Ankylosing Spondylitis Associated with Bilateral TMJ Ankylosis (강직성 척추염에 수반된 양측성 측두하악관절 강직)

  • A 31-year-old male with the complaint of severe limitation of mouth opening was referred to our department of Chonbuk National University Hospital. The physical status of the patient was hyposthenic. Extraoral examination showed no condylar movement of the both temporomandibular joints, no pain, no facial swelling or paresthesia. Intraoral examination showed several cervical caries on the upper anterior teeth, and gingival swelling on the whole dentition. Transcranial view showed no condylar movement, and narrowing of joint spaces. Chest P-A view showed straightening of thoracic, lumbar spine, and squaring of vertebrae of the same spines. Conventional lateral radiograph of cervical spine showed calcification of the intervertebral ligament. Computed tomograph showed extensive bone formation between temporal bone and the condylar head at both sides. Laboratory findings showed positive reaction on HLA-B27 histocompatibility antigen and increased level of IgA, IgG, ESR. Based on the clinical, radiographic, and the laboratary findings, final diagnosis was made as bony ankylosis of the both temporomandibular joints secondary to ankylosing spondylitis.

Changes in Lymphocyte Subsets following Open-Heart Surgery ; A Study for Changes in Lymphocyte Subsets (개심술 환자에서의 면역기능의 변화;T lymphocyte subset의 변화에 대한 고찰)

  • Cell mediated immunity is depressed following surgical procedure and the degree of immunosuppression is directly related to the magintude of the procedure, blood transfusion, and length of operation. So we would expect cardiac operations to be highly immunosuppressive, although little is konwn about their immunosuppressive effect. The nearly complete consumption of complement factors and decreased levels of IgM and IgG resulting in an impaired opsonizing capacity. Additionally, peripheral blood mononuclear cell counts including T-and B-lymphocytes and T-cell subsets are reduced. Depression of cell-mediated immunity following open-heart surgery is potentially detrimental because it could increase the susceptability of patients to viral and bacterial infection. We reviewed 20 patients after cardiac operation to search for changes in peripheral blood lymphocyte subsets. Lymphocyte subsets were measured by flow cytometer and the preoperative values of lymphocyte subsets were compared with those from the first, fourth, and seventh days after operation. After cardiac operation, total mumbers of T lymphocyte was severely depressed on the first postoperative day and returned to the preoperative level by the seventh day after operation. CD3, CD4, and CD8 lymphocytes were decreased on the first postoperative day and returned to the preoperative level by the seventh day also. There was four cases of wound infection and these patients had increased CD4 lympocyte and more decreased CD19 lymphocyte compared with the non-infected group. It is concluded from these data that cell-mediated immunity is significantly depressed for at least one week following open-heart surgery and this result was closely related to the postoperative infection.

Double-Outlet of Left Ventricle in Corrected Transposition of Great Arteries -One case report- (좌심실 이중유출로를 동반한 교정형 대혈관전위증 -1예보고-)

  • This is one case report of the extremely rare congenital cardiac malformation, Double-outlet of left ventricle in corrected transposition of great arteries. 11-year-old boy complained acrocyanosis and exertional dyspnea, the parents noticed cyanosis since birth. Physical examination revealed acrocyanosis, clubbed fingers and toes, G-III pansystolic murmur on 2nd and 3rd ICS, LSB. Right heart catheterization revealed significant $O_2$ jump in ventricular level. Right and left ventriculography showed the both catheters arriving in the same ventricle i.e. anterior chamber, morphological left ventricle was in right and anterior position, simultaneous visualization of aorta and pulmonary artery and aorta locating anterior and right side of pulmonary artery. Echo cardiogram surely disclosed interventricular septum. Conclusively it was clarified that the patient has Double-outlet of left ventricle and corrected transposition of great arteries [S.L.D.]. Operation was performed to correct the anomalies under extracorporeal circulation with intermittent moderate hypothermia. Right-sided ventriculotomy disclosed the following findings. 1. Right-sided ventricle was morphological left ventricle. 2. Left-sided ventricle was morphological right ventricle. 3. Right side atrioventricular valve was bicuspid. 4. Left side atrioventricular valve was tricuspid. 5. Aortic valve was superior, anterior and right side of pulmonary valve. 6. Subpulmonary membranous stenosis. 7. Non-committed ventricular septal defect. We made a tunnel between VSD and aorta with Teflon patch so that arterial blood comes through VSD and the tunnel into aorta. After correction the patient needed assisted circulation for 135 min. to have adequate blood pressure. Postoperatively by any means, adequate blood pressure could not be maintained and expired in the evening of operation day.

