 elevation in ECG, demonstrating a possible use in the emergency room to screen CAD patients.

The Eating Behaviors, Nutrient Intakes and Hematological Status of the Lower Grade Primary School Children in Gwangiu (광주지역 일부 초등학교 저학년 아동의 식습관과 영양소 섭취량 및 혈액성상에 관한 연구)

  • The purpose of this study was to examine height, weight. chest circumference, sitting height, hematological status. eating behaviors and nutrient intakes for the lower grade primary school children in Gwangju. The subjects consisted of 76 boys and 60 girls aged 9 years old. Height, Weight, Chest circumference, Sitting height, Hct, WBC, RBC, Hb, serum GOT, GPT and cholesterol were measured. With regard to regularity of meal time, 66.7% of the subjects has been 'regular'. With regaled to amount of eaten food, 100.0% of the subjects has been 'moderate'. With regard to nutritional balance, 66.6% of the subjects has been 'think,but do not practice'. The study also found that 100.0% of the subjects skipped breakfast, liked korean food. Their dietary intake vase assesed for 1 day by means of 24 hours dietary recall method. The mean energy intake of the subjects was 1,306 kcal. The subjects consumed 47.9g protein, 28.8g lipid, 3.9g fiber, 446.9mg calcium, 835.0mg phosphorous, 7.9mg iron, 3,721mg sodium, 1.863mg potassium, 362.3RE retinol, 0.8mg thiamin, 1.1mg riboflavin, 10.8NE niacin, 93.9mg ascorbic acrid and 173.5mg cholesterol respectively. Energy, protein, calcium, iron, retinol, thiamin, riboflavin and niacin intake was lower than the Korean RDA. There were positive correlations between meal time and protein intake or fat intake or fiber intake or iron intake or retinol intake : negative correlations between meal time and sodium intake negative correlations between saltiness and cholesterol intake positive correlations between use of perilla seeds and riboflavin intake or niacin intake : negative correlations between energy intake or carbohydrate intake or phosphorous intake : negative correlations between frequency of eating-out and protein intake or fat intake or fiber intake or iron intake or retinol intake or thiamin intake or riboflavin intake or niacin intake.