A Study on Body Satisfaction and Fitness Apparel Based on Body Type by Body Mass Index: In Women 20-50's Years of Age (BMI지수에 의한 신체유형별 신체만족도와 의복적합성에 관한 연구: 20~50대 여성을 중심으로)

  • The purpose of this study is to classify body type by BMI and to inquire about body satisfaction and fitness apparel depending on body type among women 20-50years of age. As a result, body types are classified into three groups: lean, normal, and obese figures. On front silhouette, the normal type occupies most in women belonged to lean figure group, the obese lower part of the bodytype in normal figure group, and the obese upper part of the body type in obese figure group. On the other, in side silhouette, the slender type is prevalent in lean figure group, hip obesity in normal figure group, and trunk obesity in obese figure group. In particular, women in the obese figure group were distributed among the various body types. The obese figure group had a lower fitness apparel in the measurement of circumference(e.g., chest, waist, and hip) related to obesity in comparison with measurement of length. Therefore, the development of an optimal sizing system in response to the various body types in the obese figure group is needed to provide more diversity in aesthetic design and continuity among various sizing systems.

A Study on Characteristics of Dyspnea in Patients with Chronic Obstructive Pulmonary Disease in the Emergency Department Visits (응급실에 내원한 만성 폐쇄성 폐질환 환자의 호흡곤란 속성에 관한 연구)

  • Purpose: The purpose of this study was to examine descriptors of dyspnea and clinical characteristics in patients with COPD at ED visit. Method: The study design was a descriptive survey, and 46 patients with COPD complaining of dyspnea were participated during their visits to the ED of C university hospital in G city, from October 1, 2004 to April 8, 2005. Result: The qualitative characteristics of dyspnea at the time of decision to the ED visit were short of breath (71.7%), tight (32.6%), hard to breathe (15.2%), smothering or suffocating (10.9%), gasping (8.7%), couldn't breathe (6.5%), constricted (2.2%), and hunger for air (2.2%). In the item of dyspnea checklists, My chest felt tight was the most common description among subjects. As the result of factor analysis of dyspnea checklists, the first factor was characterized rapid and shallow, the second was suffocating/smothering, the third was hunger for air, and the fourth was constricted. Conclusion: This study suggests that a checklist of dyspnea descriptors based on this findings would be utilized as a tool of initial and ongoing assessment for dyspneic patients with COPD in the ED after identifying the validity and reliability of the checklist.

  • An intelligent developer replenish system was developed for automatic film processor. The system detects the gray level of a developed film and determines the amount of the developer to be added to the system. Using the film darkness detection system, it was found that the film darkness is inversely proportional to the amount of light exposure, that the developer retrograde after developing certain number of films without replenishment and the number is dependent upon the parts of the body, and that the developer does not retrograde when 30 ml of developer is added after each chest film processing. The use of this system resulted in a decrease in the amount of developer by 40%.

Effect of Albumin Administration on Erythrocyte Crenation in Patients Undergoing Extracorea1 Circulation - A Report of 8 Case - (체외심폐순환중 알부민이 적혈구의 Crenation 에 미치는 영향)

  • The Free fatty acid [FFA] induced massive erythrocyte crenation has been reported in patients undergoing extracorporeal circulation[EGG], suggesting that the crenated cells impair microcirculatory flow and tissue oxygenation during and after the operation. Effect of albumin administration on erythrocyte crenation was examined in 24patients undergoing open heart surgery at the department of Thoracic and Cardiovascular Surgery in the Kyung Hee University Medical Center Between January, 1989 and August, 1989: 8 patients of 24 patients [group A] were given 50 gm albumin, another 8 patients of 24 patients [group B] were g en 26 gm of albumin and the other 8 patients of 24 patients [group C] were not given any albumin as a part of the priming solution. Erythrocyte crenation was almost completely prevented in the patients given 50 gm albumin. Crenated erythrocytes d urging ECC were 3.6* 0.4 % [mean\ulcornerD] of all erythrocytes and 3.1*0.6% after ECC. This was significantly lower than in patients without albumin administration [64.2*9.5% during ECC and 28.7*1.9% after ECC]. But the effect was less striking in the patients given 25 gm albumin, 37.8* 12.0 % during ECC and 27.8*3.9 % after ECC. Maintaining an adequate level of plasma albumin is important in preventing erythrocyte crenation during ECC, improving microcirculatory flow in patients undergoing open heart surgery.

  • A patient,58 years of age, with suspected 0/l pneumoconiosis since 1993, complained of a dry cough and exertioning dyspnea for 6 months. He had worked in an asbestos company for more than 20 years from 1974. He was subsequently diagnosed with an interstitial lung disease during an annual special health check-up for asbestos workers. h chest X-ray showed an interstitial lung disease and high-resolution computed tomography (HRCT) showed a round opaque asbestosis with chronic hypersensitivity pneumonitis. A pulmonary function test indicated that the patient had a mild restrictive lung disease with FEV1 1.67 litters and 82% FEVl/FVC. The bronchoalveloar larvage fluid included many asbestos bodies, indicating previous exposure to asbestos. Transmission electron microscopy (TEM) using an energy dispersive X-ray analyzer (EDX) revealed many asbestos bodies consisting of mainly crocidolite fibers (6,071$\times$$10^6$fibers/g of dry lung). The patient had an unusually high asbestos content of 6,112$\times$$10^6$ asbestos fibers/9 of dry lung.

Studies on Proteus Spp Isolated from Clinical Sources (임상검체에서 분리된 Proteus Spp에 관한 고찰)

  • Seven hundred and forty eight strains of proteus spp were isolated from clinical sources at National Medical Center during recent five years(1968 to 1972). 1. These strains identified as follows: P. mirabilis 357 strains(47.7%) P. vulgaris 140 strains(18.7%) P. morganii 134 strains(17.9%) P. rettgeri 63 strains(8.4%) P. inconstans 54 strains(7.2%) 2. About the incidence between both sexes, male is higher than female. 3. Total numbers of the age group from $20{\sim}29$ years, $30{\sim}39$ years, and $40{\sim}49$ years old respectively are higher than total numbers from other age groups. 4. Seasonal trend shows hot season e.g. August to September is more prevalent. 5. Correlation of strain source with clinical department shows the highest incidence of general surgery next to this, orthopedic surgery, chest surgery, internal medicine, neurosurgery, ENT and pediatrics etc. in descending order. 6. Correlation of strain source with clinical material shows the highest incidence of pus next to this urine, stool, sputum etc. in descending order. 7. The rate of susceptibility to several antibiotics is low in general except gentamicin(82.7%) and carbenicillin(72.9%). However, in case of gentamicin sensitive strains are 15(9.6%) and moderately sensitive strains are 114(73.1%).

  • This study was undertaken to evaluate the growth, body shape and carcass cutting yield traits of Duroc (D) and crossbred (Duroc×Pietrain×Pietrain; DPP) pigs. A total of 147 D and 101 DPP pigs were used for analyzing the growth trait, whereas 16 D and 16 DPP pigs were evaluated for carcass yields. Backfat thickness (BF) and average daily feed intake (ADFI) were significantly higher in D (14.07±0.24 mm, 2,101 g) than in DPP (12.69±3.25 mm, 1,909 g) (p <0.001). Moreover, D exhibited significantly higher body shape traits including body height (BH), chest depth (CD) and chest width (CW), as compared to DPP pigs (p <0.001). No differences were observed for body length (BL) between the two strains. Analysis of the carcass cutting yield traits determined for D and DPP were in the order: ham (HM; 31.17% and 33.43%), belly (BY; 23.40% and 19.55%), and picnic shoulder (PS; 16.54% and 16.87%), respectively. Then, HM showed a difference of 2.26% P with D(31.17%) and DPP(33.43%), while BY showed a difference of 3.85% P with D(23.40%) and DPP(19.55%). Taken together, our results indicate that DPP has a better feed efficiency than D, and therefore has the potential to increase the production of low-fat pork, targeting consumers having a high preference who have opted for a healthy lifestyle. These results can be used as basic data for developing an ideal pig breed.
